Final report into fatal outback plane crash blames pressure issue for lack of oxygen, finds operator AGAIR was aware
The Gulfstream 695A aircraft took off from Toowoomba, west of Brisbane, on November 4, 2023 and was on its way to photograph fire zones north of Mount Isa.
But the plane crashed about 55 kilometres south-east of Cloncurry in open bushland and burned up after impact.
On board were a pilot and two camera operators, including 22-year-old William Jennings from the United States, all of whom died in the crash.
The flight was operated by operated by Victorian company AGAIR.
In its final report, released this morning, the Australian Transport Safety Bureau (ATSB) found the pilot was experiencing hypoxia when the crash occurred.
ATSB Chief Commissioner Angus Mitchell said the "aircraft's pressurisation system was not reliably maintaining the required cabin altitude", which had been in issue for "many months".
"This led some company pilots to employ a variety of actions in the aircraft to manage the potential and deadly effects of hypoxia, including at times briefly descending to lower altitudes, and improperly using emergency oxygen systems," he said.
On the day of the accident, the pilot had descended from 28,000 feet to 15,000 feet for about six minutes before climbing back up to 28,000 ft, he said.
"Later, while the aircraft was ... nearing Cloncurry at 28,000 ft, both power levers were probably reduced, possibly with the intention of undertaking a similar descent," the ATSB report found.
"This caused the aircraft's speed to decay, before it ultimately entered a steep, descending, anticlockwise turn.
Mr Mitchell said it was almost certainly due to pilot control inputs made in an unsuccessful attempt to regain control.
"The ATSB found the onset of hypoxia during the flight significantly degraded the pilot's ability to safely operate the aircraft, and it is possible that at stages the pilot also experienced some loss of consciousness," he said.
The ATSB said the pressurisation defect in the plane was known to senior AGAIR management, who attempted to have it rectified.
"However, they did not formally record the defect, communicate it to the safety manager, undertake a formal risk assessment of it, or provide explicit procedures to pilots for managing it," Mr Mitchell said.
An online fundraiser to bring Mr Jennings home received over $92,625 US in donations — about $142,000 AUD.
In the fundraiser, his family said William was "a bright light extinguished too soon".
"William was a 22-year-old promising mechanical engineer who recently graduated from Northeastern University," his family said.
"He had an exciting life ahead of him, but tragically lost his life in a plane accident while surveying fires in Australia.
"William was known to bring light to any room he walked into, and his sense of humour was infectious.
"He was an avid hiker and lover of nature, always seeking new adventures in life."
The other two people on board the aircraft are yet to be publicly identified.

Great Australian Cattle Drive in Qld raises money for food charities
As the last of the day's light retreats across the paddock, boss drover Billy Prow is checking the cattle one last time before joining his comrades around the campfire. Like the weary cattlemen of days gone by, his time stewarding this mob across the stock routes of outback Queensland is coming to an end. But the scene on the outskirts of Roma, 470 kilometres north-west of Brisbane, is not from a history book — it's a modern depiction of the generosity of the bush. An epic 770-kilometre trek, the Great Australian Charity Cattle Drive has something special in common with the droves of old — it is about making sure people get fed. Anthony 'Bim' Struss's connection to the cattle industry spans generations. He previously served as the president of farm lobby AgForce's cattle board, and runs his own property, Havelock, with wife Susan. In 2021, he joined the board of BeefBank, a Brisbane-based not-for-profit that distributes meat to food charities struggling to access protein. "The BeefBank chairman Andrew Rogers mentioned to me one day an event where we could put a few cattle together, drive them down the road and sell them and we could take the profits from that," Mr Struss said. "I told him he was crazy at that point and I still think that Andrew is crazy. "It has been a lot bigger event than that." It took three years to pull the event together, but when the drovers finally set out from Longreach in May, Queensland Governor Jeannette Young and her husband Graeme Nimmo were there to send them off. "What a great couple they are, just full of laughter, full of fun," Mr Struss said. "I'll never forget Her Excellency said that she was always taught never to swing on a gate but [she said], 'Bim, I'm going to swing on the gate today and I'm the boss so I'm going to do it.' "Everyone saw her swing, it was on national TV. "It was a tremendous start for us." What followed was three months of long days and some cold nights herding 1,670 head of donated cattle, volunteers and participants along historic stock routes. Stock routes were once the circulatory system of the livestock industry, a network of arteries and veins connecting station to saleyard and pumping cattle to consumer. Following river systems, Indigenous trade routes and trails, drovers walked cattle to market in the days before trucks and trains. Some routes eventually became roads while others fell into disuse as transport improved, but many remain, providing pasture in times of need and safe passage for migrating wildlife. Head drover Billy Prow has been moving stock along these paths for three decades. "I would have done that route [Longreach to Roma] numerous times, I don't know how many times but done it quite a fair bit," he said. "Some years were flat out, and then other years I've had a couple of quieter years compared to some blokes around. "It's all about the stock routes to me ... hopefully they keep maintained so travelling stock will always get through." Participants were provided a horse, meals and transfers to the nearest town — a "bit different" to the drives Mr Prow was used to. "I've never had that many people around me before around the mob of cattle," he said. "They've been pretty good ... they always try to listen to what you're trying to say. They don't get in the middle of them [the cattle] too much." Affectionately called the "tourists" around