Shingles vaccine & lower dementia risk: what new study says

The shingles vaccine may do more than protect against the viral infection. Those vaccinated have a 23% lower risk of developing cardiovascular conditions, a recent study from South Korea found.
But perhaps even more significant is a 'natural experiment' from Wales that confirms what health records have long suggested — that getting the shingles vaccine reduces the odds of the onset of dementia, a debilitating neurodegenerative disorder which does not yet have very effective treatments.
Shingles is a viral infection that causes painful rashes. While shingles can occur anywhere on the body, it typically looks like a single stripe of blisters that wraps around the left or right side of the torso, along the path of the nerve from which the virus spreads.
It is caused by the varicella-zoster virus — the virus which also causes chickenpox. After a bout of chickenpox, usually during childhood, the virus remains dormant in one's nerve cells for life. It can reactivate and cause shingles when a person's immune system is weakened.
People who have never had chickenpox may get the infection if they come in direct contact with the fluid released by the shingles rashes, or breathe in the virus particles.
Pain can persist even after rashes go away. In severe cases, the rashes cause vision loss if they are close to the eye, facial paralysis, or inflammation of the brain.
The shingles vaccine, which prevents the reactivation of the varicella-zoster virus, is recommended primarily to individuals over the age of 50 years. It may also be recommended to adults living with a weakened immune system due to conditions such as HIV.
Previous studies shown an association between shingles vaccination and lower risk of dementia. But most of the evidence has come from patient records, which are subject to one major bias: people who are vaccinated tend to take better care of their health.
The unique roll-out strategy of the shingles vaccine in Wales created a 'natural experiment' that effectively mimicked a randomised control trial, the most credible level of evidence in medical research, much better than patient records.
The programme, which began on September 1, 2013, allowed anyone who was between 79- and 80-years-old on that date to get the shingles vaccine over the next year. While 78-year-olds could receive the vaccine the year after, those above the age of 80 would never be eligible. This created a natural cohort of people — of similar age and living in similar conditions — only half of whom were eligible to receive the shingles vaccine.
Upon analysing the health outcomes of this cohort, the Welsh study published in Nature last month found that dementia diagnoses among people who had gotten the shingles vaccine reduced 3.5 percentage points over a seven-year period. This equates to a 20% lower relative risk of getting dementia compared to the unvaccinated population.
Scientists are yet to determine what might be behind this. One theory looks at the shingles-causing virus which has been linked to long-lasting cognitive effects. Preventing its reactivation through vaccination, could directly reduce the risk of dementia. Some researchers have also pointed to changes in the immune system brought about by the shingles virus which could protect against dementia.
There are two approved shingles vaccines — both of which are available in India for a price of Rs 6,000 to Rs 17,000 per dose. Zostavax uses a live, weakened virus, while Shingrix uses a recombinant technology to develop parts of the virus that do not cause the disease.
The Welsh study was based on people who had received the older Zostavax vaccine. However, a study from Oxford University last year showed that the new recombinant vaccine may also offer protection against dementia. The study, based on the health records of 200,000 persons in the US, found a 17% reduction in dementia diagnoses in the six years following immunisation with Shingrix.
Note that there is also some evidence linking lower dementia risk with other common vaccines. Published in Alzheimer's & Dementia, a recent study covering 130 million people found a correlation between a lower risk of dementia and the hepatitis A, typhoid and diphtheria vaccines.
Another 2023 study in the Journal of Alzheimer's Disease found the diphtheria, pneumococcal and shingles vaccine to be associated with a 30%, 27%, and 25% reduction in the risk of dementia.

HIV (human immunodeficiency virus) doesn't just weaken the immune system; it can also show some of its earliest signs in the mouth. The tongue, gums, lips, and inner cheeks are especially vulnerable to infections and sores when the body's defences are low. These oral problems are not only uncomfortable but can also affect eating, speaking, and overall quality of life. In fact, certain changes in the mouth may be warning signals that HIV is progressing. Knowing how HIV impacts oral health makes it easier to spot early signs, seek timely care, and protect overall wellbeing. Common tongue and mouth conditions linked to HIV According to the National Institute of Dental and Craniofacial Research , people living with HIV often experience a range of oral problems, some of which are directly related to the virus, while others are due to opportunistic infections that take advantage of a weakened immune system. Canker sores (aphthous ulcers): These small, painful sores appear as round or oval ulcers with a red border and a yellow or gray center. They often show up on the tongue, inside the cheeks, or along the lips. For people with HIV, canker sores can be more frequent and severe. While mild ulcers usually heal on their own, severe cases may need prescription medications such as corticosteroid mouth rinses. Oral thrush (candidiasis): According to a study published in NIH, this is the most common fungal infection among people with HIV. Thrush looks like creamy white or yellow patches on the tongue and can spread to the throat, tonsils, or roof of the mouth. It may cause pain, burning, or bleeding when scraped. Since HIV weakens the immune system and often causes dry mouth, the natural balance of fungi in the mouth is disrupted, allowing yeast to grow unchecked. Antifungal medications are effective, but thrush can return if the immune system remains suppressed. Oral hairy leukoplakia (OHL): Caused by the Epstein-Barr virus, OHL produces white, fuzzy patches on the sides of the tongue. These patches may be painless but can sometimes affect taste or cause sensitivity. While not dangerous on its own, OHL is usually a sign of advanced immune suppression and is more common in untreated