An Emirati man with a rare inherited disorder has become only the second patient in the world to benefit from a pioneering $2 million gene therapy that eliminates the need for regular blood transfusions.
Khalifa Dawood, 22, from Abu Dhabi said he has been given a 'new lease on life' as he recovers from the novel gene treatment he received at Cleveland Clinic Children's hospital in Ohio, which recently won medical approval for the therapy.
The UAE has some of the regions highest rates of thalassaemia, also known as sickle cell disease, a condition that limits the body's ability to produce healthy red blood cells.
While some carriers show very limited symptoms, others experience bone deformities, shortness of breath, dizziness, organ damage and heart palpitations.
The condition can require regular blood transfusions, with research suggesting thalassaemia affects as many as 16.5 per cent of the population.
A new treatment used for the first time by the Cleveland Clinic Children's hospital has proven to modify a patient's blood forming cells, and can be delivered as a one-time treatment called Casgevy.
Mr Dawood said that he hoped the treatment would allow him to lead a normal life, without the need for regular hospital stays.
'I am hoping this gives me a new lease on life where I am energised to enjoy it,' he said.
Life-changing treatment
Since its approval by the US Food and Drug Administration in January, the therapy has been delivered to an American woman from Ohio and Mr Dawood.

Although the treatment is expensive, it is life changing for those who receive it, according to Dr Rabi Hanna, chairman of the paediatric haematology-oncology department at Cleveland Clinic Children's hospital.
It is not uncommon for adults to receive specialist care at a children's centre, particularly for conditions they have had since birth, as is the case with Mr Dawood.
'Unlike regular medicines that people take for years, Casgevy is meant to be a one-time treatment that could last a lifetime,' he said.
'It uses CRISPR gene editing, which is a very advanced science. Editing cells safely and precisely takes a lot of time, technology, and testing.
'It is also custom-made, as Casgevy is made from a person's own cells, which are collected, sent to a lab, edited and then returned. That process is complex and individualised.'
The innovative treatment uses a gene editing technology called CRISPR/Cas9 to modify a patient's stem cells.
The edited cells are then transplanted back into the patient, where they produce more haemoglobin in new healthy red blood cells.
The FDA's approval was based on results from a single arm which showed 91 per cent of patients with beta thalassaemia did not need a blood transfusion for at least a year after having the treatment.
Costly medical care
Despite the therapy's success, it is likely to remain out of reach for the majority with costs of about $2.2 million.
Gene therapies are typically the most expensive medical treatments.
Etranacogene dezaparvovec, sold under the brand name Hemgenix is a gene therapy used for the treatment of haemophilia B, a potentially life threatening disorder that prevents blood from clotting.
Priced at about $3.5 million, it was the world's most expensive drug until the arrival of Lenmeldy in March last year.
The gene treatment developed by Orchard Therapeutics costs about $4.2 million and is used to treat life threatening nerve disorders in young children.
High costs of certain drugs can be covered by insurers, government health systems or charitable foundations when there are no other treatment options.
Drug companies also occasionally offer financial support programmes for families in need.
The process of administering Casgevy begins by collecting stem cells from the patient's blood that are then sent to the lab to be edited using tiny, precise scissors.
By clipping off the DNA responsible for preventing the body making healthy haemoglobin, the repaired cells can then be returned to the body.
A course of chemotherapy is then required to clear out the old, sick marrow cells, allowing new healthy ones to grow in their place.
Recovery time in the hospital is about four weeks.
As the science behind gene editing improves, it should become faster, easier and cheaper to edit cells with higher accuracy, and fewer side effects.
'It is my hope and prediction that price will come down, but I am not sure how quickly,' said Dr Hanna.

'Right now, Casgevy is the first gene editing therapy approved for sickle cell and thalassaemia.
'But more companies are developing similar treatments. When that happens, competition can help drive prices down.
'Also as companies learn how to make these therapies at scale, they may reduce production and delivery costs.
'Think of it like early computers or smartphones – they were expensive at first, but prices dropped as technology improved.'
Global health drive
Although patients like Mr Dawood must travel overseas for expensive gene therapy, the Gulf is also emerging as a go-to destination for emerging treatments.
The King Faisal Specialist Hospital & Research Centre in Saudi Arabia is emerging as a leader of regional clinical trials and delivered a breakthrough in haemophilia care, using one-time gene therapy to restore clotting function in eight patients.
In the UAE, at Medcare Royal Speciality Hospital Al Qusais, patients are receiving the latest care for rare genetic disorders like spinal muscular atrophy and Duchenne muscular dystrophy, two life-limiting growth conditions.
Doctors there said patients were travelling into Dubai from Turkey and elsewhere in the region.
Medcare Women and Children Hospital was one of the first private clinics outside the US to administer gene therapy for spinal muscular atrophy, and has since delivered 100 infusions.
American Hospital, Al Jalila Children's Hospital and Fakeeh University Hospital also offer gene therapy Zolgensma, which can cost about $2 million per dose.
Dr Vivek Mundada, a consultant paediatric neurologist at Medcare Royal Speciality Hospital, said the drug had delivered remarkable outcomes in managing complex genetic conditions.
'Dubai is becoming a centre of medical innovation, and Medcare is proud to be leading that movement, especially in the field of rare genetic diseases,' he said.
'Through our comprehensive, patient-centred care model, we are transforming outcomes for children who previously had limited treatment options.'

