The Truth About Cane Sugar vs. High Fructose Corn Syrup—And Why It Matters for Your Health
President Trump recently suggested Coca-Cola should swap out high-fructose corn syrup for cane sugar.
Experts say that the two are really not that different, and have similar health impacts.
Here's what else the top doctors and nutritionists want you to know.
On Wednesday, President Donald Trump suggested on Facebook that he had convinced Coca-Cola to phase out high-fructose corn syrup in its products in favor of cane sugar. "I have been speaking to Coca-Cola about using REAL Cane Sugar in Coke in the United States, and they have agreed to do so,' he wrote. 'I'd like to thank all of those in authority at Coca-Cola. This will be a very good move by them — You'll see. It's just better!"
It's clear that high fructose corn syrup isn't, well, great for you. It's been linked to an increased risk of type 2 diabetes and heart disease, among other health issues. Plus, it doesn't look super appealing. In response, more and more companies have started to use more natural forms of sugar in their products, but there are a lot of questions swirling around cane sugar now that President Donald Trump is talking about its perks.
But is one actually 'better' than the other? Here's what you need to know about cane sugar versus high fructose corn syrup, according to experts.

What is cane sugar?
Cane sugar is fructose (a form of sugar) that's extracted and refined from the sugarcane plant, explains Scott Keatley, RD, of Keatley Medical Nutrition Therapy. 'It is found in everything from baked goods to cereals to beverages, essentially anything labeled with 'sugar,' 'raw sugar,' or 'evaporated cane juice,'' he says. 'It is often sourced from cane.'
What is high fructose corn syrup?
High fructose corn syrup, on the other hand, is a sweetener made from corn starch, Keatley says. 'There are different formulations, but the two most common are HFCS-42 and HFCS-55, the latter being used most often in soft drinks,' he says.
It's often found in soda, condiments, snack bars, sweetened dairy products, and shelf-stable baked goods, according to Keatley.
Why are they bad for your health?
As with everything, how much of these types of sugar you consume and how you consume it matters. 'Neither product is inherently bad, it's the way we use them which can cause health consequences,' Keatley says. 'Both cane sugar and high fructose corn syrup contribute to excessive calorie intake without nutritional benefit.'
Having too much of either one of these products can increase the amount of fat in your blood, mess with how your body processes sugar, lead to fat building up in your liver, and increase inflammation in your body, Keatley says.
Is one better for you than the other?
Nope, consuming one is not worse than consuming the other, according to Spencer Nadolsky, DO, an obesity and lipid specialist physician. (On X, he hilariously compared exchanging one for the other to "rearranging the chairs on the Titanic.")
'Cane sugar and high fructose corn syrup have very similar clinical outcomes in terms of metabolic health when consumed at similar doses,' he says. 'There doesn't seem to be much of a clinical advantage in terms of metabolic health for either one. Preference is likely dependent on taste.'
This is partly because the two aren't that different in molecular makeup. Cane sugar is sucrose, which is a molecule that connects glucose and fructose together, Dr. Nadolsky explains. 'High fructose corn syrup is a mixture of glucose and fructose.' If you want to get a little more technical, sucrose is 50/50 glucose and fructose, with the molecules connected, while high fructose corn syrup is a mixture of glucose and fructose that isn't connected, he explains. That means the product is made up of about 55 percent fructose and 45 percent glucose.
'While some feel the sucrose should be better due to the glucose and fructose being connected with slightly less fructose, the studies looking at them don't show meaningful differences in terms of metabolic health,' Dr. Nadolsky says.
At the end of the day, Dr. Nadolsky says it's better to limit any and all added sugars in your life, regardless of where they come from.

