Dorothy Readhead was hoping an innovative heart operation to replace a faulty valve would give her a new lease of life – but it turned out to be fatal.
The 87-year-old was suffering from breathlessness, but was not suitable for open-heart surgery.
A new, less invasive procedure to replace the aortic valve – called a transcatheter aortic valve implant (TAVI) – promised her a return to gardening and helping out at her local church.
But doctors at Castle Hill Hospital, near Hull, botched the procedure after trying to force the new valve through the wrong leg, causing a major artery tear and the loss of five litres of blood during a six-hour operation.
Mrs Readhead died a week later, in the summer of 2020, with her family none the wiser about what had occurred.
Now, it has been revealed that a catalogue of surgical errors, doctor in-fighting and omissions from death certificates has led police to investigate the deaths of 11 patients who had the same procedure at Castle Hill's cardiology department between 2019 and 2023.
Documents seen by The Telegraph reveal how 'a rolling power struggle' between consultants charged with running the cardiac unit distracted from caring for patients, while hospital executives alleged that doctors' abuse of the whistleblowing system had been a 'weapon of mass destruction that has harmed patients'.
All of the 11 patients were having a TAVI installed. It involves doctors replacing a damaged valve by accessing an artery, often in the upper leg, and feeding a new valve through a wire all the way to the heart, where it will sit inside the old valve and restore normal function.
At Castle Hill, operated by the NHS Humber Health Care Partnership, the mortality rate for the procedure was three times higher than the national average, according to the BBC.
Meanwhile, one of two independent reviews by the Royal College of Physicians (RCP) into the deaths raised serious concerns about death certificates failing to acknowledge the surgeries as a contributing factor, leaving families in the dark, the broadcaster said.
In the case of Mrs Readhead, she had blockages in her right leg caused by calcified arteries that made it unsuitable for access.
Despite this, surgeons went in through the right leg by mistake. Then, after realising their error, they failed to change tack and instead tried to plough through the blockage three times, eventually causing a major tear of the femoral artery.
By this point, Mrs Readhead had been on the operating table for six hours and had lost five litres of blood.
She died a week later, with her death certificate failing to declare the operation as a cause. Instead, it cited 'hospital acquired pneumonia' and 'severe aortic stenosis' –- the condition the operation was supposed to treat.
Christine Rymer, Mrs Readhead's daughter, only found out what her mother had endured after being approached by the BBC. She said: 'None of that was told to us. None of it. It just feels as if Mum was a guinea pig, which is not nice to think about.'
Mrs Readhead's care was 'graded poor' by the RCP in its 2021 report because of the use of an 'inappropriate access site' during a procedure. It said this 'unfortunately resulted in an avoidable vascular complication'.
An anaesthetist called in to help during the operation wrote in an email that the TAVI team's decisions had 'resulted in a disaster for this patient'. He said there was 'a change of plan without weighing the risks vs benefit for the patient, but having a 'have a go' approach'.
Dr Thanjavur Bragadeesh, the then clinical director of the cardiac unit, called for a serious incident to be declared so there would be a full investigation into the case, which there later was.
He was one of seven consultants who wrote a letter to Chris Long, the hospital's chief executive, and Dr Makani Purva, the medical director, saying they were 'very concerned about the safety and transparency of the TAVI service'.
Dr Bragadeesh was later removed from his position as part of a restructuring of the unit's management, and would take the trust to an employment tribunal.
He argued he had raised concerns around four deaths, but the tribunal ruled against him, saying 26 of 29 complaints had not been made within the required three-month time frame.
Documents from the hearing reviewed by The Telegraph reveal an email written by Dr Simon Thackray, who was associate medical director at Hull Teaching Hospitals at the time, in which he said: 'A rolling power struggle between key individuals has dragged in much of the time and energy that normally individuals would put into service development.'
He said: 'A huge amount of time has been spent investigating retaliatory complaints, dealing with rudeness and incivility, and trying to bring a sense of direction to teams pulling in opposing directions.'
And he added that disclosures about colleagues' wrongdoings and errors were 'a weapon of mass destruction that have harmed patient care in my department to further personal grievances'.
It also revealed that Dr Purva sought advice from the General Medical Council (GMC) about the behaviour of the doctors.
'In a further twist to the long-standing issues between 2 cardiologists, one of them has accused the other of taking away one of his patients and offering him a different treatment option which caused his death,' she wrote.
In another case, Brian Hunter, a former fisherman from Grimsby, was diagnosed with a heart problem at 83 and offered a TAVI procedure.
The RCP's second review found 'a lack of urgency' to treat him so by October 2021, when the operation took place, he was 'a high-risk case… with an increased risk of complication and little margin for error'.
Medics failed to properly deploy the device, allowing blood to leak back into the hear, and he died on the operating table.
'We were led to believe that dad had a heart attack on the table and unfortunately passed away,' Tracy Fisher, Mr Hunter's daughter, told the BBC. 'To find out three years down the line that what your father actually passed from wasn't the truth is torturous.
'I feel angry as well, and so does the rest of the family, that [the hospital] just outrageously lied. At no point do any of us find it acceptable. It's just not.'
