Cancer: World-first 'Trojan horse' therapy to be given on NHS

A "Trojan horse" therapy that sneaks toxic drugs inside cancer cells is being made available on the NHS in England in a world first.It can halt the blood cancer myeloma for nearly three times longer than current therapies.The drug is an advanced form of chemotherapy that hits cancer with a bigger dose, while reducing side-effects.Paul Silvester, one of the first people to get it, says the therapy has been "life-changing" and he's now planning history-themed adventures.
Myeloma – also known as multiple myeloma – affects part of the immune system called plasma cells. These are made in the spongey bone marrow in the centre of our bones.Paul, who is 60 and from Sheffield, was diagnosed nearly two years ago after the cancer led to broken bones in his back.He had a bone marrow transplant last year, but relapsed around Christmas. He has since been on the new therapy – called belantamab mafodotin – as part of an early access scheme.Within weeks he was in remission.
Other treatments could have left him isolating in his bedroom for months, so Paul says the therapy "is absolutely life-changing" and was "creating that opportunity to enjoy" life. Visiting Hadrian's Wall is next on the agenda for history buff Paul; and he's looking forward to one of his daughters graduating later this year. "Most people say 'you look really really well'... I have a good normal life," he told the BBC.
How does this therapy work?
Paul's therapy - belantamab mafodotin – is a lethal chemotherapy drug that has been bound to an antibody, similar to the ones the body uses to fight infection. However, these antibodies have been designed to spot markings on the outside of plasma cells. So they travel to cancerous cells, stick to the surface and are then absorbed. Once inside they release their toxic payload, to kill the cancer.
The therapy is named Trojan horse therapy after the siege of the city of Troy in Greek mythology, when a giant wooden horse was used to smuggle soldiers into the city.Myeloma cannot be cured, but clinical trials last year showed the Trojan horse therapy halted the cancer for three years, compared to 13 months with current therapies. Prof Peter Johnson, the national clinical director for cancer at NHS England, said the difference was "life-changing".He told me: "This is a really important development for people with myeloma, because although we may not be able to cure the illness, giving them time free of the disease and free of the symptoms is really important."We've seen in the last few years that using antibodies to deliver chemotherapy drugs directly into cells can make a big difference for a variety of different types of cancer."
Around 33,000 people are living with myeloma in the UK. The new drug will be used when the first-choice therapy fails, so around 1,500 patients a year could benefit.The decision comes after a review by the National Institute of Health and Care Excellence (NICE) concluded the drug was cost-effective for NHS use. NICE recommendations are normally adopted in England, Wales and Northern Ireland while Scotland has its own process. The therapy is kinder than other cancer treatments, but is not free from side-effects. After a cancer cell has been destroyed, the remaining chemotherapy drug will leak into the body. This can cause dry eyes and blurred vision.
'These are very smart drugs'
The technical name for these drugs is an antibody-drug-conjugate.This therapy was developed by GSK in the UK with early research taking place in Stevenage and the first clinical trials in London.Prof Martin Kaiser, team leader in myeloma molecular therapy at the Institute of Cancer Research, said these "are very smart drugs" and the difference in side effects compared to other drugs "is really remarkable".While myeloma is still considered an incurable cancer, Prof Kaiser says drugs like this are "an important step towards a functional cure" and he thinks long-term remission will go "above 50% in the next five years".Antibody drug conjugates are being developed for a range of cancers. The limitation is being able to design an antibody that can target the cancer alone. There is one that can target some types of breast cancer. Research is already taking place on stomach and bowel cancer.Shelagh McKinlay, from the charity Myeloma UK, said the approval would "transform the lives of thousands" and it was "fantastic to see the UK at the forefront of myeloma treatment".Health Minister Karin Smyth, said: "This ground-breaking therapy puts the NHS at the forefront of cancer innovation."

