A damning report into the operating theatres at Wales' biggest hospital has revealed crime, racism and an "unsafe" workplace.
The 22-page comprehensive service review of the theatres at University Hospital of Wales, Cardiff, reveals that staff felt serious problems were not dealt with by leaders, including a series of thefts and the discovery of illegal drugs in a staff locker. The findings include:
"Inappropriate and racist remarks" were made by a staff member who said a group of Indian nurses needed name badges because they "all looked the same".
Some anaesthetic practitioners were not always in theatre for the whole operation, instead watching Netflix in the anaesthetic room.
Female staff felt unable to leave anything of value in the changing room because of the "regularity" of theft, which included money, phones, computers and clothing going missing.
An "atmosphere of fear" had gone "a long way to destroying the bonds which allow people to work effectively together".
One manager "actively sabotaging" a colleague in a newly created role in the clinical lead team.
The report includes many more alarming findings, which we will add to the live blog below.
Cardiff and Vale University health board said the report was a "difficult read" and apologised to those affected. It vowed to implement the long list of recommendations.

CCTV captures Genevieve Chenneour, 27, fight back in front of stunned customers in the Joe & The Juice in Kensington, west London, after Zacariah Boulares, 18, had pounced. The teenage thug admitted robbery and possession of an offensive weapon and will be sentenced next month. ‌ Ms Chenneour, born in Northallerton, North Yorkshire, said: "They didn't expect me to stand up for myself – but I did. Getting a coffee shouldn't be something you need your wits about you for. I'm so grateful to the staff at Joe & The Juice – they were incredible during the incident and when I went back to see them after." ‌ It has now emerged Boulares, a serial phone thief, threatened to behead singer Aled Jones during a terrifying Rolex robbery in Chiswick, west London, in July 2023 - when the yob was just 16. He was sentenced to a two-year term at a youth detention centre, but was released after just 14 months - and able to carry out more crimes, including the one at the Joe & The Juice. Stills from the CCTV at the juice bar show the hooded Boulares lurking behind the actress, who was out with a friend walking her dog. He took the star's phone and assaulted another customer, Carlo Kurcishi. But Ms Chenneour, who has played Miss Clara Livingston in Bridgerton since season three, quickly leapt into action and took down the thief with the help of her friend. The fearless actress put her arm out to block the thief from leaving and then dislodged the phone from his hand. Retrieving the iPhone from the floor, she then used the device to strike Boulares who was left helpless on the floor. The tussle would continue for four minutes, after which the humbled yob left the café without the phone. Police were called to reports of theft and an assault on February 8. Boulares appeared at Westminster Magistrates' Court on May 29, where he pleaded guilty to stealing the phone and assaulting Mr Kurcishi. He also admitted stealing a black leather hand bag from a diner at a pizza restaurant in London's West End on 30 January this year. The teenager, an Algerian national, was remanded into custody and will now be sentenced on July 17 at Isleworth Crown Court in west London. A source was previously quoted as saying: "Genevieve showed real bravery throughout. It was a very shocking incident. It shows what is happening on London's streets on a daily basis." More than 70,000 phones were snatched in London in 2024, out of an estimated 100,000 thefts across the UK. Joe & The Juice, founded in 2002 in Copenhagen, has nearly 400 coffee shops across Europe, Asia and North America. The Mirror has approached the group for comment.

A Powys dad is lucky to still be alive after being stabbed multiple times after a drug deal went wrong, police say. Scott Weale suffered four stab wounds to the chest and lower back near his family home during the lead-up to Christmas. Ricco Douglas, 25, was jailed for more than five years after pleading guilty to wounding with intent to do grievous bodily harm using Weale's own knife which he grabbed during a scuffle. The horrifying attack unfolded in a residential street in Llethyr Bryn, Llandrindod Wells on December 14 which led to armed police attending, and the area cordoned off by crime scene investigators who were treating the incident as an attempted murder. Dyfed-Powys Police said it would "always" respond to community concerns in Llandrindod Wells, adding that Douglas' sentencing proves that their officers "will not tolerate violent on our streets". Newport Crown Court was told that Ricco Douglas travelled from his home in Birmingham on the day of the stabbing to meet with Mr Weale in order to obtain or exchange cocaine. Within 14 mins of being in Llandrindod Wells, Douglas had stabbed his victim. It was believed the attacked took place after as a result of the drug deal going wrong. Douglas was arrested three days later after police used a stinger to stop his car in the Birmingham area. An abandoned vehicle, belonging to Douglas' stepmother, was found on the night of the stabbing with traces of the victim's and the defendant's blood inside. Dyfed-Powys Police Detective Chief Superintendent Ross Evans said: 'This was a violent incident which occurred on a residential street, just before Christmas last year. "It was highly fortunate that there was no loss of life which would have deprived a family of a loved one. 'We know this incident has been a shock for the local community, in particular for those most affected. 'We welcome the sentencing of Ricco Douglas and hope it provides clear reassurance to everyone that we will not tolerate violence on our streets." DCS Evans added: "I would like to thank all the first responders and the staff at the University Hospital of Wales, Cardiff, on the night for the level of care and treatment provided to the victim, which may well have saved his life. "I would also like to thank our police officers and staff, who demonstrated great determination to bring the offender to justice on behalf of the victim and his family and the wider community.' DCS Evans appealed to the public to report any suspicious activity and crimes which can help officers gain vital intelligence, which ensures perpetrators of crime are caught and brought to justice. He added: 'To help us prevent such incidents, I am appealing to anyone with information about any type of serious crime to let us know. "You can speak to local officers, report online or do so completely anonymously via Crimestoppers. 'We will always respond to community concerns, and it is important to remember that we rely on the support of the public of Llandrindod Wells and Powys to keep our local area safe'.

Michael Ross was convicted of the murder of Shamsuddin Mahmood in 2008, with his lawyer opening up on the convicted killer in a new Prime Video documentary. Convicted murderer Michael Ross has been described as "gentle" by his lawyer in a new one-off documentary. The Orkney Assassin: Murder In The Isles premiered on Prime Video on Sunday, June 8, providing audiences insight from officers, journalists, witnesses, and Ross's parents regarding the murder of Shamsuddin Mahmood. ‌ In 2008, Ross faced conviction for the murder of the Indian restaurant waiter, which occurred 14 years prior and recieved a life sentence. ‌ Throughout the trial, Donald Findlay provided Ross' defence, but as disclosed in the documentary, with more than 10 years remaining until parole eligibility, Ross sought new representation. Lawyer Aamer Anwar was hired for the position but expressed his initial doubts about taking on the case. Anwar commented in the documentary: "So when it came in, there was some healthy cynicism. "'Why me? You know my reputation. You know I fight injustice, human rights and on race' but they said that's exactly the reason why Michael wanted me to have a look at the case. "And when I met Michael, he was well mannered, considered, gentle." ‌ Ross, since his sentencing, has consistently proclaimed his innocence and has made three attempts to escape prison, the most recent being a 2018 incident where he tried to scale a fence. Anwar added: "And the fact that in all these years, he's maintained his innocence. "That always strikes me when somebody refuses to take the easy option and go 'you know what, I'm guilty' to try and get out earlier. "But he's refused to do that, he says he cannot do that, he insists he's an innocent man. And it's never ended."
