What To Know
The VA has announced that, effective immediately, veterans no longer need a second VA physician to review and approve their eligibility to receive non-VA health care. Instead, the determination will rest solely on the veteran and their referring VA clinician.

VA Cuts
Veterans groups and policy analysts have raised concerns that deeper cuts to staff and resources could push more veterans into non-VA care, potentially increasing costs for the government and affecting care quality.
Like other departments, the VA has not been immune to staffing cuts ordered by the Trump administration as part of a large effort to reduce the size and cost of the federal government. The VA plans to reduce its workforce by more than 17 percent, about 82,000 staffers, according to Reuters.
VA Secretary Doug Collins said the cuts would be achieved without making a dent in health care or benefits to veterans.
Veterans marches were held around the country in March to protest the personnel cuts.

Now, we're making it even easier for Veterans to get their health care when and where it's most convenient for them. Everett Kelley, the national president of the American Federation of Government Employees (AFGE), said in a statement shared with Newsweek in March regarding VA staffing cuts: "The VA has been severely understaffed for many years, resulting in longer wait times for veterans in need. The DOGE plunder of career VA employees, adding to the illegal mass firings of thousands of probationary employees, can only make matters worse. Veterans and their families will suffer unnecessarily, and the will of Congress will be ignored."

Valley Fever, also known as coccidioidomycosis or "cocci," is a disease caused by a fungus that grows in the soil in some parts of California and other southwestern states. It can affect both people and animals, and is spread through spores in the air that are breathed into the lungs, causing respiratory symptoms such as cough, fever, chest pain, and tiredness or exhaustion. Given the similarity in symptoms to other respiratory diseases, such as pneumonia and even COVID, it is often misdiagnosed or undiagnosed, the Centers for Disease Control and Prevention (CDC) reported, meaning some may be given ineffective treatment or face delays in getting the right treatment. While some people may breathe in the spores and not get sick, others are at increased risk of developing a more severe illness, although this is rare. Pregnant women, those with compromised immune systems, those over 60, those with diabetes and those who tend to be more exposed to dirt and dust, such as construction workers, are all at higher risk. Between 5 to 10 percent of people who get Valley Fever may develop serious or long-term problems in their lungs, while 1 percent may have an infection that spreads from the lungs to other parts of the body, such as their skin, bones, joints, or brain, according to the CDC. Many with the disease can recover on their own in weeks or months without treatment, but some may need antifungal medication for more serious infections and may require hospitalization. Where Are Cases The Highest? Valley Fever gets its name from the Central (San Joaquin) Valley as that was where cases originally were the most rife in the state, although cases have been increasing more recently in other parts of the state. The top counties for Valley Fever cases include: Fresno Madera San Joaquin Stanislaus Kern Merced San Luis Obispo Tulare Kinga Monterey Santa Barbara Ventura As of July this year, Kern had the highest number of reported cases compared to any other county, with as many as 1,945, although this was a decrease compared to July numbers in previous years. Fresno had 576 reported cases by July 31, a marked increases on 2024 and 2023 July numbers. Cases in Monterey have been rising more than in any other county though, and it was the only state which saw an increase in cases of between 200 to 300 percent across 2023 to 2025. Why Are Cases Increasing? There are a number of factors that could be impacting the prevalence of Valley Fever fungus, including that in recent years, climate change has meant California has had higher levels of drought and rainfall. During times of drought, when there are higher-than-usual temperatures alongside lower-than-usual rainfall, Valley Fever fungus can survive in the soil even when other microbes tend to die off due to the lack of water. While the fungus is believed to be dormant during a drought, so less people are likely to breathe it in, when the rain returns, the fungus can start growing again and spreading spores in dust and dirt, meaning more people can be exposed. According to the California Department of Public Health, cases of Valley Fever in the state have historically been found to be lowest during years of drought and highest in the years immediately after a drought. As it has been predicted that California is likely to remain in a continued drought-rainfall cycle in the coming years due to climate change, prevalence of Valley Fever cases seems likely to continue increasing. There is also concern among experts that climate change will see Valley Fever fungus spreading all along the coast up to Canada.

