It's Complicated: Surgery and Lung Cancer Care

Hello. It's Mark Kris from Memorial Sloan Kettering, continuing my little series on the initial treatment of patients with lung cancers. In the earlier discussions, we talked about people with metastatic disease. I'd like to switch today to talking about people who are candidates for surgery.
The two operant terms here are operable and resectable. Operable means that the patient is medically fit to have an operation and to undergo the extent of surgery needed for a complete resection. Resectable means that, in the opinion of the thoracic surgeon who will be performing the surgery, R0 resection is likely to be achieved.
Again, these things are not perfect. Experience guides many of our decisions here, and the operative decision is made by many different folks. The surgery decision really needs to be made by a surgeon. The truth is that the question of resectability comes from the surgeon at your institution, in your multidisciplinary tumor board.
What are the stages we're talking about? Looking at the trials of perioperative therapies, the stages looked at are IB (tumors greater than 4 cm, regardless of nodal status) up to IIIB (larger primary tumors with N2 nodes). N3 nodes are a different discussion, which we'll hopefully have later on.
I think I personally am unbelievably struck by the amount of data and the absolute consistency of data about the use of neoadjuvant therapy in patients who do not have a driver. That's the first group I'd like to address.
It is critical to get sufficient tissue to make sure that there are no drivers present. If you have a driver, particularly EGFR and ALK , it sends you in a different direction for decision making. Also, if you're following the package insert, for example, you need to show that the patients are EGFR and ALK negative. To do that, you really should do next-generation sequencing in 2025. It has to be done on tissue.
The data from using blood in these situations show that less than [about] one in five of the mutations are detected in blood in these earlier-stage patients. You must have tissue. Obviously, if you have a positive blood test, that's great. You can use it. If it's negative, though, you must have tissue. You have to get the tissue, you have to get the answer, and you have to get it quickly. It's not an easy situation, and we can talk about that.
I think the data are very, very clear in the nine trials now. There are nine randomized trials that have been reported, and every single one shows an improvement in disease-free survival by giving neoadjuvant therapy with chemotherapy and a checkpoint inhibitor.
I'm struck by the consistency of the trials, by the huge amount of data, and I think that makes this the standard of care today in patients that are both operable and resectable.
What about the discussions that go on at our tumor boards about the need for immediate surgery, particularly for sick patients with IB and II disease?
Well, I have to say that there are virtually no data for the patient sitting in your office, saying that immediate surgery followed by adjuvant therapy is as good, or better than, the neoadjuvant approach. Frankly, there probably is never going to be a trial that answers that.
When you look at the trials of where people have proposed to do adjuvant therapy, up to one-third of the patients, or more, never get the adjuvant therapy. It's really an apples-and-oranges comparison here. The data just do not exist and I think are unlikely to exist just by the nature of the problems.
For people who have neoadjuvant therapy, there are two groups now as well. There are patients who have a major response, particularly a pathologic complete response or no pathologic response, and there are patients who don't.
I think for those who don't, the way forward is clear. You need an alternative therapy. I personally would advise not to give a component of the neoadjuvant program that was truly unsuccessful, particularly for folks who have a large amount of remaining disease in the resection specimen.
For people with no pathologic response or a pathologic complete response, the question there comes whether to give perioperative therapy, generally the checkpoint inhibitor, after or not. The data here are, again, not going to give you the answer.
The trial of nivolumab — not followed by a year of nivolumab after surgery — showed very, very good results and really comparable to those results. The FDA has clearly pointed out that the data supporting the use of the additional year of therapy with the checkpoint inhibitor are not proven. There is toxicity. In adjuvant situations, the data are not particularly impressive.
I do want to point out the need for consideration of postoperative therapy with radiation if you have N2 disease. When you look at the recurrences noted in the neoadjuvant and adjuvant trials, now with better systemic therapy, the recurrences are in the chest.
We must think about improving control in the chest. Frankly, the only modality we have today to do that is radiation, and it's most proven for N2 disease. I do think it's very, very important for people with N2 disease to get a radiation oncologist in the treatment planning group to see if there is a role for radiation in those patients.
To summarize, I think for patients who don't have a driver and are operable and resectable, neoadjuvant is the way to go. What you do afterward is difficult for people with a major pathologic response. You can make a good case for not giving any additional therapy. There are FDA-approved regimens to give additional therapy of the same drugs.
For people who clearly progress, you need to think about what other alternatives there are, both local and systemic.

