Meet the woman trying to make America healthy again

Vani Hari started taking packed lunches to work fifteen years ago. Her boss at the consulting firm Accenture 'would cater in for breakfast, lunch and dinner': blueberry muffins and bagels each morning, a big spread like a barbecue with banana pudding every afternoon. Evenings were spent networking over a five-course Italian meal of 'very heavy' pasta, garlic bread and tiramisu.
Hari had once tucked in happily, but a bout of appendicitis had put her off the additives and saturated fats she knew lurked on every table. So she'd sit it out and eat her own cooking – usually a kale salad or porridge oats with protein powder. In America of the 2010s, that was enough to make her an outsider and a health freak.
Over in Britain, we tend to imagine that the only clean eaters in America are lean, tanned, image-obsessed Californian types. Hari, 46, a mother-of-two from the Republican-leaning state of North Carolina, is far from it. She eats 'everything', she says, except beef, because she's a Hindu. She cooks every meal in advance for trips away with her family or for work, but 'I'd love to go to a good Italian pizzeria where they're making their dough from scratch, or eat from a bakery or a hamburger joint that is not full of man-made chemicals,' she complains. And in her sequinned white suit standing on the White House lawn, as she was in April, Hari looks anything but alternative.
Years since she made the choice to cut artificial ingredients from her diet, Hari has put the issue of food additives on the political agenda in the States. The White House has recruited a legion of friendly influencers to bypass traditional news organisations and better reach the public with its messages, and when it came to shaping how Americans eat, Hari was the obvious choice. Her blog FoodBabe, which she set up in 2011 – at first to share exercise tips and healthy recipes – has millions of readers. Search her name on any social media platform and you'll see FoodBabe, with her hair curled and pink lipstick freshly applied, pointing out the bleached white flour 'contaminated with weed killer' and 'emulsifiers and gums that wreck your gut' in the products on supermarket shelves. She wishes that they would be 'wiped from this earth,' she wrote in a caption.
These ingredients, often illegal in Britain, enrage Hari because they are 'harmful to the human body' and are there to 'improve the bottom line of the food industry', she says. She seems to have a point. The average American is more likely to be overweight than they are to be within a healthy weight range. More than one in ten are type two diabetic, heart disease accounts for one in every five deaths, and rates of colorectal cancer in young people have nearly doubled since 1995. It is no wonder that the average American's life expectancy is in decline, dropping to 76 years in 2021 from 79 years in 2019.
The country's health secretary, Robert F Kennedy Jr, promised to take on this dire situation when he was sworn in earlier this year. In his sights is not only America's food industry, however. Aside from bringing in the ban on food dyes in April, with Hari at his side, Kennedy Jr has ruled that Covid vaccines shouldn't be given to children and pregnant women, and recently suggested setting up a national autism database to account for rising autism diagnoses. Kennedy has also pushed raw milk over pasteurised milk, and has said that fluoride in America's water supply is 'associated with arthritis, bone fractures, bone cancer, IQ loss, neurodevelopmental disorders, and thyroid disease'. (In Britain, where we also have fluoride in some of our drinking water, no such disasters have occurred).
Vaccines and fluoride 'aren't my issue,' says Hari. She set up FoodBabe because 'people asked me why I try to avoid MSG in the soup at restaurants, or why I don't eat artificial food dyes,' she says, and her work is meant to help people navigate the 'unconventional lifestyle choice' of a diet free from processed foods. Her own rigorous attempts to avoid harmful ingredients are what have led to her fame. In 2012, Hari took a closer look at the ingredients used by a frozen yoghurt shop in her hometown of Charlotte, North Carolina. She discovered that the chain had been using artificial flavourings, despite claiming to be organic. Her post went viral, forcing the company's CEO to make a written apology.
