Researchers say changes in stride can ease arthritis pain

Slightly altering your stride while walking could considerably ease pain caused by wear-and-tear knee arthritis, a new study says.
Foot positioning while walking can reduce stress on a person's knee joint, researchers reported Tuesday in The Lancet Rheumatology.
People trained to angle their feet slightly inward or outward from their natural alignment experienced slower degeneration of the cartilage cushion inside their aching knees, results show.
They also reported greater reductions in knee pain and better knee function after a year, researchers said.
"Altogether, our findings suggest that helping patients find their best foot angle to reduce stress on their knees may offer an easy and fairly inexpensive way to address early-stage osteoarthritis," said co-lead researcher Valentina Mazzoli, an assistant professor of radiology at NYU Grossman School of Medicine in New York City.
This strategy could lower patients' reliance on painkillers and delay the need for knee replacement surgery, Mazzoli added in a news release.
For the study, researchers recruited 68 people with knee osteoarthritis and recorded their gait while walking on a treadmill. A computer program simulated their walking patterns and calculated the maximum stress they were placing on their knees.
The research team also generated computer models of four new foot positions angled inward or outward by 5 or 10 degrees and estimated which would best reduce stress on each person's knees.
Participants then were randomly divided into two groups. Half were trained in six sessions to walk with the foot angle ideal to them, and the other half were told to continue walking normally.
Overall, those who adjusted their gait reduced maximum loading in their knees by 4%, while those who kept their normal stride increased loading by more than 3%.
Further, those taught the new foot position experienced a 2.5-point reduction on a 10-point pain scale, equivalent to the effect of over-the-counter painkillers like NSAIDs and acetaminophen, researchers said.
"These results highlight the importance of personalizing treatment instead of taking a one-size-fits-all approach to osteoarthritis," Mazzoli said. "While this strategy may sound challenging, recent advances in detecting the motion of different body parts using artificial intelligence may make it easier and faster than ever before."
AI software that estimates joint loading using smartphone videos is now available, allowing doctors to perform a gait analysis without specialized lab equipment, researchers noted.
The team next plans to test whether those AI tools can indeed help identify the best walking method for knee arthritis patients, Mazzoli said. They also plan to expand their study to include people with obesity.

Monica Seles first noticed the symptoms of myasthenia gravis — a neuromuscular autoimmune disease she discussed during a recent interview with The Associated Press — while she was swinging a racket the way she'd done so many times during, and after, a career that included nine Grand Slam titles and a place in the International Tennis Hall of Fame. 'I would be playing with some kids or family members, and I would miss a ball. I was like, 'Yeah, I see two balls.' These are obviously symptoms that you can't ignore,' Seles said. 'And, for me, this is when this journey started. And it took me quite some time to really absorb it, speak openly about it, because it's a difficult one. It affects my day-to-day life quite a lot.' The 51-year-old Seles, who won her first major trophy at age 16 at the 1990 French Open and played her last match in 2003, said she was diagnosed with myasthenia gravis three years ago and is speaking publicly about it for the first time ahead of the U.S. Open, which starts on Aug. 24, to raise awareness about what is known as MG. The National Institute of Neurological Disorders and Stroke calls it 'a chronic neuromuscular disease that causes weakness in the voluntary muscles' and 'most commonly impacts young adult women (under 40) and older men (over 60) but ... can occur at any age, including childhood.' Seles said she'd never heard of the condition until seeing a doctor and being referred to a neurologist after noticing symptoms such as double vision and weakness in her arms — 'just blowing my hair out ... became very difficult,' she said — and legs. 'When I got diagnosed, I was like, 'What?!'' said Seles, who is partnering with argenx, an immunology company headquartered in the Netherlands, to promote their Go for Greater campaign. 'So this is where — I can't emphasize enough — I wish I had somebody like me speak up about it.' It's been three decades since Seles returned to competition at the 1995 U.S. Open, making it to the final, more than two years after she was attacked by a man with a knife at a tournament in Hamburg, Germany. 'The way they welcomed me ... after my stabbing, I will never forget,' Seles said about the fans in New York. 