For women in India, lung cancer is often in plain sight but overlooked due to diagnostic and societal barriers
Lung cancer is one of the most prevalent causes of cancer deaths among women glocally. In the United States, lung cancer surpassed breast cancer as the leading cause of cancer-related deaths among females since 1987. In India, lung cancer is also on the rise among women, emerging as a significant public health concern. According to the National Cancer Registry Programme (NCRP) data for 2012–2016, lung cancer ranks among the top five cancers in women, constituting 6.2% of all cancer cases in females. The age-adjusted incidence rate for lung cancer in women is highest in Aizawl district, Mizoram (27.9 per 100,000), followed by Mizoram state (18.0 per 100,000). Trends show a significant increase in lung cancer incidence in cities like Bengaluru, Chennai, Delhi, and Mumbai from 1982 to 2016.

Another alarming statistic is that the age-adjusted lung cancer incidence among females in India is increasing annually and approaching that of males. Approximately 32% of lung cancer cases in women occur in the 55–64 age group, and adenocarcinoma is the dominant histological subtype, accounting for 53% of cases in females compared to 34% in males. In a Kolkata-based study, more than 2/3 rd of female lung cancer patients were non-smokers, suggesting that lung cancer is not solely a smoker's disease.
Symptoms in females are frequently missed or misdiagnosed, delaying treatment and resulting in poor outcomes. The reasons for neglected symptoms are a mix of biological, sociological, and diagnostic issues specific to women. In India, around half of female lung cancer patients are diagnosed at an advanced stage, which contributes to high mortality rates.
More women are diagnosed with lung cancer who are non-smokers than ever before. Research across studies, including NCRP data, confirms that many women diagnosed with lung cancer in India have never smoked. This can lead to a bias where healthcare professionals may overlook lung cancer as a potential cause of symptoms, particularly in young, otherwise healthy women who are non-smokers. Biologically, lung cancer in non-smoker females often presents as adenocarcinoma with a particular mutation in the EGFR gene which is treated differently than a lung cancer without this mutation, whereas smokers more commonly develop other types of cancer viz. squamous cell carcinoma or small cell lung cancer, linked to tobacco-related carcinogens. Environmental factors like air pollution, second hand smoke, and indoor biomass fuel exposure are significant contributors to non-smoker lung cancer in Indian women.
The biological differences in lung cancer between non-smokers and smokers have significant treatment implications. Non-smoker females with adenocarcinoma frequently harbor EGFR mutations, making them more responsive to targeted therapies like EGFR tyrosine kinase inhibitors (e.g., gefitinib or erlotinib), which can improve survival compared to traditional chemotherapy. In contrast, smokers with squamous cell carcinoma often lack these mutations and are more likely to benefit from chemotherapy or immunotherapy, due to a higher tumor mutational burdens associated with tobacco exposure. These differences underscore the need for doing additional advanced tests to identify certain genetic mutations, at diagnosis to tailor lung cancer treatment.
Frequently observed symptoms such as fatigue, shortness of breath, back pain, or a cough lasting three weeks are often dismissed as non-serious conditions like asthma, allergies, menopause, or anxiety. In India, vague or mild symptoms may be ignored by both healthcare providers and patients, limiting opportunities for timely imaging and testing. Women also tend to present with non-specific symptoms, such as shoulder pain and hoarseness, compared to men.
Societal roles and caregiving responsibilities also delay diagnosis. Women in India often prioritize family well-being over personal health, leading to delays in seeking care. This may cause women to dismiss early warning signs or postpone medical consultations until serious symptoms develop.
Developing awareness among women and primary healthcare providers is critical. Persistent or unusual respiratory symptoms, even in non-smokers, should prompt immediate investigation with chest X-rays or CT scans. In India, where 75% of lung cancer cases are diagnosed at stages 3 or 4, early detection is vital to improve survival rates. Public health campaigns must emphasize that lung cancer affects women without observable risk factors, including non-smokers. Increasing advocacy for annual check-ups, detailed medical histories, and addressing gender biases in medical diagnoses can reduce delayed or missed diagnoses.
