NHS ordered to make improvements to patient care while cutting costs

The NHS has been ordered to drive improvements in hospital waiting times and cancer care while at the same time cutting costs.
New planning guidance for the health service in England, which sets out national priorities for the year ahead, calls on NHS providers to reduce costs by 1%.
The NHS has also been challenged to raise productivity by 4%.
The guidance stresses the need for the NHS to meet certain challenges in the year ahead, with a particular emphasis on getting the waiting list back on track.
Specific aims set out in the 2025/26 planning guidance include:
– To make sure that 65% of patients on the waiting list for routine hospital care to be seen within 18 weeks, which will help towards the Prime Minister's aim for 92% of patients to be seen within this time frame by July 2029.
– To cut the number of people waiting more than a year for routine hospital treatment to 'less than 1% of the total waiting list by March 2026'. The latest figures indicate that 221,889 people had been waiting more than 52 weeks to start routine hospital treatment at the end of November.
– For a rise in the proportion of people who have suspected cancer to be diagnosed or given the all-clear within 28 days – the target stands at 75% now but hospitals have been told to drive this up to 80% over the coming year – officials estimate that this will mean 100,000 more people will be seen in this time frame when the target is met.
– The Mental Health Investment Standard – which ensures that funding for mental health keeps pace with overall NHS spending – will be retained. The NHS has also been given specific targets on reducing the length of stay in mental health settings and giving more access to children and young people who are trying to access services.
– Hospitals have been told that at least 78% of A&E patients should be admitted, transferred or discharged within four hours by March 2026. Some 71.1% of patients in England were seen within four hours in A&Es last month. The original NHS recovery plan set a target of March 2024 for 76% of patients attending A&E to be admitted, transferred or discharged within four hours, with a further target of 78% set for March 2025.
– Meanwhile ambulance services have been told that response times for so-called category 2 incidents – which includes heart attacks, strokes and sepsis – should improve to an average of 30 minutes. In December, ambulances took an average of 47 minutes and 26 seconds to respond to these calls.
– The guidance also calls for improvements to patient access to GP surgeries, an increase to the number of urgent dental appointments and improvements in safety in maternity and neonatal services.
– There are a number of demands about productivity improvements, including a call to 'reduce agency expenditure as far as possible, with a minimum 30% reduction on current spending across all systems'.
Some 32 goals were set last year but the number has been cut to 18 this year in a move designed to give local NHS bodies more freedom to deliver care to patients, officials said.
Some organisations and charities have expressed dismay over reports that specific priorities set out in 2024/25 guidance will not be repeated in 2025/26.
Fiona Carragher, from the Alzheimer's Society charity, said: 'Alzheimer's Society is shocked to learn that dementia, including a dementia diagnosis target, has been removed from the NHS Operational Planning Guidance, meaning it is not considered one of England's healthcare priorities.
'This glaring omission is unacceptable and sends the message that dementia doesn't matter.'
Meanwhile, Jon Sparkes, chief executive of learning disability Mencap, said: 'Scrapping vital NHS targets for people with a learning disability is a dangerous step backwards and could have serious consequences.'
Amanda Pritchard, NHS chief executive, told MPs on Wednesday: 'Just because something isn't written in planning guidance doesn't mean it's not important and it doesn't mean the work won't happen.'
Health and Social Care Secretary Wes Streeting said: 'If everything is a priority, then nothing is.
'I want to empower NHS leaders to deliver the innovation and reform required to fix the NHS, rather than overload them with targets which have failed to deliver better outcomes for patients.
'This new approach will see the NHS focus on what matters most to patients – cutting waiting lists, getting seen promptly at A&E, and being able to get a GP appointment.
'We've provided significant extra investment and given clear directions to trusts on what we expect, so there is no excuse for failure. We will reward high performance and drive up standards as we deliver on our Plan for Change and build a health service fit for the future.'
The central NHS body, NHS England, plans to make £325 million in savings, including 'reducing the organisational structure by 15%' – it is thought this will lead to around 2,000 job cuts.
Ms Pritchard said: 'The NHS must go further and faster to improve and reform care, and today's guidance aims to deliver more timely treatment for hundreds of thousands of patients.
'In what will undoubtedly be another tough financial year, the NHS will continue its relentless focus on boosting productivity and driving efficiencies for the benefit of patients and taxpayers.
'Our main focus will always be on supporting frontline teams to deliver what matters most to patients – so it is also right that we prioritise and streamline the work of NHS England to ensure we maximise frontline resources.
'Together with Government, we have also honed down national priorities, allowing local leaders maximum flexibility to plan better and more efficient services for their population.'
Responding to the guidance, Matthew Taylor, chief executive of the NHS Confederation, said: 'NHS leaders will welcome the reduction in national targets and clearer priorities that are set out in the planning guidance.
'But we should be under no illusion – this is going to be one of the most challenging financial settlements of recent years and these ambitions will be unbelievably stretching for the NHS.'
NHS Providers interim chief executive, Saffron Cordery, added: 'This guidance is a stark reminder of the difficult decisions and tough trade-offs facing trust leaders as they brace for another challenging year providing vital health services.
'It tasks the NHS with tackling delays and improving patient care in every setting while also demanding stringent efficiency savings.'
Sarah Woolnough, chief executive of The King's Fund think tank, said: 'If achieved, (this guidance) will bring improvements for many patients, but emphasis in these areas will inevitably mean other services get deprioritised.
'Achieving the target of patients being seen for planned hospital care within 18 weeks will seem like a small and isolated victory in four years' time if it meant the Government took its eye off the ball in reforming adult social care, helping the NHS to turn into a prevention-focused service that helps keep people well, and reducing health inequalities between different parts of the country.
'The plan also hopes for ambitious efficiency gains to free up much-needed resources, but few people working in the NHS will think it will be delivered without harming the quality of patient care over the coming year.'
Dr Becks Fisher, from the Nuffield Trust think tank, added: 'The coming year will be particularly tough financially, with NHS trusts being asked to hit ambitious remaining targets while cutting their costs by at least 1%.
'This includes reducing the use of staff overtime, which until now has played a significant role in slowing the growth of waiting lists.'

