The deadly explosions that happened Monday at U.S. Steel's Clairton Coke Works facility come following several different incidents at the plant over the past several years, including one incident in February.
A flaring and fire at the Clairton plant in February sent two people to the hospital.
U.S. Steel said that a heating and cooling failure in one of the plant's batteries resulted in a buildup of flammable materials that ignited and caused the boom.
Leaders called it a rare occurrence and the impacted battery was taken off line.
In February of 2024, there were process and equipment failures at seven of the facility's ovens, prompting the health department to get involved, fining U.S. Steel nearly $2 million.
Two years earlier, the Allegheny County Health Department issued an order against U.S. Steel for pushing coke out of ovens without air pollution devices.
In 2018, a catastrophic Christmas Eve fire blamed a failing pipe that led to gas from the ovens igniting.
In 2010, at least 20 people were hurt when an explosion occurred during maintenance work.

Investors are eagerly awaiting trial results later this year. 10 stocks we like better than Viking Therapeutics › Viking Therapeutics (NASDAQ: VKTX) investors have seen a handsome return over the past three years, and this demonstrates that the real battle over biopharmaceutical stocks is won in the lab and clinical trials. Viking Therapeutics doesn't have a commercial product on the market yet and has never generated a dollar in revenue. Yet, it's made many investors wealthy. One thousand dollars invested three years ago is now worth an incredible $10,640 as of Aug. 11. Viking Therapeutics: The best may be yet to come The excitement around the stock centers on its lead development compound, VK2735. It's a GLP-1/GIP dual agonist for the treatment of obesity. Vikings VK2735 is following in the footsteps of Eli Lilly's GLP-1/GIP dual agonist tirzepatide (branded as Zepbound), and investors are hoping VK2735 can achieve similar levels of success. Where is this promising drug now VK2735 is in phase 3 trials in subcutaneous (injection) form, with results probably not due until 2027, and, arguably more excitingly, it's in phase 2 trials in an oral form. The latter has obvious benefits of convenience and comfort for patients, and management expects to report on the phase 2 trial this year. Investors have high hopes, as the phase 2 subcutaneous trial resulted in up to 88% of patients experiencing more than 10% weight loss. The phase 1 trials in oral form were also impressive (remember that phase 1 trials are fundamentally exploratory and often aim to understand safety and tolerability), with a mean weight loss of 8.2% after 28 days of dosing. These results are enough to encourage Viking investors that VK2735 could prove to be a hugely successful compound, and the results of the latest tests of the oral form are eagerly anticipated, not least by potential larger pharmaceutical companies that may try to buy the company and take VK2735 (oral) through phase 3. Nurses at Anaheim hospital hold 1-day strike to voice concerns about employee turnover

Registered nurses at an Orange County hospital are holding a one-day strike on Tuesday, claiming that management has refused to address concerns related to high nurse turnover. The nursing staff at West Anaheim Medical Center said in a news release that it wants a new contract to improve conditions for safe patient care, provide ways to attract new nurses and retain them, and keep up on legal requirements on nurse-to-patient ratios. "Our emergency department is often completely full with a large lobby of patients waiting to be seen," said Liz Rivera, RN in the telemetry unit. "Having a strong, experienced, well-trained workforce in all units will make a huge difference in getting our patients timely and critical care throughout the hospital." Heather Foy, a nurse in the intensive care unit, said the hospital perpetually hires new graduates, who leave for other opportunities quickly after completing their training. "In my twelve years at WAMC, nurse retention has not been this dire. We are truly exhausted and need real solutions," she said. In a statement, WAMC said it has already implemented pay increases for nursing staff that went into effect last week, and its latest proposal in contract negotiations provides benefits it calls among the best in the nation. "We value our employees and their dedication to caring for our community, and in turn, we are committed to ensuring that our mission endures," the statement reads. "West Anaheim Medical Center continues to be a trusted source for healthcare in Orange County, receiving national patient safety and quality recognitions while providing vital care for its community, including A-grades in patient safety, health equity, community benefit, and clinical outcomes from the Lown Institute Hospitals Index. None of this would have been possible without our dedicated staff. We are committed to ensuring a workplace that honors their work and continues to provide lifesaving care." WAMC said the strike will not disrupt patient care. The nurses said negotiations have been ongoing since February and have included three days with a federal mediator. It's not yet clear when the sides will return to the bargaining table. Aerial footage on Tuesday morning showed a large group of nurses gathered outside of the hospital in solidarity. The nurses are expected to return to work on Wednesday.
