Scientists believe they may have found a bacterial toxin in E.coli which leaves behind specific patterns of DNA mutation which are present in bowel cancer especially among young people
A food bug could be behind a spike in in bowel cancer cases among young people, scientists have discovered.
E. coli produces a toxin which may be driving up the rates of the cancer around the globe and especially in the UK. The bacterial toxin – called colibactin – is capable of altering DNA and is produced by a strain of E. coli, though not the strain linked to infection such as diarrhoea in some people. Researchers, including from the UK and backed by Cancer Research UK, found that exposure to colibactin in early childhood imprints a genetic signature on the DNA of bowel cells, which may then increase the risk of developing bowel cancer before the age of 50.

A previous study published in December found rates of bowel cancer in young people are rising more sharply in England than in many other countries around the world. For those aged 25 to 49, England is among the countries with the biggest rise, averaging a 3.6% increase every year in the decade up to 2017.
Data provided by Cancer Research UK up to 2019 further suggests that bowel cancer has seen a 52% increase in incidence rates for adults aged 25 to 49 since the early 1990s. There are around 2,600 new bowel cancer cases in people aged 25-49 in the UK every year and around 44,100 new cases among all ages.
Work has been ongoing to discover why more younger people are developing bowel cancer, with experts believing poor diet, more ultra-processed foods, obesity and a lack of exercise are playing a role. Now in the latest study, led by the University of California San Diego (UCSD) and published in the journal Nature, researchers have uncovered another possible culprit.

Experts examined 981 colorectal (bowel) cancer genomes from patients with both early and late-onset disease in 11 different countries. They found colibactin can leave behind specific patterns of DNA mutations that are 3.3 times more common in early-onset bowel cancer cases (in the study this was adults under 40) than in those diagnosed after the age of 70.
These mutation patterns were also particularly common in countries with a higher rate of early-onset bowel cancer. Senior author, Ludmil Alexandrov, a professor at UCSD, said: 'These mutation patterns are a kind of historical record in the genome, and they point to early-life exposure to colibactin as a driving force behind early-onset disease.'
Study first author Marcos Diaz-Gay, a former postdoctoral researcher, said: 'When we started this project, we weren't planning to focus on early-onset colorectal cancer. Our original goal was to examine global patterns of colorectal cancer to understand why some countries have much higher rates than others. But as we dug into the data, one of the most interesting and striking findings was how frequently colibactin-related mutations appeared in the early-onset cases.'

The study also found that colibactin-related mutations account for around 15% of what are known as APC driver mutations – some of the earliest genetic alterations that directly promote cancer development – in bowel cancer. Professor Alexandrov said: 'If someone acquires one of these driver mutations by the time they're 10 years old, they could be decades ahead of schedule for developing colorectal cancer, getting it at age 40 instead of 60."
The work is part of Cancer Grand Challenges team Mutographs, funded by Cancer Research UK. Researchers are now developing early detection tests that analyse stool samples for colibactin-related mutations. The director of Cancer Grand Challenges, Dr David Scott, said: 'Globally and in the UK, we're witnessing an alarming increase in some types of cancer in people under the age of 50.
'It is unlikely that there will be one clear driver but Cancer Grand Challenges scientists are racing to solve this puzzle, and our Mutographs team has uncovered a surprising clue. Many early-onset colorectal cancer patients appear to have been exposed to a toxin, called colibactin, produced by some strains of the bacteria E. coli in early life. It's unclear how the exposure originates, but we suspect that a combination of factors – including diet – may intersect during a crucial phase in the development of the gut microbiome.
'This study adds an important piece to the puzzle of early-onset cancers, but it isn't conclusive, and more research will be needed to establish a definitive link between colibactin and an increased risk of early-onset colorectal cancer. Other Cancer Grand Challenges teams, like Optimisticc and Prospect, are looking deeper into the microbiome and other environmental factors to uncover what's behind the global rise.'
Professor Sir Mike Stratton, Mutographs team lead and senior group leader at the Wellcome Sanger Institute, said: 'Our research has allowed us to generate the hypothesis that the presence of colibactin leads to an increased number of mutations in colon cells, which then causes a greater risk of colorectal cancer at an early age. If this turns out to be correct, we can explore preventive measures such as tests that tell us if the toxin, or the bacterium that makes it, is present, and finding ways to eliminate them from our bodies at a young age.
'We know that diet and lifestyle choices drive the risk of developing colorectal cancer, but this study has opened up a new and exciting route we can take when researching how to lower the rate of early-onset colorectal cancer.'

