Straddled in a unique yet precarious situation is the healthcare industry, which National Heart Institute (IJN) chief executive officer Prof Datuk Seri Dr Mohamed Ezani Md Taib described the practices as 'a very complex environment.'
He admitted in a recent interview that hospitals are traditionally one of the main culprits of carbon emitters due largely to the wide range of medical devices that they use.
'In the past, we used cloth masks so they were reusable. Due to worries of infection, there was a change to disposable masks and caps in the operating theatre.'
He said this resulted in the increase in single-use medical devices that resulted in an increased number of waste products.
'Now with the concern for sustainability, environment and ESG, there's a relook at what we're doing now, if that is correct,' he added, referring to the use of plastic containers and bags for medication.
'To make that change will result in rising costs to repackage and redeliver these sustainably, and that is challenging. That's why it's not easy for the medical line to become fully ESG, but no doubt we are moving in that direction,' he explained.

Dr Mohamed Ezani explained that IJN is recognised as a premier health centre globally, thanks to its cutting-edge technology and services. As such, it has become an example for healthcare centres within the country to aspire to become.
'As a role model, we need to be responsible for our patients, and the environment as well,' he said.
However he pointed out that the challenge arises in how to implement sustainability procedures without compromising the patient care or healthcare standards, basically: 'How to prevent infection from happening while using sustainable approaches.'
He does so by targeting sustainability 'low-hanging fruits', such as using more energy efficient LED lights and harvesting rainwater to water plants in the landscaped areas of the centre.
'We have also installed solar panels on half or more of our rooftops, which means using less from the main grid. Electricity consumed from solar generation is approximately 32,000kWh per day on average which contributes to about 2.2% of IJN's total electricity consumption per day.'
A significant challenge he identified was changing employees' mindset of using excessive resources such as electricity and papers. With his efforts, IJN has almost gone paperless for its clinical and financial operations for the environment.
'We benchmarked our performance against the highest standard possible, which is the American standard.'
Waste issues
Dr Mohamed Ezani said that IJN has been segregating its waste into either clinical or general.
'Clinical waste carries a lot of carbon footprint because it is either to burn, which has a high carbon footprint, or bury it in landfills. For this, we will ensure that the amount of waste given to our waste vendors is minimised,' he said.
Another aspect involves recycling, of which he said the hospital's food services are currently using recyclable materials.
'We're trying to cut down the usage of single-use plastics, in the wards. Now we don't use plastic spoons, instead we have recyclable items. Food containers are also recyclable as opposed to using plastic or polystyrene in the past. About 30% of the hospital's surgical cases are already almost green,' he said.
He also explained that the hospital has policies that minimise or eliminate redundancies in its operations, in order to be as green as possible.
Social awareness
Dr Mohamed Ezani pointed out that IJN has been taking the CSR road before ESG became important.
'We realised that in Malaysia, there's a lot of issues about non-communicable diseases and for the last 10 years, we have visited more than 160 rural areas, such as the islands off the East Coast, to conduct healthcare screening for more than 160,000 participants for free.
He explained that the hospital conducted such screenings to identify those at risk, then referring them to the nearest hospital when needed, or even bringing them back to IJN for treatment if necessary.
'We have been doing this as a service to return what we have gained back to the people. Our way of giving back to the community.'
He also mentioned that patients can expect the best care possible and that the accessibility to this standard is important.
'When you talk about being sustainable, it also means that any Malaysian should have access to IJN. Our target is that 75% of new patients should receive their first appointment within two weeks. We will also ensure IJN is free of risk of infections—these are the things that we monitor.'
Matter of governance
IJN has been diligent in ensuring it is accredited in its standing, from ISO 55001:2014 Asset Management System to MS ISO/IEC 27001:2013 in Information Security Management System and more.
'Way before anybody had been talking about this, we've been looking at topics like anti-bribery, net zero carbon charges, cybersecurity and of course digitalisation. We've been careful to ensure that SDG Goal 5 in upholding transparency is observed all the way up to the board of directors.
'We have to report to our audit risk and compliance committee to make sure these are performed.'
In light that World Ocean Day is in July, he shared that IJN will embark on more rural community work, which will include cleaning out the beaches from plastics and other marine litter.

