The truth about junior doctors' pay

Wes Streeting has said he will try to keep disruption to a minimum, but previous strikes in 2023 led to more than a million appointments being cancelled. The doctors' union, the British Medical Association (BMA), is asking for a 29 per cent pay rise for resident doctors, who have so far received an uplift in pay of almost 30 per cent since 2022-23, according to the Nuffield Foundation.
That sounds like a lot. Are resident doctors (formerly known as junior doctors) being reasonable?
The BMA says this is a question of 'pay restoration'; salaries have not kept pace with inflation, and a newly qualified hospital doctor is now paid significantly less, relative to the price of everything else in the economy, than they were in 2008. There are lots of ways to slice the data. Starting at 2008 gives the biggest possible drop in real-terms pay, which is why the BMA is using it for its negotiations. It also uses the RPI measure of inflation, because it says student loans grow by RPI (which is true, but RPI is also, conveniently, higher than the inflation measures used by everyone else). However, it is still true to say that doctors' salaries have reduced in real terms since the peak of their pay growth in 2008.
To give a sense of by how much, the numbers start to change: institutions such as the Institute for Fiscal Studies and the Nuffield Foundation prefer to measure from 2010, because that's as far back as proper comparable data goes, and to use the more standard inflation measures of CPI or CPIH. Doing so still shows doctors' pay being compressed, even after the recent rise, by somewhere between 4.1 per cent for those in core training and 10.3 per cent for those in their second foundation year.
Some critics of resident doctors point to their generous pensions – the NHS contributes a further 23.7 per cent of their salary to their pension – but this is just another part of their salary; it too can be compressed by inflation.
The problem I see in the BMA's logic was revealed by a series of adverts the union created in 2023, which claimed that resident doctors were paid less than people who make coffee in Pret a Manger. 'You can make more serving coffee than saving patients,' the ads read. This was a misrepresentation. In reality, Pret workers could have made up to £14.10 an hour if they got their full bonus and location allowance, while a first-year junior doctor could have been said to be making as little as £14.09 per hour if only their basic wage was counted across a 40-hour week. In fact, a first-year foundation resident doctor was on average taking home considerably more (their average pay when the ads went out was more than £37,000) than a barista.
But that wasn't the most revealing thing about those ads. More important was the outrage they implied: it was presented as shocking that a mere milk-frother should be paid an amount comparable to a doctor (although, to be clear, they weren't). Shocking, and counterintuitive: surely being a doctor is a better career than being a barista?

And, of course, it is. Walk into any Pret and ask any member of staff if they would like to have the job security and pay progression that are effectively mapped out in the career of a doctor. All of them would. The BMA would argue that doctors earn this by spending a long time getting very expensive qualifications, then doing a hard and socially useful job. Which is true. But it's also well rewarded by the standards of the wider economy. It is still true there is no degree that pays as well as a medical degree five years after graduation.
The barista, after a year, might be paid more if the minimum wage rises. The foundation year one doctor (average pay £43,400 according to Nuffield) will receive a significant pay increase because they are now a foundation year two resident doctor with a different pay scale (average pay £51,600), and then they will enter core training (£67,400).
This is one of the issues with using inflation as the main negotiating point. It is true to say that the average wage of a doctor at a given point in their career is lower in real terms than it was in the past. But this does not mean that a given doctor is actually being paid less. Data collected by the Office for National Statistics from 2003 to 2017 suggests that, on average, fewer than one in five British workers are promoted each year. So while the BMA can argue that the average British worker has achieved pay restoration while newly qualified doctors haven't, lots of people remain average British workers for their whole working lives, while doctors don't remain newly qualified.
The other issue is that inflation is not the same for everyone; it matters less the more you are paid. No full-time doctor makes less than the median wage, even in their first year after qualifying, and their pay progression is rapid from that point; by the sixth to the eighth year after qualifying they are typically making around double the median wage. A GP partner on £160,000 a year experiences inflation very differently to a supermarket worker or barista on £25,000 a year. Lower-income households spend more on essentials, and the lower your income, the more of the price of your consumption is affected by the cost of energy and materials (basic foods such as pasta and vegetable oil increased more sharply in 2022, for example, doubling in price in some cases). People on higher incomes are much more likely to have savings – which benefit from inflation. By using inflation (and the highest inflation measure) to justify its pay demands, the BMA is pretending doctors are just like other people, but they're not.
