Sierra Leone has taken a critical step toward advancing Universal Health Coverage (UHC) with the convening of a high-level policy dialogue on the draft Sierra Leone Agency for Universal Health Coverage (SLAUHC) Bill. Organized on 6 May 2025 by the Ministry of Health with support from the World Health Organization (WHO), the one-day engagement brought together over 60 senior leaders in Freetown, including ministers, directors, and technical heads from across the health sector.
The proposed SLAUHC Bill outlines the establishment of a dedicated agency that will integrate and manage two major national health financing mechanisms, the Free Healthcare Initiative (FHCI) and the Sierra Leone Social Health Insurance Scheme (SLeSHI). The unified governance structure aims to address current fragmentation, improve the efficiency of health financing, and accelerate the country's progress toward achieving UHC.
'This Bill is a transformative step in Sierra Leone's journey toward sustainable health financing,' said Dr. Ibrahim F. Kamara, speaking on behalf of the WHO Country Representative. 'It will strengthen institutional capacity, enhance accountability, and ensure equitable access to health services, particularly for the most vulnerable populations.'
The engagement served three key objectives: a comprehensive review of the draft legislation, consensus-building among stakeholders, and alignment with the Ministry of Health's UHC Roadmap and SLeSHI implementation framework. The outcome is a consolidated and informed policy position ahead of the bill's submission to the Inter-Ministerial Committee (IMC).
Chief Medical Officer Dr Sartie Kenneh emphasized the importance of a comprehensive and inclusive approach to UHC: 'the name and scope of the bill must reflect the broader dimensions required to achieve UHC. While health financing is a critical pillar, it is only one part of the equation. No healthcare service is truly free, while it may be free at the point of delivery, someone ultimately bears the cost. Therefore, we must collectively design a sustainable health financing model that ensures long-term viability. It is also prudent to allow the Free Healthcare Initiative and the Social Health Insurance Scheme to operate in tandem, to optimize coverage and ensure the full spectrum of healthcare costs is addressed.'
The proposed SLAUHC Agency responds to longstanding structural challenges in the health financing landscape. Currently, out-of-pocket payments account for 56% of total health expenditure, well above the sub-Saharan Africa average of 30%. Less than 1% of Sierra Leone's population is covered by any social health protection scheme, exposing many to catastrophic health spending and pushing households further into poverty. Moreover, with 75% of health financing reliant on donor contributions, ensuring coherence with national priorities remains a pressing issue.
The draft bill is grounded in the Ministry's Health Financing Strategy 2021–2025, which calls for the creation of a Universal Health Coverage Fund, integration with SLeSHI, and the development of operational and regulatory systems to support long-term health sector sustainability.
WHO has reaffirmed its full technical support to the Ministry in the finalization and operationalization of the SLAUHC Bill. This includes support for institutional design, capacity strengthening, cost-containment mechanisms, and the establishment of care quality and priority-setting frameworks.
Once enacted, the SLAUHC Agency will serve as a central institution for resource mobilization, regulation of financial flows, and oversight of major health benefit programs. It is expected to play a pivotal role in improving efficiency, transparency, and equity in health service delivery.
This high-level dialogue marks a significant milestone in Sierra Leone's health sector reform agenda. It paves the way for the establishment of a National Health Insurance Scheme and reinforces the country's commitment to achieving health for all, leaving no one behind.

The Ministry of Health, through the National AIDS Control Program (NACP), on Monday, July 28, 2025, kicked off a nationwide hepatitis-B awareness and testing campaign in observance of World Hepatitis Day. This year's event is celebrated under the global theme 'Hepatitis: Let's Break It Down,' with a slogan ' Hepatitis Can't Wait.' The campaign began today with simultaneous and massive free testing at the Ministry of Health in Congo Town, the Ministerial Complex, and the NACP office at John F. Kennedy Memorial Hospital in Sinkor. It seeks to raise awareness, encourage early testing and treatment, and highlight the deadly impact of viral Hepatitis, particularly Hepatitis B and C, on Liberians, especially young people. Speaking at the launch of the campaign on Monday, Assistant Minister for Preventive Services, Dr. Cuallau Jabbeh-Howe, issued a passionate appeal to the public to embrace testing as a first step toward defeating the disease. 'We are here today for a very important program. We are celebrating World Hepatitis Week, and today, July 28, is being observed globally as World Hepatitis Day, ' Dr. Howe emphasized. She continued, 'It is good for you to know your status so you can better plan and know how to take good care of yourself. Don't get ill unknowingly. It is always good to know your health status.' Dr. Howe warned against the dangerous culture of health denial and silence in Liberia. 'A lot of us don't want to know our status, which is very bad and dangerous. If you don't know, you won't be able to plan properly, and you won't have someone to treat you properly. That's why today, we are calling upon you to do your free testing so that you can be advised on how to move forward.' According to her, Hepatitis is emerging as a major killer disease in Liberia, with young people most at risk. 