The United States is seeing a rise in Covid-19 activity, with the sharpest increases recorded in the west. The Centres for Disease Control and Prevention (CDC) says that nationwide wastewater virus levels have climbed from 'low' to 'moderate' in the past week.The highest readings are concentrated in Alaska, California, Colorado, Nevada and Utah, all classified as 'high'. Louisiana leads in the southeast, where levels are also elevated.'Wastewater monitoring can detect viruses spreading from one person to another within a community earlier than clinical testing and before people who are sick go to their doctor or hospital,' the CDC says. 'It can also detect infections without symptoms. If you see increased wastewater viral activity levels, it might indicate that there is a higher risk of infection.'Wastewater surveillance is being used as an early-warning system for new waves, with California's own data now showing higher levels than during last winter's peak.As of Tuesday, the CDC estimates cases are rising or likely rising in 45 states, up from 40 last week. Only Alaska, North Dakota, Maine and New Hampshire showed no change. Wyoming's trend was not available.While the pace of increase is slower than in past surges, health officials say immunity from previous infections and vaccinations varies widely, making it hard to predict how severe the current wave could be.The XFG variant, nicknamed 'Stratus,' is now the third most common strain in the US this summer. First detected in Southeast Asia in January, it went largely unnoticed in America until May. By late June it accounted for 14 per cent of cases, behind NB.1.8.1 and LP.8.1.XFG is a combination of the F.7 and LP.8.1.2 variants, the latter being the second most prevalent in the country. It carries mutations that may help it evade immune defences, though scientists say it is less likely to be highly contagious than other dominant strains.'There is currently no clear evidence that XFG causes more severe disease or significantly different symptoms than earlier Omicron variants,' said Subhash Verma, professor of microbiology and immunology at the University of Nevada, Reno. 'Importantly, there are no immediate public health concerns associated with this variant.'In June, the World Health Organization placed XFG on its watchlist but rated its global public health risk as low. 'Currently approved COVID-19 vaccines are expected to remain effective to this variant against symptomatic and severe disease,' the WHO said in its June report.The CDC's shift to longer reporting timeframes is partly due to lower data submissions from states, which means the real spread may be higher than official estimates.The CDC lists the common Covid-19 symptoms as fever or chills, cough, shortness of breath, sore throat, congestion, loss of taste or smell, fatigue, muscle aches, headache, nausea or vomiting. Hoarseness has been reported anecdotally with XFG, though there is no confirmed link.People are advised to seek urgent medical attention for breathing difficulties, persistent chest pain, confusion, inability to wake, or changes in skin colour such as pale, grey or blue lips and nail beds.The increase in cases comes shortly after the US Department of Health and Human Services announced it will terminate 22 federal contracts for mRNA-based vaccines. The move has sparked debate about the future of a technology credited with saving millions of lives during the pandemic.During a CBS interview in July, physician Jon LaPook said the pattern of rising summer cases is now familiar. 'We now know that there's a winter spike and then there's a summer spike,' he said. 'And every year, the number of deaths, the number of hospitalisations, is gradually going down each season. So that's the good news.'However, experts say it is still too soon to tell how big the late-summer wave will be, especially with changes to vaccine recommendations and varying levels of immunity across the population.

Dr Ajay Nair, Sr Consultant, Internal Medicine, Narayana Hospital, Jaipur shares all you need to know: Understanding Dengue and Its Carrier Dengue fever is caused by the bite of mosquitoes carrying the dengue virus (DENV) and is transmitted primarily by Aedes aegypti and, to a lesser extent, Aedes albopictus mosquitoes. These vectors are well-adapted to urban environments, thriving in stagnant water around homes, offices, and public spaces. Once infected, the mosquito can remain a carrier for its lifetime, transmitting the virus to humans every time it bites. Unlike malaria, which is spread by Anopheles mosquitoes active at night, Aedes mosquitoes cannot be linked to a single time frame, as their biting patterns vary. This often causes confusion about the timing of their bites. The Feeding Habits: Science Over Assumptions The peak activity of the Aedes aegypti mosquito has been recorded in the early morning and late afternoon, as documented in multiple entomological studies, including those published by the World Health Organization (WHO) and the National Vector Borne Disease Control Programme of India. The highest activity typically occurs for about two hours after sunrise and several hours before sunset. However, their bites do not occur only in the morning. While early daylight hours see the most concentrated activity, Aedes mosquitoes can bite throughout the day, particularly in shaded indoor areas with low light. Dengue cases typically rise during the monsoon, when breeding sites multiply and mosquito activity remains high almost throughout the day until evening. This flexibility increases the chances of virus transmission, especially in densely populated areas. Clearing the Misconception The belief that dengue mosquitoes bite only in the morning likely stems from early observations showing higher bite counts after sunrise. While this is statistically accurate as a trend, it is not the whole picture. People are still at risk during other active hours, particularly in the late afternoon and early evening. Prevention: Beyond the Clock Since the Aedes mosquito's activity is not limited to one specific time of day, dengue prevention must be a continuous effort. Prevention during travel When visiting dengue-hit areas or places experiencing heavy rains, ensure you are protected from bites regardless of the time of day. Why Awareness Matters Millions worldwide suffer from the effects of dengue fever, with India bearing a significant share of the global burden. Misunderstanding mosquito behaviour can lead to carelessness, inadvertently increasing infection rates. The idea that dengue mosquitoes bite exclusively in the morning oversimplifies their complex behaviour. While Aedes aegypti activity may peak just after sunrise and before sunset, bites can occur at other times too. Goa's emergency medical services are a model for India