This doesn't mean doctors don't have a right to be annoyed about the compression of their pay. As I've written previously, our tax system disproportionately raises money from people in exactly the sort of pay brackets doctors are in, with some very high marginal rates, while they have very large student loans to pay off and their salaries no longer have the same power in the housing market. Their story is part of a wider story of the middle classes in Britain remaining fairly static in real earnings while people on lower incomes rise to meet them and the pay of a small elite takes off. And in that sense the doctors' strike, while its numbers may be debatable, augurs something very real and more widespread: the creeping spread of middle-class rage at the disappearing prospects of professional life.

People seeking cheap Botox-like injections have been warned by experts against doing it themselves due to the risk of 'eyelid droops', infection and even botulism. There are growing concerns over the availability of medication called Innotox that is being sold illegally online in the UK. Unlike Botox, which comes as a powder that must be reconstituted for use in an injection, Innotox is a ready-to-use liquid – making it easier to self-administer. Wes Streeting, the health secretary, announced plans this week to introduce legislation cracking down on England's cosmetic 'wild west', where there is scant regulation of who can deliver treatments such as dermatological filler and Botox. Experts say Innotox is not licensed for use in the UK, unlike some other liquid Botox-like injections, meaning its quality and safety has not been assessed. Aenone Harper-Machin, a consultant plastic surgeon and spokesperson for the British Association of Plastic, Reconstructive and Aesthetic Surgeons (BAPRAS), said the online availability of Innotox was frightening and appalling, and she cautioned against DIY jabs. 'People could be giving themselves eyelid droops and all sorts of weird asymmetries by injecting it in the wrong place, too deeply, too superficially. You can inject it into your blood vessel and give yourself botulism,' she said. Products such as Botox and Innotox contain type A botulinum toxin, which blocks nerve signals and prevents muscles from contracting. While botulinum toxin is a prescription-only medicine in the UK, Innotox can be readily bought online without one, amid longstanding concerns about untrained and unregulated providers, as well as unlicensed or counterfeit products. Health officials have said 41 recent cases of botulism poisoning in England have been linked to unlicensed jabs. Harper-Machin has had Botox-like injections but said she would not self-administer them. 'I wouldn't have it done by anybody other than a consultant plastic surgeon who has intimate knowledge of facial anatomy,' she said. Botox jabs have become a routine procedure for many people. The government estimates that about 900,000 such injections are carried out in the UK each year. Despite now being available in high street clinics, their high cost and the regular need for top-up treatment has fuelled a desire for cheaper alternatives. The Medicines and Healthcare products Regulatory Agency (MHRA) says it is unlawful to sell unauthorised medicines, although in certain circumstances unauthorised medicines may be supplied under the supervision of a healthcare professional. Nora Nugent, the president of the British Association of Aesthetic Plastic Surgeons (BAAPS), said: 'While rare, BAAPS has become aware of isolated but concerning cases of individuals experiencing serious complications following self-injection of botulinum toxin and dermal filler products. 'These include infections, tissue damage and, in extreme cases, the need for multiple surgical interventions. We are aware of cases that have resulted in significant tissue damage and tissue loss needing multiple surgical interventions and resulting in permanent scarring.' Sophie Cooper, the managing director of the Harley Street Skin Clinic, said: 'Content creators on TikTok have been sharing their results from self-injecting Innotox, which has been encouraging viewers to jump on the bandwagon. 'Innotox has been reported to have a simpler injection process, be faster working, longer lasting and be lower in price. In reality, self-injecting Innotox as a Botox alternative comes with many dangers.' Tom Albright, the chief executive of Luvantas, a subsidiary of Medytox, which produces Innotox, said the company became aware in June of the unauthorised importation of Innotox to the US, UK and other countries and alerted Medytox management to the situation. 'Medytox has opened an investigation into this matter and have engaged experts to assist in shutting down the sources of importation,' he said. 'Patient safety is our highest priority. We strongly caution healthcare providers and consumers against purchasing unapproved or misrepresented botulinum toxins through unauthorised channels. Products distributed outside authorised supply chains may not meet safety, efficacy or storage standards.' He added: 'Botulinum toxins should be administered only by qualified healthcare professionals in a medically appropriate setting. Administration requires a deep understanding of facial anatomy and aesthetic principles, which cannot be replicated in consumer-administered or unregulated environments.'