Hari then turned her attention to Starbucks. She convinced a barista to give her the recipe for a pumpkin spice latte and shared it online for the first time, along with criticisms of the ingredient list. Worst of all, she said, was an ammonia-based caramel-colour food dye. 'It's already brown, so in what world is that necessary?,' she asks, clearly recalling the outrage a decade on. Starbucks later pulled the dye from its drinks. Next came Subway. 'I thought I was eating fresh when I ate there, but when you look closer at what's in a Subway roll, there were nearly 60 ingredients in there, and one ingredient was azodicarbonamide. It's found in yoga mats and shoe rubber.' Another petition, hand-delivered, led to the ingredient's removal. Then it was Kellogg's: in 2015, the company said it would remove an additive called butylated hydroxytoluene from its American products after Hari launched a petition that attracted hundreds of thousands of signatures.
The food dye ban announced earlier this year 'was a huge achievement' for Hari, she says. Greater still, though, she was invited back to the White House by the president himself just weeks later, to witness the release of the first Make America Healthy Again (Maha) report. Listed as problems were the things Hari has railed against for years: sweeteners, high-fructose corn syrup, seed oils and pesticides. 'Our food system is poisoning us, and we finally have leaders who want to make changes,' she wrote in her weekly newsletter. Her role in that has been to 'communicate different things to the public and make sure our leaders do what they say they're going to do,' she says.
Hari has a legion of Maha moms behind her, who she calls her 'FoodBabe army'. She wasn't always so comfortable being a figurehead for additive-free living. 'I wanted to call the blog 'eat healthy, live forever', but my husband, the tech geek in the family, thought it was a terrible name,' she says. 'Foodbabe' was her husband's suggestion. 'People think that I was calling myself food babe, but I didn't think of myself as a babe at all. I had these cartoon characters that I hid behind for a year and a half, while I still had my corporate job. Describing myself in that way felt very foreign.'
Growing up, in one of just a handful of Indian families in Charlotte, Hari ate a standard American diet. 'I grew up with two immigrant Indian parents, and when they came here to the United States, they really adopted the American food system and they were very trusting of it,' Hari says. Her parents wanted her and her older brother to fit in. 'My mom was cooking medicinal, Indian spiced cooking at home for her and my dad, but they didn't require me and my brother to eat it.' She ate fast food several times a week and 'as many processed foods as I possibly could'.
Hari believes that this is why 'my brother and I both had severe health issues,' she says, 'but when they took me to the doctor, no one asked what I was eating'. She was put on nine different prescription drugs in her early twenties, shortly after she finished her computer science degree at the University of North Carolina. 'I hit rock bottom after that. I ended up having surgery to remove my appendix, when I got appendicitis, and I had to have surgery for endometriosis a few years later too.'
Doctors told Hari that her appendix wasn't an essential organ, as our bodies have 'evolved not to need it'. She wasn't satisfied with that. 'It really baffled me that God would put this organ in your body that you don't need. I did my own research and found that your appendix is really there to populate your gut with good bacteria.' It was then that Hari 'started to investigate why this was happening to my body, what was causing those problems and what exactly I was eating,' she says. Processed foods were contaminated with dangerous chemicals, she found, so she 'decided to opt out of that system' and eat only whole foods. Hari also came off her medications. She lost weight, her eczema vanished, 'and I started to feel better and have better energy'.
Ever since she began her work, Hari has been criticised as a scaremonger. As early as 2014, she was accused of generating controversy to drive traffic to her blog and sell copies of her healthy eating guide. Many of the chemicals she discusses are safe, if unnecessary, say some experts, like the bread ingredient that Hari's petition forced Subway to scrap. But people would understand if they 'put themselves in my shoes, and thought about how I had two surgeries in my early twenties,' she says. 'I deeply am passionate about helping Americans to avoid what I went through. I want them to know the truth about what they're eating, and that nature has provided everything we need.'
In 2017, Hari set up Truvani, a food company that makes protein products. Her powders and bars are stocked in Whole Foods in the US. 'I became a mother, so instead of taking on the food industry full time, I kind of took a backseat to that, and decided to start my own thing, using ingredients that you would find in your own kitchen.' Her children are now four and eight. Then, in late 2024, Hari 'got out of retirement as a food activist', as she puts it. 'An old friend called me and asked me to help take on the food industry. When you really have a passion for something, it keeps calling.' There was another target in her sights, too: 'a really important issue that I didn't think was being addressed, which is that American companies are using better, safer ingredients in other countries and using dangerous ingredients here.' Last September, she spoke at a Washington roundtable about the issue, presenting flipcharts filled with comparisons between the American and British versions of different foods.
