
Trapped in terrain: how a range of factors complicate Gadchiroli's fight against malaria
Despite contributing only about one per cent to Maharashtra's population, Gadchiroli, a region affected by Maoist insurgency, accounts for approximately 50 per cent of the state's malaria cases. It faces a significant malaria problem, consistently reporting an Annual Parasite Incidence (API) above 2. An API of 2 means there are 2 or more confirmed malaria cases per 1,000 population in a year.
This year, over 2,060 malaria cases have been identified here and three deaths have been reported so far, said Avishyant Panda, District Collector, Gadchiroli. The district reports malaria deaths every year – 13 in 2022; 12 in 2023 and 14 in 2024. The year 2021 saw the most cases in the last five years – 12,326, though only eight deaths were reported that year. Over the last five years between 2019 and 2024, a total of 51 deaths have been reported, as per the data provided by the malaria task force.
Starting April 1, this year, Gadchiroli launched a comprehensive malaria eradication plan. A Malaria Eradication Task Force was initiated under the Public Health Department and chaired by Dr Abhay Bang, a public health expert and the Founder and Director of Society for Education, Action and Research in Community Health (SEARCH). Chief Minister Devendra Fadnavis also announced to allocate Rs 25 crore for the initiative through the Tribal Development Department.
'This year, the rain started in May. Usually, rain starts in mid-June. And so, small water collections have occurred in the forest everywhere. Hence, mosquito breeding has started much earlier. That is why the task force has cautioned the district administration that there is a likelihood of an early upsurge in malaria cases this year. And we have to be prepared to face a rise in cases this year on a war footing,' said Dr Bang.
Terrain and other key challenges
The district faces several geographical and environmental challenges – 76 per cent of Gadchiroli is covered by forests and hills, hindering development. Dense forests, rivers, and hilly terrain create ideal conditions for mosquito breeding, say sources.
High transmission of Plasmodium falciparum (PF) – one of the two primary species of malaria parasites commonly found in India – is frequently observed, particularly in tribal blocks, and malaria demonstrates persistent and perennial transmission, with peaks occurring in July-August, according to the task force data.
While Plasmodium falciparum is known to cause more severe and potentially fatal cases of malaria, Plasmodium vivax is generally considered milder.
Panda told The Indian Express, 'The district is working on the availability of drugs and logistics for which we are specifically targeting three talukas that get cut off.'
'In malaria-positive cases, there has been a difficulty regarding whether to take blood smears. Every year, nearly 10 lakh blood smears have to be taken in the district. But really, microscopy of 10 lakh blood smears is a Herculean task. Then, a backlog develops. Hence, the task force has decided that all fever cases should be instantly examined by what is called RDT, Rapid Diagnostic Test. So, within 15 minutes, the result is available. And if it is positive for malaria, the patient is treated. So, every fever case must be tested by RDT as a regular practice,' informed Dr Bang.
He added, 'There is an implementation committee to carry out this plan, under the chairmanship of the collector. So, this has not remained merely a health department programme. The collector, zilla parishad CEO, and the health departments are all involved, with the collector chairing it. The implementation would therefore involve various other sectors of the administration as well.'
Socio-cultural factors
There are other socio-demographic and cultural factors contributing to the issue. Ninety-seven per cent of the population lives in rural and tribal villages, which are often scattered and difficult to access. Locals often prefer traditional healers or pujaris for fever, leading to delayed malaria diagnosis and treatment. Risky practices such as outdoor sleeping and non-use of long-lasting insecticidal nets (LLINs) further increase the risk of malaria. Poor socio-economic conditions, low literacy, and lack of hygienic conditions are also contributing factors.
'We are trying to increase the testing efforts and routine surveillance. The RDK (Rapid Diagnostic Kit) method is being used for diagnosis in the field, and when a case is found positive, the first anti-malaria dose is administered. IEC (Information, Education and Communication) activities are also being carried out in Marathi and Madia languages,' Panda said.
'A school IEC programme is also being launched, with nodal teachers being appointed, even in ashram schools, because these are also used as shelter homes during floods. It is also being ensured that the school IEC programme benefits the parents as well,' Panda said.
'Some never reach institutions'
Talking about factors associated with delay in treatments, Dr Bang said. 'There are several delays. There are various superstitions linked with fever in tribal areas, and people usually don't seek help immediately. They first go to the pujari. Then the pujari does his process, and so two–three days, sometimes even five days, are wasted there. From there on, most of the patients can actually be treated in the village itself by the ASHA workers (Accredited Social Health Activists) or the ANMs (Auxiliary Nurse Midwives).'