The RCP also highlighted serious concerns about the failure to put the failed surgeries on death certificates.
The death certificate of a 73-year-old man who had the valve implanted in the wrong position also failed to contain 'an accurate description' of what happened. An initial version that included 'failed TAVI' was amended to just state 'pneumonia'.
In two other cases, women who died within six weeks of each other had crucial details missing from their death certificates, making them inaccurate, the RCP said.
A spokesman for Humberside Police said: 'An investigation is in the very early stages in relation to deaths following TAVI surgery at Castle Hill Hospital. Inquiries are being carried out and at this time, we can confirm no arrests have been made.'
A spokesman for NHS Humber Health Partnership said: 'We would never discuss an individual patient case in the public domain, but we understand families may have questions and we are happy to answer those directly.
'We have previously written to families who have lost a loved one following TAVI treatment with an invitation to meet and discuss the specific circumstances of their case, and we would reiterate that offer.'
They added that the service has the confidence of regulators and claimed that external reviews had 'shown that mortality rates associated with TAVI are similar to national mortality rates over a four-year period'.
The spokesman said: 'The Royal College of Physicians was invited to review the service in 2021, at the request of the Trust's chief medical officer. The Royal College report concluded that the TAVI service is essential for the Humber and North Yorkshire region and needs to be expanded.
'It stated however that the design of the service should be reviewed and invested in. The report offered a number of actions for improvement and we have delivered against all of those since it was shared with us.
'A key improvement has been the dramatic reduction in the length of time patients wait to have their TAVI procedure, which was shown in the Royal College Report to have been too long, like many other TAVI services across England.'

Though the claim is not new (archived), it circulated in recent years on Facebook (archived), Threads (archived), Bluesky (archived) and X (archived). The story, in all its "right place at the right time" magic, is true. Dorothy Fletcher was 67 years old when she suffered a heart attack while flying to Orlando, Florida, on Nov. 7, 2003. Also on that plane: several doctors headed to a Heart Association conference in the Florida city. Fletcher, originally from Liverpool in England, gave her account of events to the BBC, which ran an article (archived) about the incident in early 2004. Other (archived) British (archived) and American (archived) news media also reported on the incident at the time. That article said Fletcher was traveling to attend her daughter Christine's wedding when she fell ill. We reached out to a company registered in Christine's name where an employee confirmed that Christine was Fletcher's daughter and that Fletcher was still alive and well in April 2025. We could not reach Christine directly for this story. According to the BBC, Fletcher told BBC Breakfast she realized she was having a heart attack while on the plane but was comforted by the number of doctors who sprang to her aid: I only realised when they said is there a doctor on board and all the lights above their heads began lighting up and they all came running towards me. I can't believe there were so many doctors - never mind cardiologists - on a plane. It was so frightening. I had never experienced a heart attack. But to see so many people helping me took the fright away. The article said that Fletcher's flight landed in North Carolina instead of Florida, where Fletcher spent five days in the hospital before attending her daughter's wedding. Fletcher told the BBC she didn't get the names of any of the doctors and only knew they were traveling to Orlando for a conference. "I wish I could thank them but I have no idea who they were other than that they were going to a conference in Orlando," Fletcher said in 2004. Though we did not manage to track down any of the doctors who helped Fletcher, we did confirm with the American Heart Association that it held an event for cardiologists in Orlando around the time Fletcher fell ill. A recent BBC report suggests that cerumen, commonly known as earwax, may harbor biomarkers to aid in the diagnosis and monitoring of conditions such as cancer, Alzheimer's disease, and metabolic disorders. Cerumen Profile The primary function of cerumen is to keep the external auditory canal clean and lubricated, preventing invasion by bacteria, fungi, and insects. Beyond this, cerumen may reflect systemic metabolism by concentrating a broader array of compounds than blood, urine, sweat, and tears. Its relative stability allows the accumulated cerumen to provide long-term snapshots of metabolic changes. Disease Associations Researchers have defined genetically determined wet and dry cerumen phenotypes. In the US, Caucasian, African American, and German women with wet cerumen faced roughly four times the risk of dying from breast cancer compared with Japanese and Taiwanese women with dry cerumen. The study found that Japanese women with breast cancer were more likely to carry the wet-cerumen allele than were healthy controls. However, large-scale studies in Germany, Australia, and Italy failed to confirm these associations. A recent analysis reported that patients with Ménière's disease had lower levels of three fatty acids in the cerumen compared with healthy controls — the first biomarker identified for that disorder. In a 2019 study led by Nelson Roberto Antoniosi Filho, PhD (Federal University of Goiás Goiânia in Brazil), researchers analyzed cerumen samples from 52 patients with lymphoma, carcinoma, or leukemia and 50 healthy volunteers, using a method that identifies volatile organic compounds (VOCs). The researchers identified 27 VOCs that served as diagnostic fingerprints for cancer, predicting the cancer status with 100% accuracy. The assay could not