It was this that put her on Kennedy's radar, she says. The health secretary, who unsuccessfully ran against Trump in the 2024 election (before dropping out and endorsing him), 'was really giving attention to the issues I've been working on, getting the food industry to be cleaner, use less ingredients and be more transparent, and he wanted to make that a primary focus of his mission,' Hari says. She first met the health secretary at a dinner the night before the Washington event in September, and has been there 'to support him' since. The two have 'a spiritual connection,' Hari says. Like her, he 'has been vilified for his ideas, and he's been called everything under the sun for the work he's done to try to protect citizens from the pharmaceutical industry and from the chemical industry.' Now, the state of Texas is behind them, too. A bill signed yesterday by the state's governor demands that warning labels are added where foods contain additives that are banned outside of America.
Even though many nutritionists would agree with Hari that UPFs (ultra-processed foods) are bad for our health, she has joined the health secretary and a cast of other characters in a group that has been called anti-science, and that is certainly anti-convention. One such character is Dr Casey Means, the incoming surgeon general, who holds a PhD in functional (alternative) medicine rather than a traditional medical degree, and like Hari has a high profile on social media. Her brother Calley Means, now a special government employee, is an influencer, health startup entrepreneur, and former lobbyist for both major food and pharmaceutical companies. He is credited with encouraging the health secretary and the president to combine forces in the first place.
Hari herself has no formal qualifications as a nutritionist. She doesn't see that as an issue. 'I don't know how many books you need to read or to write about eating healthy to make you eventually qualified to talk about this,' she says. Rather than being a Republican, however, Hari says she is an independent — and she used to be a Democrat. In 2008, she became a delegate for the Democrats, meaning that she was selected to represent her local area at the party's convention and could vote on their presidential nominee.
Hari backed Barack Obama because he promised to force the labelling of genetically modified foods. In 2012, after this didn't come to pass, she sat in the first row on the convention floor and held a sign that read 'label GMOs!' during the then-agriculture secretary's speech. Going against her fellow Democrats was not a difficult decision. 'Poison is not partisan,' she says. 'When a child gets cancer, she doesn't know whether she's Democrat or Republican, or she doesn't care. Politics doesn't matter, because you have nothing other than your health.'
Does Hari see a prime spot for herself in the White House, alongside Calley and Casey Means? She is a talented campaigner, with a powerful story, and the ability to communicate ideas about healthy living to a population that loves its fast food. 'It's unacceptable that we haven't had more clear communications and guidelines coming from our government, not until now, anyways,' she says. She hopes that Maha 'outlasts this President and continues to go on.' Future leaders are going to 'have to have these issues as part of their platform, or they're not going to get voted in, because Americans care about them,' Hari believes.
But 'what I am first is a mom,' says Hari. 'I've got two small kids, so my business is being there for them, and then I'll continue to use my platform to inform Americans about the food industry and the food that they're eating. So I'm just going to keep doing that.'

