Flooding Triggers Major Melioidosis Outbreak in Australia

Australia has recorded its highest number of cases of the tropical bacterial infection melioidosis, which is thought to have been triggered by major rainfall and flooding. The outbreak has placed pressure on already strained hospitals in the area.
The northeastern state of Queensland has recorded an unprecedented 215 cases of melioidosis so far this year compared with 59 cases over a similar period in 2024 and 46 cases in 2023. The state also recorded 31 deaths.
'We've had about four times more [cases] in Cairns and the hinterland area compared to the year-to-date average, and about six times more here in Townsville than the year-to-date average,' Jeff Warner, PhD, a microbiologist at the Australian Institute of Tropical Health and Medicine at James Cook University in Townsville, told Medscape Medical News.
Both cities border the Great Barrier Reef. Townsville experienced massive floods in January and February caused by record-breaking rainfall, which is likely to have contributed to the surge in infections. Cairns has also experienced its wettest March on record.
'On top of what was already probably a spreader event back from Cyclone Jasper flooding, there's now been this additional significant weather event this year with a lot of water and a lot of flooding,' said Aileen Traves, MD, a general practitioner in Cairns. 'That obviously has made a huge difference to seeing the numbers really spike.'

The increase in cases has put pressure on hospitals in the area, Traves told Medscape Medical News . 'There have been some additional pressures, like on intensive care here, because some of the people are so sick that they're actually ending up in the ICU.'
'Great Imitator'
Melioidosis is caused by the bacterium Burkholderia pseudomallei , which is found in soil and water. The disease is endemic in tropical regions around the world, including northern Australia, and outbreaks are commonly tied to major rainfall events such as the monsoon.
The infection can present as pneumonia, skin infections, or septicemia and has a mortality rate that ranges from around 10% in Australia to as high as 40% in Thailand.
'Clinically, it's one of the great imitators, along with tuberculosis and syphilis,' said Steven Donohue, MD, director of the Townsville Public Health Unit in Queensland. 'In other words, it can present in very different ways,' It can also remain latent or subclinical for a long period, potentially even decades. This feature led to its being labeled the 'Vietnamese time bomb' by US military personnel who were infected during their service in the Vietnam war.

Those most vulnerable to severe infection tend to be older individuals with preexisting comorbidities, such as diabetes and kidney disease, and those with impaired immune systems. But one unusual feature of this latest outbreak is the number of younger, otherwise healthy individuals — particularly those working on building sites — presenting with the infection.
'They're often young, fit, and male, but they're working in conditions where they are being exposed potentially to the bacteria in their job,' Traves said.
Potential Routes of Exposure
How patients get exposed to and infected with B pseudomallei is still uncertain, Warner said. Bore water has been implicated as a source of infection through ingestion, as has subcutaneous inoculation via cuts or abrasions. Another possible source is inhalation. 'It's almost certainly true that it is aerosolized during the extreme weather.'
Given that the organism usually resides in the soil, soil disturbance from development may be contributing to some of the clusters. 'The hypothesis up in Cairns is that the Bruce Highway extension being pushed through just south of Cairns has disrupted the environment to such an extent as to release B pseudomallei from layers of soil that weren't [normally exposed] to people,' he said.
Another theory is that the cleanup from flooding in Townsville has exposed more people to the bacterium than usual. 'People are often having to clean up after floods or heavy rain in the wet, and often people are doing this with bare feet,' Donohue said.
That cleanup process also sometimes includes the use of pressure hoses, which can aerosolize the water and soil and increase the likelihood of inhalation by people who are 'not necessarily wearing a mask or boots or gloves or all of the stuff they should,' he noted.
Another factor could be the demographics of Australians in the area, Donohue said. 'We've got an aging population with a high rate of chronic diseases and also human movement into new suburbs and new settlements, so a lot of changes in behavior, a lot of changes in the environment.'
Australia has experienced numerous extreme flooding events in recent years, particularly along the eastern coast, that have been attributed to climate change. This latest outbreak highlights the human health risks for climate change, Traves said. 'People don't always think about the associations with human health and planetary health, but I think this is a classic example.'

