New drug based on gene-editing tech could treat rare, potentially fatal heart disease
SINGAPORE – A new drug based on gene-editing technology could soon provide respite to patients suffering from a rare and potentially fatal genetic heart disease.
Transthyretin amyloid cardiomyopathy (ATTR-CM) is a rare disease of the heart muscle that involves deformed transthyretin (TTR) proteins building up in the heart, nerves and other organs.
As a result of the protein build-up, the walls of the heart become thickened and stiff. The heart cannot then relax properly and get filled with blood, and it cannot pump it efficiently to the rest of the body.
The symptoms of ATTR-CM can be vague and may include numbness in the hands and feet, lethargy and dizziness ; but left undiagnosed and not treated promptly, it could lead to heart failure and death.
The disease affects about 150 people in Singapore, but Assistant Professor Lin Weiqin – clinical director for the heart failure and cardiomyopathy programme at the National University Heart Centre, Singapore (NUHCS) – believes the number could be much higher.
This is because those affected often dismiss the symptoms – including swelling in the legs, numbness in the hands and unsteady walking – as normal consequences of ageing, Prof Lin said, noting that such symptoms can also be attributed to other conditions.
'Not many patients know about this condition, and not many doctors can diagnose this condition accurately as well,' he said, noting that NUHCS first set up a registry for ATTR-CM patients in 2019.
Nexiguran Ziclumeran, a new drug that is also known as nex-z or NTLA-2001, seeks out and removes a portion of the TTR gene, slowing down the production of the disease-causing protein.
It is based on Crispr-Cas9 technology, the Nobel Prize-winning gene-editing tool which has been used to alter DNA sequences.
US biotechnology firm Intellia Therapeutics, which developed nex-z, is now conducting a trial involving 765 patients with ATTR-CM and a history of heart failure worldwide to evaluate its efficacy.
Recruitment for the double-blind trial – where neither the participants nor the researchers know who received the actual treatment and who received a placebo – in Singapore began in 2024.
Eight patients have already been recruited from NUHCS and the National Heart Centre Singapor e, and Prof Lin, who is leading the study here, said they hope to recruit a total of 14 patients by end-2025 .
Prof Lin noted gene editing therapy has already been approved in other countries for treating certain neuromuscular conditions, cancers and inherited blood disorders.
'If this trial is successful, it will be the first DNA-altering treatment used in the field of adult cardiology and offers new hope to patients living with ATTR-CM,' he said.
Patients in Singapore who wish to take part in the trial must be aged between 18 and 90, and diagnosed with heart failure due to ATTR.
They must also be on medication for heart failure and have experienced heart failure in the past year.
As a precaution, only women who can no longer have children are eligible to participate in the trial, as the edited genes could be passed down, said Prof Lin.
One of those recruited is Mr Chua Ah Hai, who began experiencing numbness and weakness in his hands following a car accident in 2018.
Doctors originally diagnosed Mr Chua, who was working as a baker then, with carpal tunnel syndrome – a common condition among those who work with their hands.
An abnormal heart scan, however, suggested a deeper problem, and a heart biopsy confirmed that the father of two had ATTR-CM.
'I couldn't carry heavy things after my condition deteriorated. I stopped work in March 2024, as my employers encouraged me to rest,' said the 61-year-old, noting he often fell down and sustained fractures as a result of his condition.
He took part in the trial as he believed it could benefit his condition.
'I wanted to give myself a new hope, a new lease of life,' he said.
Mr Chua's son and daughter, aged 27 and 26, were tested and found to be positive for the gene mutation that causes the disease, though they have shown no symptoms and have been asked to monitor their conditions.
A parent with the gene mutation that causes familial ATTR-CM has a 50 per cent chance of passing it to each of his or her children, according to the Cleveland Clinic; but not every child who inherits this gene mutation develops ATTR-CM.
Though the outcomes of the study will only be known in about two to three years, early phase clinical trials – whose findings were published in the New England Journal of Medicine in November 2024 – showed 'consistent, rapid, and durable reductions' in the abnormal protein that causes ATTR-CM, with minimal side effects.
Patients enrolled in the trial here have also not reported any side effects, Prof Lin noted.
Another trial will begin to study the effectiveness of nex-z on patients with transthyretin amyloid neuropathy, where the accumulation of TTR proteins results in nerve damage.
The National University Hospital (NUH) and the Singapore General Hospital are recruiting patients for this, with Dr Kay Ng – a senior consultant at NUH's neurology division – leading recruitment.
Should the trials prove successful, those who initially received a placebo will also receive a free dose of nex-z, said Prof Lin, adding that he expects the drug to become commercially available by the end of the decade.