Healthcare Access, Geography Predict HIV in Black Women
Limited healthcare access and sociostructural factors were stronger predictors of HIV than behavioral risk factors among Black women in the southern United States, based on modeling data from more than 300,000 women in or near Atlanta, Georgia. The findings were presented in a late-breaker study at the International AIDS Society Conference on HIV Science. Nonbehavioral factors such as structural and social determinants of health can complicate HIV risk assessment, wrote Meredith Lora, MD, of Emory University, Atlanta, and colleagues. Previous risk prediction models have underperformed in Black women, whose HIV risk is approximately 20 times that of White women, the researchers wrote. To better assess HIV risk in Black women, the researchers used machine learning to create a model that analyzed electronic medical records data from all women treated at a single center in Atlanta between 2012 and 2022. The study population included 333,263 women; 617 (0.19%) had incident HIV diagnoses and 89% of those with HIV identified as Black individuals. The model included features with a prevalence of 5% or higher in the electronic medical records data and included novel structural determinant features and healthcare utilization features. Overall, the top predictors of HIV included younger age, Black race, residing in high HIV incidence zip codes, and at least one change in phone number or address prior to HIV diagnosis. In addition, women who underwent more HIV screening tests were more likely to be diagnosed with HIV, and women with HIV were more likely than those without HIV to seek care in an emergency department (ED) vs primary care or women's healthcare settings. Although more women with HIV tested positive for sexually transmitted infectious compared to those without HIV in the 2 years before diagnosis, 'seeking sexual health was more important to the model than STI positivity,' the researchers wrote. The model achieved a test area under the receiver operating characteristic curve (AUROC) of 0.90 and area under a precision-recall curve (AUPRC) of 0.14. The appearance of frequent HIV testing as a predictor of risk suggests the presence of undisclosed risk factors that may merit further investigation, the researchers noted in their abstract. EHR models designed for racially diverse female populations may identify more candidates for pre-exposure prophylaxis (PrEP), but that is not enough, the researchers emphasized. 'Evaluating how these models are implemented to support PrEP uptake and behavior change is critical for real-world impact,' they concluded. Assistance in Risk Assessment The current study is important given the traditional and historical difficulty of women in the United States in predicting their own risk of HIV, said Monica Gandhi, MD, director of the University of California San Francisco Bay Area Center for AIDS Research, and a professor of medicine at UCSF, in an interview. Gandhi cited a recent study from the CDC showing that more than 2.2 million individuals in the US need PrEP, but only 336K have received prescriptions. 'Women may not be able to adjudicate their own risk of HIV, as that risk depends on the risk in their male sexual partners,' said Gandhi, who was not involved in the current study or the CDC study. 'Machine learning using electronic medical records [EMR] can make risk prediction more accurate by reporting both health and social factors,' she noted. The current study identified predictors of HIV risk using extracted EMR data, that the healthcare system may be able to mitigate, she said. 'I was surprised by the fact that seeking sexual health was a stronger predictor of HIV than STD positivity, which indicates that women actually are more aware of their risk for HIV in the US than suggested in previous studies that usually involved smaller sample sizes, Gandhi told Medscape Medical News . 'I was also surprised that women with frequent changes in address or phone number had higher risk,' she said. This suggests mobility as a risk factor, which has been identified more commonly in sub-Saharan Africa, she noted. However, Gandhi was not surprised by the preference for ED care or the increased HIV risk among Black women vs other races, both of which have implications for HIV prevention, she said. The preference for sexual health care in the ED setting in the study population suggests that the ED is an important setting in which to perform STD testing and start PrEP or set up an individual with HIV prevention services, said Gandhi. 'Designated PrEP services for women in historically Black neighborhoods are indicated, and women who are mobile and change their address or phone number frequently should be counseled on HIV prevention modalities, specifically PrEP,' she added. Looking ahead, qualitative research is needed to ask a subset of women with HIV who presented to the ED for sexual health services why they chose the ED; this could help inform how to re-engage women in care, Gandhi said. Additional research on women's knowledge of PrEP and whether women with HIV were offered PrEP in the past also would help identify more opportunities for HIV prevention, she said.