Colorectal Cancer Linked With Adverse Sexual Health Outcomes

As new colorectal cancer (CRC) cases and related mortality continued to decline among Canadians older than 50 years, a new finding emerged: An increased risk for sexual health conditions in women with CRC, especially those with early-onset CRC (diagnosed in women younger than 50 years). These patients have as much as a 90% higher risk for dyspareunia compared with cancer-free peers. Other striking findings of the recently published study included elevated risks for premature ovarian failure, pelvic inflammatory disease (PID), and endometriosis among women with CRC. Study co-author Mary De Vera, PhD, a pharmacoepidemiologist and assistant professor of pharmaceutical sciences at the University of British Columbia (BC) in Vancouver, was diagnosed with early-onset CRC at age 36 years. 'The motivation for me to look at sexual and reproductive health outcomes stemmed from my own lived experience,' she told Medscape Medical News. Mary De Vera, PhD The study was published online on June 1 in the Journal of the National Cancer Institute . Sexual Health Challenges The researchers conducted a population-based cohort study to evaluate the impact of CRC on the following six outcomes: Dyspareunia, abnormal bleeding outside pregnancy, PID, endometriosis, premature ovarian failure (ie, early menopause in women younger than 40 years), and at least one hormone replacement therapy prescription. Eligible participants were identified in BC administrative health data (which contains linked healthcare visits and the BC Cancer Registry) and included patients assigned female at birth who were diagnosed with CRC between January 1, 1985, and December 31, 2017 (n = 25,402). These participants were matched by age and sex 1:10 to cancer-free controls (n = 254,020). The participants were further stratified by age (≤ 39 years, ≥ 40 years), and sensitivity analyses were conducted to explore the association between sociodemographic and cancer-related factors (eg, cancer site or treatment type) on sexual health outcomes. The most common cancer sites were the left colon (39.8%) and rectum (28.8%), and the most frequent treatments were surgery (42.9%) and chemotherapy (35.5%). Overall, participants with CRC had more healthcare encounters in the previous year than matched control individuals did (15.1 vs 10.9; P < .0001). Women with CRC had a 67% higher risk for dyspareunia compared with women without cancer. 'When we further stratified them by age, the risk actually rose to 90% in females diagnosed before age 40,' said De Vera. Treatment-related effects were common. Sensitivity analysis demonstrated an increased dyspareunia risk with surgery (hazard ratio [HR], 1.23), chemotherapy (HR, 1.25), and radiation (HR, 1.24). Women receiving chemotherapy had an increased risk for abnormal bleeding (HR, 1.24). CRC was associated with increased PID risk (HR, 3.42). Cancer site and treatment were independently associated with increased risk. For example, the risk for PID increased by 80% in participants diagnosed with CRC in the left colon and by 121% for rectal CRC. Radiation treatment was associated with a 56% increased risk. Additional notable findings included a 95% higher risk for endometriosis across all ages, with an 80% increase noted in women who had received surgery. Risk for premature ovarian failure appeared to increase more significantly in younger women with CRC than in those without (HR, 1.75). Chemotherapy was associated with a 164% higher risk. Causation Unclear De Vera pointed to a surprising but important finding related to the timing of diagnosis. 'We were finding that a lot of sexual health issues were diagnosed in the 3-4 years after the original cancer diagnosis, which showed me that a lot of these issues were surfacing during the survivorship stage,' she said. However, the study is not without limitations. Though findings suggested that there is an effect of cancer itself and that many of the conditions were being driven by cancer treatments, these effects have not been clearly delineated or defined. De Vera also said that the increase in health encounters amongst women with CRC suggested that certain drivers (eg, pain during or after sex) might not have been captured in the medical records. Davis Jewish General Hospital in Montreal, told Medscape Medical News that although pain disorders like endometriosis or dyspareunia can affect pleasure during intercourse, it's important to note that not all patients face the same situation. Jaclyn Madar, MD 'When you have a woman with sexual pain, you really have to look into what the cause of it might be, regardless of age,' said Madar, who was not involved in the study. 'What's front of mind for a lot of these women is to treat the cancer, and then afterward deal with the aftermath,' she said. 'It doesn't seem to me that it is integrated into the care, and I believe that it's important to warn that when they go through certain types of chemo, for example, they'll probably enter early menopause or have menopause symptoms.' Madar noted that the prevalence of many of these conditions is difficult to estimate, namely because patients don't always volunteer the information, and diagnosis is often delayed. For CRC in particular, radiation can have significant effects on vaginal tissues and cause vaginal stenosis, while surgical treatments have a domino effect: Post-op pain near the rectum can lead to pelvic floor muscle spasms and subsequent dyspareunia. 'The findings will lead to greater awareness and better communication,' said De Vera hopefully. The study was funded by grants from the Canadian Institutes of Health Research. De Vera and Madar reported having no relevant financial relationships.

