
Fractal proposes indigenous AI medical models to improve healthcare

, medical education, and clinical decision support. Srikanth Velamakanni, co-founder of Fractal Analytics, said the estimated cost of the project would be around Rs 150 crore.
Fractal proposed building a comprehensive family of foundation models—from base language models to specialised medical reasoning systems with multimodal capabilities to advance AI in healthcare, aimed at transforming the medical landscape.
"The PG NEET test is taken by 200,000 people every year. Indian doctors take it annually for their post-graduation in medicine. On that test, we scored 83, which is our benchmark. All the other open-source models we accessed scored much less than that. We launched

to elevate it to the next level by building India's biggest breakthrough medical model," Velamakanni said.The Mumbai-based company developed India's first multimodal medical assistant, which outperformed frontier language models like GPT-4o, Gemini, and Llama 3.2 on the PG NEET benchmark.
Trained extensively with more than 650,000 images and 200,000 text samples, Vaidya.ai exhibits proficiency in processing various Indian languages, such as Hindi, Kannada, Tamil, and Telugu.
Fractal's proposal said that multimodal reasoning systems possess the capability to recognise patterns of symptoms, enhancing diagnostic assessments through analysis and verification across X-rays and CT scans. These systems function by validating signals between different modes of data collection, such as establishing connections between minute features of lung nodules visible in CT scans, alterations in respiratory measurements from wearable devices, and descriptive information documented in clinical records.

India's healthcare system demonstrates a pressing need for technological advancements. The disparity in healthcare access is particularly evident in rural and semi-urban regions, where the ratio of doctors to patients surpasses 1:1,500, significantly higher than the WHO's recommended standard of 1:1,000.
He said the medical reasoning model supports doctors and patients with diagnosis and health-related questions.
"The model can aid medical reasoning. It can assist a doctor in reaching a diagnosis. It could help a patient understand their symptoms or determine what their side effects might be. It can also provide guidance on what nutrition they should follow." The tech roadmap has two phases, with the first one incorporating a foundational language model trained from scratch on 18T tokens, STEM content, coding, and educational material.
It also includes a multimodal foundation integrating the vision encoder with the language model. The second phase includes meta-cognitive abilities, self-reflection, and expert-level medical reasoning capabilities.
Velamakanni said Fractal has an extensive collection of educational materials, including PG NEET and MBBS examination books covering the five-year medical programme. The dataset underwent thorough preparation through text standardisation, medical term identification, and contextual parsing before being utilised to calibrate Vaidya, ensuring its foundation in reliable, examination-focused medical knowledge and clinical analysis.

SJM/Norms relaxed, residents of registered homes for the mentally ill to be enrolled under CMCHIS
All inmates of registered homes for the mentally ill will be enrolled under the Chief Minister's Comprehensive Health Insurance Scheme (CMCHIS) without requiring the mandated ration card and income certificate. Issuing an order to this effect, the State government permitted the Project Director of Tamil Nadu Health Systems Project (TNHSP) to enrol all inmates in registered homes under CMCHIS without the mandated ration card and income certificate, subject to satisfying the criteria prescribed in Government Order 16 dated January 8, 2022 [on continuation of implementing CMCHIS with effect from January 11, 2022 to January 10, 2027 through United India Insurance Company at a premium of Rs. 849 per family per annum]. The government sanctioned Rs. 51,05,896 for the enrollment of 5,944 inmates in 108 homes under the Welfare of the Differently Abled Persons Department without the need for the two documents. It was at the sixth monitoring committee meeting chaired by the Chief Secretary in early 2024 that it was decided to issue orders to approve issuance of CMCHIS cards to inmates residing at homes for the mentally ill in the absence of other individual identity cards such as ration card, income certificate that are mandated as eligibility criteria for enrolment under the scheme. Listing out the mental health establishments in the State, the Health department noted that there are a total of 54 government-run and an equal number of NGO-run homes for mentally ill persons in the State. There are six halfway homes - one with the Institute of Mental Health and five with the Welfare of the Differently Abled Persons Department. There are eight government-run and eight NGO-run Emergency Care and Recovery Centres in the State. Officials said that providing CMCHIS coverage for persons with mental illness residing at mental health establishments would help them get medical or surgical treatment that they required in both government hospitals and empanelled private hospitals. 'There may be inmates who suffer from cardiac ailments requiring bypass surgery or stenting, or from fractures or hernias or medical management for any illness,' an official said, adding that one of the highlights measures taken was to do away with the need for the mandated documents to enrol under CMCHIS. Doctors said that most of the residents do not have social support, and relaxation of these norms would facilitate access to immediate medical care especially in emergencies. It may be recalled that in mid-2022 a total of 520 residents of IMH were brought under CMCHIS after the State government relaxed these norms.

AI errors: RBI panel calls for 'tolerant supervision'
MUMBAI: An RBI panel examining the responsible use of AI in finance has urged regulators to adopt a "tolerant supervisory stance" towards mistakes made by AI systems. The idea is to allow institutions some leeway for first-time errors if they have adequate safety measures in place. The aim, the panel argues, is to encourage innovation rather than stifle it. Such tolerance is justified, the report says, because AI is inherently probabilistic and non-deterministic. A strict liability regime that penalises every misstep could make developers overly cautious, limiting AI's ability to deliver novel solutions. This approach could be controversial as it may be seen to be shielding institutions at the expense of customers who suffer losses from AI errors. The framework rests on seven "sutras": maintain trust; keep people in control; foster purposeful innovation; ensure fairness and inclusion; uphold accountability; design for transparency; and build secure, resilient, energy-efficient systems that can detect and prevent harm. Its 26 recommendations span building better data infrastructure, creating sandboxes for AI testing, and developing indigenous models to help smaller players. Regulators are advised to draft flexible rules and apply liability proportionately. Banks are told to adopt board-approved AI policies, implement strong data governance, and safeguard customers through transparency, effective grievance systems, and robust cybersecurity. Continuous monitoring, public reporting, and sector-wide oversight are proposed to keep AI use safe and credible.