Supreme Court says states may bar women on Medicaid from using Planned Parenthood clinics

The Supreme Court ruled Thursday that states may exclude Planned Parenthood clinics from providing medical screenings and other healthcare for women on Medicaid. The court's conservative majority cast aside the longstanding rule that said Medicaid patients may obtain medical care from any qualified provider. In a 6-3 vote, the justices ruled the Medicaid Act does not give patients an "individual right" to the provider of their choice. The dispute turned on abortion, even though federal funds cannot be used to perform the procedure. Medicaid is funded by the federal government and the states. For decades, conservative states have sought to "defund" Planned Parenthood and argued they did not want to subsidize a leading provider of abortions. But until recently, the federal government and most courts had held that Medicaid patients may go to any qualified provider for healthcare. The legal battle hinged on whether the Medicaid Act gave a patients a right that could be protected in court. The answer was no, said Justice Neil M. Gorsuch speaking for the majority. The court's three liberals, all women, dissented. Justice Ketanji Brown Jackson said the decision "will deprive Medicaid recipients in South Carolina of their only meaningful way of enforcing a right that Congress has expressly granted to them. And, more concretely, it will strip those South Carolinians — and countless other Medicaid recipients around the country — of a deeply personal freedom: the ability to decide who treats us at our most vulnerable." Justices Sonia Sotomayor and Elena Kagan agreed. In theory, a Medicaid patient could file a complaint with the Trump administration and tell the Department of Health and Human Services that the state is failing to comply fully with the Medicaid Act. Planned Parenthood clinics provide cancer screenings, birth control, medical screenings, pregnancy testing, contraception and other healthcare services. Congress pays most of the state's costs for Medicaid, and the law says "any individual eligible for medical assistance" may receive care from any provider who is "qualified to perform the service." Last year, the 4th Circuit Court of Appeals rejected South Carolina's contention that it could exclude Planned Parenthood from the Medicaid program. "We reaffirm that a Medicaid beneficiary may ... vindicate her right under the Medicaid Act to freely choose among qualified healthcare providers, of which Planned Parenthood is one," wrote Judge J. Harvie Wilkinson, a prominent conservative and a Reagan appointee. But the court agreed to hear the state's appeal in Medina vs. Planned Parenthood. Lupe Rodríguez, executive director of National Latina Institute for Reproductive Justice, called Thursday's decision "an attack on our healthcare and our freedom to make our own decisions about our bodies and lives. By allowing states to block Medicaid patients from getting care at Planned Parenthood health centers, the Court has chosen politics over people and cruelty over compassion." Sen. Dick Durbin (D-Ill.) said it was "yet another shameful ruling that inserts the government directly between a patient and their doctor — just like Dobbs three years ago and Skrmetti last week. Intimate, personal decisions about health care shouldn't require sign off from extremist politicians." He was referring to the 2022 decision that overturned Roe vs. Wade and last week's ruling upholding state laws that ban hormone treatment for transgender teens. Carol Tobias, president of the National Right to Life Committee, praised the decision as a landmark. "We are grateful the Supreme Court has recognized the right of states to direct taxpayer dollars toward life-affirming healthcare providers,' she said. 'No one should be forced to subsidize the abortion industry with their tax dollars.' After the Supreme Court overturned the constitutional right to abortion, South Carolina made most abortions a crime. But the state continued its legal fight to prevent Medicaid patients from receiving care at Planned Parenthood's clinics in Charleston and Columbia. Gov. Henry McMaster, who issued the ban on Planned Parenthood in 2018, said he did so to protect "his state's sovereign interests." Critics of the move said the state has a severe shortage of doctors and medical personnel who treat low-income patients on Medicaid. Get the L.A. Times Politics newsletter. Anson Mount's Captain Pike continues to be one of the most compelling leads in modern Trek, blending warmth, authority, and vulnerability in a way that sets him apart from his predecessors. This season adds new layers to Pike's journey, particularly as he grapples with the aftermath of the Gorn conflict and the growing weight of command. Ethan Peck's Spock leaves a lasting impression, paying tribute to Leonard Nimoy's legacy while confidently carving out his own take on the iconic role. The rest of the bridge crew—Rebecca Romijn's Number One, Christina Chong's La'an, Jess Bush's Chapel, Celia Rose Gooding's Uhura, and Babs Olusanmokun's M'Benga—each get moments to shine throughout the season. But it's Melissa Navia's Ortegas who finally steps into the spotlight. The effects team has outdone themselves this season, crafting alien worlds and creatures that feel immersive and tangible. The blend of practical sets and digital backgrounds is, for the most part, seamless, and the attention to detail—from the uniforms to the bridge consoles—grounds everything in a believable reality. Thematically, the season is as complex and compelling as ever, with a healthy dose of classic Trek charm woven throughout the production. While the show continues to embrace familiar tropes—exploring strange new worlds, seeking out new life and new civilizations—it also isn't afraid to engage with timely, contemporary issues. Ethical dilemmas, questions of identity, and the burdens of leadership are all seamlessly integrated into the narrative. Its willingness to balance the silly with the serious remains one of Strange New Worlds' greatest strengths. Ultimately, from what I've seen of Star Trek: Strange New Worlds Season 3, it's shaping up to be another standout entry in the franchise. This is a season that feels both nostalgic and forward-thinking—a celebration of what makes Star Trek special, while boldly pushing its boundaries. With sharp writing, stellar performances, and top-tier production values, Strange New Worlds continues to prove why it's one of the best modern entries in the Trek universe. With its blend of action, humour, and heart, Star Trek: Strange New Worlds Season 3 isn't just the best Trek in decades—it's a powerful reminder of why we keep looking to the stars. The Enterprise is back, and it's never been in better hands.