'But a small percentage, maybe 5 to 10 per cent, have more severe malaria and they need hospitalisation. So they are appropriately selected and referred to the hospital for treatment. There are delays that occur at every level. Besides, some people never reach institutions. And so certain measures have been decided on how we can avoid this,' Dr Bang said, adding that the task force also plans to seek pujaris' cooperation to tackle this issue.
'I recently held a meeting on anti-superstition practices with the police and revenue machinery. Here, malaria patients are often taken to pujaris. We have almost 2,000 pujaris in the district, so we have mapped them, and we want to start a programme where pujaris encourage people to go to medical facilities rather than treat them on their own. Deaths occur when medicines are not administered in time,' said Panda.
Dr Bang adds, 'Of all the malaria deaths reported in Gadchiroli, nearly half of them are in children below 15 years of age. And world over, children are more vulnerable to malaria and to more serious forms of it.'
Small villages, manpower shortage
Three hundred villages lack Accredited Social Health Activists (ASHAs). There are also long-standing vacancies in vital manpower positions like Multi-Purpose Workers (MPWs), Auxiliary Nurse Midwives (ANMs), and technicians.
'The peculiarity of the Gadchiroli district is that, especially in the tribal areas, village sizes are very small. There are nearly 100 villages with a population of less than 100. So, for Gadchiroli, the ASHA norm has been relaxed by the Government of Maharashtra, and smaller villages are allowed to be included. So, an additional 191 ASHAs will be recruited for this need,' informed Dr Bang. He also informed that 200 positions for multi-purpose workers were filled recently.
He added, 'Yet, nearly 100 villages remain where you can't imagine having an ASHA because the population includes hardly 10 to 15 homes. So there, we have decided that a resident in the village who is literate will be trained to use RDT and diagnose malaria.'
Identifying villages that are more prone
In 2024, 159 villages were classified as Category A (Annual Parasite Incidence (API)>50), indicating a very high malaria burden. Additionally, 357 villages were Category B (API 5-49.99), and 1,183 villages were Category C (API <4.99). Across six tribal blocks, 19 Primary Health Centres (PHCs) are classified as High Malaria Endemic with API>2, affecting 3.2 lakh people. Similarly, eight PHCs across six non-tribal blocks are High Endemic with an API >2, affecting 1.3 lakh people. This makes a total of 27 priority PHCs.
Bhamragad, a tribal block in Gadchiroli which alone had over 174.3 API in 2021, contributes over 40 per cent of the cases in the district. Dr Supriyalakshmi Totiger, Deputy Director, Community Health Research at SEARCH, and a member of the Malaria Task Force in Gadchiroli told The Indian Express, 'Bhamragad has very thick central Indian forest. With that kind of ecosystem, you tend to find a lot of breeding sources easily available in the natural ecology, so removing mosquitoes and reducing mosquito density is difficult in such places. That is a major contributor to malaria in Bhamragad.'
Taskforce sets 2027 target
'The last stage in malaria elimination is eliminating it in tribal regions. The task force identified 29 PHCs (Primary Health Centres) as high-risk areas in six tribal blocks and eight PHCs in non-tribal blocks. This covers around the population of 4.75 lakh, which is high risk. Hence, Bhamragad tops the list, followed by Etapalli and then Dhanora,' said Dr Totiger.
'One of the powerful strategies in tackling malaria, also recommended by the World Health Organization (WHO), is the use of LLINs, that is, long-lasting insecticide-treated bed nets. Under the Malaria Mukta Gadchiroli Abhiyaan, the task force will distribute 5 lakh LLINs, so that every family will have one mosquito net per person,' informed Dr Bang.
Collector Panda added, 'For identified high-risk areas, we are conducting full-scale Indoor Residual Spraying and LLINs distribution. Fifty thousand LLINs have been procured and distributed so far.'
The task force aims to achieve zero deaths due to malaria by 2027 and an API of less than one. The task force has also been supported by the Bill & Melinda Gates Foundation, Foundation for Disease Elimination and Control of India under Sun Pharma, Godrej Foundation, Indian Council of Medical Research (ICMR), and the National Institute of Malaria Research (NIMR).
'The main season of malaria starts from July onwards. So, we have to wait and see what happens this year. It depends so much on the climate, rain, etc. Worldwide, malaria occurs cyclically. Every five or six years, you get an upsurge. The last upsurge was in 2021. So, one would expect, as a part of the natural cycle, another upsurge in 2026 or 2027,' Dr Bang said.

