Exercise Can Help Protect Against Cancer Fatigue, Depression

Lingering fatigue and depression are more common among women than men cancer survivors and often lead to a decrease in recreational physical activities in all patients, new data showed.
However, moderate physical activity was linked to an almost 50% lower risk for cancer-related fatigue, and both moderate and vigorous physical activity were associated with a two- to fivefold reduced risk for depression among cancer survivors, according to the analysis presented at the American Association for Cancer Research (AACR) Annual Meeting 2025.
The findings 'highlight the importance of providing special attention and tailored interventions such as exercise programs, support groups, and mind-body behavioral techniques for vulnerable groups to help effectively manage fatigue and improve participation in recreational activities as they are an essential aspect of quality of life,' Simo Du, MD, a resident at NYC Health + Hospitals and Jacobi Medical Center/Albert Einstein College of Medicine, New York City, said in a news release.
Du noted that, during her residency, cancer-related fatigue was a common complaint among patients, affecting 'not just their daily activities but also their overall quality of life and mental health, making tasks like climbing stairs, doing groceries, or laundry overwhelming.'
Cancer-related fatigue affects more than 80% of patients who receive chemotherapy or radiation therapy, while depression affects around 25% of patients. Unlike typical fatigue, cancer-related fatigue can linger for weeks, months, or even years after treatment, Du explained.
Despite its high prevalence, cancer-related fatigue remains 'overlooked and undertreated,' she noted during a conference press briefing. In addition, cancer-related fatigue can affect men and women differently.
To investigate further, Du and her colleagues analyzed National Health and Nutrition Examination Survey (NHANES) data from 1552 cancer survivors (736 men and 816 women).
After adjusting for age, race, socioeconomic status, and comorbidities, women cancer survivors were more likely to experience fatigue (odds ratio [OR], 1.54; P < .017) and depression (OR, 1.32; P = .341) related to their cancer compared with men cancer survivors.
Du said there are likely multiple reasons behind the sex differences observed.
Women may, for instance, be more likely to experience side effects from chemotherapy, radiation, and long-term use of endocrine treatments because of slower drug clearance, which can lead to higher concentrations and a stronger immune response that may heighten inflammatory side effects.
In multivariate logistic regression analysis, cancer-related fatigue (OR, 1.93; P = .002) and depression (OR, 2.28; P = .011) were both strongly associated with reduced moderate recreational activities, such as brisk walking, biking, golfing, and light yard work.
The data also showed a protective role for physical activity. For patients who engaged in moderate physical activity, their risk for cancer-related fatigue (OR, 0.52; P = . 002) and depression (OR, 0.41; P = .006) was significantly reduced, Du reported.
For depression (but not cancer-related fatigue), 'the higher the intensity of physical activity, the higher the protective effects, with almost 4-5 times the reduction of the depression,' Du noted.
Although the NHANES uses standardized protocols designed to minimize biases, Du said a limitation of the current study is the use of self-reported data and the fact that women could potentially overreport fatigue symptoms and men could potentially underreport symptoms of depression.
Looking ahead, Du and her colleagues are planning studies to assess the effectiveness of tailored interventions on cancer-related fatigue and explore the connection between cancer-related fatigue and different mechanisms, such as inflammatory markers, to see if gender modifies the association.
Commenting on the study for Medscape Medical News , Jennifer Ligibel, MD, a senior physician at the Dana-Farber Cancer Institute in Boston, said that, because the dataset is cross-sectional, it is unclear whether people who were more tired weren't exercising or if people who weren't exercising were more tired.
However, Ligibel explained, a huge body of literature has demonstrated that exercise is 'the most efficient remedy for fatigue,' and it likely helps with depression too.
In fact, in a recent survey of cancer patients conducted by the American Society for Clinical Oncology, slightly more than half of patients reported that their oncologist talked about exercise and diet during clinic visits, Ligibel said. Provider recommendations for exercise and diet were associated with positive changes in these behaviors.
'Roughly half of oncologists now give exercise advice; that figure is a lot more than it was a few years ago, but it's still not universal,' Ligibel said.