NHS App now informs patients in England when their medicine is ready so they can avoid unnecessary trips or leaving it late to collect from chemists Hundreds of thousands of NHS patients have started using a new 'Amazon-style' service via its app so people can track their prescriptions. ‌ NHS England says 400,000 are using the tracking feature ten weeks after it was launched and health chiefs are calling on more to follow suit. It provides near real-time updates in the app that lets you know when your medicine is ready so you can avoid unnecessary trips or leaving it until the last minute to collect. It comes after five million patients had a consultation with their pharmacist instead of their GP since an NHS scheme to take pressure off family doctors. ‌ Currently around 45% of phone calls to community pharmacies are from people chasing prescriptions and the feature has been hailed by pharmacists as a 'game changer' in freeing up their time to see more patients. ‌ Health Secretary Wes Streeting said: 'We are driving the reform necessary to make the NHS as easy and convenient to use as doing your shopping or banking online. 'Nearly 400,000 people are already using our new prescription tracker through the NHS App because it saves them time and hassle – no more queuing up at the pharmacy only to find their medicines aren't ready and no more time-consuming phone calls with pharmacy staff. Get the NHS App now - track your prescriptions, skip the in-person lines, and free up both yours and your pharmacist's time.' ‌ Since the launch 4.4 million people have viewed their prescription in the app, with 394,000 also using the app's new tracking feature to check whether their prescription was ready. Around 1,650 high street chemists – including every Boots in England – are now offering the service, which enables patients to check if their medicines are 'ready to collect' or 'dispatched by pharmacy' if they are being delivered. The service is also expected to expand to almost 5,000 pharmacies over the next 12 months. ‌ Ryan's Pharmacy in Brighouse, West Yorkshire, is the second busiest pharmacy dispensary in Yorkshire fulfilling more than 30,000 prescription items each month. Previously the online pharmacy needed five phonelines to field all the calls from patients with prescription enquiries, but since integrating with the NHS App, phone calls have dropped by around 80%. Pharmacist Yasif Salim said: 'We thought the lines must have gone down at first. Normally the phones ring off their hooks with patients asking for updates on their prescriptions. A normal day for us is noisy. ‌ 'Before the digital prescription updates were available on the NHS App the phone would ring constantly. And when it wasn't ringing, we were just waiting for it to happen. Some days it felt quite stressful and overwhelming. Some nights I went to sleep and dreamt of the phones ringing. And I know that feeling ran across the whole team. 'The beauty of it is that in pharmacy we didn't need to do anything. There wasn't any switch on preparations that we needed to make, and it didn't add any additional steps into the dispensing process. We could just carry on as usual and everything was handled by Pharmacy Manager.' ‌ Health service data suggests the NHS app has 37.4 million registered users, with an average of 11.4 million people using it to manage their healthcare every month. The app also allows patients to use a barcode to collect their prescription. Some 61.5 million repeat prescriptions have been ordered via the NHS App in the last 12 months – a 46% increase on the previous year. There were also 87.4 million views of patient test results during the same period. Dr Vin Diwakar, Clinical Transformation Director at NHS England, said: 'It's brilliant that so many people are making use of the new Amazon-style tracking feature in the NHS App, which means you can now be sure your prescription is ready before setting off to the chemist. 'For those who rely on regular prescriptions, the tracker offers near real-time updates on when their medicines are ready, like you can get for your online shopping, and this is all part of our shift to give people more control over their care through the NHS App. 'We're seeing millions more people using the app to easily order repeat prescriptions and get faster access to their test results – I'd encourage anyone who hasn't logged in for a while to take a look and see how the new features can help them manage their healthcare from wherever they are.'