Medical Education Scam Busted: Godman, Ex-UGC Chief, Among 34 Officials Named
Last Updated: The alleged corruption spans multiple states, pointing to a nationwide racket involving approvals, inspections, and recognitions granted to several private medical colleges. In what it has called as one of the largest medical education scams in India, the Central Bureau of Education (CBI) has busted a nexus of top government officials, intermediaries and representatives of private medical colleges, who were allegedly involved in 'egregious" acts, including graft and manipulation of the regulatory framework governing medical colleges. The alleged corruption spans multiple states, pointing to a nationwide racket involving approvals, inspections, and recognitions granted to several private medical colleges. As per the agency, the corruption involves top officials from the union health ministry, the National Medical Commission (NMC), middlemen, private college representatives, prominent educationists, and even a self-styled godman, news agency PTI reported. High-Profile Names Under Scanner: The scam, unearthed by the Central Bureau of Investigation, has opened the lid on a number of deep links the officials had with each other. The agency named several high profile names in its FIR including, The central probe agency arrested eight people in the case recently. These include three doctors of the NMC team who were held for allegedly taking a bribe of Rs 55 lakh for giving a favourable report to the Naya Raipur-based Rawatpura Institute of Medical Sciences and Research. The agency has alleged that Ravi Shankar of the Rawatpura Institute wanted advance information about the inspection. A director of the institute, Atul Kumar Tiwari, also named in the FIR, got in touch with Raval to get the information unlawfully. Ravi Shankar also got in touch with DP Singh to get a favourable inspection report in lieu of bribes, the CBI alleged and added that Singh delegated the task to one Suresh. There was no immediate reaction from Singh. Raval had allegedly demanded Rs 25-30 lakh for the information and gave the names of assessors and the date of inspection to the institute officials. The syndicate has its roots in the Union Health Ministry, where the eight accused officials ran the sophisticated scheme facilitating unauthorised access, illegal duplication and dissemination of highly confidential files and sensitive information to representatives of medical colleges through a network of intermediaries in exchange for huge bribes, the FIR alleged. These officials, along with the intermediaries, manipulated the statutory inspection process conducted by the NMC by disclosing inspection schedules and identities of the designated assessors to the medical institutions concerned well in advance of the official communication. The CBI has named the Union Health Ministry's Poonam Meena, Dharamvir, Piyush Malyan, Anup Jaiswal, Rahul Srivastava, Deepak, Manisha and Chandan Kumar as accused in the FIR. These people allegedly located files and clicked photographs of notings and comments made by senior officers and sent these to middlemen and representatives of medical colleges. The crucial information of internal processing of medical institutions in the ministry gave an alarming degree of leverage to colleges. The agency has alleged that several middlemen and representatives of medical colleges, including Bhadoria, Joshy Mathew, Udit Narain, Virendra Kumar, and Manisha Joshi, were tapping accused Health Ministry officials for information on NMC assessments. All of them have been named in the FIR. 'Such prior disclosures have enabled medical colleges to orchestrate fraudulent arrangements, including the bribing of assessors to secure favourable inspection reports, the deployment of non-existent or proxy faculty (ghost faculty), and the admission of fictitious patients to artificially project compliance during inspections, and tampering with the biometric attendance systems to falsify," the FIR said. The CBI has alleged that Bhadoria was cloning artificial fingers to create a fake biometric attendance of doctors in the hospital. The agency has mentioned bribes running into lakhs of rupees being exchanged between NMC teams, intermediaries and representatives of medical colleges, being routed through hawala and used for multiple purposes, including the one in the name of construction of a temple. It has also alleged that Jeetu Lal Meena, a wholetime member of the Medical Assessment and Rating Board in NMC, was in touch with Indra Bali Mishra 'Guruji', a resident of Varanasi, who in turn was a pointsman for Virender Kumar, an alleged middlemen for a number of colleges in southern India. It is alleged that in lieu of information, Kumar gave bribes in lakhs to Guruji, who got it collected through his brother-in-law Shivam Pandey and got it handed to Meena. Meena allegedly used a portion of the bribe money in the construction of a temple in Rajasthan's Dausa. One Hari Prasad in Ananthpur, Andhra Pradesh, also worked through Kumar. Prasad, who arranged dummy faculty and handled NMC affairs for some medical colleges for a fee, had two more partners working for him, Krishna Kishore and Ankam Rambabu who had taken bribes from Venkat, a director of Gayatri Medical College, Visakhapatnam and Father Joseph Kommareddy of Father Colombo Institute of Medical Sciences in Warangal. AI helps couple achieve pregnancy after 18 years of failed IVF cycles: Fertility expert explains how tech can make this happen