'Hepatitis is silently killing many of our young people and is now becoming one of the top killers in Liberia. So please, get tested. Know your status. Call your family. Share the word,' she urged. In continuation of the campaign, free Hepatitis B testing will last up to the 31st of July 2025 at the Ministerial Complex in Congo Town, targeting civil servants, government employees, and the general public. Health workers will be stationed on the grounds to provide testing, counseling, and basic information about Hepatitis B prevention and treatment. The week-long commemoration is not just about testing, but also about breaking the silence and breaking down stigma, which aligns with this year's national slogan, 'Let's Break it Down.' The Ministry hopes to demystify Hepatitis B by educating citizens about its symptoms, transmission routes, prevention methods, and treatment options. World Hepatitis Day is recognized every year on July 28 to honor the birthday of Nobel Prize-winning scientist Dr. Baruch Blumberg, who discovered the Hepatitis B virus and developed the first vaccine for it. The day is used to intensify global and national efforts toward Hepatitis elimination, which the World Health Organization (WHO) aims to achieve by 2030. Following a press release published on 25 July by the non-governmental organisation MSF (Médecins Sans Frontières (Doctors Without Borders), mentioning alleged 'massive cuts' by the European Union (EU), among other international donors, in the response to the nutrition insecurity in Nigeria, the EU Delegation in Nigeria would like to communicate that this information is unsubstantiated and inaccurate, as the level of EU humanitarian support granted by the EU Civil Protection and Humanitarian Office ECHO in Nigeria in the first half of 2025 has in fact been similar to the one provided in the first half of 2024 and, as done in previous years, the EU is working to increase the amount with additional top ups. Like in 2024, a significant part of ECHO assistance in Nigeria focuses on nutrition, health and food assistance emergency. The EU Delegation takes note of the revised version of the press release published today by MSF, which eliminates the mention of the European Union when speaking of massive cuts by international donors. While commending the work performed by MSF teams, partly through EU funding, to help malnourished children and their mothers in Nigeria, as witnessed by the EU Ambassador during his recent visit to Sokoto State on 19-20 July, the EU Delegation underlines the importance of communicating verified and accurate information on this matter of utmost importance for the Nigerian people. It reiterates the call launched by the EU Ambassador, during his 22 July joint press conference with the Honourable Minister for Humanitarian Affairs and Poverty Reduction as well as humanitarian partners to urgently enhance resources from domestic and international sources to address the emergency nutrition situation particularly in North-West and North-East Nigeria. President Wavel Ramkalawan, accompanied by First Lady Linda Ramkalawan and Vice-President Ahmed Afif, officially inaugurated the newly constructed Baie Lazare Health Centre on Monday afternoon, marking a significant milestone in the district's healthcare infrastructure development. The modern healthcare facility replaces the original health centre built in 1938. Entirely funded by the Government of Seychelles at a cost of approximately 9.2 million Seychelles rupees, the project demonstrates the administration's commitment to enhancing community healthcare services. Construction commenced in February 2024, with temporary healthcare services provided at Baie Lazare Primary School to ensure uninterrupted medical care for residents. The comprehensive health centre offers an extensive range of services with particular emphasis on preventive care, early detection, and health promotion. The facility features state-of-the-art amenities including public health services, physiotherapy units, dual consultation chambers, an observation unit, an emergency unit, equipment sterilization facilities, antenatal care and family planning consultations, pharmaceutical services, and storage facilities. Additionally, the centre is equipped with an advanced dental service unit featuring a dental X-ray system with high-end equipment generously sponsored by the German Dental Association, demonstrating international partnership in Seychelles' healthcare advancement. Minister for Health Peggy Vidot expressed great honour at the facility's opening, emphasizing its significance beyond mere infrastructure. "The inauguration represents more than just a new building, it symbolizes a transformative chapter in the lives of individuals and families within the district," Minister Vidot remarked. "This vibrant facility will propel the community forward through collective spirit, creating success stories that empower families to embrace healthier lifestyles." The Minister highlighted the government's proactive healthcare approach, emphasizing both treatment and prevention strategies. "We will offer programs and services focused on early prevention, healthcare education, early detection programs, vaccination, and health promotion initiatives to equip residents with the knowledge and resources needed to take charge of their own health," she explained. Healthcare Agency Chief Executive Officer Dr. Danny Louange reinforced the agency's dedication to delivering exceptional healthcare services throughout communities. "Our objective is to offer healthcare services of standard, respect, quality, and dedicated care. This centre is part of a broader national vision for the healthcare agency," he explained. "We are working toward accrediting all our healthcare services, therefore we must ensure our healthcare facilities meet the standards required for accreditation." President Ramkalawan's address underscored his administration's commitment to ensuring equitable healthcare access across all districts of Seychelles, with the Baie Lazare Health Centre representing one of several new healthcare infrastructure projects under the current administration. The ceremony featured a testimony by Baie Lazare resident Mr. Berard Esparon, along with songs and poem recitals by Baie Lazare Primary School students.