distinguish among cancer types, indicating that these VOCs represent a general response to malignant cells. Another investigation by the same group showed that cerumen analysis can detect metabolic disturbances in premalignant stages when cells exhibit dysplastic changes that may lead to cancer but remain nonmalignant, potentially enabling much earlier intervention. The team is also investigating whether metabolic alterations caused by neurodegenerative diseases, such as Parkinson's disease and Alzheimer's disease, can be detected in the cerumen. Diagnostic Tool Antoniosi Filho and colleagues proposed that cancer, as a mitochondrial metabolic disorder, releases VOCs that accumulate in cerumen, allowing differentiation between healthy and cancerous individuals via an assay they call the cerumenogram. The aim was to develop a cerumenogram as a diagnostic tool to accurately predict certain cancers from a small cerumen sample. Clinical Adoption Hospital Amaral Carvalho in Jaú, São Paulo — a national reference center for oncology and bone marrow transplantation — has recently implemented cerumenograms for cancer diagnosis and monitoring. Future Directions An April 2025 paper in Scientific Reports by Antoniosi Filho concluded that the cerumenogram could: 1. Identify oncologic risk by detecting premalignant cells before cancer onset, introducing a novel screening modality 2. Show that mitochondrial impairment in premalignant cells — such as hypermetabolic inflammation and dysplasia — produces the same VOC biomarkers as malignant cells, distinct from those in benign lesions, opening new paths for risk management and early intervention 3. Correlate cerumenogram findings with established imaging techniques such as 2-deoxy-2-[fluorine-18] fluoro-D-glucose PET/CT (18F-FDG PET/CT) and gallium-68 PSMA PET/CT, demonstrating alignment with clinical results while offering a noninvasive, lower-cost alternative 4. Monitor treatment response and cancer remission, supporting assessment of therapeutic efficacy and cellular return to normal 5. Confirm metabolic indicators of malignancy to guide clinical decisions alongside imaging and biopsy 6. Drive the development of targeted therapies aimed at metabolites overproduced in malignant conditions. In conclusion, the cerumenogram may serve as a valuable assay for evaluating precancerous indicators, cancer progression, and remission, with the potential to reduce mortality, alleviate patient suffering, and lower disease-related costs. Opposition to a bill which would legalise assisted dying in England and Wales is growing among MPs, the BBC has found. The Assisted Dying Bill was supported by 330 MPs last year, passing its first major vote in the House of Commons with a majority of 55 MPs from a wide range of political parties. But at least a dozen MPs who backed it or abstained in November have now said they are likely to vote against it. "I would also be very concerned if legislation produced a situation where people who considered themselves a burden on their families and friends felt pressured to end their life." Fellow Labour MP Josh Fenton-Glynn also plans to vote no, having previously abstained. "If you'd asked me at any point in my life what I thought of assisted dying, I'd have always said in favour," he told the BBC. He has suggested a number of amendments to the bill, and claims he is "still open to supporting" the plans, but doubts it will change after Leadbeater blocked previous similar attempts. "We see quite a few of the amendments which are specifically aimed at stopping coercion being opposed by the supporters of the bill," he said, adding: "I don't think chances to make the bill safer have been taken." The two MPs have added their names to a growing list of their colleagues who have suggested they will switch sides including: Tory former minister George Freeman, Tory MPs Mike Wood and Andrew Snowden, Lib Dem Brian Matthew, Labour's Jonathan Hinder, Reform UK chief whip Lee Anderson and ex-Reform MP Rupert Lowe. Labour MP Debbie Abrahams and Tory MP Charlie Dewhirst, who abstained previously, will vote against. Labour's Karl Turner, who voted in favour at second reading, is now abstaining. Tory MP David Davis has not announced how he plans to cast his next vote, but has publicly backed a number of amendments that would make sweeping changes to the bill. Some MPs have also changed their stance, by supporting the bill. Minister Chris Bryant is understood to be backing it, having abstained in November. Labour MP Jack Abbott, who voted against last November, is also now likely to vote in favour. Others who were hesitantly supportive are now more firmly in favour. Liz Saville Roberts, the leader of Plaid Cymru at Westminster, said she was initially concerned but supported the principle of assisted dying. "I think the safeguards have got stronger," she said. "They have been improved through the bill committee and through the evidence that we heard." One of the changes made as the bill was scrutinised was allowing Senedd members a vote on whether it will apply in Wales. Despite opposition, supporters expect the bill to clear the Commons and go on to the House of Lords. Campaigners backing the bill do not expect many MPs to reverse their views - but with such tight margins, efforts to shore up support are in full swing. One major threat, for supporters of the bill, is MPs deciding to return home to their constituencies instead of voting. Due to a quirk of the parliamentary system, private members' bills are normally debated on Fridays - a day when MPs are normally in their constituencies. There is also growing concern that the government cannot afford to keep giving it parliamentary time. One Labour source told the BBC: "Given the huge amount of difficult issues the government face, from winter fuel payments and Personal Independence Payment to defence and special educational needs and disabilities spending, a tight vote on assisted dying becomes a real problem. "Lots of MPs think No 10 would be unwise to expend political capital continuing with this very divisive issue rather than choosing to fix the stuff we got elected to sort."