The use of robotic surgery is set to soar in the NHS in England under plans to expedite care for patients. Officials said that the range of robotic-assisted procedures has widened in recent years and spans multiple areas of medicine. Speaking ahead of a speech at the NHS ConfedExpo conference in Manchester, NHS England boss Sir Jim Mackey said: 'The NHS has pledged to return to shorter elective waiting times by 2029 and we are using every tool at our disposal to ensure patients get the best possible treatment. 'Expanding the use of new and exciting tech such as robotic surgery will play a huge part in this. 'Not only does it speed up the number of procedures the NHS can do, but it also means better outcomes, a faster recovery and shorter hospital stays for patients.' Mr Streeting said: 'Innovative treatments and technologies that help fast track better outcomes for patients is how we transform our NHS and make it fit for the future. 'I know myself how important this is, when the NHS saved my life from kidney cancer with an operation led by a world-class surgeon being helped by a robot. 'Whether it's robotic surgery, our new health data research service to accelerate the development of new medicines, or announcing new artificial intelligence that detects skin cancer, our Plan for Change is driving forward new ways to help cut waiting lists and get patients treated on time again.' John McGrath, consultant surgeon at North Bristol NHS Trust and chair of the NHSE Steering Committee for Robotic Assisted Surgery, added: 'Robot-assisted surgery is a perfect example of innovation improving patients' care and transforming the way the NHS works – the number of procedures being carried is set to rapidly grow over the next 10 years according to our analysis. 'As keyhole surgery continues to develop and scale up in the NHS, it is likely that many of these procedures will be provided with degrees of robot assistance in the future. 'Faster recovery and shorter hospital stays are not only hugely important benefits for patients undergoing surgery, if used efficiently they can have a positive impact on the rest of the system by relieving pressure on services and therefore helping to reduce waiting times.'

