
Taupō Hospital doctor, staff shortages spark closure contingency plans
RNZ has obtained a copy of the Escalation and Closure Plan for Taupō Hospital, which outlines what could happen if the hospital were forced to shut down because of not finding enough staff.
No closures have happened, and Health New Zealand Te Whatu Ora's top manager for the area, regional deputy chief executive Cath Cronin, has told RNZ she wouldn't allow this and was instead focused on keeping the hospital open.
But closures aren't unprecedented. Last year, Westport's hospital shut its doors several times because of short staffing.
The plan's introduction outlines the gravity of a closure: 'Temporary closure of Taupō Hospital poses a potential risk to the population and, as such, Health NZ Lakes has a responsibility for managing the risk with a contingency plan, to safeguard the public to the best of its ability.'
The plan
The document said its purpose was to set out an agreed process when there weren't enough senior doctors to staff the hospital's emergency department.
It said authority to consent to a closure sat with the regional deputy chief executive – Cronin – and 'every possible option for covering vacant roster shifts must be exhausted' before the plan was put into action.
If closure were required, contingency measures would swing into action three days before the unstaffed shift, allowing time to tell the public and other affected parties.
If the emergency department (ED) were to close, but some services continue, security would be stationed at the hospital entrance with a list of patients allowed inside.
A February memo from senior clinical staff to management outlined further details.
The senior staff cited upcoming vacancies and difficulties in getting locums.
'Consequently, we are unable to guarantee that the Taupō Hospital will be able to cover every shift in the ED or the inpatient ward. Therefore, we thought it prudent to have contingency options documented and agreed in advance, which could be employed if and when this situation arose.'
Six scenarios
The memo described possible situations in which the plan would be enacted.
'Although none of these scenarios are considered acceptable under normal circumstances, we may be forced to implement one or more of them to ensure continuation of service provision,' it said.
Two scenarios involved using doctors from Rotorua to keep open Taupō's general ward, or its ward and emergency department.
A third scenario suggested closing the Taupō general ward, which usually has about 15 patients.
'All inpatients requiring admission will be transferred to Rotorua, increasing the workload for Rotorua physicians and registrars/nurses.'
Option four was to use telehealth for emergency department patients.
Option five tabled closing the department, which sees an average of 50 patients a day.
This would also potentially involve telehealth, but otherwise, emergency cases would have to travel to Rotorua.
The memo said risks for this were: 'No access to emergency care for Taupō-Turangi communities. No on-site clinician for ward. No on-site support for birthing unit.'
The sixth option was the status quo, with a reliance on locums. This was time-consuming to sort and expensive.
Pressure building
Emails released to RNZ show the hospital's battle to find staff.
On May 15 last year, then clinical lead Jared Bayless said there were five emergency department shifts, including four overnight ones, unstaffed in the coming week.
Another email from Bayless, a month later, discussed juggling staff to cover vacancies during the week, which stretched the weekend roster thinly. There was concern that the hospital would have to operate at decreased capacity. Bayless subsequently informed emergency services about the possible staff shortfall.
In other messages, Bayless stated what would happen if staffing wasn't found to cover all shifts, options that the Escalation and Closure Plan covered.
A draft memo to Cronin in October from Health NZ's Lakes district group director of operations, Alan Wilson, again talked about the possibility of closure when staff couldn't be found, and outlined the risks of this, including having to rely on an already under-pressure Rotorua Hospital.
The memo said $1.29 million was spent on locums for Taupō Hospital in 2023-24.
It recommended changing the staffing structure away from senior doctors working 24/7 and employing more doctors to allow for round-the-clock coverage.
Cronin found out about the memo from doctor unions and emailed them to say she had concerns about the issues at Taupō Hospital's emergency department and how they'd been addressed.
'All discussions, planning or other communications regarding Taupō ED are now on hold.'
Cronin also emailed Wilson expressing her disappointment about the memo's contents and that he let it happen.
