
Cancer Patients Are Overmedicated To Enrich Health Systems

Are patients being prescribed more medications not for their health, but to generate profit? A new economic study published in The Journal of Health Economics reveals that cancer patients treated by physicians participating in the 340B Program receive more medications—including those outside clinical guidelines—without improvements in survival.
The federal program, named after Section 340B of the Public Health Service Act, was created by Congress in 1992 to support hospitals and clinics serving low-income communities. It allows eligible entities to purchase outpatient drugs at steep discounts and receive full reimbursement, retaining the difference. The Affordable Care Act expanded eligibility criteria, accelerating the program's growth.
Since 2012, the number of 340B-eligible entities has tripled to more than 60,000. Between 2013 and 2023, discounted drug purchases under the program rose from $8 billion to $66 billion, largely driven by eligible hospitals and their affiliated outpatient facilities. Today, nearly 60% of U.S. pharmacies serve as contract pharmacies, dispensing drugs for 340B-eligible entities. This rapid growth reflects the lucrative 'buy-low, sell-high' opportunities created by the program.
Dr. Danea Horn at the University of California, San Francisco, studied the prescribing behavior of physicians treating breast cancer patients before and after their practices began participating in the 340B Program. Her findings are striking: the likelihood of prescribing outpatient drugs per patient increased by over 25%, and the intensity of drug treatment rose by 40%, including medications not aligned with clinical recommendations.
Despite increased medication use, the study found no evidence of improved patient survival—consistent with research indicating that 340B participation does not enhance hospitals' inpatient care quality. As Dr. Horn concluded, the 340B Program is fundamentally a 'cash transfer program' that distorts healthcare markets by inflating drug profit margins, an incentive to which providers actively respond.
Recent research found that 340B-eligible hospitals secured substantial profit margins on outpatient drugs and that the program discouraged biosimilar adoption, favoring more expensive brand-name drugs with higher 340B profit margins. Additionally, hospitals strategically manage their patient mix to game the 340B eligibility criteria and expand into wealthier neighborhoods to capture lucrative outpatient drug margins.
In effect, the 340B Program gives eligible entities a substantial financial advantage, creating a deep moat that fends off competitors. Independent physician practices and for-profit hospitals, regardless of their patient mix, are ineligible for the program but must compete with eligible nonprofit hospitals.
This uneven playing field has driven vertical integration, prompting hospitals to acquire physician practices and expand clinics to increase drug revenue. These actions raise commercial prices and worsen the financial burden on patients, directly contradicting the 340B Program's original intent.
It's tempting to blame hospital greed for their relentless expansion of 340B-driven operations. However, hospitals—like all other players in healthcare—respond to the incentive structure around them. Why should they be expected to leave easy 340B money on the table? Bad rules ruin the game, often in ways unforeseeable to well-intentioned rulemakers.
Reforming the 340B Program is politically challenging, as it has evolved into an indispensable cash cow for eligible entities, creating entrenched financial and political interests committed to sustaining and expanding it. However, as the public becomes increasingly aware of the nature and implications of this rapidly growing program, a tipping point may emerge for reforms that prioritize and directly empower patients rather than the healthcare entities serving them.

Proposed federal spending cuts to health care in Republicans' "One Big Beautiful Bill Act" may increase some families' medical debts by as much as $22,800, according to a new report from Third Way, a Washington, D.C.-based think tank. The Republican budget bill proposes $1.1 trillion in cuts to health care that target both Medicaid and Affordable Care Act coverage. Without coverage, families may see their medical debts increase by as much as $22,800, according to Third Way's report. About 87% of households that previously had no medical debt would accumulate an average of $22,800 in balances. Meanwhile, 13% of households may accumulate an additional average of $8,790 in medical debt on top of $13,490 in existing balances. "That's going to put people's dreams back, if they're hoping to go to college or hoping to have a solid retirement or hoping to buy another house," said David Kendall, senior fellow for health and fiscal policy at Third Way. "Medical debt stands in the way of the American dream, and we shouldn't make it worse." The White House said proposed federal spending cuts are aimed at eliminating "waste, fraud and abuse" in government programs including Medicaid. The Trump administration has said the "big beautiful" bill is a potential "economic windfall for working and middle-class Americans" through tax cuts, higher wages and higher take-home pay. In a Monday letter that cites the Third Way report, Sen. Jeff Merkley, D-Ore., ranking member of the Senate Budget Committee, and Democratic Sens. Cory Booker of New Jersey, Chuck Schumer of New York and Ron Wyden of Oregon, urged Republican leaders to reconsider the proposed health-care cuts. Addressing medical debt is a "national priority" with "bipartisan support," the senators wrote in a letter to Senate Majority Leader John Thune, R-S.D., and House Speaker Mike Johnson, R-La. Currently, 16 states have moved to either cancel medical debt or eliminate medical debts from credit reports, they wrote. "Medical debt is a complex problem, but having health insurance coverage makes a measurable difference," the senators wrote. They pointed to a 2013 study in The New England Journal of Medicine that found Medicaid coverage reduces medical debt rates by 13.28 percentage points. The study, published ahead of state Medicaid expansion under the ACA, looked at the effects of Oregon's 2008 Medicaid expansion. Americans with unpaid medical balances may face "dire" consequences, which may include delaying or going without needed care, cutting back on food or other necessities or taking on additional debt, the lawmakers wrote. In addition to personal setbacks, medical debt also affects consumer spending, which may prevent economic growth, they said. "If the Republican reconciliation bill passes these drastic health care cuts into law, working class families across America risk going further into medical debt," the senators wrote. "It is not too late to stop these cuts," they wrote.