MSPs vote to introduce scaled-back social care reforms

MSPs have unanimously passed legislation which will allow people in care homes to receive visits from a named loved one even in restricted measures. The Care Reform (Scotland) Bill will also introduce changes to social care procurement and a new right to breaks for unpaid carers. The legislation was backed by 116 votes to none. Plans to introduce a national care service, which were initially part of the bill, were dropped in January after unions withdrew support and a number of health boards and care organisations expressed concerns. One of the big changes planned under the new law is a legal right to breaks for unpaid carers. This mean councils will have a duty to decide whether a carer is able to take sufficient breaks from their caring role. If they are not, then the local authority will provide support to enable this, such as providing funding for short respite breaks. This policy, given Scotland has around 700,000 unpaid carers, will cost between £196m and £315m by 2035/36, according to the Bill's financial memorandum. However, it remains a fraction of the £13.9bn that unpaid care is currently saving Scotland every year. Improvements to the way information is shared in health and social care - to make it less likely that people will have to repeat their information - as well changes to procurement rules in the sector are also planned. Beefing up the powers that watchdogs can take against failing care providers is also part of the bill. The most high-profile part of the Care Reform (Scotland) Bill is Anne's Law, which allows people in care homes to receive visits from a named loved one even in restricted measures. It is named after Anne Duke, who died aged 63 in November 2021 after being cut off from her family while battling early-onset dementia during the Covid pandemic. The original proposal for a National Care Service, inspired by the NHS, was to take social care provision and staff away from local authorities into a new national agency. That was then dropped in favour of creating a national care board to supervise service delivery and improve consistency - but this failed to win over a growing number of critics. Council body Cosla and trade unions then withdrew their support for the project, while a number of health boards and care organisations also expressed concerns. The plan, which was also subject to a series of delays, was eventually scrapped in January after £30m was spent on the process. Social Care Minister Maree Todd said at the time she was "still committed to the ambitions of the National Care Service" but added the SNP no longer had the support it needed in parliament to pass its original plans into law. Groundbreaking cancer treatment which ‘destroys tumours with sound' to be rolled out on NHS

NHS patients will be the first in Europe to benefit from a pioneering new cancer treatment which uses ultrasound to destroy tumours. The technology, known as histotripsy, offers a non-invasive treatment capable of breaking down liver cancer tissue without surgery, radiation or chemotherapy, with minimal damage to the surrounding organs, according to the Department for Health and Social Care (DHSC). First approved by US regulators in October 2023, histotripsy uses pulsed sound waves to create 'bubble clouds' from gases present in the targeted tissue and tumour. When these bubbles pop, within microseconds, a shockwave is released which is capable of breaking up a mass and killing cancerous cells, while avoiding the harms of radiation and reducing the risks of bleeding, infection, and damage to surrounding non-targeted tissue. Treatment is delivered via a single session – potentially taking just 30 minutes – with limited or no pain, a quick recovery, and can be performed as a day case, according to the DHSC. The first NHS patients are set to be treated this summer at Addenbrooke's Hospital in Cambridge, where the technology – which uses a device called an Edison System, created by US firm HistoSonics – is being debuted after a donation from the Li Ka Shing Foundation. Cancer Research UK describes liver cancer as the 17th most common cancer in the UK and the 8th most frequent cause of cancer death. More than 8,000 people received a new diagnoses in 2022, with liver cancer incidence across the UK having surged by 42 per cent over the past decade, according to the British Liver Trust. So far, more than 1,500 patients worldwide have received treatment using histotripsy, mainly in the United States – following approval by the US Food and Drug Administration in late 2023. According to 12-month follow-up data from HistoSonics' previous clinical trials, which involved patients for whom other treatments had been either unsuccessful or unavailable, overall survival rates after one year were 73.3 per cent for primary liver cancer, and 48.6 per cent for those with secondary tumours. The paper, published in the Annals of Surgery journal in April, noted that both tumour control and survival rates were similar to those among other current treatments. The UK's new device is expected to be fully installed in Cambridge later this year, where it will be used initially to treat patients with primary and secondary liver tumours. Wider research on how this could potentially treat tumours in other organs – such as the pancreas and kidneys – is underway. The technology has been approved in the UK via the innovative devices access pathway, which aims to enable faster approvals of medical devices and treatments which evidence shows are safe, efficient and can fill unmet need. 'This is a strong example of smart, agile regulation in action,' said James Pound, of the Medicines and Healthcare products Regulatory Agency (MHRA). 'It's a major step forward for patients with liver cancer and shows how the UK can be a frontrunner in supporting responsible innovation that meets real clinical need.' While 'regulation is vital to protect patients', the government is 'slashing red tape, so game-changing new treatments reach the NHS front line quicker – transforming healthcare', said health secretary Wes Streeting. Hailing histotripsy as 'an exciting new technology that will make a huge difference to patients', Roland Sinker, chief executive of Cambridge University Hospitals, said: 'By offering this non-invasive, more targeted treatment we can care for more people as outpatients and free up time for surgeons to treat more complex cases. 'The faster recovery times mean patients will be able to return to their normal lives more quickly, which will also reduce pressure on hospital beds, helping us ensure that patients are able to receive the right treatment at the right time.' Fiona Carey, a kidney cancer patient who co-chairs Cambridge's patient advisory group, added: 'This is seriously good news. A new, non-invasive option to treat these cancers is very welcome indeed. 'For patients for whom ordinary surgery is no longer an option, this could make all the difference.'