camp, there were plenty of authentic droving moments for volunteers along the way, such as bad weather and the herd spooking. For "tourist" Dean Nalder, the event was a chance to get back in the saddle for the first time in 20 years. "I grew up in the bush and we had horses on the farm when I was a kid," he said. "A lot of people back home in Perth were horrified that I was doing such a thing. "But I've been pleasantly surprised. I've got a great horse, he was quiet, the cattle are fat and happy, so it was just a quiet ride." Given a unique opportunity to work alongside Mr Prow, he felt like an apprentice watching a master ply his trade. "We admired what he could do with his dogs and the horses and his awareness of the cattle," Mr Nalder said. "We could walk up amongst the cattle on horseback and they were quite comfortable with us being there, it was just really fantastic. "I would do it just for the adventure, but the fact that it helps people at the same time is what, for me, takes it over the top and makes it something quite special." Jaide Donnellan, 16, has only been riding horses for three years, but was part of the droving team that brought the cattle into Longreach from Muttaburra. "The kids made it fun ... I learned a bit from them too, how young they are and how they can ride so easily because they've been doing it their whole life. "It's pretty amazing to watch." Mo Bassett spent seven weeks volunteering on the drive. "We just had to do our job, help move gear along to the next spot. That was our job and we did it. I loved it," he said. As the cattle settled into their pens at the Roma saleyards, the final count of how much money was raised along the way is still being tallied. The animals will either go back to the feedlots and pastures they came from, or be sold to meatworks, eventually making their way onto dinner tables around the country. Whatever the total raised turns out to be, the collaborative effort is enough to make even stoic cattlemen like Mr Struss a little sentimental. "Sometimes it's just that one or two meals that gives that person the dignity to get up and get going again. "I think it's important that we all understand that we have a responsibility to look after these people. "The whole thing has been an extraordinary adventure for us all."

It has been a week since Gregory Vaughan, Kim Worner and their dog disappeared in a light plane
It was a mild day on the northern Tasmanian coast when a couple and their dog climbed into a small green aeroplane and flew away. But alarms sounded when pilot Gregory Vaughan, 72, his partner, Kim Worner, 66, and their dog, Molly, failed to arrive at their destination in central New South Wales four hours later. Despite an extensive search across Bass Strait and nearby islands, there has been no trace of the missing trio or their plane since the Saturday flight a week ago. It has left many questioning how, in 2025, an aircraft and its occupants can simply disappear. Last Saturday at 12:45pm, Mr Vaughan and Ms Worner departed from George Town airport, about an hour north of their home in Deloraine, in a two-seater Bristell S-LSA plane purchased a few months earlier. They were due to make a fuel stop in regional Victoria, before continuing to Hillston in New South Wales, west of Condobolin. George Town Airport Association president David Brewster said Mr Vaughan had been planning the journey for some time and texted a friend before leaving, telling him he was all set to go. But Mr Brewster said the friend sensed trouble when Mr Vaughan's departure time came and went without a departure call being heard on the radio. Mr Brewster said he and the friend contacted Airservices Australia at about 1:15pm, after noting the plane had left the airport. They then prepared their own planes to conduct a search for Mr Vaughan's aircraft. The formal alarm was raised by a family member after 5pm, when the travellers failed to land at Hillston airport. An official aerial and marine search began soon afterwards. Police said Mr Vaughan had not made a distress call and believed there were no active tracking devices on board. Australian Maritime Safety Authority (AMSA) said the pilot made no contact with authorities during the flight. "The best-case scenario we can only really hope for is they have made land somewhere," Tasmania Police Inspector Nick Clark said on Tuesday. Wynyard Aero Club president David McCarthy said that unless entering controlled airspace, there was no requirement for private or recreational pilots to log their flight plans, make radio calls to air traffic controllers during a flight or have a transponder. In aviation, transponders — a radar-dependent surveillance device — play a vital role in identifying aircraft to air traffic control and other aircraft. "But you should at least notify a responsible person of where you're going and when you're expected back." He said pilots could also nominate a SARTIME (time search action required) with Airservices Australia, which is when search and rescue procedures should be initiated, if the aircraft fails to check in. A Civil Aviation Services Authority (CASA) spokesperson said when flying long distances over water, pilots were required to leave a flight note with a responsible person, or notify authorities through a flight plan or SARTIME. Mr McCarthy said it would be "highly unusual" for an experienced pilot not to make any calls. CASA rules state private or recreational pilots must carry a registered distress beacon when flying more than 50 nautical miles (93 kilometres) from land. Bass Strait at its narrowest point is about 220 nautical miles wide. Recreational Aviation Australia chairman Michael Monck said pilots "should carry life jackets and appropriate sort of safety equipment". Mr Monck confirmed satellite trackers were not required in any type of airspace. He said transponders were only required in some controlled airspaces. Mr McCarthy and Mr Monck fear increased regulation would add to costs and detract from the joy of flying. They said aviation was generally incredibly safe. Mr McCarthy said pilots underwent substantial training. In addition, he said most private pilots performed a flight review with an instructor every two years. "Once I'm in the air, I'm in a much safer environment because of the way that it operates and the way that we're trained." But there would always be a certain level of risk. "The bottom line [is] things can go wrong," he said.