Nirsevimab Cuts Bronchiolitis Cases in Young Infants

Nirsevimab implementation in Catalonia, Spain, was associated with a 44% reduction in emergency department (ED) visits for bronchiolitis in infants younger than 6 months and a 48% reduction in hospital admissions compared with previous seasons. The multinational study analyzed 1,574,392 ED visits and 255,689 hospital admissions across Spain, the United Kingdom, and Italy. METHODOLOGY: Researchers conducted a multinational retrospective analysis of ED visits and admissions at 68 hospitals in Catalonia (Spain), one hospital in Rome (Italy), and four hospitals in the United Kingdom from May 1, 2018, to April 30, 2024. Analysis included data for all diagnoses, respiratory diagnoses excluding bronchiolitis, and bronchiolitis diagnoses for different age groups (< 6 months, 6-11 months, and 12-23 months). A generalized linear model in Poisson regression was utilized to obtain risk ratio and 95% CI of bronchiolitis in the 2023-2024 season compared with a mean of previous seasons, excluding 2020-2021 (as a COVID year). TAKEAWAY: In Catalonia, the risk ratio was 0.52 (95% CI, 0.48-0.55) for bronchiolitis-related hospital admissions in infants aged less than 6 months in the 2023-2024 season compared with previous years. ED visits for bronchiolitis in Catalonia showed a risk ratio of 0.56 (95% CI, 0.54-0.58) for infants younger than 6 months and 0.93 (95% CI, 0.89-0.97) for infants aged 6-11 months. No significant reduction in risk ratio for ED visits or admissions was observed in the 2023-2024 season at other study sites in the United Kingdom and Italy. According to the authors, the effect of nirsevimab was less clear in older infants aged 6-11 and 12-23 months. IN PRACTICE: 'Nirsevimab had a clear impact in reducing attendances and admissions for infants with bronchiolitis aged < 6 months in Catalonia. However, the impact on older infants was less clear, making it unrealistic to imagine a substantial change in the epidemiology of infants accessing EDs or inpatient wards, at least in the near future,' wrote the authors of the study. It was published online in The Lancet Regional Health – Europe . LIMITATIONS: As a retrospective analysis, the study faced several limitations. The proportion of visits and admissions coded as bronchiolitis varied widely across countries, potentially due to coding differences or health system factors rather than true epidemiological differences. The researchers could not directly match all diagnoses in International Classification of Diseases, 10th Revision, to Systematized Nomenclature of Medicine Clinical Terms codes. Limited virology testing of ED visits prevented determination of the relative contribution of respiratory syncytial virus toward the disease burden in the studied seasons. A significant number of adults in the U.S. don't know that they have high blood pressure, diabetes or bad total cholesterol numbers, which all contribute to the risk of death from cardiovascular disease. And awareness may be even worse now because COVID-19 disrupted preventive screenings. That's according to a new study in the journal JAMA Cardiology from researchers at Beth Israel Deaconess Medical Center, the Pritzker School of Medicine at the University of Chicago and the Harvard T.H. Chan School of Public Health. The research involved 15,269 nonpregnant adults 20 or older (average age 56.8 years) who had cardiovascular risk factors between 2013 and 2023, using the National Health and Nutrition Examination Survey. The researchers looked specifically at how many didn't know their own health status for those conditions, which was learned through a household interview and then a medical examination. 'The proportion of U.S. adults who were unaware of having hypertension increased significantly over the study period, particularly in young adults and women, while diabetes and high cholesterol level unawareness remained stable. By the 2021 to 2023 cycle, approximately 1 in 6 adults with hypertension and 3 in 10 with diabetes were unaware of their condition,' per the study. The adults who had been identified as having a cardiovascular risk factor but who reported not being told by a health professional that they had the factor were classified as unaware. 'Given declining cardiometabolic health in young adults,' the authors expressed concern that about one-third of those with hypertension, 2 in 5 with diabetes and 1 in 4 with high cholesterol are not aware of their own situation. They added that 'policy efforts to address these gaps in awareness are needed to prevent future cardiovascular events.' The researchers found that across the entire age range studied, 17.8% who had hypertension didn't know it in 2021-2023, compared to 14.6% in 2013-2014. The proportion of adults who didn't know they had diabetes did not change significantly, going from 27.5% to 28.9%. The share with high cholesterol who didn't know was also stable, at 11.5% and 11.9% respectively. But age made a difference. The proportion of young adults ages 20-44 who didn't know they had hypertension rose from 21.9% to 37.1%. Awareness of that didn't change for those 45 and older, or among different age groups who had undiagnosed diabetes or high cholesterol. Lack of awareness of having high blood pressure rose significantly over the study period in women, to 16.5%. All three of those conditions are treatable. Left untreated, however, they can cause severe problems or kill. They are considered 'modifiable risk factors' for cardiovascular disease. The Centers for Disease Control and Prevention reports that heart disease is the leading cause of death in the United States. It includes stroke, heart failure and atrial fibrillation. High cholesterol can lead to a buildup of plaque on artery walls, narrowing those blood vessels and limiting or blocking blood flow to and from the heart and other organs, per CDC. That can cause chest pain or a heart attack. It also raises the risk for heart disease and stroke, which are leading causes of death. High blood pressure or hypertension doesn't usually have symptoms, but can damage the heart, brain, kidneys and eyes. Making lifestyle changes or taking blood pressure medicines can reduce the risk, per CDC. People who have diabetes have twice the risk of heart disease and the risk climbs over time. High blood sugar damages blood vessels and the nerves that control the heart, per CDC. But medication and lifestyle changes can help there, too. Those with diabetes are also more apt to have high blood pressure and so-called 'bad' cholesterol — along with the increased risk those conditions bring. They are also more likely to experience heart failure, where the heart pumps less efficiently, which can also lead to a buildup of fluid in the lungs, making breathing both miserable and difficult. Cleveland Clinic reports that about half of U.S. adults have some form of cardiovascular disease and that 1 in 3 women die from it. Among the risk factors — besides hypertension, high cholesterol and type 2 diabetes — are tobacco use including vaping, genetics, lack of exercise, an unhealthy diet, overuse of alcohol, misuse of prescription and recreational drugs, some chronic autoimmune or inflammatory conditions, chronic kidney disease, and certain pregnancy-related complications. As reported by Medical News Today, researchers at the University of Copenhagen in Denmark found that those who don't know they have forms of heart disease and who have undetected obstructive coronary atherosclerosis have an eight-fold risk of heart attack. That same study suggested that nearly half of adults over 40 'may have such 'hidden' heart conditions.' The American Heart Association noted that there are other conditions that often go undiagnosed, including a 'potentially fatal buildup of abnormal proteins in the heart and other organs.' That condition, called transthyretin amyloidosis, is considered rare, but studies now suggest that it's a fairly common cause of heart failure in older adults, with the proteins found in the hearts of one-fourth of older adults at autopsy. That condition, like hypertension, diabetes and high cholesterol, is also treatable. But it has to be diagnosed and addressed. Undiagnosed heart disease is also responsible for a very large share of heart attacks that aren't caused directly by a blood clot, the association separately reported.
