
Cognitive Therapy Offers Sustained Relief of Low Back Pain
The three-armed RESTORE trial assessed adult patients in Australia with chronic disabling low back pain, showing effectiveness for CFT with or without motion sensor biofeedback compared with usual care for up to 1 year.
In this new extension analysis of RESTORE, both CFT treatments were still more effective than usual care at 3 years, with greater reductions in both pain intensity and activity limitation.
Interestingly, no differences in outcomes were found between the CFT-only group and the CFT plus biofeedback group.
'It shows that if we empower patients with knowledge and skills to manage back pain, including future flare-ups that are almost inevitable, then we can markedly reduce the massive personal and societal burden of back pain,' lead author Mark Hancock, PhD, Spinal Pain Research Center, Macquarie University, Sydney, Australia, told Medscape Medical News.
'We hoped to see these results, given the intervention design, but we were still somewhat surprised how well the effects lasted over time,' he added.
The findings were published online on August 5 in The Lancet Rheumatology .
Building on 'Unusual' Initial Findings
Hancock noted that most treatments for low back pain produce small, short-lasting benefits.
'The CFT intervention is specifically designed to have long-term effects, as it gives people the skills, knowledge, and confidence to self-manage. The results we previously published showed the effects were sustained at 1 year, which is unusual, and we wanted to see if it remained effective at 3 years,' he said.
In the original RESTORE trial, the investigators recruited adult patients who had low back pain lasting more than 3 months and randomly assigned them to receive one of three treatments: CFT only (n = 164), CFT plus biofeedback (n = 163), or usual care (n = 165).
The CFT groups received up to seven treatments over 12 weeks plus a booster session at 26 weeks, all of which were delivered by trained physiotherapists.
Investigators noted that CFT targets biopsychosocial barriers to recovery and has three components that are based on an individual patient's goals. These components include 'making sense of pain,' exposure with control, and lifestyle change.
Both CFT groups wore movement sensors during their sessions, but only the CFT plus biofeedback group and their physiotherapists had access to the data.
Usual care involved treatment that was chosen by the patient themselves and/or recommended by their healthcare provider.
The new 3-year follow-up included 312 of the original participants (mean age, 48 years; 60% women) and between 60% and 65% of each of the original treatment groups.
The extension's primary outcome was self-reported pain-related physical activity limitation, as measured on the Roland-Morris Disability Questionnaire. The secondary outcome was pain intensity, as measured by the numeric pain rating scale.
Long-Term Pain Reduction
Results showed that CFT with biofeedback was more effective in reducing activity limitation at the 3-year mark vs usual care (mean difference, -4.1; P < .0001), as was CFT alone (mean difference, -3.5; P < .0001).
Additionally, CFT with or without biofeedback was more effective in reducing pain intensity vs usual care (mean differences, -1.5 and -1.0, respectively; P < .0001 for both).
For both measures, the differences between the two CFT groups were not significant.
Sensitivity analyses showed similar, although slightly smaller, effects.
In addition, 49% of the CFT plus biofeedback and 43% of the CFT-only groups maintained recovery level scores between the 1-year and 3-year follow-ups compared with 17% of the usual care group.
'These long-term effects are novel and provide the opportunity to markedly reduce the effect of chronic back pain if the intervention can be widely implemented,' the investigators wrote.
They noted that this would include an increase in clinician training and replication studies in other populations.
'This type of biopsychosocial intervention is widely recommended in guidelines, but the evidence [for it] is now becoming much stronger,' Hancock said.
'Many clinicians aim to deliver these interventions, but they often find it hard, especially if they were trained in a more pure biomedical way of thinking. There are now more resources to assist clinicians in upskilling in these approaches,' he added.
'Cautious Optimism'
In an accompanying editorial, Dimitrios Lytras, PhD, Department of Physiotherapy, International Hellenic University, Thessaloniki, Greece, applauded how the study was conducted.
'Methodologically, RESTORE is exemplary: a pragmatic, multicenter trial embedded in routine care [and] supported by thorough therapist training,' Lytras wrote.
He added that the results 'offer cautious optimism' and a shift from passive care models to ones that are more patient-centered.