'The proposed plan is not a direction I will endorse without further discussion, so don't progress any further planning or discussions with the team.'
However, the Escalation and Closure Plan was circulated early this year in further emails.
'Not an active plan'
Cronin told RNZ this week the Escalation and Closure Plan wasn't an 'active plan'.
'My approach is always to work on plans to keep hospitals open and EDs open,' she said.
'This wasn't an approach that I endorsed. It got a life of its own, with the team wanting to do the right thing but not in quite the right way.
'When I found out about it, I did stop that approach to making a plan to close the ED, and instead we reconvened to work together on how we keep our ED open.'
Cronin said she'd never been asked to consider closure, as outlined in the plan, although it was challenging to fill shifts.
'But we always get there, one way or another,' she said. 'We always manage to cover that. We haven't had to close.
'We've got a particularly tough time in the next couple of months, not only in Taupō but across the whole country.
'We're getting right into the middle of winter, with lots of sick leave, but everyone's endeavouring to do what they can to maintain that access for patients.'
There were plenty of other mitigations before closure would even be considered, Cronin said.
'We take that week by week when we plan, then day by day, or shift by shift if we need to.'
Cronin said this year she met with Taupō's medical staff weekly and would regularly check in with the lead clinician.
Asked about shifting patients to Rotorua if required, Cronin said moving patients to ensure they received the proper standard of care was something that happened nationwide.
This week, there were 4.7 fulltime-equivalent senior doctor vacancies in Taupō, which would drop to 2.7 in October when two staff members joined.
Two extra junior doctors had recently joined and one would soon start.
Another two were due by January, which left two vacancies.
Cronin said recruitment would focus on how Taupō was a great place to live and on the quality of the clinical team at the hospital.
Plan a message to management
Rural hospital medicine specialist Ralston D'Souza has just taken over as Taupō Hospital's clinical lead.
He said the lack of permanent staffing at Taupō wasn't new, and he and the other clinicians developed plans in response.
'It's probably been known about for a couple of years,' D'Souza said.
'With that short staffing, there's going to be gaps in the normal, everyday roster. So, as a group, we were trying to [say] to management: 'Look, this is a risk to the organisation'.
'If permanent staff are unable to fill these gaps, if locums are unable to fill these shifts, we have to have a contingency plan on what we need to do.
'There's a huge amount of people affected in the hospital and in the community if there's no doctor in the hospital, so we wanted to get something on paper or get protocols in place if that were to occur because of the vulnerability of our workforce.'
D'Souza said additional junior doctors were welcome, although it would take some time until they were trained to fill overnight shifts, with their varied responsibilities.
But he was more hopeful than previously and he said Cronin was working closely with the hospital on staffing issues.
Clinicians, management and the community were working together to find solutions, D'Souza said.
Sarah Dalton, the executive director of the senior doctors' union, the Association of Salaried Medical Specialists, said clinicians drawing up the Escalation and Closure Plan showed there weren't enough doctors to staff the hospital.
'Between 40 to 50 per cent of their roster at any given time is filled either by locums or their employed staff being prevailed upon to do extra shifts, so do extra work for extra pay to keep the place open.'
She said the arrangements to fill the roster were 'hand-to-mouth, subsistence stuff'.
Dalton criticised Health NZ for paying little attention to the wellbeing of permanent staff, while spending plenty on locums.
Patient Voice Aotearoa's Malcolm Mulholland said the Escalation and Closure Plan showed how concerned clinicians were.
'It's pretty confronting seeing that plans have been drafted to actually shut down one of our hospitals in New Zealand, and to know that it serves a community of 40,000 people or more.
'It's extremely concerning.'
Mulholland said the advocacy group held a public meeting in Taupō earlier this year, where he heard from clinicians' concerns about staff shortages and the prospect of patients transferring to Rotorua.
It was planning to hold a further public meeting in the town on July 30 at 6pm, at Taupō's Hilltop School.