One afternoon in May 2025, 48-year-old Lebohang Pitso* from Lesotho's capital, Maseru, felt her life slipping away. Weakened from a bout of severe bleeding caused by cervical cancer, she sought emergency care at the city's renowned Senkatana Oncology Clinic. A few hours after being admitted and receiving a blood transfusion, she felt a powerful transformation take place in her body. 'It was like a heavy cover was removed from my face,' she says. 'The blood gave me life.' Moved by the experience, Pitso asked her family to give back. 'Four of my family members went to the blood bank and donated blood,' she says. 'Blood donors are doing a great job. If it were not for my condition, I would be a regular donor. I encourage those who can, to give life to others.' Lesotho Blood Transfusion Services is ramping up efforts to recruit and retain blood donors. It has implemented several strategic approaches, such as promoting blood donor clubs in schools and conducting mobile outreach blood donation drives, which have led to improved blood collection. In 2024, the institution carried out 165 mobile blood donation drives. Overall, there has been an almost 70% increase in the number of units collected between 2017 and 2024, from 5583 to 9411. 'But demand is higher than supply,' says Manaledi Makhetha, a phlebotomist at Lesotho Blood Transfusion Services. 'For example, the 180 units we collected during a week-long campaign this year were all used in just two days,' she says. The total number of units collected per year, although improved, still falls short of the annual target of 10 000 units. Makhetha screens donors, collects blood and ensures its safe delivery to various hospitals. 'The primary need for blood transfusions is maternal- and cancer-related cases and road traffic crash survivors, among others,' she says. On average, Senkatana Oncology Clinic uses 2‒3 pints of blood a day. Many cancer patients require frequent transfusions, as both the illness and treatment compromise blood production. Rethabile Makhetha, an oncology nurse at the clinic, recalls one morning when four patients needed blood, but the clinic only had sufficient units for three. 'Cancer is a long journey,' she says. 'The patients become family. When we can't help them due to blood shortages, it's devastating.' The Ministry of Health is increasing public awareness, advocating for regular donations and expanded infrastructure to meet increasing demand. 'With World Health Organization (WHO) and other partners supporting the Ministry of Health with guidelines, policies and campaign events, Lesotho is making strides,' says Stephen Monkoe, Director of Laboratory Services at the Ministry of Health. 'We are grateful to partners and donors alike. Blood donors are our heroes.' Every year on 14 June countries mark World Blood Donor Day, as a tribute to unpaid blood donors who save lives, strengthen communities and show solidarity with others. The day is also an opportunity to highlight the ongoing need for safe, regular blood donations. 'It is imperative that everyone, from citizens to community leaders, private and public partners, and government, redouble our efforts to strengthen the national blood service,' says Thato Mxakaza, Health Promotion Officer in WHO's country office in Lesotho. 'By investing in vigorous collection, safe processing, and equitable distribution, we ensure that the profound gift of blood translates into sustained life and renewed hope for every recipient.' In Maseru, 32-year-old Nthomeng Leuta encourages others to become regular blood donors. Curious to know her blood type, she began donating blood at high school. She discovered she was type O negative, the only blood type that can be transfused to any recipient and is therefore critical during emergencies when a recipient's blood type is unknown. Motivated by her special status as a 'universal blood donor' she jokingly refers to the blood bank her 'second home.' Leuta's most precious memory is when she responded to a social media post from a man whose wife urgently needed a blood transfusion for an emergency caesarean section. Leuta responded, and the result of her donation was a healthy mother and baby boy. 'The husband called me to inform me how I had saved two lives that are so dear to him,' she says. Predicting and managing clinical risk Beyond operational and access improvements, Medshield proactively uses technology to manage clinical risk. By applying artificial intelligence and predictive modelling with clinical biomarkers, the Scheme identifies members who may be at risk of hospital admission within the next three to six months. Once these members are identified, the Scheme provides clinical support, including clinical coaching or referral to a general practitioner or specialist. The goal is to prevent unwarranted admissions, reduce the need for emergency care, and improve health outcomes for our members and their families. Technology is also being used to identify members likely to be readmitted post-discharge, for example, due to wound infections. Early engagement helps to identify clinical risk, prevent complications and avoid unnecessary re-hospitalisation. In addition, Medshield is piloting facial scan technology that can analyse up to 20 clinical health parameters from a person's facial image. Medshield is utilising this as a mass digital health screening tool to direct members to appropriate care to reduce high-cost episodes and to support earlier intervention. Behaviour change and lifestyle health Lifestyle-related diseases remain one of the most significant cost drivers for medical schemes. Conditions such as type 2 diabetes, high blood pressure, and obesity are closely linked to behaviours such as inactivity, poor diet, and unmanaged stress. Medshield is working to address this through behaviour change programmes like Medshield Mind and Medshield Movement. These initiatives support members with educational materials, coaching, and practical advice for managing their health, including child and family-focused interventions. One proof of concept currently underway has shown that members living with type 2 diabetes can reduce or stop medication with consistent lifestyle improvements while under medical supervision. This programme aims to empower members to improve their quality of life. Interventions are tailored based on a member's clinical risk profile, ranging from low-touch digital education to personalised nurse coaching. Due to current regulations under the Medical Schemes Act, the Scheme cannot offer financial incentives for improved health outcomes. However, behaviour change remains a key focus area in chronic disease management. Remaining accessible beyond digital tools Although the demand for access to digital channels continues to increase, many members still prefer to meet in person. Medshield has made enhancements to our regional offices and walk-in centres and recently relocated our Cape Town and Durban branches after conducting a survey and engaging with members to understand their needs. Looking ahead, the Scheme is reviewing its benefit design for the 2026 cycle with a strong focus on prevention. With one of the highest solvency ratios in the industry, Medshield is well-positioned to consider enhancements to its preventative benefits while aiming to limit contribution increases. Additional efforts are focused on predicting avoidable healthcare episodes and ensuring members are guided toward appropriate care earlier in their treatment journey.