Women With ILD Fare Better After ICU Care

Women admitted to ICU for interstitial lung disease (ILD) had shorter hospital stays and a lower risk for death than men, based on a new analysis of more than 800,000 individuals. Although previous studies have shown gender-based disparities in disease progression and severity for ILD based on subtype, data on the effect of gender on ICU outcomes in these patients are limited, according to Matthew Viggiano, MD, an internal medicine resident at Temple University Hospital, Philadelphia, and colleagues. In a study presented at the American Thoracic Society (ATS) 2025 International Conference, the researchers analyzed data from the National Inpatient Sample (NIS), part of the Healthcare Cost and Utilization Project for the period from 2016 to 2018. They identified 810,295 adults aged 18 years or older hospitalized with ILD, of whom 42,080 received ICU care. Of these, 46.7% were women. Female patients were significantly younger than male patients (mean age, 66.9 vs 69.1 years), more likely to be African American (17.0% vs 10.9%), and less likely to be Caucasian (63.7% vs 69.2%; P < .001 for all). Mortality was significantly lower in women than in men (40.5% vs 48.1%) even after adjusting for confounders including age, race, and comorbidities, and this difference was the most striking finding, Viggiano said in an interview. 'It also surprised us that these women tended to have a shorter length of hospital stay, given many came from lower-income areas,' he said. ICU stays were defined using International Classification of Diseases (ICD) codes for central line placement and mechanical ventilation. Overall, hospital stays for female patients lasted 1.15 days less than hospital stays for male patients. Female patients also were significantly more likely than male patients to come from lower-income ZIP codes (38.3% vs 33.2%) and less likely to have a history of tobacco use disorder (35.0% vs 43.9%; P < .001 for both). The reasons for the disparities remain unclear, but new studies suggest that hormones may play a role in disease progression and severity, Viggiano told Medscape Medical News . 'For example, estrogen has been implicated in modulating immune responses and fibrotic processes in the lungs via downregulating profibrotic pathways,' he said. 'Additionally, women may have lower threshold to seek medical attention or follow-up, leading to earlier intervention and management of ILD,' he noted. Other comorbidities unrelated to ILD also may contribute to morbidity and hospital length of stay, he added. 'Overall, recognizing these disparities is a key step toward more personalized treatment strategies, and our hope is that this research will prompt further studies to fully understand and address the underlying causes,' said Viggiano. Not Time for Gender Neutral Treatments Although the results suggest that clinicians should be aware that gender could influence ILD prognosis, the data do not suggest a need to advocate for entirely separate protocols as yet, Viggiano said. 'Instead, we encourage clinicians to recognize that men may have unique risk factors and might require more aggressive monitoring or early interventions; further studies will help refine specific management strategies,' he said. 'We believe evaluating for mortality and hospital stay in different subtypes of ILD would be an immediate future direction for the project,' said Viggiano. The investigation of specific biological, immunologic, and social factors also must be an area of focus, he said. 'Understanding why women fare better could lead to targeted therapies, especially for men who are at higher risk of poor outcomes, and ultimately to more personalized approaches to ILD care,' he added. To that end, Viggiano and colleagues intend to conduct prospective studies to explore specific biological markers and social determinants in men and women with ILD. 'We'll also look at the influence of treatment interventions, medication use, and rehabilitation services on outcomes. Ultimately, we'd like to identify targeted strategies to reduce the mortality gap and enhance care for both genders,' he told Medscape Medical News . Data Reinforce Differences 'As more treatments for interstitial lung diseases emerge, it is important that we now start focusing on which populations get the greatest benefit for specific treatments,' said Anthony Faugno, MD, a pulmonologist at Tufts Medicine, Boston, in an interview. To that end, the authors of the current study used data from the NIS to ask important questions about how sex, demographics, and socioeconomic factors affect patient outcomes, said Faugno, who was not involved in the study. Were You Surprised by Any of the Findings? Why or Why Not? Biologically important differences in hormones between men and women are known to affect the way a given disease behaves; therefore, it is important to have representative samples of diverse sex and race in clinical trials to ensure the generalizability of therapy, Faugno told Medscape Medical News . The current study findings were not surprisingbut reinforce the value of a diverse population using a large, nationally representative sample, he said. The current study findings may not directly affect clinical practice, as the results were based on ICD codes that cover many different diagnoses, Faugno noted. However, as the authors suggest, 'I do think it informs additional research directions, such as doing a similar analysis in specific interstitial diseases,' he said. The current study addresses a global catch-all term of ILD, which may include many different pathologies that respond to different treatments, said Faugno. 'A future analysis that addressed the gender disparities in more specific diagnoses would add to our understanding and help patients better understand how they may respond to a specific therapy,' he said.

Gut Microbiome Changes in Chronic Pain — Test and Treat?