An aortic aneurysm can be caused by many factors, such as high blood pressure, genetic conditions or aortic valve issues. The condition is often under-diagnosed, thus increasing the risk that the bulge in the aorta continues to grow and eventually ruptures along the elastic wall, resulting in rapid, life-threatening blood loss. 'The open-mesh sleeve used to support the weakened aorta in Pears is custom-shaped to a 3D-printed model of a patient's aorta from their CT scan,' said Adjunct Associate Professor Vitaly A. Sorokin, who heads the adult cardiac surgery division at NUHCS' cardiac, thoracic and vascular surgery department. 'Due to the customised nature of this sleeve, it provides personalised long-term structural support to the heart vessel, which is crucial in stopping the aneurysm from growing larger,' he added. Pears is developed by British medical manufacturing firm Exstent, whose founder, Mr Tal Golesworthy, has Marfan syndrome – a genetic disorder that affects the body's connective tissue and can weaken the aorta's walls. An engineer by training, Mr Golesworthy designed Pears as a solution to treat his own aorta, becoming the first person to have the device implanted in 2004. Though the procedure has been around for more than two decades, it has only become more widely adopted internationally over the past 10 years, amid advancements in CT scanning and 3D printing, said Prof Vitaly. NUHCS is believed to be the first centre to introduce Pears in South-east Asia, first performing the procedure in November 2024. Six patients, aged between 40 and 55, have undergone the procedure since it was introduced, all of whom are faring well. Among the first few patients to undergo the procedure was 56-year-old Long Foo Pieng, who in 2004 was diagnosed with congenital bicuspid aortic valve – a heart defect present at birth, where the aortic valve has only two leaflets instead of three. This can result in aortic stenosis, where the valve narrows, or aortic regurgitation, where the blood flows backwards from the aorta into the left ventricle. These conditions can be fatal if severe and left untreated. The retired company director initially managed the condition using medication and lifestyle adjustments, as surgery would require that the valve be replaced with a prosthetic substitute, which would require significant changes to his lifestyle, said Adjunct Associate Professor Low Ting Ting, a senior consultant with the cardiology department. However, his condition eventually led to aortic dilation, requiring surgery. 'When this Pears procedure came along, it allows us to manage to constrict the size of his aorta without replacing the aortic valve,' said Prof Low. After undergoing Pears, Mr Long was discharged from the hospital within a week. He recovered well and in six months was able to resume his normal day-to-day activities. 'I was very happy back at the golf course again, going to the driving range almost two to three times a week,' he said.

Coroner's Court holds inquiry into death of local actress Alice Ho, Entertainment News
An investigation into the death of local actress Alice Ho from post-op bacterial infection began on Monday (July 28), two years after her death. Alice, who debuted in local showbiz in 1997 and is most well-known for playing Taugeh Soh (Beansprout auntie) in local long-form TV drama Holland V (2003), died at the age of 72 on July 9, 2023. According to a report by Lianhe Zaobao on Monday, she underwent surgery for uterine prolapse at Singapore General Hospital on July 6, 2023, but died from sepsis and peritonitis a few days later on July 9. The Coroner's Court opened an inquest on Monday following questions her family raised about the medical and post-operation treatment she received prior to her death. The court heard that Alice had visited the hospital in April 2023 and was attended to by Dr Lin Shaojiang (transliteration). Dr Lin testified that Alice had complained of a lump between her legs that had troubled her for a decade and affected her urination. Upon further examination, the doctor found her uterus to have prolapsed and proposed three options, including undergoing surgery to remove her uterus and repair the pelvic floor, which Alice decided to go for after discussing with her family. Dr Lin added that the operation was successfully completed on July 6, 2023, and when he checked on her the next day, she was awake and able to communicate normally. However, Alice's condition worsened later, and she was found to have bacterial infection after a CT scan. The medical team decided to perform surgery to find the source of the infection. They found a small amount of fluid in her pelvis and there were no abnormalities in her surrounding internal organs. She was prescribed antibiotics, but her condition did not improve, and she died in the evening of July 9. The court learnt that investigation found Alice's cause of death to be bacterial infection by E. coli, which was translocated during the removal of her uterus, fallopian tubes and ovaries. A medical expert who testified at court said that the bacteria was likely transmitted from a laceration in her intestines or from the medical devices that were used. On the second day of the hearing on July 29, the court heard that Alice's examination results corresponded to an abdominal infection, which they treated with antibiotics. However, she didn't respond to the treatment and her condition deteriorated. The case will be concluded by the Coroner's Court at a later date. [[nid:720780]]
