Sometime between now and June — the exact date is yet to be determined — Randy Olson will take down the framed honors hanging on the walls of his fifth floor office, soak in one final look at the panoramic view of the Salt Lake Valley below, and walk away from the John A. Moran Eye Center to join the ranks of the newly retired.
He'll be 78 in April, so no one's suggesting this is premature, but, still, no one's entirely sure the building will remain standing once he's gone.
Few, if any, institutions in the state are as entwined with one person as the Moran Eye Center is with Dr. Randall J Olson. Would it be there without him? Or would it still be a parking lot? And would Utah be home to one of the greatest eye care centers in the world?
Probably not.
'I hope you've got good equity in your house because I'm sure as heck not paying for it.'
That's how the chair of surgery at the University of Utah Medical Center responded when Olson asked if he could add a third ophthalmologist to the staff. Only the chair didn't say heck.
This wasn't ancient history. This was the fall of 1979. A few months earlier, Olson, 32 years old and six years removed from graduating from the U. of U. Medical School, had arrived in Salt Lake to head up the hospital's ophthalmology division. Although 'head up' is being generous, as is 'division.' Olson was the only ophthalmologist.
It had taken some hard talking for him just to get the job, because the med center was thinking seriously about not replacing Olson's predecessor, who had departed for greener pastures. Eye care at the U. was losing money, a perennial drain on the system. They reluctantly gave Olson an $80,000 loan for startup operational expenses and an office so tiny it had a pocket door just so a desk could fit inside.
Yet here he was, a few months later, threat of losing his house notwithstanding, wanting to triple the faculty when they couldn't even afford more staff.
He talked a former med school intern named Mano Swartz to hire on for below-market pay, and the first thing the two of them did was take a road trip. They loaded into Swartz's car, a Chevy Monte Carlo, turned up the volume on the eight-track, stayed at the cheapest motels they could find and visited ophthalmologists throughout northern Utah, Idaho, Wyoming, and parts of Montana.
Their message was a simple one: refer your patients to us, and they'll get excellent care.
If the gamble hadn't worked, you wouldn't be reading this.
But as it turned out, word of mouth, and the doctors' power of persuasion, proved to be a momentum turner. By the end of that first fiscal year, in June of 1980, the $80K loan was paid back and ophthalmology at the U. was in the black.
Solvency meant survival, and that meant Olson could begin to concentrate on his long-range dream: to bring world-class eye care to the Intermountain West, something similar to the facilities he'd seen while training at UCLA and LSU.
It would take volumes to detail all that's happened in the decades since to make the dream come true — something Dr. Olson says he plans to tackle when he writes his memoirs — but suffice it to say the history is full of Olson being told 'no' and him proving to be pathologically incapable of hearing it. That and generous people appearing on the scene just when they were most needed.
Most significant of these donors was John Arthur Moran, a successful investment banker and devoted University of Utah graduate (Class of 1954) who was introduced to Olson by former university president Chase Peterson.
Olson remembers what would prove to be a most auspicious event:
'We met in the president's office. Chase was a consummate fundraiser. We had a dog and pony show for John, after which John said, 'I like it, I want to be involved. What do you need?' I had no idea what to say and Chase just came up with a number. He said, 'John, we need $3 million.' John said, 'Can you do what you need to do to get this center up and going?' and Chase said, 'yes, we can,' and I'm thinking, 'I'm not sure if we can or not.'
'Then John asked if we would like that in installments or one single donation. Chase said, 'one single donation,' John said 'OK,' and just like that we were off and running. We broke ground in 1991.'
The first John A. Moran Eye Center, built for $17 million thanks to other donors who joined Moran, opened in 1993, a gleaming new 85,000 square foot building next to the Huntsman Cancer Institute.
In less than a decade it was too small.
Again, Moran stepped up, this time agreeing to donate $18 million toward a bigger headquarters. The donations from the old building were put back into the new one, while the Sam Skaggs and Ezekiel R. Dumke families contributed an additional $10 million each to top off the fundraising. In 2003 construction began in a parking lot south of Primary Children's Hospital on the second John A. Moran Eye Center.