It is also increasingly being diagnosed in young people globally. Here's what you need to know about bowel cancer, including symptoms and signs to watch out for. What is bowel cancer? 'Bowel cancer, also known as colorectal cancer, affects the large bowel, which is made up of the colon and rectum,' explains Dr Lisa Wilde, director of research, policy, and influencing at Bowel Cancer UK. 'Most bowel cancers develop from pre-cancerous growths, called polyps. But not all polyps develop into cancer. 'If your doctor finds any polyps, they can remove them to prevent them becoming cancerous.' Here are some symptoms to look out for… Bowel changes A change in your pooing habits could be an early warning sign. 'You might be going more or less often, or have diarrhoea or constipation that might come and go,' says Wilde. Blood 'One of the most recognisable symptoms to look out for is blood in your poo or bleeding from your bottom,' highlights Lindsay Easton, CEO of Bowel Research UK. 'This can be a sign of a less serious condition, but it's best to get this checked out with your GP as soon as you can.' Pain or lump in your stomach 'A pain or lump in your tummy is a symptom of bowel cancer,' says Wilde. Weight loss Losing weight when you are not trying is also common in people with cancer. 'This may because of reduced appetite, but not always, some people lose weight even when they are eating normally,' explains Dr Angela Rai, GP at The London General Practice. 'This is called cachexia, and the body may not be absorbing all the protein, fat and carbohydrate from the food you eat. 'Cancer may release chemicals in the body that contribute to the loss of fat and muscle.' Unexplained tiredness Feeling constantly tired and not knowing why is also a symptom. 'There are many causes of fatigue and this should be investigated,' says Rai. 'Bowel cancer can cause anaemia, and the lack of healthy red cells causes fatigue. 'A simple blood test can detect this. Anaemia can be an early warning sign of bowel cancer and occurs when the tumour starts to bleed causing a reduction of red blood cells and a reduction in a protein called haemoglobin, which carries iron.' When should you see a GP about these symptoms? 'Not everyone will have all the symptoms of bowel cancer and sometimes symptoms may come and go,' says Wilde. 'If you have any of the above symptoms, or if your pooing habits just don't feel normal for you, our advice is to contact your GP straight away. 'They will want to hear from you and may give you an at-home test to help decide whether your symptoms need further investigation.' When should you ask for an urgent GP appointment or get help from the NHS 111 or go to A&E? 'The NHS advises people to call 111 if your poo is black or dark red or you have bloody diarrhoea, and to go to A&E or call 999 if you're bleeding non-stop from your bottom and there's a lot of blood,' says Easton. Can you get an at home test? How does this work? 'If your GP wants to rule out bowel cancer, they may ask you to do a Faecal Immunochemical Test (FIT), which is a test you do at home that that looks for blood in your poo,' explains Easton. 'These are the same tests sent out to people as part of the bowel cancer screening programme, and we would encourage anyone eligible for screening to take part when they are invited because early diagnosis is so important.'

Scientists celebrate bowel cancer breakthrough in bid to tackle surge in young people

Scientists have discovered that bowel cancer cells can transform into skin or muscle cells, allowing them to spread more aggressively. This breakthrough offers hope for treating the increasing rates of the disease, especially among young people. A study conducted by the Cancer Research UK Scotland Centre and the University of Edinburgh revealed that a critical step in aggressive bowel cancer involves cells losing their original identity, a process known as cellular plasticity. Researchers found that the disease spreads when colonic cells begin to resemble squamous cells, which form skin, or muscle cells. Bowel cancer is the second most common cause of cancer deaths in the UK. It claims the lives of 16,800 people in Britain, including 1,700 in Scotland, every year and is increasingly being diagnosed in younger people internationally. A recent study by the American Cancer Society published in The Lancet Oncology showed early-onset bowel cancer rates in adults aged 25-49 are rising in 27 of 50 countries studied, and increasing faster in young women in Scotland and England than in young men. Scotland is disproportionately affected with around 4,000 people diagnosed each year overall, according to Cancer Research UK. The latest study found bowel cancer cells can adapt to resemble skin cells, which can tolerate much harsher day-to-day conditions due to their role and position protecting the outside of the body, and also muscle cells, both of which are more 'robust'. Cellular plasticity was found to be an important element in bowel cancer metastasis – when it spreads and becomes harder to treat. Researchers hope identifying this and preventing it could help make current treatments more effective and stop the disease from spreading. The study also examined a particular gene called Atrx which was already associated with aggressive forms of bowel cancer. Using mice and human tissue samples, researchers found the loss of this gene resulted in increased metastatic tumours which spread from the bowel to the liver, lymph nodes and the diaphragm. Key to the ability of these cells to spread is that they shed their identity of colonic cells and resembled squamous cells which form skin, or cells that resemble muscle. The paper, 'Loss Of Colonic Fidelity Enables Multilineage Plasticity And Metastasis', is published in Nature. The research received funding from the Medical Research Council and the European Research Council. Dr Kevin Myant, of the Institute of Genetics and Cancer at the University of Edinburgh and the Cancer Research UK Scotland Centre, said: 'With more and more younger people being diagnosed with bowel cancer, it's vital we understand how this disease grows and develops. 'Our research has discovered one way that aggressive bowel cancer is able to spread is by 'shapeshifting' to resemble skin or muscle cells rather than bowel cancer cells. 'This finding will hopefully allow us to develop new treatments to stop these cells changing and prevent the cancer spreading, when it becomes much harder to treat.' Lead researcher Dr Patrizia Cammareri said: 'Skin cells can tolerate much harsher day-to-day conditions than other types of cells – due to their role and position protecting the outside of the body – so this may be a strategy to help the bowel cancer cells become more robust and enable them to spread around the body. 'Metastasis is a leading cause of cancer death and a key focus for cancer research, so this finding could be pivotal in halting the progression of aggressive cancer and providing better outcomes for patients.' Cancer Research UK director of research, Dr Catherine Elliott, said: 'Diagnosing and treating cancer early and preventing spread to other parts of the body offers the best chance of a positive outcome for patients so research like this, which could lead to new ways to stop that spread, offers great hope. 'Bowel cancer is of increasing concern globally, which is why we invested £5.5m to the CRC-STARS initiative (Colorectal Cancer – Stratification of Therapies through Adaptive Responses) jointly led by our Cancer Research UK Scotland Institute, which will bring together more than 40 bowel cancer experts, including researchers who worked on this project, to find new and kinder ways to tackle this disease.'