RFK Jr and Dr Oz announce insurers' ‘pledge' to reform prior authorization

The US health secretary Robert F Kennedy Jr and Dr Mehmet Oz announced a voluntary agreement with insurance companies to change prior authorization practices – where private health insurers require patients to ask for permission before they can receive medical treatment. The majority of Americans get health insurance through a private company, whether through an employer, or a privatization of public health insurance programs, such as Medicare Advantage. Prior authorization is an insurance company practice that is both common and abhorred. There are whole social media accounts devoted to egregious examples of it, campaigns for change built around it, and, in Oz's words, there is 'violence in the streets' over prior authorization – an allusion to the broad daylight killing of an insurance company CEO. Oz said repeatedly in a press conference on Monday that 85% of Americans or their loved ones had experienced a delay or denial of care thanks to prior authorization. 'The pledge is not a mandate, this is not a bill or rule – this is an opportunity for industry to show itself,' said Oz, who heads the enormous federal health insurance bureaucracy, the Centers for Medicare and Medicaid (CMS). CMS oversees the health insurance of about 68 million seniors through Medicare and about 71 million low-income and disabled Americans through Medicaid. 'It's a good start,' said Oz, 'and the response has been overwhelming.' Kennedy called the agreement 'momentous' and said it would help make the health system, 'work to make our country healthy again'. The announcement of voluntary measures echoed one made earlier in the year by Kennedy, who announced the administration had an 'understanding' with food companies to phase out synthetic dyes. Food companies later told reporters there was no agreement. Concurrently, Republicans are working to push a bill through Congress that is expected to result in at least 16 million Americans losing health insurance in the next decade. The bill would add red tape to Medicaid and, advocates say, 'punish' states that expanded care to the low-income. With Biblical references and a bullet point chart, Trump administration officials, two Republican members of Congress and even an actor who played a doctor on TV – Eric Dane of Grey's Anatomy – laid out their hopes for insurers to implement this voluntary agreement that they said covered 275 million Americans. Should they do so, insurers would work to standardize the prior authorization approval and deliver decisions faster and near real-time (not over the course of, say, weeks). Additionally, insurers would reduce the number of procedures and drugs subject to prior authorization, honor existing prior authorization approvals in the event a patient switches insurers in the course of care and build a 'public dashboard' of how the industry is doing which would allow 'medical professionals' to review every denial. Notably, insurance companies made a similar pledge to doctors, hospitals and Americans in 2018, during the first Trump administration. In a press release announcing that agreement, insurers pledged to work with doctors and hospitals to 'eliminate' prior authorization for some procedures, 'minimize care delays' and 'protect continuity of care for patients'. By 2022, the American Medical Association (AMA), which signed onto that agreement, was arguing publicly that insurers failed to live up to their end of the bargain. A 2023 survey by the AMA of 1,000 doctors found 7% of physicians had a prior authorization lead to 'a patient's disability or permanent bodily damage, congenital anomaly or birth defect or death'. Trump administration officials did acknowledge that the practice could be egregious and warranted change. 'A vaginal delivery,' often requires prior authorization, said the Trump administration Medicare director Chris Klomp, 'Why is that a question mark in this day and age?' The insurance industry often argues insurers 'target its use' to prevent wasteful testing by doctors. However, prior authorization is known to be incredibly widespread: in 2023, a spokesperson for a lobbying group told FierceHealthcare that 93% of beneficiaries were in plans that required prior authorization for nearly a quarter of services. Beneficiaries of the federal privatization program Medicare Advantage, which allows private health insurers to manage beneficiaries of the public program Medicare (and is widely regarded as more expensive for taxpayers), issued about two prior authorization requests for every one of its 32 million beneficiaries in 2023, according to the Kaiser Family Foundation. The practice has even spawned cottage industries: a ProPublica investigation found one company contracted by major insurers sold a product called 'the dial' that used an algorithm backed by artificial intelligence to control denial rates. Unlike the federal government, some states have found the nerve to legislate. In just one example, New Jersey required insurers to turn decisions around faster, required peer-to-peer conversations between doctors about the insurers' decision and required insurers to share denial rates and reasons – at least some of which one Republican congressman at the dais said he wanted, but was not in the agreement. 'I will say this being a surgeon: I'm a skeptic, the proof is going to be in the pudding,' said Dr Greg Murphy, a Republican from North Carolina, who added that he would be open to regulations, but questioned whether insurers would abide by the agreement: 'Are they doing something to placate an audience?'