Since then, an additional 11 remote clinics have been added, along with an outreach program that provides humanitarian eye care aid throughout the world.
Today, the Moran is considered among the world's top eye care institutions. Its researchers and clinicians are perennials on The Ophthalmologist magazine's Power List ranking of the 100 most influential people in the industry. A business that had two employees and $180,000 net revenue in its first year now has more than 500 employees and a yearly net revenue of $90 million. It is incalculable how many eye patients have been helped in the past 46 years, just as it is incalculable to compute how much the Moran Eye Center means to the Intermountain West.
All traced back to the vision of one man.
Postscript: As the final day approaches, and the University of Utah wraps up its nationwide search for his successor as CEO, no one is surprised that Olson is using his pending retirement as a vehicle to — what else — raise money.
Brochures with his smiling face on the cover have been mailed to prospective donors near and far, hailing 'a visionary' and highlighting the remarkable story of Randall J Olson and the Moran Eye Center.
'I'm trying to see that there's a nice buffer for my successor,' says the soon to be retired ophthalmologist. 'There's so much still we want to do, so much more to accomplish.' Then, with a smile in his eye he adds, 'and you know, we are running out of space.'

This is the warning of a new study led by the University of Michigan (U-M) Medical School, which looked into the problem known as "boarding." "When patients come into an emergency department (ED) and undergo treatment, many will require ongoing care in the hospital for days. Those patients are 'admitted' and treatment teams in the ED request a hospital bed," study author and U-M Health emergency physician Alex Janke told Newsweek. "When they wait for long periods of time, usually from a treatment space in the ED, this is what we mean by boarding." Janke said the types of health conditions people might be waiting for emergency hospitalization with include sepsis, pneumonia, kidney infection, heart attack, stroke and injuries like a fall with a broken hip. While it may be exacerbated at certain points, the problem doesn't only occur in the winter months—when viral infections rise and inevitably lead to more emergency hospitalizations—the team found. In the last three years, more than 25 percent of all patients who came to a hospital's ED during a non-peak month and got admitted to the same hospital waited four hours or more for a bed. During the winter months this was closer to 35 percent. Patients shouldn't board in an ED for more than four hours for safety and care quality reasons, as per national hospital standards. The study also found that by 2024, nearly five percent of all patients admitted to a hospital from its ED during the peak months of winter waited a full day for a bed. During the off-peak months, 2.6 percent still waited that long. The research team analyzed 46 million emergency visits that led to hospitalizations at the same hospital. The data came from the electronic health record systems of 1,500 hospitals in all 50 states, from the start of 2017 to the end of September 2024. Busy hospital waiting room. Boarding can lead to delays in needed care, and resultant crowding in the ED can increase the risk of medical errors," Janke explained. Boarding also makes it difficult for EDs to see new patients as they arrive. "Sustained high levels of boarding, as we have seen over the past three years, suggest the health system is at risk of collapse in the event of another pandemic," Janke warned in a statement. By 2024, even in the months with the lowest rates of boarding patients, the percentage of patients who waited four or more hours for a bed was higher than it had been during the worst times of year in 2017 to 2019, according to the study. While less than five percent of patients waited more than 12 hours for a bed even at the peak times in pre-COVID years, it now rarely goes below five percent even at the lowest times of year. The worst point was January 2022, when 40 percent of patients boarded in an ED for more than four hours, and 6 percent boarded for 24 hours or longer. "The primary driver of boarding is insufficient 'downstream' hospital capacity," explained Janke. While boarding has grown nationwide and in all patient groups, the Northeast had the highest rate of boarding for 24 hours or more. Boarding during peak months also rose especially quickly for people aged 65 and over, those whose primary language is something other than English or Spanish and Black patients. "This may reflect differences in the hospitals where those patients seek care, or may reflect a bias in the care they receive in the hospital. Our study did not address this," said Janke. The researchers say their