The AIDS Crisis Offers a Warning About Trump's Research Cuts

During a sweaty night in Houston 33 years ago, on Aug. 19, 1992, I spoke to the Republican National Convention and, via television, to millions of others. My speech, 'A Whisper of AIDS,' took 13 minutes of the four or five years I was told I had left. I had AIDS. Everyone said it would kill me. However, I did not die. Thanks to incredible medical research, AIDS was converted from certain death to possible life for those with access to new drugs. Today about 1.2 million Americans live with HIV/AIDS and 50,000 or so are added to this total each year. Thanks to drugs many people can't afford, an AIDs diagnosis is no longer a death sentence. Medicaid is redemptive. Federal resources save lives. And I am alive to bear witness to the danger that still lurks in our communities, to the enormous cost already paid in money and lives, and to the tremendous advancements we are making against this disease. If we're willing to learn, our experience with AIDS offers some lessons. For example: Science, if persistently supported, can generate miracles. Science has kept me alive all these years. Science has virtually eradicated vertical (mother-to-child) HIV transmission for a few pennies per person. The miracles are within reach. But if scientific funding is stopped, so are the miracles. The Trump Administration has gutted America's AIDS eradication program and HIV research initiatives. Republicans have simultaneously provided a historic tax cut for wealthy Americans. The unpleasant truth is that these policies are a reflection of a broader belief that some lives are more valuable than others. As the philosophy goes: Infants in, say, Sudan can be allowed to die because their lives aren't as important as American's. And funding for the Supplemental Nutrition Assistance Program (SNAP), which feeds hungry families and school kids, can be cut because their lives don't matter as much as wealthy Americans need a tax break. Burdening the poor with greater poverty while lightening the tax load of the most wealthy, to paraphrase Bill Gates, smacks of the richest people in the world killing the poorest children in the world. The dilemma we faced with the AIDS crisis, and we face again today with President Donald Trump's reckless dismantling of domestic and international programs, is that those setting the rules believe they themselves will not be impacted. What it will take to change minds, as we learned in the AIDS epidemic, is a personal encounter with the truth—and with the repercussions of their actions. When a Republican Congressman who voted for Trump's budget learns that his mother's rural nursing home has been closed, and there's no other one within a hundred miles, then he may care. When his eight-year-old daughter is given a terminal diagnosis, and his prayers for a miracle are not answered because research has been starved of needed funds, then he may care. When the consequences are close enough, personal enough, painful enough, we begin to care. When we care enough about hunger, we can and will solve it—just like when we cared enough about AIDS, we were able to make huge strides. But right now, we simply do not care enough. The AIDS epidemic taught us that until we are personally touched by the truth, we're not likely to care; and until we care, we'll stand by, hands in pockets, looking the other way. Cuts in USAID programs alone will result in the deaths of 14 million people, maybe more, who might have otherwise lived. But if their deaths are in another place, somewhere we won't be bothered by seeing them, we just don't care. If we wait until we care enough, we'll learn the lesson of Pastor Niemoeller who said of Nazi Germany: 'They came after the Jews and I was not a Jew, so I did not protest. They came after the trade unionists and I was not a trade unionist, so I did not protest. They came after the Roman Catholics and I was not a Roman Catholic, so I did not protest. Then they came after me, and there was no one left to protest.' It's an old warning, one I cited in a speech to Republicans 33 years ago. If we wait too long, and if we refuse to care about the lives of others, we will all eventually feel the consequences. Then we will care.

More than 200,000 US veterans are living with prostate cancer. About 1 in 5 men who have served in the military will be diagnosed with the disease. Many of these survivors have experienced a range of physical and psychological effects, including urinary incontinence, erectile dysfunction, posttraumatic stress disorder, and treatment regret. In this panel ReCAP, produced in partnership with the Association of VA Hematology/Oncology (AVAHO), Drs Michael Goodman, Michael Chang, and Jeffrey Jones discuss how healthcare professionals can address the needs and concerns of these patients. The panelists begin by reporting that veterans are disproportionately diagnosed with prostate cancer earlier than men in the general population, and typically they have more aggressive cancers. The doctors next examine treatment options for these men and how related effects of surgery, radiation, and androgen deprivation therapy affect those decisions. They also discuss the psychosocial effects of prostate cancer, including the risk for embarrassment from incontinence and erectile dysfunction as well as the psychological burden relating to risk for recurrence. Finally, the experts emphasize the importance of multidisciplinary care and shared decision-making. They outline how urologists, oncologists, and radiation oncologists need to work together with their patients to ensure the physical and psychological needs of veteran prostate cancer survivors are met.
