
High A1c Fluctuations Linked to Mortality Risk in Diabetes
METHODOLOGY:
The importance of glycaemic control, as reflected by average A1c levels, is well-established in diabetes management guidelines; however, growing evidence suggests that A1c variability may also be important for various microvascular and macrovascular outcomes.
Researchers in England carried out a population-based cohort study to examine predictors of high A1c variability and evaluate the relationship between high A1c variability and all-cause mortality in patients with diabetes.
They included 20,347 patients with T1D (mean age, 52.9 years; 57.4% men) and 409,821 patients with T2D (mean age, 67.5 years; 55.8% men), all aged 31 years or older, from the Clinical Practice Research Datalink database. All participants had at least four A1c measurements taken at least 30 days apart during 2011-2014 and were followed from 2015 to 2017 for subsequent mortality outcomes.
The A1c variability score was calculated by measuring the frequency of A1c fluctuations of at least 0.5% (or 5.5 mmol/mol) between successive measurements over time, expressed as a percentage; A1c variability score estimates were categorised into four levels: 0 to < 20, 20 to < 50, 50 to < 80, and 80-100.

The impact of average A1c levels on mortality was less pronounced than that of A1c variability, as evidenced by higher population attributable fractions for variability (A1c variability score ≥ 20) than for average A1c levels in both T1D and T2D.

The duo highlighted the study's importance in filling a research gap on puberty timing in boys. They noted: "Boys who were exclusively formula-fed had a 16 percent increased risk of CPP compared with those who were exclusively breastfed, and [excess body fat] mediated this association in a pattern similar to that observed in girls." The experts pointed out that this increase in CPP risk observed in boys contrasts with the 60 percent increase observed in girls—which might help explain the lack of similar reports on puberty timing in boys. Ultimately, the study's findings support the view that breastfeeding during early infancy may help protect against CPP in both sexes. A man who went blind after drinking alcohol in Laos is working with other affected families to campaign for awareness of methanol poisoning. Calum Macdonald had a night out in Vang Vieng in November last year and the next day his vision was 'engulfed in this blinding white light', he told BBC Breakfast. Police in Laos have previously detained several people in connection with the deaths of six people who are believed to have been poisoned, including Simone White, 28, from Orpington, Kent. Methanol is sometimes used by disreputable bars as a cheaper alternative to ethanol but can cause severe poisoning or death. Mr Macdonald was at a hostel where free whisky and vodka shots were offered, and he mixed them with soft drinks, he said. He told how he was on a bus crossing into Vietnam when he noticed something was wrong. He told BBC Breakfast: 'The point at which I really started to notice some strange side effects was we briefly got off the bus to cross the border into Vietnam, and we went through the border office – we had to fill out a number of forms to get across to the other side – and at that point, sort of my entire vision was engulfed in this blinding white light. 'And I just thought, you know, this is very strange. I couldn't actually see where I was walking.' He added: 'We arrived in Hanoi, and at that point, the kind of white light in my eyes had diminished, and it wasn't until we got to our hotel room and my friends and I were sitting in the room, and I said to my friends, why are we sitting in the dark here? Let's turn on a light. And they informed me that the light was, in fact, already on.' Mr Macdonald then went to hospital and said on the journey there he 'really started to struggle to breathe' and could 'barely get my words out'. He said a number of tests were carried out initially but it was not until he got back to England when he was in hospital and they 'really sort of indicated that it was definitely related to something I'd been drinking'. Mr Macdonald is now speaking out as he wants information to be available to people about methanol poisoning, especially students who might be preparing to go on a gap year. He said: 'I think I definitely feel a sense of responsibility as someone who's been lucky enough to survive this, to try and get the message out. Because I know certainly, if I'd known about it, I wouldn't be in this situation. 'And I know I have a lot of friends that have also done similar sort of gap years in travelling around even that particular area. And so definitely, I think it's quite important. 'I've had the opportunity to speak to a number of other families that have been affected by this, and they're far more knowledgeable and have been campaigning for awareness for a lot longer than I have been involved. And what they tend to say generally is that sometimes the advice isn't put in strong enough terms. 'And so I think, I guess if I was to put out a recommendation to people, although I'm certainly not an expert or a medical professional, I would probably say that if you did want to drink alcohol in some of the affected countries, probably just to avoid spirits entirely. 'I think it would be nice, even if we could get some more information directly into universities, because I think that's the age of people that are really considering doing these kinds of trips, I think that would be a great thing.' Diabetes Tied to Slower Brain Recovery After TBI
TOPLINE: Diabetes was associated with reduced global cognition 2 weeks after traumatic brain injury (TBI) and less improvement in processing speed throughout the first year post-TBI compared to not having diabetes, a new cohort study showed. METHODOLOGY: The prospective cohort TRACK-TBI study included more than 1000 participants aged 17 years or older with TBI (mean age, 39 years; 67% men; 76% White individuals) who presented to one of 18 level 1 trauma centers in the US between 2014 and 2018. Participants underwent cognitive assessments at 2 weeks, 6 months, and 1 year post-injury that evaluated verbal episodic memory, executive function, and processing speed alone and combined into a global cognitive factor score. Researchers analyzed associations between cognitive outcomes and the vascular risk factors of hypertension (17%), diabetes (7.5%), hyperlipidemia (9%), and smoking (30%). They adjusted the analysis for age, sex, race, ethnicity, education, severity of TBI, and histories of TBI and psychiatric conditions. In sensitivity analyses, participants with treated diabetes had worse cognitive outcomes than those without diabetes, including lower global cognitive factor scores at 2 weeks (mean difference, -0.32) and less improvement in global cognitive function over 1 year (mean difference, -0.29). IN PRACTICE: 'Given the modifiable nature of comorbid vascular risk factors, future work interrogating postinjury changes in vascular risk factor burden, over longer follow-up, is warranted,' the investigators wrote.