The precious 'golden hour' — those critical first 60 minutes after an emergency — becomes a race against time that too many families lose. This healthcare gap hits India's poorest the hardest, where medical emergencies bring double tragedy: Fear of losing a loved one and crushing medical bills that destroy generations of progress. Before 2008, Goa faced typical emergency care challenges despite advanced infrastructure. Unpredictable delays, fragmented services, and economic barriers particularly affected migrant workers and daily labourers. Geographic complexities, narrow roads, the tourist influx, and scattered communities compounded these problems. The journey of revolutionary transformation began in September 2008 with the establishment of Goa's 108 Emergency Medical Services through partnerships with EMRI Green Health Services. Starting with 18 Basic Life Support ambulances, this initiative reimagined emergency care beyond simple transport to integrated response systems. This transformation needed more investment and a dedicated control system which got more prominence after 2014 when Prime Minister Narendra Modi envisioned a path towards affordable and accessible healthcare for all. Today, Goa has a fleet of 103 vehicles, including Advanced Life Support ambulances, Cardiac Care units, Neo-Natal ambulances, and bike ambulances that navigate Goa's unique landscape. This diversification addresses geographic, economic, and social barriers that lead to healthcare exclusion. The state-of-the-art Command and Control Centre in Bambolim operates 24/7, with an average response time of less than 12 minutes. GPS-enabled vehicles, Computer-Aided Dispatch systems, and Electronic Patient Care Records create seamless coordination between responders and hospitals. So far, 16 Emergency Care Centres across Primary and Community Health Centres have handled over 5,24,000 emergencies, proving distributed care effectiveness. The 104 Health Helpline, launched in 2018, processed over 79,334 calls during Covid-19, serving as a vital public health communication bridge in Goa. The Community First Responder programme trains one family member per household in emergency medical assistance, creating distributed networks of first responders. This community-based approach recognises that sustainable emergency response must build local capacity, not just rely on professional services. The numbers tell a powerful story: Over 8,10,000 emergencies handled, more than 60,435 lives saved, and over 1,38,000 road accident victims transported safely. Beyond statistics, the service has gained national recognition, with multiple states studying Goa's model for adaptation. When emergency care becomes universally accessible, families gain freedom to pursue business ventures previously considered too risky. The World Health Organisation found that over 60 million Indians fall into poverty annually due to healthcare expenses — Goa's model breaks this destructive cycle. Women, especially, benefit from accessible emergency childbirth care, gaining confidence in family planning decisions. The peace of mind from healthcare security enables families to dream bigger, invest in the future, and pursue opportunities that drive prosperity. Emergency medical services work as powerful economic liberation tools. When comprehensive emergency coverage provides financial protection, small business owners, daily wage workers, and migrant labourers can engage more confidently in productive work. Communities with reliable emergency services typically experience increased economic activity as residents feel secure enough to invest locally. The Janani Shishu Suraksha Karyakram (JSSK) which provides free transportation for pregnant women and sick newborns from public health facilities back to their homes after delivery or treatment have assisted over 50,660 pregnant women and facilitated more than 5,000 emergency deliveries in Goa. The STEMI initiative has substantially reduced heart attack death rates through coordinated rapid response protocols. Goa's success demonstrates a practical implementation of the Prime Minister's healthcare vision on a larger scale. The key elements that enable successful replication across India include treating emergency care as a public service rather than a market product, ensuring universal access regardless of economic status. The integration of advanced technological systems with skilled personnel creates powerful combinations that maximise effectiveness, while community empowerment initiatives build local capacity alongside professional services. Continuous improvement through evidence-based enhancement and systematic monitoring ensures sustained quality and responsiveness to evolving needs. This comprehensive approach has earned the model international recognition, positioning India as a global leader in innovative emergency healthcare delivery and attracting valuable opportunities for knowledge sharing and technical cooperation with international health organisations. The state health department is now focusing on artificial intelligence applications in the health sector, expanded telemedicine integration, and enhanced climate change adaptation. Integration with broader digital health systems will enable transition from reactive emergency response toward proactive health promotion and disease prevention. Goa's Emergency Medical Services transformation addresses systematic challenges while advancing inclusive development. This initiative goes beyond traditional healthcare to become a powerful tool for social justice and economic progress in the state. The model validates treating healthcare as strategic investment in human potential rather than welfare spending. By ensuring universal access to excellent emergency care regardless of economic circumstances, the service makes real the constitutional promise of healthcare as fundamental right while contributing to poverty reduction and social mobility.