How to stay fit (and injury free): the midlife guide

By 50, one can feel as if injury is inevitable if one sneezes wrong, never mind stays active. And if we do tweak something by lunging or lifting, our impulse is to do less. We're afraid to let ourselves go. Ballistic and plyometric training — explosive, powerful movements — in particular are a no-no. Jumping, with my knees, back, hips — are you kidding? The short of it is that in midlife, most of us are brittle and lack bounce. And so it was for Henry Abbott, award-winning journalist and amateur sportsman. British-born but raised in the US, he enjoyed a successful career covering basketball, reporting on the springiest elite athletes of America's NBA. Meanwhile, physiotherapists and doctors couldn't fix his worsening hip and lumbar issues. Yet Abbott was aware that in recent decades, prevention of injury in professional sport has advanced in leaps and bounds, driven by the pioneering work of the Harvard-trained physician Dr Marcus Elliott, founder of P3, the Peak Performance Project, in Santa Barbara. He believed he could assess and predict an athlete's injury risk by how they moved. His intuitive, science-based approach irritated conservative coaches who reckoned that every time a player ended their career on a stretcher, 'there's nothing anyone could have done'. But Elliott continued his precision research into biomechanics. Early on, working with the New England Patriots (of the NFL) he reduced their incidence of hamstring injury from about 23 a season to 3. Abbott's new book, Ballistic — The New Science of Injury-Free Athletic Performance, tells the inspiring story of Elliott and P3, and is a trove of information for anyone who is keen to stay springy and robust. P3's 3D motion-capture of thousands of athletes jumping and moving has given its scientists an invaluable database of evidence as to what can cause injury — and how to prevent it. The DIY version? Film yourself in slow motion stepping off a high box and you'll acquire decent information about how you land, Abbott says. 'Maybe you'll see your knees cave in; maybe one foot lands before the other; maybe your heels slap down; maybe your pelvis is uneven.' He recommends taking the footage to a trusted personal trainer. Halfway through writing his book, in agony and desperate, Abbott, 50, acceded to a P3 assessment himself. The instruction enabled him to bounce back to peak fitness and function. He's speaking on Zoom from his home in New Jersey, and flaps three soft-worn sheets of exercises at me, beaming. His regime includes strength training, plyometrics and yoga. 'You're fighting your body's natural inclination to reduce your power and range of motion every year after 25,' he says. But if you know what to do, it's a battle you can win. Below are some scenarios that many of us have experienced (or are likely to experience) — and advice on how best to support your body in each case. My knees are stiff and ache when I run. What can I do to offset the pain? Beyond a common finding — 'You're a bit short of cartilage and, whoops, you're ageing' — P3's research shows that almost every serious knee issue comes from the hips or ankle, Abbott says. 'You're counting on your ankles, knees and hips to attenuate the force of landing. And if they are twisted or rotated, the knee just has to cope with whatever's coming from the ground or the hips. The knee is the unfortunate middle man.' P3 studied the knees of 380 NBA players over two years and 'found that people who end up with a catastrophic knee injury' — usually an anterior cruciate ligament tear, but also meniscus tears — 'all had a habit of landing on the outside of their foot and rolling to the inside'. This poor landing position is associated with weak soleus and tibialis posterior muscles (below the knee). Strengthen them by standing on one leg and lifting and lowering your foot, for example, or skipping. The second leading cause of knee injury is what Abbott calls 'dishrag': femoral rotation. 'As you squat, your upper leg bone rotates inwards, like twisting the turkey leg off the carcass. It's from weak hip muscles.' Side planking — with weights and using leg raises — targets the gluteus medius, which is key in stabilising the hips. I want to play f ive- a-side in midlife. How can I avoid injury? Abbott cheekily queried whether warm-ups were 'a real thing'. A warmed-up muscle is two to six times less likely to be injured, Elliott told him. But sprints won't prepare your body for football's range of movement. P3's recommended warm-up — for any workout — incorporates rotating, lunging and bouncing. Exercises include a heel walk; lunging and twisting; inching from a plank to a standing position and back again; single-leg Romanian deadlifts; quad stretches; knee hugs; leg swings (sideways and front to back); and 'a bit of lying on the ground, more rotating'. Why save it for sport? 