Dying teen transferred, family told to arrange return of his body
The family of a teenager who died from complications after an operation never got the chance to say goodbye. They were further distressed by being told they needed to organise his body's repatriation to his hometown. A complaint about the 19-year-old's care was referred to Health and Disability Commissioner by the coroner. The young man – who died in 2015 – had undergone an operation in January that year related to his type 2 neurofibromatosis – a genetic condition that causes benign tumours to develop on nerves, particularly those in the skull and spine. There were complications due to a post-operative infection and meningitis, which was treated successfully at a secondary hospital. ADVERTISEMENT The man – who was referred to in the commissioner's report as Mr B – continued to suffer from fluid building up around the brain and required regular release of cerebrospinal fluid through lumbar puncture. He was admitted to hospital with ongoing headaches and vomiting in April. A decision was made to hold off on further lumbar punctures due to concerns it could cause a hernia and to transfer him to another hospital via an air retrieval team. The transfer was delayed due due to staffing issues and a lack of an available air ambulance. Deputy commissioner Dr Vanessa Caldwell said Mr B was neurologically stable at the time and his transfer was scheduled for the next day. However, while waiting he collapsed and his heart stopped. He was then urgently transferred to another hospital, but his condition deteriorated and at the second hospital he was declared brain dead. ADVERTISEMENT Mr B's family told the commissioner they did not understand why he was not transferred by road when the air retrieval team was not available, and they did not understand why he was transferred to another hospital when his prognosis was poor. The transfer meant they did not have a chance to say goodbye to him before his death. The family also said they were asked if they would donate his organs only minutes after being told he was brain dead, which left them little time to consider their options. They were also told by a social worker it was up to them to organise transport of his body back to where they lived, even though he qualified for travel assistance. Health NZ apologised for the distress caused by the discussion related to organ donation and the miscommunication regarding transporting Mr B's body. Dr Caldwell said the care provided to the man was at an appropriate standard and decisions, such as the air transfer, were made appropriately based on the information available to the team at the time. Incorrect and minimal information was provided to the family once the man died and this had been particularly distressing for them, she said. Health New Zealand breached the patient's right to information under the Code of Health and Disability Services Consumers' Rights, the commissioner said. Northland's first milk bank opens, aiding premature babies
'It makes me quite emotional because I would have accepted milk when my baby was in there and needed it... those babies are quite vulnerable, and I'd do anything to help them.' Bree compared the milk bank project to a pregnancy, albeit one with a more than two-year gestation period. It had been made possible by 'incredible generosity' from the community, which had raised the almost $33,000 needed to buy the pasteuriser. So, we recognised this was going to be a really beneficial thing for our sick and vulnerable pēpi [babies].' Small, premature babies would be prioritised, but donated milk could also be given to babies in the maternity ward with low blood sugar levels. In the past, premature babies had mainly been given formula, made from cow's milk, which was essential but not ideal. She expected hundreds of babies a year would benefit. Midwife Louise Rowden said a mother's breast milk was the perfect food. 'But if the mother can't get her own breast milk, then the next best thing is another mother's breast milk,' she said. 'Terrified and confused': Baby dies after overdose
By Sam Sherwood of RNZ Warning: This story has details of the death of an infant A two-month-old baby died following an overdose after she was allegedly given medication at an adult dosage by a pharmacy, RNZ has revealed. Her grieving parents are calling for a law change that would make it mandatory for medication to be checked by two people before it is dispensed. The revelations have prompted the Ministry of Health and Health New Zealand to "urgently" undertake a joint review into the incident, with Medsafe visiting the pharmacy to ensure they are safe to continue operating. The Pharmacy Council, which is also investigating, says it is "clear that an awful error has occurred". Bellamere Arwyn Duncan was born at 31 weeks and five days at Palmerston North Hospital on 2 May. Her parents Tempest Puklowski and Tristan Duncan