After 20 years, a couple in the US is expecting to be parents as the woman has achieved pregnancy after a series of unsuccessful attempts of in-vitro fertilisation (IVF), where the eggs and sperm are fertilised outside the body in a laboratory and the resulting embryo is transferred into the woman's uterus. That was because the man had very low sperm. And even the little that was there was hidden and undetectable by an embryologist. But now thanks to an artificial intelligence system, previously undetectable sperm can not only be identified but harvested for use. The STAR (Sperm Tracking and Recovery) system, developed at Columbia University Fertility Centre in New York, is expected to help men with azoospermia, a condition where no sperm is easily found in the semen. This technology is a breakthrough because it can harvest hidden semen, has a non-invasive extraction method and can offer an option to men battling fertility issues. This method costs under $3,000 (Rs 2.6 lakh). 'This is an example where technology multiplies human efforts. Even embryologists currently do the same thing. But now AI can assist them in identifying the sperm even better,' says Dr Anjali Malpani, IVF specialist and founder of Malpani Infertility Clinic. A typical semen sample usually has millions of sperm. But some men have really low counts, so much so that they can't be seen even after centrifugation (spinning the semen sample to concentrate the sperm) and hours of meticulous searching under a microscope. This is different from ejaculatory duct disorders, which prevent sperm from being ejaculated despite being produced in healthy numbers in the testes, usually due to a blockage. Then we use a needle to extract the healthy sperm. Azoospermia is diagnosed when no sperm can be found in at least two separate semen samples analyzed after centrifugation. So we study multiple ejaculates, which means collecting and analyzing multiple semen samples to confirm the absence of sperm. We usually collect the samples in quick succession, within an hour or two. This is done because sperm production can fluctuate, and a single semen analysis might not accurately reflect the overall situation. Multiple samples help account for these variations. Then an embryologist studies them to see if they can be used for IVF or not. Now the semen sample is placed on a chip under a microscope. AI connects to the microscope through a high-speed camera and with high-powered imaging technology scans it. The STAR system takes more than 8 million images in under an hour to find what it has been trained to identify as a sperm cell. It can isolate sperm faster in the sample, allowing embryologists to recover cells that they could not have just with their eyes. So AI is amplifying the embryologist's vision. Of course, each IVF case has different complexities which require the experience of a trained embryologist. But in resource-scarce settings and in the absence of a good embryologist, AI can scan sperm and eggs based on the dataset of thousands of samples that have been fed into them. Still I would say that an embryologist can grade embryos — assessing the quality and developmental potential of embryos — better.

Haryana's vigilance dept arrests Palwal Chief Medical Officer for accepting bribe
The Vigilance and Anti-Corruption Bureau of Haryana on Thursday arrested Dr Jai Bhagwan, Chief Medical Officer (CMO) of Palwal, for allegedly accepting a bribe of Rs 1 lakh. The Gurgaon unit of the Vigilance department also recovered an additional Rs 3 lakh in cash from a cupboard at the doctor's official residence in Palwal during a raid. Dr Bhagwan has been booked under Section 7 of the Prevention of Corruption Act and Section 308(2) of the Bharatiya Nyaya Sanhita (BNS) that deals with punishment for extortion, at the State Vigilance and Anti-Corruption Bureau Police Station in Faridabad. According to the complainant who runs Sunrise Trauma Hospital in Palwal along with three others, Dr Bhagwan had been threatening to shut down their hospital, citing operational deficiencies. The CMO allegedly demanded Rs 15 lakh to allow the hospital to continue functioning, claiming he needed to pay off other officials. The complainant stated that he had already paid Rs 6 lakh about 20 days ago and an additional Rs 1 lakh on Wednesday. Dr Bhagwan then demanded the remaining Rs 8 lakh, of which the complainant arranged Rs 1 lakh for payment Thursday night, leading to the sting operation by the Vigilance Bureau. The Vigilance department has urged the public to report bribe demands by government officials to the toll-free numbers 1800-180-2022 or 1064 for prompt action. In January, the Haryana Anti-Corruption Bureau (ACB) had recovered Rs 3.65 crore and gold worth Rs 6.5 lakh from the residence of a retired Haryana Development and Panchayat department official. The recovery came after the ACB conducted a raid at the Panchkula residence of the official in connection with a scam worth Rs 50 crore and arrested the accused, identified as Shamsher Singh, who had retired on November 30 last year. ACB officials had said that currency notes were found in a bed and almirahs at Shamsher's house at Panchkula Sector 28. The ACB had registered a case on January 24 this year against junior officials of Haryana Development and Panchayat department posted in Faridabad apart from a outsourcing services firm. An audit report of Comptroller of Auditor General (CAG) had earlier found that funds were disbursed to the Palwal District Development and Panchayat Officer for the financial years 2023-24 and 2024-25 without approval of Chief Accounts Officer of the panchayat department.