work highlights the need to prepare for winter peaks and to address the year-long issue of mismatches between acute care demands and available resources. They cite a report from an ED boarding summit held last fall by the Agency for Healthcare Research and Quality, which notes this mismatch, as well as proven solutions for ED boarding. These include smoothing out surgical schedules across the week to allow more rapid movement of ED patients to inpatient beds, streamlining discharges to earlier in the day and on weekends, using discharge lounges, using bed managers and providing alternative services for patients experiencing mental or behavioral health emergencies. The summit's participants called for more measurement and public reporting of ED boarding, more sharing of data about bed availability within regions, help for rural hospitals including telehealth consults and transferring patients needing higher-level care and efforts to reduce the need for inpatient behavioral health care. "Among the most important next steps is infrastructure to support regional capacity and load-balancing during periods of strain. Fundamentally, though, we have to build more hospital care capacity. We cannot 'optimize' our way out of this," said Janke. With the number of patients seeking emergency care rising, new short-stay units, home-based hospital-level care and specialized providers in the triage area have all been taken into account by U-M Health. A new 264-bed Michigan Medicine hospital currently under construction and due to open later this year—the D. Dan and Betty Kahn Health Care Pavilion—is intended to provide more patient space for patients admitted from the ED. Dr. Prashant Mahajan, professor and Michigan Medicine emergency medicine chair, emphasized the need for more studies on boarding and related issues nationwide, to inform policymaking. "We need rigorous research, to better understand this problem and identify sustainable solutions," he said in a statement. Angela Alsobrooks (D-Md.) and Cynthia Lummis (R-Wyo.) will introduce a bill to award federal grants to study uterine fibroids as one of several competing bipartisan measures to support study of the noncancerous growths common in women of childbearing age. The bill would establish a new grant program of unspecified amount and duration to support research on early detection of and intervention for uterine fibroids, including screening procedures. Alsobrooks is also a cosponsor on a separate bipartisan measure, introduced by Sen. Cory Booker (D-N.J.), that would carve out $30 million over four years to study uterine fibroids, although not necessarily through a direct grant program. A spokesperson for the Maryland senator's office said the two 'approaches need to work in tandem — supporting direct research dollars, while also empowering the [Health and Human Services] Secretary to support innovative programs across the country that can reach patients where they are in community.' Eligible topics under the grant program would include disparities in pain control in surgery for uterine fibroids, as well as conditions like Asherman's Syndrome, where scar tissue builds up inside the uterus. Alsbrooks and Lummis's bill would also support programs to raise awareness for uterine fibroids. 'For too long, uterine fibroids have plagued women. We need to invest in research, raise public awareness, and develop programs for early treatment,' Alsobrooks said in a statement. 'I am proud to reach across the aisle and partner with Senator Alsobrooks to ensure women in Wyoming and across the country suffering from uterine fibroids no longer suffer in silence,' Lummis said in a statement. 'Our bipartisan legislation will expand access to screenings and intervention, providing hope to the millions of women who endure this painful condition.' The push to study the health condition, estimated to affect 40 percent to 80 percent of women, comes after the Trump administration has pushed to cancel research grants related to 'diversity, equity and inclusion,' health equity and other topics it deemed to be wasteful. The Department of Health and Human Services has pulled at least two grants this year directly studying uterine fibroids, including a $1.5 million study at the University of Texas and a doctoral student's fellowship at Columbia University. The bill, officially called the Uterine Fibroid Intervention and Gynecological Health Treatment (U-FIGHT) Act, also has a companion in the House introduced by Rep. Shontel Brown (D-Ohio) as part of a larger legislation package on women's health. Brown's group of bills includes a law to study the relationship between hair straighteners and uterine cancer. Lummis is the first Republican senator to support the U-FIGHT Act. The bill has also won the support of actress Lupita Nyong'o, who was diagnosed with uterine fibroids and had surgery to remove them.