The addition of biofeedback not leading to added benefit could be explained by the fact that CFT 'already incorporates rich feedback mechanisms, making supplementary sensor input redundant,' he noted.
Lytras wrote that the intervention is also low risk and high value.
Durable Functional Gains
Commenting for Medscape Medical News, Sean Mackey, MD, PhD, Redlich professor and chief of the Division of Pain Medicine at Stanford Medicine, Palo Alto, California, said the study showed durable functional gains and modest pain changes.
The dose of seven visits over 12 weeks plus a 26-week booster was 'lean,' but the effects held, which was 'pleasantly surprising,' said Mackey, who was not involved with the research.
However, he did note a few caveats, including that only 63% of the randomized cohort contributed to the 3-year data. In addition, 'those followed were less severe at baseline and did better at 1 year, though loss to follow-up was nondifferential by arm,' he said.
In reflecting on the study design, Mackey noted that an 'attention-matched active comparator to blunt performance and expectancy bias' would have been helpful. He pointed out that the researchers themselves acknowledged that usual care was not contact-matched.
He added that it would also have been valuable to include data on 3-year adverse events, healthcare utilization, and costs.
If future research confirms its generalizability to other countries, Mackey said he would recommend clinicians 'adopt the CFT package' of graded exposure with a control and lifestyle coaching, schedule 6-8 visits plus a 6-month booster, measure function as the primary outcome, and set expectations for about a one-point reduction in pain. He recommended against adding sensor biofeedback because 'it didn't help.'

Nearly a million patients hit by DaVita dialysis ransomware attack
Healthcare institutions have become a favorite target for bad actors, largely because of how easy they make it for attackers. In June, researchers discovered a healthcare data breach that exposed the personal information of around 8 million patients. All of this information was publicly accessible online without any passwords or authentication protocols. The latest healthcare organization to fall victim to a breach is DaVita, which has put nearly a million people at risk. The breach, which the company disclosed in state filings, compromised names, Social Security numbers, dates of birth, health insurance details, medical records, tax ID numbers, addresses and even images of checks made out to the company. DaVita says the incident disrupted internal operations and primarily affected its laboratories. In its latest notice to victims, the company says the cyberattack began March 24, 2025, and continued until April 12. It has not confirmed whether a ransom was paid. Ransomware gang Interlock claimed responsibility on April 25, posting screenshots of alleged stolen documents and saying it took 1.5TB of DaVita's data. The group lists the company on its public leak site, where it pressures victims by threatening to sell or release stolen files. DaVita is offering eligible breach victims free identity restoration services through Experian, with a Nov. 28 enrollment deadline. The company has not confirmed how attackers gained access to its network or the size of the ransom demand. CyberGuy reached out to DaVita for comment but did not receive a response before publication. Interlock, which first appeared in October 2024, has claimed responsibility for the DaVita attack and at least 23 other ransomware attacks, plus dozens more that remain unverified. Healthcare targets have included Texas Digestive Specialists, Kettering Health and Naper Grove Vision Care, all of which reported data breaches in 2025. The DaVita incident is the second-largest U.S. healthcare ransomware attack by number of records this year, behind Frederick Health's January breach. According to Comparitech, there have been 53 confirmed ransomware attacks on American healthcare providers in 2025 alone, compromising more than 3.2 million patient records. A toxicologist's guide to poison ivy's itch and bee stings' burning pain – 2 examples of nature's chemical warfare
Enjoying the outdoors carries the danger of running into nature's less-friendly side: toxic plants and animals. As toxicologists at the University of Virginia's Blue Ridge Poison Center, we see many patients each year suffering from itchy rashes from poison ivy and stings from wasps or bees. Plants and animals deploy toxins most often in order to defend themselves. Learning how they do that and what happens when the human body is exposed to these substances can offer insights on how to prevent or manage these encounters with nature. The goal is not to scare people away from the outdoors, but to equip them with the knowledge to appreciate these organisms' intricate self-preservation strategies and to protect themselves in return. Whether in a remote state park or on a city playground, most people have encountered poison ivy. This plant is recognizable by its characteristic arrangement of leaves growing in groups of three with edges that vary