India's cholesterol crisis: why dyslipidemia is the next big public health challenge

Changing lifestyle patterns have given rise to a host of non-communicable diseases (NCDs) in India, including diabetes, hypertension and dyslipidemia. A recent cross-sectional study conducted by the Indian Council of Medical Research–India Diabetes (ICMR-INDIAB), assessed a representative sample of individuals aged 20 years and older, drawn from urban and rural areas of 31 States, Union Territories, and the National Capital Territory of India found that dyslipidemia was prevalent in 81.2 % of the total of 1,13,043 individuals (79,506 from rural areas and 33,537 from urban areas) who participated in the survey.
This presents a worrying scenario, one where individuals as young as 20 are already showing signs of abnormal lipid profiles, significantly increasing their risk of developing heart disease and other complications later in life. If left unchecked, this can snowball into a major public health challenge for our country.
Understanding dyslipidemia
To understand dyslipidemia, we need to understand what cholesterol is. Cholesterol is a fat-like substance found in all cells of our body. Cholesterol travels through the body in small packages called lipoproteins. Lipoproteins are of three types: LDL or low-density lipoprotein known as 'bad cholesterol', HDL or high-density lipoprotein known as 'good cholesterol', and triglycerides.
LDL is bad because it leads to a buildup of cholesterol in our arteries, which can lead to plaque build-ups and potentially cause blockages, which can eventually cause a heart attack or a stroke. HDL or high-density lipoprotein is good for the body because it carries cholesterol from other parts of the body back to the liver which removes the cholesterol from the blood. Triglycerides are also bad for the body; they are a type of fat stored in your body and used for energy. They increase when you consume more calories (especially sugar and fat) than your body needs. High triglyceride levels can contribute to hardening or thickening of the arteries, increasing the risk of stroke, heart attack, and pancreatitis.
To check for dyslipidemia, all three types of lipids are tested: if LDL and triglycerides are more than 100 mg/dl and 150 mg/dl respectively and HDL is lower than 40 mg/dl in men and 50 mg/dl in women, it means your body has high cholesterol levels that need to be brought under control.
The South Asian problem
The South Asian population in general, is more prone to NCDs such as dyslipidemia and diabetes due to a genetic risk, lack of physical activity, and suboptimal dietary habits. Our population tends to have low HDL (good cholesterol) levels, which puts it at a markedly increased risk for heart disease.
HDL is even more important than LDL. Low HDL is three times more common than high LDL in patients with premature heart disease. Thus, there is an urgent need to lower LDL levels and increase HDL levels to protect the heart from diseases, and the best way to do this is through a good diet and active lifestyle. Those with dyslipidemia and increased LDL levels (bad cholesterol) should restrict saturated fat to less than 7% of their total calorie count and cholesterol intake to less than 200 mg/day. They should add more fibre through dietary strategies such as increasing oats or psyllium, as well as fruits and vegetables. This can help decrease bad cholesterol. Indian foods such as rice, puri, naan, white bread and fried items are all high in non-saturated fats and can hike triglyceride levels.
Combating obesity
Obesity is strongly linked to dyslipidemia and weight management is an essential part of the recovery plan to lower lipid levels. Physical exercise for at least 45 minutes a day, five days a week is a must. For those who are not able to control their cholesterol levels with lifestyle changes such as diet and exercise, effective prescription medications are available and should be strictly adhered to.

Owing to a lack of control and supervision, the dog bit the child on the chin, causing injury," said Mumbai Police. Based on the detailed statement of the complainant, an offence has been registered against Khan under Sections 291, 125, 125(a) of IPC (BNS) at Mankhurd Police Station. 3.17 lakh people sought treatment for dog bite cases in the State in 2024 A notice has been served to the accused under Section 35(3) of BNS. Earlier, on July 10, the Central Government aimed to eliminate Rabies by 2023. According to the study done by the Indian Council of Medical Research (ICMR)-National Institute of Epidemiology (NIE), on Human rabies death and animal bite burden estimates in India, 2022-2023, more than 5,000 people die every year owing to dog bites. 'Nearly 9.1 million animal bites and 5,726 human rabies deaths due to dog bites are estimated to occur every year in India,' states a study done by ICMR-NIE. 'Eighty per cent of the dog bite victims reported taking at least one dose of the vaccination. In order to eliminate dog-mediated rabies by 2023, India must continue to fast-track its action through a focussed health approach,' states a study done by ICMR-NIE. The Ministry of Fisheries, Animal Husbandry and Dairy, based on a release issued by the Ministry, mentioned that States/UTs have reported, as per the data reported on the Integrated Disease Surveillance Programme (Integrated Health Information Platform) portal under the Ministry of Health and Family Welfare, that 21,95,122 dog bite cases in India during Jan-Dec 2024.

