
Are IV Proton Pump Inhibitors Overused in Upper GI Bleeding?
Nearly 70% of patients with acute upper gastrointestinal (GI) bleeding and low-risk or no peptic ulcer disease received intravenous (IV) proton pump inhibitors (PPIs) inappropriately, leading to longer hospital stays.
METHODOLOGY:
Current guidelines recommend transitioning from IV to oral PPIs after esophagogastroduodenoscopy (EGD) if no high-risk stigmata are found and stopping PPIs in patients with upper GI bleeding from causes other than peptic ulcer disease.
Researchers conducted a retrospective cohort study of patients hospitalized for acute upper GI bleeding between January 2014 and December 2019.
All patients were started on continuous or intermittent IV PPI and underwent EGD that showed a gastric or duodenal ulcer with low-risk signs or lesions unrelated to ulcer disease.
Inappropriate use was defined as any continuous IV PPI infusion lasting more than 4 hours after EGD or any intermittent dose administered after the procedure.

Among patients with lesions unrelated to peptic ulcer disease or with no lesions at all, those receiving twice daily IV PPI dosing averaged six extra doses, and those on continuous infusion received an average of 34 additional hours of therapy.
At baseline, hemoglobin levels were slightly lower in the inappropriate therapy group than in the appropriate group (7.9 vs 8.7 g/dL).
Patients receiving appropriate therapy had a shorter hospital stay than those receiving inappropriate therapy (4 vs 7 days; P = .001).

'We believe that educating practitioners on the appropriate use of PPI therapy in the context of the endoscopic lesion that was believed to have caused bleeding is a critically important next step,' the authors wrote.

Louisiana scraps coastal erosion project designed to rebuild the state's wetlands
New Orleans — Louisiana's coast is under threat as a quarter of the state's wetlands, an area about the size of Delaware, have disappeared in the last 100 years, according to the U.S. Geological Survey. The wetlands are a critical buffer that shield Louisiana communities from monster hurricanes. While massive flood walls and human-made marshes are helpful, coastal scientist Alisha Renfro and campaign director Simone Maloz with the nonprofit group Restore the Mississippi River Delta say say those projects alone are not enough to rebuild and counteract future projected land loss. "You can reduce storm surge up to a foot with just a mile of coastal wetlands," Renfro told CBS News. "A tough lesson that we learned after Katrina: we have to think long term. We also have to think bigger," Maloz added. It's been 20 years since Hurricane Katrina exposed the extent of the state's coastal erosion. It was the largest residential disaster in U.S. history, killing more than 1,300 people and displacing one million others across the Gulf Coast, according to the Louisiana independent research nonprofit The Data Center. Since Katrina, satellite images from the NASA Earth Observatory show that the Louisiana coastline has lost even more marsh land — an estimated 200,000 acres, according to the USGS. Land loss is caused by a number of factors, including sea level rise, shipping channels, levees, natural subsidence, invasive species and severe storms, according to Restore the Mississippi River Delta. Without the protection of wetlands, Maloz and Renfro say the future would be unsustainable for coastal Louisiana communities. "I think that's a future we don't want to think about," Maloz said of the impact on the region if the wetlands were to disappear. "It is a loss of culture," Renfro said. "It is a loss of place and it is a loss of home for a lot of people." Last month, Louisiana officials canceled a $3 billion project — known as the Mid-Barataria Sediment Diversion — that sought to rebuild the marshes. Renfro and Maloz spent years planning the diversion project with other state and federal leaders and scientists. The project was unique, because instead of a human-made marsh built by dredged sediment, it would have diverted sediment flowing down the Mississippi River to naturally rebuild wetlands, the way the Mississippi River Delta was first formed centuries ago. Renfro and Maloz saw it as the best solution to save Louisiana. They argue that other projects wouldn't be nearly as effective, nor would they have rebuilt as much land. "If you don't change the forces that took that away from you in the first place... if you don't have a systemic change, then you're going to lose this too," Maloz said. "And so that's what Mid-Barataria represented. It represented that systemic change that returned back to how we were created that gave us that long-term future. If you want to talk about dollars and cents, you're talking about investing money over 50 years, rather than investing X amount of money over 10 years or 20 years. But really it's the time and scale." The project would have diverted Mississippi sediment to rebuild 17,000 acres of land over the next 30 years, enough to potentially absorb up to 10 feet of storm surge, according to Restore the Mississippi River Delta. "We are very short on money," Maloz added. "We're short on natural resources. And we are short on time." Regardless, Louisiana's Coastal Protection and Restoration Authority scrapped the project this summer, at the direction of Republican Louisiana Gov. Jeff Landry, even though about $600 million of the $3 billion had already been spent. The project faced stark opposition from many community members in Plaquemines Parish, a small parish south of New Orleans. Mitch Jurisich, a third-generation oysterman, local businessman, and Plaquemines Parish councilman, filed a lawsuit to stop the project. He told CBS News he did so "because I know what it would have done to our fishing industry, our commercial fishing industry here. And when you look around at this harbor, full of commercial boats, full of life, it would have just totally wiped us out." Documents for the project published by the U.S. Army Corps of Engineers show it would have had some negative impacts on fisheries near the project site at first. But Renfro says that, down the road, the fisheries would have come back stronger. Government records also showed the project would have increased flooding and storm surge to some homes south of the project site. The project would have compensated those impacted, but Jurisich believes there are better solutions that wouldn't sacrifice his community for the rest of the state. "I tell them all, 'It's not my fault that y'all wasted money,'" Jurisch said of state officials. "'It's your fault, because you pushed this down our throat knowing we didn't want it.'" Jurisich says smaller, human-made marshes could be cheaper and wouldn't sacrifice his fishing community for the rest of the state. But Renfro argues that is only a temporary fix and that the fishing industry will ultimately pay a steep price for the continued loss of the wetlands. "They (the fishing industry) are dependent on those coastal wetlands that are disappearing," Renfro said. "And once those wetlands are gone, the fisheries collapse. Doing nothing isn't an option."