Sarawak looks to Danish pig farming system to boost production

MIRI (Aug 20): Sarawak has conducted a strategic study mission to Denmark to boost the pig farming industry, which has been identified as a key driver for the state to become a net food exporter by 2030. The move aims to drive Sarawak beyond the current model of live pig exports to Singapore and Selangor with a complete value-added production ecosystem. 'This visit studied the pig farming system in Denmark to formulate a strategy for the development of the pig farming industry in Sarawak,' said Food Industry, Commodity and Regional Development Minister Dato Sri Dr Stephen Rundi Utom in a statement. He said the nine-day fact-finding mission from Aug 8-17 sought to position Sarawak as a major food producer in the region. The delegation, which included Deputy Minister for Modernisation of Agriculture and Regional Development II Datuk Martin Ben and Department of Veterinary Services Sarawak director Datu Dr Adrian Susin Ambud, focused on identifying investment opportunities that would transform the industry. Sarawak currently exports 122,400 live pigs valued at RM197.6 million to Singapore but the ministry is looking to expand the pig production industry to RM1 billion in export value by 2030. Learning from Denmark's success model, the mission explored investment opportunities across the entire pig production value chain, from breeding and farming to processing and export, positioning Sarawak to compete with established producers globally. Dr Rundi said Denmark's pig industry serves as an ideal benchmark as it contributes 30 per cent to the country's agricultural sector with a population of 11.45 million pigs in 2024. The Nordic nation exports 90 per cent of its production to 140 countries, demonstrating the potential for a well-developed pig industry to drive export revenues. The Sarawak delegation visited six strategic facilities including Big Dutchmen for livestock equipment technology; DanBred headquarters for breeding expertise, modular slaughterhouse manufacturers; and Danish Crown's state-of-the-art processing facility in Horsens. Key areas of study included advanced slaughter technology and best practices, disease prevention strategies for foot-and-mouth disease (FMD) and African Swine Fever (ASF), robotic technology applications in livestock farming, and modern pig breeding techniques. The strategic shift represents a fundamental change from Sarawak's current approach of primarily exporting live pigs to developing a complete domestic pig industry ecosystem capable of producing value-added products for export. This transformation would involve establishing modern breeding facilities, implementing advanced biosecurity measures, developing processing capabilities, and creating supply chain networks that could serve both domestic and international markets. Digital devices impacting M'sian children's eyesight
KUALA LUMPUR: Vision problems are becoming increasingly common among Malaysian children, yet many remain undiagnosed until they begin to affect behaviour, school performance, or overall development. Sunway Medical Centre Velocity (SMCV) Consultant Ophthalmologist, Paediatric Ophthalmologist and Strabismus Surgeon Dr Fiona Chew Lee Min said most of these cases stem from refractive errors, a condition where light is not focused properly on the eye due to the shape of the eyeball, resulting in blurry vision. She said Myopia (nearsightedness) and astigmatism (a common condition that causes distortion of vision) are the most common refractive errors among Malaysian children, and both are becoming increasingly prevalent. "Nearly one in two Malaysian children is affected by refractive errors, with a local study reporting a prevalence of 47.8 per cent. Myopia affects 30.2 per cent and occurs when light focuses in front of the retina, causing distant objects to appear blurry. "Astigmatism, which affects 16.3 per cent, is caused by an uneven curvature of the cornea, resulting in blurred vision at all distances. While hyperopia, or long-sightedness, is less common at 1.2 per cent and occurs when light focuses behind the retina, making near objects appear blurry," she said in a statement. With August marking Children's Eye Health and Safety Awareness Month, Dr Fiona reminded parents to prioritise regular vision checks, even when there are no obvious symptoms. The surgeon said that while the conditions are common, they are often overlooked by parents because children rarely complain of vision problems and tend to adapt to their limitations, leading parents to believe nothing is wrong. "Symptoms may be subtle, such as squinting to see distant objects, frequent blinking or rubbing of the eyes, or even holding books and devices unusually close to the face. Dr Fiona also added that while behaviours like sitting too close to the television are often mistaken for bad habits, they could indicate that a child is struggling to see clearly and may have myopia. "If left untreated, refractive errors can lead to more serious vision problems, including lazy eye which causes permanent poor vision, eye alignment issues, headaches from eye strain, problems with depth perception and even difficulties in concentration," she explained. These risks are further compounded by modern lifestyle habits such as prolonged screen use, especially as reliance on digital devices has increased significantly over the years post-COVID-19 pandemic. As a result, more than half of Malaysian students experience headaches linked to extended periods in front of screens. Regarding the recommended screen time for children, Dr Fiona said it should be introduced as late as possible, preferably from the age of seven, adding that children over the age of three should have no more than one hour of screen time per day, while those under three should avoid screens entirely. Dr Fiona said spending more time outdoors can help protect children's vision by giving their eyes a break from digital devices, allowing them to focus on varying distances and exposing them to natural sunlight, which reduces the risk of developing myopia. With that, Dr Fiona encouraged the adoption of the 20-20-20 rule, which is beneficial for people of all ages, noting that it is a simple yet effective habit she practises herself - taking a 20-second break every 20 minutes of near-distance work to focus on something 20 feet away. Alongside preventive habits, she said appropriate corrective measures, including prescription glasses, are key to protecting a child's vision, as they remain the most effective way to correct refractive errors in children, and those who require them should wear their glasses consistently to prevent eye strain and help stabilise the refractive error. She also noted on the concerns about glasses will weaken a child's eyesight and said such thing is unfounded, as the correct prescription actually helps the eye focus properly. For older children from the age of 10 to 12, contact lenses may be introduced as an alternative, but strict hygiene must be practised to avoid eye infections. Dr Fiona said routine eye screening also plays an important role in maintaining children's eye health, starting as early as at birth, followed by a formal eye assessment at the age of three. Should there be any signs of vision problems in between, parents are encouraged to take proactive steps and have their children's eyes assessed earlier. Meanwhile, a registered dietitian at SMCV, Chow Yu Fei, said nutrition is a critical yet often overlooked factor in protecting children's vision, as a balanced diet during early childhood supports optimal visual development, eye muscle coordination and overall eye function. She said key nutrients such as vitamin A, lutein, zeaxanthin and omega-3 fatty acids play an essential role in maintaining clear vision and protecting the retina from harmful light exposure, adding that deficiencies during this stage may not only affect children's vision now but also increase the risk of potential eye conditions in the future. "These nutrients can be found through everyday foods such as carrots, sweet potatoes, eggs, spinach, broccoli, corn, salmon, mackerel, local favourites such as ikan tenggiri, ikan kembung, ikan patin, walnuts and chia seeds. Vitamin A deficiency can significantly impair a child's ability to see in low-light conditions and, in severe cases, may cause permanent damage to the cornea. "Insufficient lutein and zeaxanthin intake reduces the retina's natural defence against harmful blue light, increasing its vulnerability to long-term damage. While lack of omega-3 fatty acids can hinder the development and function of the retina, affecting the clarity and efficiency of visual processing," she said.