HYDERABAD: Soon, dietary advice by Hyderabad-based ICMR-National Institute of Nutrition (NIN), based on the Union health ministry's recent regulation on oil and sugar, will be displayed across central institutions in India. These 'model posters' by NIN were prepared in collaboration with the Food Safety and Standards Authority of India (FSSAI). Their recommendation: 25 grams of sugar (about five teaspoons) and 30 grams of edible oils, ghee, and butter (six teaspoons) every day. This calculation is based on a healthy calorie intake of 2,000 per day. 'In the case of oil, a family that now uses two to three teaspoons each day can cut it down by just 10% — to about a quarter of a teaspoon. 'That has more health benefits,' he said. 'Added (direct) sugar, which we put in our coffee, tea, is only required in minimal amounts, as it provides empty calories and no nutrition,' said Dr Swetha. 'While often used for taste or as an instant energy boost we tend to consume it far beyond what is needed through honey, biscuits and other food items. In some households, sugar is added in vegetable curries too.' Watch out for hidden fats in food Similarly, people end up consuming both visible fats, which we put in the curries and invisible fats taken from nuts, seeds, dairy, and processed foods. 'Often, invisible fats are overlooked, leading to excessive total fat intake. Oil and ghee are also added to chapatis, rice, and even dough,' the dietician said. According to her, five tablespoons of sugar and six for oil is ideal. 'Though actual requirements can vary based on an individual's activity level and metabolism. The key lies in maintaining a balanced intake to avoid non-communicable diseases (NCDs). Even consumption of excess sugary items converts into fat,' said Dr Swetha. ACTIVITY AND GENES CRITICAL TOO Dr Avula Laxmaiah, a public health nutrition expert and retired ICMR-NIN scientist said that the rise in lifestyle diseases is not only due to unhealthy eating but also due to limited physical activity. 'In the past, lifestyle diseases were thought to mostly affect rich people. But now we see a huge increase among low-income groups as well,' he said. 'Another reason is that Asian Indian genes tend to store fat more easily,' he explained. 1 IN 3 HOMES IN HYD HIT BY NCD The Union ministry's initiative aims to curb the rapidly growing count of NCDs such as diabetes, obesity, high blood pressure, heart diseases, certain cancers, and other lifestyle-related illnesses. The awareness campaign, experts hope, will encourage people to reduce their intake of HFSS foods, and ultra-processed foods. Citing a 2025 study of the medical journal The Lancet, the central health ministry's directive states that obese adults in India are projected to rise from 18 crore in 2021 to 44.9 crore in 2050. This, alarmingly, will put India in the second spot globally. In Hyderabad, a survey by the city-based Helping Hand Foundation found that one in three households is affected by lifestyle diseases.

ICMR invites partners to launch and sell its new malaria vaccine

The Indian Council of Medical Research (ICMR) has invited the country's vaccine manufacturers to partner with it to launch and sell a malaria vaccine that its Regional Medical Research Centre, Bhubaneshwar, has developed. Called AdFalciVax, it is a recombinant vaccine, meaning parts of the genes that encode for targets are inserted into a cell, the target proteins are expressed, and then used to trigger an immune response. Malaria is a parasitic infection transmitted by mosquitoes. While the current vaccine protects against the most common one that causes infection in humans called plasmodium falciparum, there are at least four other parasites from the same family known to cause infection in humans. P. vivax is the other one that commonly causes malaria infections, especially in India. The others — p malariae, p ovale, and p knowlesi — cause fewer infections. While the pre-clinical studies have already shown positive indications, the company that collaborates with the ICMR will be responsible for further development, human clinical trials, and scale-up for commercial production. The vaccine contains the genetic material of two targets — one for a major protein called CSP found on the surface of the malaria parasite and another for a combination of parts of two proteins that affect the pathogen's lifecycle. 'There are some key differences in the ICMR vaccine as compared to the other two currently in the market. One, this vaccine used the DNA for the whole CSP protein instead of just a few parts, meaning it is likely to produce a stronger immune response. Two, the other target that is used prevents the development of the p. falciparum in the mosquito's midgut during its lifecycle. What this essentially means is that it prevents the infection not only in the vaccinated person, but stops the further spread of the disease by disrupting the lifecycle of the pathogen even when it has already been picked up by a mosquito from an infected person for further transmission,' said Dr Subhash Singh, project manager for development of the vaccine. Senior scientist Dr Susheel Singh and director Dr Sanghamitra Pati were part of the development process. The pre-clinical validation was done in collaboration with ICMR-National Institute of Medical Research and the National Institute of Immunology.