A new study adds to what has been emerging in the literature — namely that there appear to be gut microbiome 'signatures' for various pain conditions — suggesting that microbiome-based diagnostics and therapeutics may one day be routine for a broad range of pain conditions. 'There is now a whole list of pain conditions that appear to have these signatures, including postoperative pain, arthritis, neuropathy and migraine to name a few,' Robert Bonakdar, MD, director of pain management, Scripps Center for Integrative Medicine, San Diego, told Medscape Medical News . Fibromyalgia and complex regional pain syndrome (CRPS) are also on the list. A team led by Amir Minerbi, MD, PhD, director of the Institute for Pain Medicine, Haifa, Israel, and colleagues published one of the first articles on gut changes in fibromyalgia. They noted that the gut microbiome could be utilized to determine which individuals had the condition and which did not — with about a 90% accuracy. The team went on to show that transplanting gut microbiota from patients with fibromyalgia into germ-free mice was sufficient to induce pain-like behaviors in the animals — 'effects that were reversed when healthy human microbiota were transplanted instead,' Minerbi told Medscape Medical News . Further, in a pilot clinical study, the researchers showed that transplanting microbiota from healthy donors led to a reduction in pain and other symptoms in women with treatment-resistant fibromyalgia. Most recently, they found significant differences in the composition of the gut microbiome in a cohort of patients with CRPS from Israel, compared to matched pain-free control individuals. Notably, two species — Dialister succinatiphilus and Phascolarctobacterium faecium – were enriched in patients with CRPS, while three species — Ligilactobacillus salivarius, Bifidobacterium dentium, and Bifidobacterium adolescentis – were increased in control samples, according to their report published last month in Anesthesiology . 'Importantly,' these findings were replicated in an independent cohort of patients with CRPS from Canada, 'suggesting that the observed microbiome signature is robust and consistent across different environments,' Minerbi told Medscape Medical News. Causal Role? 'These findings collectively suggest a causal role for the gut microbiome in at least some chronic pain conditions,' Minerbi said. However, the co-authors of a linked editorial cautioned that it's 'unclear if D succinatiphilus or P faecium are functionally relevant to CRPS pathophysiology or if the bacteria increased in healthy control samples protect against CRPS development.' Minerbi and colleagues also observed that fecal concentrations of all measured short chain fatty acids (SCFA) in patients with CRPS were lower on average compared to pain-free control individuals, of which butyric, hexanoic, and valeric acid showed significant depletion. Additionally, plasma concentrations of acetic acid showed significant depletion in patients with CRPS vs control individuals, while propionate, butyrate, isobutyrate and 2-methyl-butyric acid showed a trend toward lower concentrations. The quantification of SCFA in patient stool and serum is a 'notable advance' in this study, Zulmary Manjarres, PhD; Ashley Plumb, PhD; and Katelyn Sadler, PhD; with the Center for Advanced Pain Studies at The University of Texas at Dallas, wrote in their editorial. SCFA are produced by bacteria as a byproduct of dietary fiber fermentation and appropriate levels of these compounds are important to maintain low levels of inflammation in the colon and overall gut health, they explained. This begs the question of whether administering probiotic bacteria — many of which are believed to exert health benefits through SCFA production — can be used to treat CRPS-associated pain. It's something that needs to be studied, the editorialists wrote. Yet, in their view, the 'most notable achievement' of Minerbi and colleagues is the development of a machine learning model that accurately, specifically and sensitively categorized individuals as patients with CRPS or control individuals based on their fecal microbiome signature. The model, trained on exact sequence variant data from the Israeli patients, achieved 89.5% accuracy, 90.0% sensitivity, and 88.9% specificity in distinguishing patients with CRPS from control individuals in the Canadian cohort. Interestingly, in three patients with CRPS who underwent limb amputation and recovered from their pain, their gut microbiome signature remained unchanged , suggesting that microbiome alterations might precede or persist beyond symptomatic phases. Test and Treat: Are We There Yet? The gut microbiome link to chronic pain syndromes is a hot area of research, but for now gut microbial testing followed by treatment aimed at 'fixing' the microbiome remains largely experimental. At this point, comprehensive gut-microbiome sequencing is not a routine, guideline-supported part of care for fibromyalgia or any chronic pain condition. 'Unfortunately, even for doctors interested in this area, we are not quite at the state of being able to diagnose and treat pain syndrome based on microbiome data,' Bonakdar told Medscape Medical News . He said there are many reasons for this including that this type of microbiome analysis is not commonly available at a routine lab. If patients do obtain testing, then the results are quite complex and may not translate to a diagnosis or a simple microbiome intervention. 'I think the closest option we have now is considering supplementing with commonly beneficial probiotic in pain conditions,' Bonakdar said. One example is a preliminary fibromyalgia trial which found that supplementing with Lactobacillus , Bifidobacterium, and Saccharomyces boulardii appeared to have benefit. 'Unfortunately, this is hit or miss as other trials such as one in low back pain did not find benefit,' Bonakdar said. Addressing gut microbiome changes will become 'more actionable when microbiome analysis is more commonplace as well as is the ability to tailor treatment to the abnormalities seen on testing in a real-world manner,' Bonakdar said. 'Until then, there is no harm in promoting an anti-inflammatory diet for our patients with pain which we know can improve components of the microbiome while also supporting pain management,' he concluded.