NASA offers dazzling new sights (and sounds) of the Andromeda galaxy

Even a century after Edward Hubble confirmed its existence, astronomers learn new details about the Andromeda galaxy that help us better understand our cosmic neighborhood and the wider universe. Earlier this week, NASA released its latest detailed images of the Milky Way's spiral sibling, as well an ethereal sonification of its energy wavelengths. Attaining an outside view of the Milky Way galaxy is a bit like trying to examine the entire planet from your backyard—that is to say, it's impossible from humanity's current vantage point. The next best option for astronomers is gazing at similar nearby spiral galaxies, the closest of which is Messier 31. Also known as Andromeda, the Milky Way's most immediate neighbor is about 2.5 million light-years away, and provides an excellent option for studying how spiral galaxies form and evolve over time. It's also where a team led by astronomer Vera Rubin first detected the anomalous material now known as 'dark matter' in the 1960s. The newest glimpses at Andromeda are based on composite data collected by an international array of the world's most powerful telescopes, including the Chandra X-ray Observatory, the ESA's XMM-Newton, and even optical information from a pair of astrophotographers. The various kinds of light span the visible, infrared, radio, and ultraviolet wavelengths. When layered, they depict a vibrant and active galaxy reminiscent of our own—and the information is already helping experts expand on Andromeda's ongoing life story. 'For example, Chandra's X-rays reveal the high-energy radiation around the supermassive black hole at the center of M31 as well as many other smaller compact and dense objects strewn across the galaxy,' NASA explained in its announcement. Astronomers aren't limited to studying visual representations of Andromeda's energy; they can also assess them through sound. In addition to the images, NASA researchers compiled the galaxy datasets into a sonification by separating out each wavelength, rotating them, and stacking them on top of one another in order of their frequency. From top to bottom, that means X-rays, ultraviolet, optical, infrared, and finally radio waves. These are next assigned a range of corresponding notes, with brightness designating volume while spectrum location determines pitch. The result is a dreamlink chorus of tones as the space telescopes traverse Andromeda's 152,000 light-year diameter. There's still an untold wealth of information to learn from the Milky Way's neighbor, possibly even the means to finally understand the dark matter first detected by Rubin. That's at least what NASA hopes to achieve with the upcoming Nancy Grace Roman Space Telescope currently scheduled to go into operation in 2027.