Study sheds light on how reams of fake scientific papers are getting into literature
Fraudulent scientific research is now being produced and published on a large scale, with some unethical researchers colluding with unethical editors to attain the prestige that comes with publication, according to a new study in the Proceedings of the National Academy of Sciences. Large groups of editors and authors appear to have cooperated in what it called "the tide of fraudulent science." Among their efforts, the researchers who conducted the study obtained about 47,000 retracted articles. They collected reports of the same image used in multiple publications. They compiled 33,000 papers of suspicious origin. Making use of the fact that editors' names are public at some science publishers, they looked at whether some editors handled disproportionate numbers of problematic scientific papers, ones that were later retracted or noted negatively by other scientists. At the journal PLOS One, they were able to link 30.2% of the retracted articles to 45 editors. Of these editors, 25 had their own papers retracted. The 45 editors represented 0.25% of the total number of editors at the journal. PLOS One did not respond to a request for comment. Researchers also found clusters of articles accepted in less than a month, often involving the same editors and authors. 'They found cases where people submitted papers and those papers got accepted extremely fast, and when you looked at the editors, they were just sending them to each other,' said Luís Amaral, a systems biologist at Northwestern University and senior author of the study. 'There are people who believe that there is widespread fraud,' said Reese Richardson, a postdoctoral researcher in the Amaral Lab at Northwestern and lead author of the study. 'What this paper does is give a method and a starting point and the data to show that this is actually happening, and that the current mechanisms are not equipped to stop it.' The study's findings confirm the suspicions of many researchers, including Elisabeth Bik, a microbiologist and independent scientific integrity consultant who has spent years identifying fraudulent research. In one case, she found 125 papers that reused parts of the same image. 'It was the same photo, but different crops of the same image," she said. "They didn't generate the photos themselves. They got the photos from a third party — a broker, a paper mill.' Researchers have been using the term "paper mill" to describe organizations that sell mass-produced low quality and fabricated research articles. Many of these fraudulent papers, Bik added, seem to come from doctors or researchers in countries where promotions are tied to publication metrics. They see it as an investment, she explained, where a couple of thousand dollars gets them a paper, and a fast track up the promotional ladder. This institutional pressure is especially common in India and China, where promotions, medical licensing or graduation are linked by policy to publication counts, several experts said. In a survey of medical residents in China three years ago, 47% admitted to buying and selling papers, letting other people write papers, or writing papers for others. When the study authors analyzed an archive of articles from a business offering services to "research professionals who are desperate" for publication, they found 26% of the authors were from India. Although the "publish or perish" culture is also common in the U.S., it manifests more in expectations around prestige, funding and tenure, rather than fixed quotas. India and China are the world's most populous nations and both are scientific powerhouses. The paper notes that science fraud can happen anywhere. The accumulation of fake literature has turned some scientific fields — RNA biology, for example — into what Richardson called an academic "minefield," making it difficult for researchers to identify which studies are reliable. Some fraudulent studies have even made it into meta-analyses that shape the way doctors treat patients. They found evidence that this field of research has been targeted by bad actors. Experts say growing awareness of fraud could feed broader skepticism of science, especially if institutional action doesn't keep up. 'The more polluted the record becomes, the harder it is to clean up, and the harder it is to rebuild trust inside and outside the scientific community,' said Stephanie Kinnan, a longtime member of the Committee on Publication Ethics (COPE). The scientific community has tools to fight back. It fines and excludes researchers and universities. Journals retract articles. Aggregators can sideline problematic journals. But the authors of the paper found the amount of "research" from suspected paper mills has been doubling roughly every 1½ years. The actions are not keeping up. For Amaral, and many other scientists, the implications are deeply personal. 'I dreamed of being a scientist since I was 12,' he said. 'Seeing the thing that I've dreamt of being a part of, that I cherish, being potentially destroyed is really enraging.' All research is built on previous research, Amaral explained. That collapses without trust. "This is the great fear — that the entire scientific enterprise that gave us vaccines, that gave us medicine for cancer, that gave us, X-ray machines, computer scanning devices — would just disappear,' he said.