Postoperative Pain Management: 5 Things to Know
There has been significant evolution in postoperative pain management in recent years, with an increasing recognition of the variability in patient responses. This variability may limit the effectiveness of standardized protocols and contribute to both undertreatment and overtreatment. Although opioids remain a cornerstone for moderate-to-severe postoperative pain, concerns about adverse effects, prolonged use, and opioid use disorder (OUD) have intensified the push toward individualized, multimodal approaches. Biologic and psychosocial variables such as central sensitization, catastrophizing behavior, and lifestyle factors also can influence outcomes and increase the risk for chronic postsurgical pain. Advances in pharmacology and drug delivery technologies, including long-acting local anesthetics and novel nonopioid agents, are helping address these challenges. Nonpharmacologic modalities such as cognitive-behavioral therapy (CBT) and patient education are also gaining traction as adjuncts in comprehensive care. As clinicians seek to improve outcomes and reduce harm, the focus is shifting from reactionary prescribing to proactive, patient-tailored strategies. Here are five things to know about postoperative pain management: 1. Standardized pain management does not work for everyone. There is growing recognition that pain sensitivity and response to therapy exist along a spectrum, rather than fitting binary classifications of 'tolerant' or 'naive.' Recent guidelines increasingly advocate maintaining the home dose perioperatively, with potential dose adjustments for major surgeries. To ensure adequate pain relief, clinicians often add multimodal strategies such as regional anesthesia, nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen, and adjuncts such as ketamine or dexmedetomidine. Important to note: Discharge plans should include communication with the patient's buprenorphine prescriber , a taper plan for any additional opioids, and strategies to minimize relapse risk. A multidisciplinary approach is essential. Surgeons, anesthesiologists, and addiction medicine providers must collaborate early in the surgical planning process to tailor pain management to the patient's needs. Recent publications emphasize the importance of this coordinated care model, which aligns with broader efforts to reduce opioid-related harm and support long-term recovery in individuals with OUD. 3. Virtual reality, CBT, and other nonpharmacologic modalities can meaningfully reduce postoperative pain and opioid use. Evidence increasingly supports the integration of nonpharmacologic modalities into perioperative pain care. Virtual reality (VR), CBT, and mindfulness-based interventions have been shown to reduce pain scores, opioid consumption, and anxiety in pediatric and adult surgical populations. VR has been especially promising in managing procedural pain, distracting patients from acute discomfort, and enhancing engagement in rehabilitation. CBT can modulate central pain processing by addressing maladaptive thought patterns such as catastrophizing. In response to growing concerns about opioid-related adverse effects and the risk for long-term dependence, clinicians are increasingly embracing multimodal analgesia approaches that combine agents with complementary mechanisms of action to optimize pain control while minimizing harm. Nonopioid agents are at the core of this shift. Medications such as intravenous acetaminophen, NSAIDs, gabapentinoids, ketamine, and dexmedetomidine have become foundational components in contemporary postoperative pain protocols. These agents target different pain pathways — peripheral inflammation, central sensitization, N-methyl-D-aspartate receptor modulation, and alpha-2 adrenergic receptor activation — providing synergistic analgesia without the sedation, respiratory depression, or tolerance associated with opioids. For example, low-dose ketamine infusions have demonstrated efficacy in reducing acute postoperative pain and opioid requirements, particularly in opioid-tolerant patients or those with chronic pain conditions. Emerging drug delivery systems are also transforming postoperative care. Evolving technology focused on transdermal patches, subcutaneous implants, and iontophoretic devices enables continuous or patient-controlled analgesia with improved precision and fewer adverse effects. Recently launched or in-development devices aim to integrate real-time monitoring and adaptive dosing algorithms to personalize analgesia and prevent overmedication. Together, these pharmacologic and technological advancements are ushering in a new era of postoperative pain management that prioritizes individualized care, safety, and functional recovery. The overarching goal is not merely to control pain but to do so in a way that accelerates healing, preserves patient autonomy, and mitigates the risks associated with opioid use. As new agents and delivery platforms continue to emerge, clinicians must remain informed and adaptable, integrating these tools thoughtfully into evidence-based practice. 5. Preoperative anxiety and preexisting pain are predictive of worse postoperative pain outcomes. Numerous studies confirm that patients with chronic pain, preoperative anxiety, or depressive symptoms are at higher risk for severe postoperative pain and prolonged opioid use. Central sensitization phenomena, such as elevated temporal summation of pain and reduced pressure thresholds, are also predictive. Psychological factors such as catastrophizing can amplify pain perception and interfere with recovery. Preoperative screening and interventions, including patient education and behavioral therapy, have been shown to mitigate these effects and improve pain control. Multidisciplinary teams are increasingly encouraged to assess and address perioperative risks early in the surgical planning process as part of a broader shift toward personalized, patient-centered care. This proactive approach brings together surgeons, anesthesiologists, nurses, pharmacists, physical therapists, and pain management specialists to collaboratively evaluate each patient's unique risk factors — including medical history, medication use, psychological status, and functional baseline — before surgery occurs. Early engagement allows the team to identify potential complications, such as adverse drug reactions, postoperative delirium, or prolonged opioid dependence, and implement tailored strategies to mitigate them.