Woman's easy to dismiss problem was deadly cancer linked to common infection that's soaring in young people

A mother-of-two has told of the shock of being diagnosed with deadly mouth cancer while seven months pregnant, despite no family history or obvious risk factors. When Holly Stubbs, from County Durham, noticed a small lump in her tongue, she assumed it was related to pregnancy hormones. She first visited a dentist, who referred her to an oral health specialist, who confirmed her suspicions that it was nothing to worry about. The doctor assured her that, as she didn't drink alcohol or smoke, it was likely a 'pregnancy tumour, known medically as a pregnancy granuloma. The non-cancerous overgrowth of tissue is found in five per cent of pregnant women's gums and tongues. However, the doctor removed a sample of the growth and sent it off for a biopsy just to be sure. At the beginning of May, the results revealed the devastating truth—she had mouth cancer. She said: 'It's just bad luck. It's one of those things. I have no risk factors, but here we are, and we know we haven't removed it all.' Ms Stubbs' diagnosis, shared with 50,000 viewers on TikTok, comes amid a rise in cancers of the mouth and throat in younger people over the last decade. Cancer Research UK data shows a 60 per cent rise in women and a 34 per cent rise in men aged 25 to 49, between 1993 and 2019, developing head and neck cancers, including those of the oral cavity. And a 2024 report by the Office for Health Improvement and Disparities (OHID) and the University of Sheffield found the rise in head and neck cancers was driven by a rise in oropharyngeal cancer. This is disease that starts in the part of the throat just behind the mouth and includes tonsil cancer and cancer in the back of the tongue. Doctors have found that oral sex is the biggest risk factor for this type of cancer — outpacing smoking, alcohol consumption and an unhealthy diet. This is because the acts can lead to an HPV infection at the back of the throat or near the tonsil. HPV is a common virus spread through close contact, including sex, and is usually harmless, but in some cases—for reasons not fully understood—it can trigger cancerous changes in healthy tissue. Some 70 per cent of cases of mouth and throat cancers are caused by HPV, according to Cancer Research UK. Experts say those with multiple oral sex partners have an up to nine-fold increased risk of throat cancer. Currently, mouth cancer is the sixth most common type in the UK, with 8,846 new cases diagnosed each year, and around 3,000 deaths. There is a vaccine for HPV which is known to help slash the risk of oral cancer. It is more than 80 percent effective and available in much of the developed world. It is a two-dose vaccine now available for children between ages 11 and 12. The shots come 12 months apart. For people who missed that window, a three-dose shot is available to people 15 to 26. The vaccine has been offered to girls in England and Wales since September 2008 before being widened to include boys in 2019. On the day of Ms Stubbs' diagnosis, the business owner had seven hospital appointments. This included giving her blood thinners as both surgery and pregnancy put her at a higher risk of blood clots. To protect her unborn baby's lungs, she had to have steroid injections as her child had to be delivered sooner than expected. The mother also needed an operation to remove part of her tongue—including the cancer—at 33 weeks pregnant, which left her temporarily unable to talk or eat. The mother revealed the heartbreak of the moment she broke the news to her two young children. 'We've had to give them as many details as we can, without, you know, traumatising them—We're going to just take one day at a time,' she said.' A week ago, during an appointment with her cancer specialist, she learned that the cancer was larger than expected. While doctors were confident they removed all the cancer from her tongue, they said there is a 30 per cent chance it has reached the lymph nodes, meaning it is likely to spread to other areas of the body. The surgeons will perform a second operation to remove them and send the samples off for testing. Ms Stubbs is now awaiting the birth of baby before her next surgery. It is important to catch the early signs of mouth cancer for a better chance of survival as it can spread rapidly. Dentists can help spot the early signs, as can doctors. Tell-tale symptoms include ulcers that last longer than a fortnight, swelling or lumps, red or white patches, loose teeth, chronic sore throat and numbness.