knew from the scans she was going to be "quite tiny", and were told she would be early but no one expected she would come as early as she did. "That was definitely on her own accord," Puklowski, a first-time mum, told RNZ. "She sort of just made up her mind, and was like 'I'm coming out'." Bellamere, who weighed 1023 grams when she was born, spent about two months in the neonatal unit. Puklowski says she could not wait to bring her baby home and was excited when she was discharged on 24 June. While in hospital, Puklowski gave Bellamere her drops for Vitamin D. Nurses also administered her phosphate. When they left hospital they were given some Vitamin D in a little bottle, and a prescription for iron and Vitamin D. The following day Duncan went to a Manawatu pharmacy with the prescriptions. He was given the iron, but says the pharmacy refused to give the Vitamin D as the staff thought the dosage was "too high for her age and her weight". The staff said they would call the neonatal unit and follow-up. A few days later Puklowski received a call from the unit to organise a home care visit. During the call she was asked if she had any concerns, and Puklowski asked if they had been contacted about the Vitamin D. They had not, and said they would follow up and rewrite the prescription along with a prescription for phosphate. The homecare visit went well. Bellamere had put on weight, and was "doing well", Puklowski recalls. "She was settling in perfect." A day after the phone call, on 2 July, Duncan went to the pharmacy to collect the medication and came home with just the phosphate. Unbeknown to the parents, they had allegedly been given an adult dosage of phosphate. The label on the medication directed them to dissolve one 500mg tablet of phosphate twice daily in a glass of water. Puklowski said given the pharmacy's refusal to give them the Vitamin D, they did not even think to question the dosage. That evening they gave Bellamere her first dose of the medicine in her formula water. They would give her three bottles in 24 hours as was recommended. The couple noticed in that period that her eating was off, and thought she was "extra gassy", Puklowski says. "She was still feeding fine. She just wasn't may be going through a whole bottle compared to what she was," she recalls. Then, the day after she got her first dosage, Bellamere suddenly stopped breathing. "We were like oh shit, I went straight into panic," Puklowski says. "Tristan had to start administering CPR, and I was on the phone to the ambulance which arrived very quickly, within at least five to 10 minutes." Bellamere was taken to hospital and rushed to the emergency department. Once she was stabilised she was taken to the neonatal unit where she stayed overnight before she was flown to Starship Hospital. "We were definitely terrified and more confused than anything about what was going on," Puklowski says. The couple told the doctors they were worried they had overfed her, and her body was struggling to get it out. "I was trying to think of what had changed in the past 24 hours, which was her phosphate," Puklowski says. The couple had taken a bottle of the medicine with them to Starship Hospital. She gave it to the staff who saw that they had been given an adult dose. The staff then requested the original prescription which confirmed the script had been written with the correct dosage, but somehow the pharmacy had given the wrong dosage, Puklowski says. "I keep thinking about how much she ended up having and it just makes me feel sick." Once at Starship Hospital the couple were told they would "have to make some hard decisions". "But then we went and saw her. She was still moving and her eyes were still opening. "So we were like, 'No. She's our strong little fighter. I mean, look at how well she's done so far'." Tragically, Bellamere died at Starship Hospital on 19 July. "It was completely horrible," Puklowski says of having to say goodbye to her baby. A preliminary coroner's opinion is that Bellamere died of phosphate toxicity, her parents confirmed. A week on from their daughter's death, the couple are still in shock. Puklowski says she is in "disbelief". "They're just numb," Puklowski's mother, Rachelle Puklowski says. "It's completely traumatised them. They just watched Bella pass twice, once in their home and then again up at the hospital." Pharmacy responds The owner of the Manawatu pharmacy that dispensed the medication said in a statement to RNZ the baby's death was "a tragedy". "Our sympathy is with the family and whānau. This is a very difficult time. "We are looking into what has happened to try to understand how this took place. There will also be external reviews which we will work with." RNZ asked the owner how the medication was given at the wrong dosage, whether they disputed the allegations, when the pharmacy became aware the wrong dosage had been given, and what confidence people could have about