For women in India, lung cancer is often in plain sight but overlooked due to diagnostic and societal barriers. Dr Rhythm Bhalla A well-off, educated couple from Canada walked into my clinic last week, seeking consultation for in-vitro fertilization (IVF). 'After ten years of marriage, we've been yearning for a child and would value your guidance,' the wife said. It was a standard conversation—until she quietly added, 'I would like a male child.' Her husband, though slightly hesitant, nodded in agreement. As a gynecologist, such requests are, unfortunately, not rare. IVF, while a medical marvel offering hope to countless childless couples, has also opened the door to something far more problematic: sex selection. It prompts a painful question—why this persistent preference for boys? From the operating table to the boardroom, women have proved themselves over and over. Whether it is medicine, law, aviation, science, or governance, women are thriving. They run countries, lead corporations, and redefine success across every domain. Yet, a slice of society continues to believe that a male child holds higher value—a belief not just outdated but dangerous. In the Indian context, this preference has deep cultural roots. Traditionally, lineage and inheritance have been perceived as the domain of the male child. Whether it's lighting the funeral pyre or passing down the family name and business, the responsibility has always been reserved for sons. 'A father cannot exist without a mother,' I remind my patients, yet many still hesitate to envision a daughter performing these roles. Underpinning these decisions are centuries-old expectations and pressures. Indian women are often burdened—silently or overtly—with the expectation of giving birth to a boy. I've seen couples devastated by the birth of a second girl. I've met women emotionally blackmailed—threatened with abandonment or divorce—because they failed to provide a 'male heir.' And I've had educated, affluent couples ask for male sex selection in IVF simply to appease family elders with ideas of inheritance and legacy. To such couples, I ask: Why not a girl? Can't she lead your legacy, inherit your business, care for aging parents, and bring the same joy, prosperity, and pride? Yet, most often, my questions are met with awkward chuckles and patronizing smiles. We remain shackled by beliefs that a male child is central to carrying forward the family line—beliefs that desperately need to change. Denied legal sex selection in India, many such couples head to international clinics in Dubai, Thailand, or Singapore, where the practice is not illegal, thus legitimizing their biased preferences. But regardless of venue or legality, the problematic sentiment remains the same. Here in India, our laws are clear. The Pre-Conception and Pre-Natal Diagnostic Techniques (PCPNDT) Act, amended in 2003, strictly prohibits sex selection before or after conception. While illegal centers do continue to operate, the government has intensified scrutiny to curb this menace. However, this vigilance, while necessary, often leads to undue pressure on ethical doctors and innocent couples navigating the IVF journey the right way. Beyond legal enforcement, however, we must shift mindsets. Education and public awareness campaigns should aim to dismantle these damaging beliefs. Legal reform alone won't suffice; societal expectations, especially those that quietly coerce women into choosing male offspring, must be publicly challenged. Empowering women economically can create ripples of change. IVF may be a gift of science, but how we choose to use it reveals who we are as a society. A recent study alleging widespread antibiotic contamination in Indian rivers is based on "simulations and assumptions" and therefore, no conclusion on the issue can be drawn from it, the government has said. Replying to a question by Congress MP Pramod Tiwari in the Rajya Sabha on Friday, Union Minister of State for Environment Kirti Vardhan Singh said the study has cited its "own limitations of data availability". Study findings The study, conducted by researchers from McGill University, alleged that 80 per cent of the total length of rivers in India could be posing environmental and health risks due to antibiotic pollution. The media report, published on April 24, 2025, is based on a study titled 'Antibiotics in the global river system arising from human consumption' conducted by researchers from McGill University, Canada, and its "findings are based on simulations and assumptions", the minister said in a written reply. Furthermore, the minister informed that the government has not conducted any study on antibiotic contamination of rivers. Regarding the steps taken to regulate pharmaceutical waste and strengthen pollution control, Singh informed that the government launched the National Action Plan on Antimicrobial Resistance (NAP-AMR) in April 2017 to develop and implement strategic interventions to reduce the environmental impact of antimicrobial resistance through effective waste management and safe disposal of expired antimicrobials. He mentioned that the environment ministry issued a notification for the bulk drug and pharmaceutical industry in August 2021, classifying chemical and biological sludge or any residue generated from wastewater treatment as hazardous waste. These shall be managed in an environmentally sound manner in accordance with the Hazardous Waste and Other Wastes (Management and Transboundary Movement) Rules, 2016, Singh added. He highlighted that state pollution control boards have been empowered to prescribe additional relevant parameters and monitoring frequencies based on local conditions. The expired antimicrobials are disposed of by manufacturers or suppliers through incineration as per the Bio-medical Waste Management Rules, 2016, while sludge from effluent treatment plants is sent to common biomedical or hazardous waste treatment facilities for incineration or safe disposal. "To reduce antibiotic residue from effluent sources, industries are adopting reduce, recycle and reuse of treated effluent to maximise the extent of Zero Liquid Discharge to minimise the risk of pharmaceutical compound residues," the minister said.

A global investor with net assets of USD 266.3 billion as of December 31, 2024, Ontario Teachers' Pension Plan Board (Ontario Teachers') acquired a majority stake in Sahyadri in 2022. The Pune-based hospital chain is now one of Maharashtra's largest, with 11 hospitals and over 1,400 beds.
