Panic attacks are a mental health condition characterised by unexpected, intense episodes of fear and physical discomfort, which can feel utterly disabling.

July marks Mental Health Awareness Month in South Africa, and on 10 July, we pause to spotlight something many people live with but few talk about openly: panic attacks.
But here's the thing, panic doesn't always look the way you might imagine.
Sometimes it's loud and visible; other times, it hides behind a composed face, a polite smile or an empty chair at the dinner table.
This year, the South African Depression and Anxiety Group (SADAG) has chosen a theme that couldn't be more real: 'The Panic You Don't See.'
It reminds us that panic disorder and anxiety are not always about gasping for air or clutching your chest in public. It's a mental health condition marked by unexpected, intense episodes of fear and physical discomfort that can feel utterly disabling.
Often, it's the silent dread, the racing heartbeat in the middle of a meeting, or the countless invitations quietly declined.
'Panic doesn't always look like gasping for air, it can be the quiet no‑shows, the life slowly shrinking behind closed doors, says Dr Colinda Linde, SADAG board chairperson and clinical psychologist.
"On Panic Day, let's speak up about the panic you don't see, because naming it is the first step to overcoming it.'
Panic attacks can manifest loudly and visibly, or they can be concealed behind a composed expression, a polite smile, or even an empty seat at the dinner table.

What does panic really feel like?
If you've never had a panic attack, it can feel almost impossible to picture. Imagine suddenly feeling an overwhelming wave of fear or dread, even when nothing around you seems obviously wrong.
You might feel your heart race, your chest tighten, or your breath catch.
Some people feel dizzy, shaky, or disconnected from reality, like watching life through frosted glass. And while these feelings are intensely real and terrifying, the outside world might see someone who looks completely fine.
That gap between what's happening inside and what's visible outside can make living with panic disorder deeply isolating.
Why can panic stay hidden?
Linde explains that people with panic disorder often become experts at masking their distress, especially in public or professional spaces.
Every day, moments can feel overwhelming. But from the outside, it just looks like someone who is composed.
And for many South Africans, especially men, the pressure to 'hold it together' can be even greater. This silent struggle can lead to self-isolation, as individuals avoid situations that might trigger an attack.
Over time, their world shrinks, impacting relationships, work, and overall well-being.
SADAG founder Zane Wilson, who battled panic attacks for over a decade, shares: 'Panic Disorder can be a terrifying illness that causes great distress, isolation, and limits your life. For years, I suffered in silence.
"But with the right treatment and support, I was able to recover fully and take back control of my life.'

The National Institute for Communicable Diseases (NICD) has confirmed that 10 travel-related cases of chikungunya virus have been diagnosed in South Africa this year. The cases were identified through the NICD-National Health Laboratory Service (NHLS) and Lancet Laboratories. All are linked to individuals who had recently travelled to regions with active outbreaks. But what exactly is the chikungunya virus, and why is it of growing concern to South Africans? "The disease is caused by a mosquito-borne virus and in South Africa is typically reported in returning travellers," said the NICD. Transmissions primarily come from the Aedes aegypti and Aedes albopictus species of mosquitoes. These are the same that are known for spreading dengue and Zika viruses. "Its transmission cycle is quite similar to that of dengue viruses, with mosquitoes facilitating human-to-human transmission in urban settings." While the disease is rarely fatal, it can cause intense symptoms that disrupt daily life. In some cases, arthritis-like symptoms can persist for weeks or even become chronic. At present, there are no licensed vaccines for chikungunya available in the country.