'There have been many days when I wake up feeling 102 years old and then I just do these steps and I feel fine,' Abbott says. I've joined a netball team. How can I protect my back? Planning to leave the ground? Whether you jump, run, lunge or leap, land on the balls of your feet — do not land toes down. P3's research-based instruction started with its observation of elite runners. '[Elliott] noticed that everyone running in the Olympics has their feet dorsiflexed — a little bit toes-up,' Abbott says. 'If the force of landing goes into the ball of your foot, with your toes up, this communication happens all through soft tissue — to your Achilles, to your calf, to your quad, to your glutes. With that chain of communication you can take huge forces without injury.' Whereas if you land with your toes down, 'the next thing is your heel slams down', Abbott says. It exerts huge forces through your body (P3's measured record is nine times a person's body weight: 'Like catching a falling bison'), absorbed by your knees and possibly your lower back. Train so that landing well becomes a habit. 'I do hip bridges with my toes up, just on my heels, or marching movements with a ball over my head and with my toes up,' Abbott says. My posture is dreadful. What can I do to straighten up? 'Computer back' or kyphosis — where your upper back is curved forward — is the most common problem Elliott sees. In athletes it makes injury more likely, limits lateral explosive power and 'contributes to lower back and hip issues, because as your head goes forward you'd fall over if you didn't compensate in your lower spine', Abbott says. To reverse it, P3 recommends a tricky move called the snap squat press. 'Put a broom across your shoulders and squat, so your hips are level with your knees. Stay in this squat and push the broom up. If you have kyphosis, the broom will hit the back of your head — at first,' Abbott says. (Avoid this by elevating your heels on a rolled-up towel.) 'You're rehearsing the movement that will fix the problem', and will ultimately progress from broom to bar. I jog in a low shuffle. If bouncing along is better, how do I train? 'So much of life comes down to glutes, it turns out,' Abbott says. 'The shuffle is the lie that you can land safely without using your glutes, so now the forces are going places they aren't supposed to go.' How to train yourself to be springy? If you skip with a rope, it cues the idea that 'I want the force of the ground to travel smoothly and efficiently into my glutes when I land from running or jumping'. After practising himself, Abbott noticed that rather than use his leg muscles to push off the ground, 'I would land with my toes up a little bit, and my legs slightly flexed and the force would go boing, boing, boing into my glute and it took almost no energy. That's the rehearsal for running. That's the springier, safer thing in action.' Understanding hip flexibility is key GETTY IMAGES I have hips like the Tin Man. Does it matter that I can't touch my toes? 'Every single pair of hips P3 has assessed needs help with either mobility or stability, and you should know which group you're in,' Abbott says. 'People with stable hips just love lifting weights and accept that they can't touch their toes, and people who are really good at the half-moon position just keep going to yoga.' Broadly, everyone who does yoga should lift weights and vice versa, he adds. Without both mobility and stability, you could suffer hip pain, hip-related lower back pain, or knee pain. Abbott suffered incapacitating back pain. His P3 assessment included stepping off a box on to a force plate, which revealed that 'when my right foot came off, my right hip would drop — it's supposed to stay level — and when I hit the ground, I'd have 30 per cent more force on my left side than on my right'. He wasn't absorbing enough force in his hips nor using his glutes, which explained his lumbar problems. Recommended exercises included the stork press: stand on one leg, the other leg pulled up high, knee bent, then hold a heavy weight in your hand on your elevated-leg side. 'You're balancing, trying to keep your hips level, and deploying the glute of the standing leg,' Abbott says. 'Most of what I learnt was in a cold sweat because it was like learning a new language.' My training is strictly gym-based. I'd love to surf but I'm scared I'll wrench something We train to prepare ourselves for life. 'Even if you just chase your dog when it gets off the leash, you're going to have to move aggressively,' Abbott says. 'It's weird that we don't jump more.' If you only stick to the elliptical, you do none of that 'complex navigating in space'. 'Humans have the DNA and the musculature and range of motion and the strength capabilities to be pretty darn free moving. That's the goal.' Aim for the joyfulness of 'puppies playing on the beach'. Abbott's grandmother set a great example: she bodyboarded in Devon. 'She had a wooden board, she'd wear a cap and a frilly, flowery World War Two swimming suit. It was a thrill: the sun and the breeze and cold and the dogs and the puffins.' I confess that my wildest move is a bear crawl. 'Bear crawl is 100 per cent something that Stone Age humans did all the time,' Abbott says. 'We cooked on the ground — there was so much scrambling around.