Sharing burdens

But these kidney donors are now thriving together with the recipients of their selfless acts. It has been 15 years since Siti Aesahah Mohd Nor donated her kidney to her nephew Abdullah Bukhari Abdul Rahim, who is now 50. 'It's precious to be able to donate such a gift to someone. By doing so, God will return our deeds tenfold,' she said. It has been a long journey, though. Blood tests in 2009 confirmed the 'compatibility', thus enabling the kidney transplant to take place. But other hurdles came their way. 'CLICK TO ENLARGE' That year, there was the H1N1 flu outbreak. And when an operation was finally scheduled in July 2010, the doctor suffered from exhaustion as a result of performing emergency transplant surgeries the night before. The operation eventually took place a month later on Aug 5, 2010, at Hospital Selayang. All this was done with Siti Aesahah having to travel back and forth from Kelantan where she was a teacher. 'Transplant operation is not like what you see on TV. There are many steps, starting from thorough health checks over several months including MRI and cross-check blood tests, which had to be done again and again due to the delays,' said the single mother of two children and two grandchildren. These days, Siti Aesahah and her nephew are grateful because both are in good health without any complications. 'There had been sceptics. But both of us have been surviving with one kidney and this is a testament to the (importance of) organ donation,' she said. She said Abdullah, a religious teacher who is married with four children, had often been an active person. 'So being able to donate a kidney has allowed him to continue his work,' said Siti Aesahah, 65. In the case of communications manager Melissa Ananthraj, the pain of seeing her husband Manvir Victor tied to a machine, going through dialysis each day, prompted her to act out of love. 'Although dialysis kept him alive, he wasn't living a life,' she said of Manvir, a healthcare portal director who is now 58. 'My husband was already on dialysis for 10 years before I knew him. When I met him, I saw what kidney failure and dialysis was doing to him,' said Melissa, 49. 'So, for me, there was no question (about donating to him).' The couple had married in July 2011. The transplant took place in June 2012. 'From the beginning, my family knew this wasn't a conventional path, but they saw how deep our bond was. 'Once they understood the situation, the realities of kidney disease and the strength of our bond, they supported me wholeheartedly,' she said. Melissa said her family did not even try to discourage her from doing so. 'They simply asked thoughtful questions and made sure I had all the information to make the decision safely. 'Their support gave me even more confidence in the life I was choosing,' she said. She also spoke to the transplant unit at Hospital Kuala Lumpur (HKL) to find out more. 'There were many tests, tissue matching and scans because the doctors need to make sure it's safe for both the donor and recipient. 'Mentally, it wasn't always easy. Sometimes, I asked myself, what if something goes wrong? But I had the best doctors and they guided me every step of the way,' Melissa said. In fact, the HKL team provided a psychiatrist to speak about their mental health before the transplant. 'The surgery went well. The recovery was painful at first, but when I saw him doing better and life coming back to his face, everything was worth it,' she said. Post-transplant, Melissa said her life is completely normal as she could work and travel. But she goes for regular check-ups. The organ transplant has made the couple appreciate life more. 'Today, my husband and I are both active in organ donation awareness. We share our story because we know that many people are still unsure. 'We want to show them that donation is not only possible – it changes lives.'