'Those are the moments that stay with you.' She talks about learning to live a 'new normal' nowadays and characterized her health as another in a series of life steps that required adapting. 'I had to, in tennis terms, I guess, reset — hard reset — a few times. I call my first hard reset when I came to the U.S. as a young 13-year-old (from Yugoslavia). Didn't speak the language; left my family. It's a very tough time. Then, obviously, becoming a great player, it's a reset, too, because the fame, money, the attention, changes (everything), and it's hard as a 16-year-old to deal with all that. Then obviously my stabbing — I had to do a huge reset," Seles said. 'And then, really, being diagnosed with myasthenia gravis: another reset. But one thing, as I tell kids that I mentor: 'You've got to always adjust. That ball is bouncing, and you've just got to adjust,'' she added. But reality might take some shine off such stories, a new study says. Participants in combat sports like boxing or mixed martial arts who grew up in poor neighborhoods might be more likely to suffer brain changes associated with degenerative brain disease, compared to fighters who come from affluent backgrounds, researchers report. Specifically, they had smaller brain volumes in regions important for movement and memory, including the thalamus, cerebellum and hippocampus, according to findings published Aug. 13 in Neurology Open Access. "It is well-known that high-impact sports such as professional fighting are at a greater risk of repetitive head injuries," lead researcher Brooke Conway Kleven, a postdoctoral scholar at the University of Nevada, Las Vegas, said in a news release. "We found that socioeconomic status may influence how the brain responds to these repetitive impacts." For the study, researchers performed MRI brain scans on 100 professional combat sport athletes. All participants had a minimum of 10 professional fights and were currently active or recently retired from competition. During five years of follow-up, 20% of the fighters developed thinking and memory problems. Analysis revealed that athletes from impoverished neighborhoods had a thinner outer layer in certain brain areas, which can be a sign of early brain degeneration. They also had experienced greater shrinking in several key brain regions, including the thalamus and hippocampus, compared to fighters from well-off neighborhoods. This link remained even after accounting for factors like the fighters' race, ethnicity, income and overall number of fights. "Stressors related to income, employment, education and housing quality may compound the risks of repeated head trauma," Conway Kleven concluded. "Athletes from marginalized or under-resourced communities may face a heavier burden when it comes to brain health." However, researchers noted that because this was an observational study, it could not prove a direct cause-and-effect link between a fighter's economic background and their risk for brain disease, only an association. Further study is needed to validate the results and explore why upbringing might play a role in risk of brain damage, researchers said. "The findings support a broader understanding of brain health -- one that considers not just training or fight history, but also the social and economic context in which people live," Conway Kleven said. "Our study suggests that where someone grows up matters. Recognizing stressors can help inform more equitable approaches to protecting long-term brain health." File Photo by Keizo Mori/UPI | License Photo Aug. 14 (UPI) -- Apple will roll out an update to the Apple Watch on Thursday to add its new pulse oximeter feature that has been stalled because of an intellectual property dispute. Apple said in a press release that it will update Apple Watch Series 9, Series 10 and the Apple Watch Ultra 2. These models will get the redesigned Blood Oxygen feature. "Apple's teams work tirelessly to create products and services that empower users with industry-leading health, wellness, and safety features that are grounded in science and have privacy at the core," Apple said in a statement. "Apple Watch Series 9, Series 10, and Apple Watch Ultra 2 also offer users irregular rhythm notifications, the ECG app, Sleep Apnea Notifications, Fall Detection, sleep tracking, wrist temperature sensing, the Vitals app, the Noise app, the Medications app, and the Mindfulness app, among many other helpful tools, helping users live a healthier life." The International Trade Commission in October 2023 ruled in favor of a request to halt sales of the Apple smartwatches by medical technology firm Masimo Corp. Masimo officials say Apple co-opted its patented technology and poached several workers who helped to develop it. Apple then paused the sale of some of its watches and began selling modified versions of them without the blood-oxygen feature. The update is possible thanks to a recent U.S. Customs ruling, the company said. There will be no effect to Apple Watch units previously purchased that include the original Blood Oxygen feature or to Apple Watch units purchased outside of the U.S., Apple said.