from smooth to jagged. It can take many forms: a single small plant, a mass of ground cover, a small bush, or a climbing vine reaching many feet up a tree or building. READ: The molecule that makes you itch also stops inflammation Poison ivy – its scientific name is Toxicodendron radicans – and its close relatives poison oak and poison sumac contain an oily substance called urushiol. This chemical is found in every part of the plant: the leaves, roots, stems and even the small white berries it produces in late summer. About 75% of people will develop an allergic reaction on contact with urushiol. Urushiol has antimicrobial properties, and scientists think its job in the poison ivy plant is to protect it from diseases. Because it is so oily, urushiol spreads easily. It can transfer from the plant to your skin, clothes, garden tools or even your pets. Direct plant contact isn't the only risk: If urushiol is on your clothing or a pet's fur and your skin later brushes against it, you can develop the same rash as you'd get from directly touching the plant. Urushiol triggers a delayed allergic reaction. When the oil touches your skin, it binds to skin cells, changing their shape. A molecule called CD1a then clocks urushiol as a foreign substance, prompting the immune system to mount an attack on the cells – hence the rash. READ: Are twins allergic to the same things? The symptoms do not appear instantly; the rash usually appears 12 to 48 hours after exposure. It often starts as redness and itching, then develops into small bumps or fluid-filled blisters. The reaction can be mild or severe, depending on how sensitive you are and how much urushiol got on your skin. The rash itself isn't contagious. Fluid from the blisters doesn't spread it. What spreads the rash to other areas of your body or to others is the urushiol lingering on your skin, clothing, tools or pets. Once the oil is adequately washed away, the rash can't spread to other people or to other areas of your body. If you have touched poison ivy, wash the area as soon as you can with soap and water and change your clothes if possible. After that, the rash will eventually resolve on its own. You can help alleviate symptoms by using a topical steroid or anti-itch cream on the rash. In severe cases, or if the face is affected, patients may require oral steroids to treat the symptoms. Bees and wasps are most active in the late summer. Because of this, we receive more frequent poison center calls about them during this season. Bees and wasps generally sting to defend their hives or nests or to protect themselves from perceived threats. They store venom in their abdominal sacs. When they sting, the venom flows through their stinger and is injected into their target's skin. This venom is a clear, slightly acidic liquid loaded with various active ingredients. For example, it contains enzymes such as phospholipase A2 that break down cell membranes, and peptides such as melittin that cause pain. The venom also contains natural chemicals such as histamine and epinephrine that affect blood vessels and the immune system. Unlike with poison ivy, where the immune system's reaction to the substance causes irritation, with bee and wasp stings it's primarily the substance itself that causes pain – although immune response can still play a role. As soon as the venom enters a person's skin, their body reacts. READ: Are you really allergic to penicillin? A sharp, burning pain comes first as the components of the venom begin to inflict damage, followed by redness and then swelling of the area. Symptoms commonly peak within a few hours and fade within a day. However, some people have stronger reactions with larger areas of swelling that can last for several days. This is because everyone's immune system is slightly different, and some people tend to have stronger reactions than others to foreign substances. In rare cases, the immune system overreacts, releasing large amounts of histamine and other chemicals all at once. Histamine is most often released in response to a foreign substance, causing symptoms of an allergic reaction. This can lead to anaphylaxis, a severe allergic reaction that can make breathing difficult, lower blood pressure and cause airway swelling, and which can quickly become life-threatening. READ: How to manage hay fever in your pets Getting stung multiple times at once can also be life-threatening due to the sheer amount of venom injected, even in people without a bee venom allergy. If you're stung and the stinger is stuck in the skin, it should be removed immediately by the quickest means available. Bee stingers are barbed and can continue to deliver venom for up to a minute. Most bee or wasp stings require only symptomatic treatment, such as an over-the-counter steroid cream or oral antihistamine to reduce itching and swelling. However, people who begin to develop more severe symptoms such as full body hives, vomiting or difficulty breathing should immediately seek emergency care. Anaphylactic reactions require rapid treatment with a medication called epinephrine and close monitoring in the hospital. Christopher P.