HIV. Antiviral drugs or topical treatments may help, but the underlying viral infection cannot be cured. Oral herpes: The herpes simplex virus can flare up more often in people with HIV. Painful blisters or clusters of sores appear on the tongue, lips, or roof of the mouth. These sores are contagious and may spread through kissing or sharing utensils. There is no cure for herpes, but antiviral medicines can shorten outbreaks and reduce recurrence. Oral warts: Caused by human papillomavirus (HPV), oral warts may appear as small, raised bumps or flat, cauliflower-like growths on the tongue or inside the mouth. They are usually painless but can be persistent. Cryotherapy or surgical removal can help, although warts may return. People aging with HIV or those on long-term antiretroviral therapy are at higher risk. Oral melanin hyperpigmentation: Dark brown or black patches may develop inside the mouth, especially on the tongue and gums. These patches are usually harmless and are often linked to certain HIV medications such as zidovudine (AZT). While treatment is not usually required, medication changes can sometimes reduce pigmentation. Why oral health is important in HIV management Oral problems in HIV are more than surface-level discomforts. Painful sores, fungal infections, or warts can make it difficult to eat, speak, and maintain good nutrition. Persistent lesions are also early warning signs that HIV may be progressing or that current treatment is not fully controlling the virus. Because some oral conditions—like oral thrush and OHL—are linked to specific infections, they can also help healthcare providers gauge the strength of a patient's immune system. Preventing oral health complications with HIV Good oral hygiene is one of the most effective ways to prevent complications. Brushing twice a day, flossing daily, and visiting a dentist at least every six months can reduce infections. People with HIV should also take these extra steps: Stick to HIV treatment: Antiretroviral therapy strengthens the immune system and reduces the risk of oral complications. Prevent dry mouth: Dryness increases the chance of infections. Avoid tobacco, alcohol, and overly salty foods, and drink plenty of water. Specialized mouth rinses or toothpastes can also help. Limit triggers: Smoking, poor diet, and lack of hydration make oral problems worse. Using a humidifier at night may also help maintain moisture in the mouth. Not all mouth sores are serious, but in the context of HIV, they should never be ignored. If you notice persistent ulcers, white patches, warts, or unexplained discoloration, schedule a medical or dental evaluation. Oral changes may indicate that your immune system is weakening or that your treatment needs adjusting. Other signs that HIV may be progressing include fatigue, fever, swollen lymph nodes, night sweats, and unexplained rashes. Addressing oral health early can prevent more serious complications such as bacterial infections or sepsis, which can be life-threatening with a weakened immune system. Oral health plays a vital role in HIV management. From canker sores and thrush to herpes and warts, many oral conditions arise because of a weakened immune system. By practicing good hygiene, keeping up with antiretroviral treatment, and seeking timely care for new symptoms, people with HIV can reduce the risk of oral complications and protect their overall health. Despite medical advances, men are more likely than women to die from common diseases like hypertension, diabetes, and HIV/AIDS, a new global study has revealed. Researchers linked this to higher-risk behaviours—such as smoking, alcohol use, and unsafe sex—and men's lower engagement with healthcare, including fewer check-ups, late diagnoses, and poor adherence to treatment. Published in PLOS Medicine, the research titled Sex-disaggregated data along the gendered health pathways: A review and analysis of global data on hypertension, diabetes, HIV, and AIDS examined data from 204 countries to uncover why men continue to die younger than women worldwide. What makes men more vulnerable to diseases like hypertension and diabetes? The study shows men smoke more than women in 86 per cent of countries, putting them at greater risk for hypertension, diabetes, and cardiovascular complications. Men also drink more alcohol, engage in unsafe sex more often, and tend to ignore early warning signs of illness. Women, on the other hand, had higher obesity rates in about two-thirds of countries, but were still more likely than men to get diagnosed and treated for chronic conditions. Which diseases are driving higher male mortality? Hypertension High blood pressure is a silent killer, often without symptoms until it's too late. Men not only develop hypertension earlier, but in over half of countries, they also had higher death rates from it compared to women. Diabetes Men in 30 per cent of countries had higher diabetes prevalence, and in nearly half the world's nations, they faced higher diabetes-related deaths. Late diagnosis and poor treatment adherence were key factors. HIV and AIDS Men were more likely to contract HIV in 56 per cent of countries and had higher AIDS-related deaths in 64 per cent of countries. Reluctance to get tested, fear of stigma, and lower engagement in treatment programs worsened outcomes. Why men avoid healthcare The study highlighted that men are less likely to seek medical advice, attend routine screenings, or continue treatment compared to women. Cultural factors, work commitments, and even traditional ideas of masculinity discourage men from prioritising health. Role of biology and gender roles The study said biology plays a role as women's higher estrogen levels protect them against heart disease, while testosterone in men may increase cardiovascular risk. But gender norms matter too. Societal expectations often push men towards risky behaviours while discouraging preventive care. How can men's health outcomes be improved? The researchers recommended several key steps: Encouraging regular screenings – early diagnosis can save lives. Promoting healthier lifestyles – cutting down smoking, alcohol, and poor diets. Making healthcare accessible – with flexible hours and male-friendly clinics. Designing gender-sensitive health policies – recognising men's unique risks and barriers. The researchers in the current global analysis stressed that being male itself has become a health risk factor, not biologically, but behaviourally and socially and so, tackling the gender health gap requires not just better medical care but also cultural shifts that encourage men to look after their health before it's too late.