A schoolboy's incurable disease was written off as dyspraxia by doctors for two years before he was diagnosed with a life-changing terminal condition. Eddie Worman, then six, first caused concern after his mother Rachel, 39, noticed he was struggling to walk upstairs. After two years of GP and physiotherapy appointments, Eddie's worried family finally received clarity and in April 2024 were told their son had Duchenne Muscular Dystrophy (DMD). The debilitating neuromuscular disease leads to muscle wastage and weakness and Eddie is one of just 2,500 sufferers across the whole of the UK. Now Eddie's family, including his sisters Maisie, 13, and Olive, 10, have launched a GoFundMe page to raise money for his ongoing care and to buy a specialised wheelchair before his condition worsens. The mother-of three and her husband, carpet fitter Adrian, 46, first became concerned more than two years ago after noticing their little boy appeared to struggle with his mobility. Rachel told MailOnline: 'I took him to the GP initially, because I was worried about the fact that he was really struggling with stairs. He was struggling with his mobility in terms of having to clutch hold of his legs and his thighs in order to climb the stairs. 'I just thought "that cannot be normal for a three to four-year-old should be tearing up and down the stairs". And obviously I had his two older sisters to compare him to. 'He wasn't complaining of pain per se, but it was general little things like, if he was sitting on the floor he wouldn't just jump up like a child would normally, he would clutch hold of his side, or he'd have to hold onto a piece of furniture.' She added that it became even more noticeable when he went to school and she was regularly called in after he had been injured while playing. 'When he got to school I was getting called constantly that he was in and out of the medical bay, and he was sustaining head injuries where he wasn't able to duck out of the way when he'd have children of his own age hurtling towards him and playing games', she said. Rachel explained she went to the GP and raised her concerns but explained the doctor did not seem particularly worried. The mother-of-three persevered and her little boy was eventually seen by an occupational therapist. After a three-hour assessment Rachel was given the news that Eddie had dyspraxia but that it would need to be confirmed by a paediatrician. Dyspraxia is a common developmental co-ordination disorder (DCD) that affects movement and co-ordination according to the NHS. In a follow-up appointment in March 2024, Rachel was told it wasn't dyspraxia but was instead DMD. She nurse said she was left in complete shock and said: 'I never thought for one second it was anything muscular related because even me in a medical profession, I hadn't really heard of Duchenne.' She explained that she went in to 'complete panic mode' and was angry at the length of time it had taken Eddie to be diagnosed. She added that the terminal diagnosis 'devastated' their family and now they're focusing on how Eddie's life can be made comfortable. Those with DMD have a significantly shorter life expectancy, with the average person only living to between 22 and 30 years of age and many dying of heart and lung failure. She said: 'As a parent you can't bear the idea that there is anything physically wrong with your child, or that they're going to struggle in any way, shape or form throughout their life.' Following his diagnosis, Eddie has been on a strict routine of medication, physio and hospital appointments. The eight-year-old takes six doses of steroids a day and has 45 minutes of intense physio in the morning and another session in the evening as well as wearing leg braces each night. Normal activities are difficult for the family-of-five and Rachel explained that they struggle to go out to the cinema and says she didn't realise what she took for granted before. She said: 'It's grueling. Everything sort of happened and changed overnight.' Rachel added that she finds it difficult to navigate the world with Eddie and said it has become 'glaringly obvious' to her the world is 'not designed for children with disabilities'. She said the family can't even make a quick trip to Tesco because Eddie becomes too tired and needs to be carried through the supermarket. Rachel also confessed she is worried about school and told MailOnline that while his friends make sure to include him in games where they can there are some things he can't do. 'At sports day they make sure he is still included but even playing hide and seek with friends, he just can't keep up,' she said. Now Rachel and Adrien are crowdfunding to help them with the increased costs of looking after Eddie. The worried mother told MailOnline that they're looking in to moving to a more accessible house because their current family home is simply not suitable for his needs. Changes they need to make include converting a garage in to a downstairs wet room, adding a lift and even converting their loft. Rachel said the couple are looking at a 'financially difficult future' and the nurse added she now 'lives to work' to make sure they can afford specialist equipment including a power assisted wheelchair for Eddie. Eddie's parents explained that he is not eligible for one under the NHS and they can cost upwards of £1,000. Rachel has also found support from other parents whose children are battling the condition and added that Facebook and Whatsapp communities have been invaluable. She said: 'My biggest support was from those parents, because they understand they understand exactly what you're going through. I reached out to them as soon as Eddie was diagnosed because I was so lost.'

