Grealish, 29, joined City from Aston Villa for the club-record fee in 2021.
He has won three Premier League titles at the Etihad and played a key part in the 2022-23 Treble-winning season.
However, Grealish managed just 715 Prem minutes last season and made seven starts - chipping in with one goal and one assist.
The England international added four goal involvements in the FA Cup but was noticeably snubbed in favour of debutant teenager Claudio Echeverri when City trailed Crystal Palace in the final.

SunSport exclusively revealed Grealish was told he is no longer part of Guardiola's plans - and would be snubbed from the squad at the Club World Cup.
Guardiola was quizzed on his stance and the latest on Grealish when he spoke to the media ahead of their Wednesday Group G opener against Wydad.
And he confirmed it was decided for the best that the playmaker did not travel to the United States.
Asked if Grealish's future is away from the Etihad, the Spanish manager said: "I don't know. Now we decided 'don't come here' and what happens in the end I don't know.

"If we don't find [a transfer], he's a player for Man City and he will be back.
"He had a conversation between him and the club and they decided it was best [to miss Club World Cup].

"Jack is an exceptional player. The only reason why he didn't play last season is of course my decisions.
"We decide that he has to play. The club was honest, he was honest.
"Incredible love and respect and we decided the best is to stay [behind] and have a place that he can feel like he can come back to be the player like he was in the year of the Treble or all his career in Aston Villa.
"Without him, it would have been more difficult in the year of the Treble."
SunSport also revealed both Newcastle and Bayer Leverkusen are considering moves for the former Aston Villa captain.
Everton may also test the waters to lure him to their new stadium.
Now we also understand Napoli could make a £45m bid for Grealish where he would reunite with Kevin De Bruyne.
And Guardiola has given Grealish the blueprint to reigniting his football fire.
The boss advised: "The fact is in the last two seasons, he didn't play many minutes.
"He has to come back to play and have the butterflies in his stomach that he can play every three days, every three days and show again the quality he has."

Jose Mourinho at centre of Turkish scandal as leaked texts force resignations
Turkish football has been rocked by a leaked messages scandal in which it is alleged governing body chiefs were plotting to "make Jose Mourinho pay" after the Fenerbahce boss was "tolerated too much." TFF committee president Celal Nuri Demirturk and the entire board of directors have resigned en masse after a series of communications were leaked in relation to Mourinho's complaints about refereeing standards in February following a draw with closest rivals Galatasaray. The former Chelsea and Manchester United boss was banned for four games and fined £35,000 for 'derogatory and offensive statements towards the Turkish referee" and accusing "Turkish football of chaos and disorder with insulting and offensive statements towards both the Turkish football community and all Turkish referees.' Mourinho subsequently said that he was suing Galatasaray following an accusation of racism from the Istanbul rivals after he said their bench 'jumped around like monkeys' during the scoreless draw. He had previously claimed Turkish referees were "worse than I thought" and "smell bad." But the leaked messages have prompted fury with Fenerbahce releasing a statement questioning the impartiality of the sport. A club spokesperson said: 'Our club has made an official application to the Turkish Football Federation in response to the correspondence that has been made public today and is claimed to belong to members of the Professional Football Disciplinary Board. 'We believe that this hostile mentality, which clearly violates the principle of impartiality and is based on showdowns and revenge, has no place in Turkish Sports.' The club's general secretary Burak Kizilhan added: "If these allegations are true, not only would the impartiality of the relevant board be compromised, but the institutional reputation and public trust in the Turkish Football Federation would be seriously damaged. "For individuals who serve on one of the most important boards with the duty to establish justice in Turkish football to be involved in such statements would render the concept of 'discipline' ineffective and constitute a clear attack on the principles of impartiality and equality." In another meeting between Fener and Galatasaray in early April, Mourinho approached opposition manager Okan Buruk at full time and grabbed his nose - leading to another three-game ban. Mourinho had also linked himself with the Rangers job and said he would be keen on taking over a Premier League struggler so he could avoid Turkish referees. Rio Ferdinand believes Victor Osimhen could be 'waiting' for a call from Manchester United after turning down a lucrative transfer move to the Saudi Pro League.