Indigenous Canadians Lack Access to Cardiologic, Stroke Care
Canada's Indigenous population continues to face major disparities in heart and stroke care. These disparities result from a combination of neglect, inaccessible resources, and lack of cultural sensitivity when it comes to community values and healthcare practices. Cardiovascular disease is the leading cause of death among Canada's Indigenous population, which includes First Nations, Métis, and Inuit patients. According to a 2022 study in the Canadian Journal of Cardiology , incidence, prevalence, and mortality rates for cardiovascular disease are higher among Indigenous peoples than among non-Indigenous Canadians. First Nations patients have approximately 2.5 times the cardiovascular disease prevalence compared with that of non-First Nations patients. Complicating the situation, access to and inequality in cardiovascular care for Indigenous Canadians remain poorly studied and understood, the study noted. Isolation in remote communities, lack of access to emergency services, negative past experiences with the medical system, and resulting mistrust are factors contributing to this long-standing and growing healthcare crisis. 'The disparities in care for Indigenous Canadians are due to a mix of historical, systemic, and social factors,' Christine Faubert, vice president of Health Equity and Mission Impact at Heart & Stroke in Toronto, told Medscape Medical News. 'The legacy of settler colonialism has left deep scars, creating conditions that lead to significant health disparities. This includes trauma and socioeconomic disadvantages from policies like residential schools and forced relocations.' Bridging the Cultural Divide Many patients delay medical care because they've had such negative experiences in the past, and this decision often results in advanced disease by the time they seek care, Heather Foulds, researcher and professor of kinesiology at the University of Saskatchewan in Saskatoon, Saskatchewan, told Medscape Medical News. 'Also, when patients undergo stroke or cardiac rehabilitation, healthcare practitioners place great emphasis on the individual patient. However, Indigenous communities are much more family-oriented, and the patient might have grandchildren for whom they're caring and can't simply leave behind,' Foulds noted. She is a former Heart & Stroke and the Canadian Institutes of Health Research Indigenous Early Career Women's Heart and Brain Health Chair. 'Some patients have 3-day delays following a stroke because there are no healthcare providers. Most rehabilitation centers are in major cities, and patients have no way of getting to them.' Conflicting life priorities also play a role. 'A patient might say to themselves, 'What's the most important challenge I'm facing today? Maintaining my blood sugar levels might not be the most important thing right now when I'm trying to find a place for my kids to live.'' In addition, Western and Indigenous views of medicine differ significantly. For Indigenous Canadians, living within a healthcare paradigm so different from their own can be challenging. Some Indigenous community members are carving out their own academic paths as a way of focusing on their people's distinct needs. Pathways for Change Originally from the Pimicikamak Cree Nation in northern Manitoba, Margaret Hart is a doctoral student in the University of Manitoba's Faculty of Education, Winnipeg. Hart has been seeking to rebuild the occupational therapy program to incorporate Indigenous ways of knowing, being, and doing. 'Across Canada, First Nations and other Indigenous communities are drawing on generations of knowledge, relational teachings, and community-based values to promote health and well-being,' said Hart. 'Due to the historical and ongoing impacts of colonization, many First Nations people face elevated cardiovascular risk and reduced access to timely, appropriate stroke care. We know stroke is a leading cause of death and disability and that women are at higher risk and often experience different pathways of recovery.' Hart's mother, Phylis, of the Nisichawayasihk Cree Nation, died of a hemorrhagic stroke in September 2024 with no care or prevention. Heart & Stroke is tackling the challenges of health reconciliation and striving to provide better access to equitable healthcare in culturally safe environments for Indigenous populations. One of their initiatives is StrokeGoRed, which was developed for women in the north who have no access to basic care. It's the first formal research network in Canada dedicated to studying stroke in women. Together with Hart, they're working to understand how stroke affects women and men differently and to develop personalized treatments to improve outcomes for women. In partnership with community members in Whitecap Dakota First Nation, Stacey Lovo, PhD, a researcher with the School of Rehabilitation Science in the University of Saskatchewan's College of Medicine, has developed a virtual reality hub to provide remote treatment for northern Saskatchewan communities that can't easily access healthcare. The program will allow patients to be seen much more quickly. 'This is a very exciting model of access. It takes place on Dakota First Nation land, so it's an example of First Nations people taking leadership and ownership of the situation,' said Foulds. Faubert, Foulds, and Hart reported having no relevant financial relationships.