Scientists pinpoint way to check if you're going to get early-onset colon cancer with up to 92% accuracy

A little-known at-home test can detect early-onset colon cancer before symptoms develop, increasing the chance of surviving a disease that is surging among young people. The fecal immunochemical test (FIT) costs $49 and screens a person's feces for flecks of blood, a warning sign of the cancer — as tumors cause bleeding. The test is FDA-approved and allows younger people a chance to screen for colon cancer before they reach the recommended age for a colonoscopy, 45 years. If blood is detected, a colonoscopy is performed, increasing the chances of catching colon cancer early when it is more treatable. And the American College of Surgeons says it is generally accurate, able to detect about 80 percent of all cases. If someone opts for the $600 FIT with DNA screening, which checks for abnormal changes in cells' DNA, accuracy rises to 92 percent. A colonoscopy is about 99 percent accurate at detecting all cancers. A recent study found people that use the FIT method between the ages of 40 to 49 years old are 39 percent less likely to die from colon cancer than their peers who wait to get the test until they are 50 years old or above. An increasing number of younger people are seeking screening, as the US experiences a surge in colon cancer among people under 50, with young patients today about twice as likely to be diagnosed with the disease than their grandparents. The rising rates are prompting some physicians to urge Americans to start getting screened for the disease before the age of 45 years, which is when US doctors recommend. For the test, which can be ordered online, customers are sent a package containing a collection tube. They are asked to collect a sample of their feces from the toilet bowl, place it in the tube and mail it to a lab. Results take about five days to come back. In the lab, scientists expose the feces to antibodies that can bind to human hemoglobin - a protein found in red blood cells. If they bind to an area of the feces, a positive test result is issued - and patients are advised to undertake further screening. In the FIT with DNA, scientists also analyze cells found in the stool for abnormal changes - which could indicate cancer. Patients over 45 years old who have an average risk — meaning no family history of the disease — may be recommended the test, which is free for the group under health insurance plans. But people can also buy the test, which is sold by labs like Everlywell for $49, or opt for other tests like Cologuard, which is priced at $121. While the test can be extremely helpful, doctors caution that about five to 10 percent of patients who get a positive result do not have colon cancer or advanced polyps. The number of false positives after a colonoscopy is below one percent. It is important to follow up with a doctor to discuss results. Patients undertaking the test are recommended to do it every year, as this can help to catch early-onset cancers which are typically slower growing. Among patients to have their cancer caught after using an at-home stool-based test is Christine Bronstein, who was diagnosed with the cancer at age 48. The Hawaii local exercises regularly, and avoids sugar and alcohol. But after seeing blood in her stool in 2021, she became concerned and ordered a stool-based test. After results came back positive, she rushed to see her doctor and was eventually approved for a colonoscopy - which led to a stage three colon cancer diagnosis with a two-inch tumor found in her rectum. Bronstein told TODAY: 'I'm very lucky that I did my test when I did. This thing really takes people down right in their prime... I think the problem why this is becoming the No.1 killer for younger people is because their symptoms get denied.' Colon cancer cases are surging among under-50s in the US in an uptick that has stunned researchers, with adults born in the 1980s having double the risk of suffering from the cancer compared to their parents. Among adults aged 25 to 29 years, colon cancer cancer rates have spiked 85 percent in two decades according to CDC data. At the same time, over the last 30 years cases have dropped among over-50s by 30 percent.