Research shows that Gen Zs are the loneliest generation, despite having it all. Gen Z is the first generation that has grown up completely immersed in a digital world. But it's not just a world of convenience and technological progress. Instead, of simply getting dopamine hits from swiping, doomscrolling, cancel culture and a host of other anxiety-causing conditions have surfaced in substantial volumes. Loneliness, disillusionment and depression are becoming the mental health scourges of modern times. Clinical psychologist and medical doctor Dr Jonathan Redelinghuys said the signs are no longer subtle. 'They are not just moody teens. Many of them are genuinely unwell. Anxiety and depressive disorders are presenting earlier and more severely than we've seen in previous generations,' he said. Gen Zs were born between 1997 and 2012. Smartphones, social media and streaming culture have shaped their reality from birth. But hyperconnectivity, what Gen Zs grew up with, has come with a toll. A 2023 Pew Research Centre report found that nearly one in three teens said social media had a negative effect on their mental health. 'It's not just the content, it's the comparison. Gen Z is constantly measuring themselves against filtered, curated lives online. That's incredibly damaging,' Redelinghuys said. His observations match published research in the American Journal of Health Behaviour in 2020, which found that the more time adolescents spent on social media, the more likely they were to report mental health challenges. The American Psychological Association reported that 91% of Gen Z adults aged 18 to 23 experienced at least one physical or emotional symptom of stress in the past year. That's a lot higher than the general adult population, where the figure was pegged at 76%. Overstressed Gen Z's In South Africa, the numbers tell a scary story. Youth unemployment remains high, with StatsSA's 2024 data showing nearly 45% of South Africans aged 15 to 24 are without work. 'There's a sense of hopelessness. Many Gen Z's feel like they are climbing a ladder that is missing rungs,' Redelinghuys said. Add to this the escalating cost of living, student debt, and an increasingly competitive academic environment, he noted. 'The consequence can be mental burnout before life's even hit the fast lane.' ALSO READ: 7 reasons Gen Zs choose friends with benefits The American College Health Association's 2023 assessment noted an increase in academic anxiety, insomnia and emotional exhaustion among US college students. 'The pressure to succeed is immense. They are expected to perform like machines, while also navigating identity, climate fear and digital overload,' Redelinghuys said. A 2021 global study published in The Lancet Planetary Health found that 59% of young people surveyed were 'very' or 'extremely' worried about climate change, with over 45% saying that those fears affected their daily functioning. One respondent noted, 'It makes me anxious, like I have no future.' Redelinghuys said this kind of future-looking dread is increasingly common. 'They are not imagining it. They're growing up in a world where adults seem paralysed, and the problems feel insurmountable.' Gen Z – the loneliest generation Gen Z has also been labelled the loneliest generation. Research from Harvard's Graduate School of Education in 2021 found that more than 61% of Gen Z adults in the United States reported profound loneliness. Remote schooling, lockdowns and a lack of in-person social development during crucial years have left many of them socially fragmented. 'It's likely no different in South Africa,' said Dr Redelinghuys. It's a generation pedestalled for its openness, inclusivity and focus on self-expression. The McKinsey & Co report on generational trends noted that while many Gen Zer's explore diverse identities, they also face backlash, online harassment and internal conflict. It can become a major source of stress though. 'They are brave and honest about who they are, but society is still catching up. That creates tension,' Redelinghuys said. According to Dr Redelinghuys Gen Z's need a different approach to how generations prior was managed and psychologically healed or counselled. 'In a sense, they have inherited a rotten world of real-time conflict, environmental collapse and social disintegration. Couple this with a hyper-connected digital multiverse, and there can only be challenges,' he said. 'Interventions need to go beyond mindfulness apps and social media breaks. Zimbabwe Human Rights Commission chairperson, Jessie Majome, has urged South Africans to show compassion toward Zimbabwean migrants. Majome has reminded all of the historic solidarity between the two nations during South Africa's liberation struggle. 'During the struggle for South Africa's independence, a lot of African countries, including Zimbabwe, hosted South Africans,' Majome said in an interview on Newzroom Afrika . Her comments come amid growing outrage over the denial of healthcare to undocumented Zimbabweans in South Africa. In recent months, protests have intensified, with groups like Operation Dudula blocking access to hospitals such as Kalafong in Gauteng. Protestors have stationed themselves outside clinics, demanding identity documents and turning away those unable to prove legal status. This is despite South Africa's Constitution guaranteeing access to emergency medical treatment for all, regardless of nationality, under Section 27. In July 2025, the South African Human Rights Commission reaffirmed that healthcare is a constitutional right. The commission condemned the unlawful gatekeeping and the ongoing policy confusion that enables some hospitals to ignore legal obligations. Legal challenges are now in motion to address exclusionary practices, particularly in Gauteng. Majome's message was clear: Africans must return to a spirit of empathy and justice. 'Some of the Zimbabweans who are in South Africa are in South Africa regularly and legitimately,' Majome said. 'But if there is intolerance and xenophobia, even those who are contributing to the economy of South Africa also bear the brand of negative attitudes, which are not helpful for South Africa.