other medication received from the pharmacy. The owner said the pharmacy was "devastated about what has happened and are investigating to find out how this occurred". "It is not appropriate to comment further at this stage." Duncan describes what happened to his daughter as "negligence". "How was it overlooked?" Puklowski wants to know. "Not even just in the initial making of it, but in the handing of it to us. They denied us the Vitamin D because they thought the dosage was too high for her age and weight, but can proceed to give us a full adult dosage of phosphate, like it just makes no absolute sense," she said. "They have to make sure they realise the kind of mistake that they have made, and that something has been done about it." The couple are adamant they want changes to the system for giving out medication. "It's the sort of thing that can't really be overlooked. "There needs to be something better in effect, rather than just relying on one person to make sure you're getting the right prescription, having at least a few eyes." Pharmacy Council chief executive Michael Pead said in a statement to RNZ the council's "heartfelt thoughts" were with Bellamere's family following the "absolute tragedy". "It is clear that an awful error has occurred, and as the regulator for pharmacists, we are working on understanding every detail of what happened, what went wrong, how it went wrong, and who was involved. The Pharmacy Council is working promptly to take any immediate steps required to ensure public safety. "Our enquiry and investigation processes are currently underway and, until these are complete, we cannot provide any further details. At the end of the process, we will make any recommended changes to ensure as best as possible an event like this does not happen again." The Pharmacy Council operated within a wider framework of organisations responsible for the protection of public health and safety, Pead said. "We have also referred the incident to other relevant organisations to ensure they can act on any matters that may fall into their remit. "We would emphasise that situations of this nature are extremely rare. No health practitioner goes to work aiming to cause harm, and New Zealanders can have faith that the pharmacists working in their communities and hospitals are vigilant about medicine dosage and patient safety." Pead said the council set the standard that all pharmacists follow a "logical, safe and methodical procedure" to dispense therapeutic products. Every pharmacy would have their own standard operating procedures that covered the dispensing and checking process, he said. "It involves checking the prescription for legality and eligibility, clinical assessment and accuracy check. "The check by a second person (separation of dispenser and checker roles) is considered best practice and is often built into the checking process in a pharmacy's procedures. The pharmacist is responsible for the final check. Sometimes this may not be possible for a sole charge pharmacist, working alone in the dispensary. It is recommended that a second self-check should be carried out, taking a few moments between the prescriptions to 'reset' and performing the final check with care." Health New Zealand and the Ministry of Health released a joint statement to RNZ, extending their "heartfelt condolences" to Bellamere's family. "Health New Zealand and the Ministry of Health take very seriously incidents like these, which while rare, are always thoroughly investigated to identify any lessons that can be learned. "Both agencies involved are acting urgently in undertaking a joint review into this incident, exploring all aspects of the care provided." Health New Zealand is undertaking a serious incident review and the Ministry of Health will be looking at actions taken by health services in the community. "This will occur alongside providing any information requested by the coroner." On Friday, a Ministry of Health spokesperson told RNZ that Medsafe had visited the pharmacy where the medicine was dispensed and completed an initial assessment that the pharmacy was safe to continue operating. "That initial assessment is part of a rapid audit and site visit of the pharmacy which is being carried out by Medsafe, aimed to provide additional assurance that the pharmacy is meeting expected standards." The actions taken by Medsafe and the Pharmacy Council would help inform the ongoing review, the spokesperson said. "These measures are occurring alongside providing any information requested by the coroner." Health Minister Simeon Brown said in a statement to RNZ he was "heartbroken" for Bellamere's family who had "inexplicably lost their baby in tragic circumstances". "My thoughts are with them at this incredibly difficult time. "I am advised that the Pharmacy Council has taken immediate action and is investigating this incident. The council has also advised other appropriate regulators."
