An inquest docket has been opened following the deaths of 15 people in a head-on collision between a minibus taxi and a bakkie, the Eastern Cape Transport Department said yesterday.
'A total of 15 people have died and five others sustained injuries in a head-on collision involving a fully loaded minibus taxi and a bakkie. The accident occurred on the R63 road between Adelaide and Maqoma (Fort Beaufort) at about midnight on Saturday,' said the department in a social media post on Facebook.

It is alleged that the white Quantum, travelling from Qonce (King William's Town) to Cape Town, collided head-on with an Isuzu double cab that was travelling from Adelaide to Kwamaqoma.
According to the department, the driver of the Isuzu died on the scene and 13 passengers in the Quantum were also declared dead on the scene, including the driver.
The six other passengers in the minibus taxi were rushed to hospital for treatment. One of them later died, taking the number of deceased to 15.

The Durban High Court learnt this on Thursday in a trial that involves four males accused of murder, public violence, and inciting violence against Africans during the unrest. The men charged are Ravine Naidoo, Trevor Gopal, Tyreece Govender, and Timothy Govender. The victim, Nkosikhona Madlala, 27, from Nanda, told the court that when he was assaulted on July 12, 2021, his things were taken from him. 'I wish to clearly state that they took a Christian Dior leather bag, a 201 Nokia, a 2019 Y5 Huawei, two necklaces, an orange wool hat, size 8 boots from Zara, and a watch from Fossil,' Madlala told the court. So far, he has told the court that on the fateful day, he and his friends Linda Khuzwayo, Sandile Sambo, Njabulo Dlamini, and Sanele Merase, travelled in a Toyota Quantum to Tees Tavern in Phoenix to purchase cocaine. He said they passed through a number of roadblocks to get to Tees. According to him, they used cocaine with his friends when they were drinking alcohol so that they would not fall asleep. After purchasing the cocaine and getting ready to leave, he said they were blocked by a white Ranger bakkie and a crowd, which also came in cars and on foot. The crowd told them to get out of the taxi. He said that after they refused to sit on the ground after getting off the taxi, the driver of the bakkie fired a firearm, and they all ran away in different directions. However, Dlamini remained in the car as he was sleeping. After running away and finding himself back to where the Quantum was parked, Madlala told the court that he was forced to get inside the taxi, which was burning. He said that upon refusal, he was assaulted, adding that Sambo was also assaulted with golf clubs, baseball bats, sticks, and sjamboks. He said that at that moment, he also saw about three males dragging Dlamini, who was not moving. 'They placed him near the taxi at the rear wheel,' he said. Madlala said the assault was intense at this stage despite his cries asking for an apology. He said both Dlamini and Sambo were not moving during this time. He stated that this assault lasted for approximately 45 minutes. Afterwards, each one of them was placed with three pallets, and a liquid believed to be diesel or petrol was poured over them.

The armed robbers stole three vehicles from the church congregants - a Mercedes Benz, a Volkswagen Polo and an Isuzu KB. Police have since recovered all the three stolen vehicles in Mpumalanga and Gauteng Image: SAPS A church service in Klipfontein, Witbank, was violently interrupted on Wednesday evening when a group of armed suspects stormed the premises, robbing and kidnapping nine congregants, including the pastor. Two suspects have since been arrested, and the victims have been safely recovered. 'According to a report, on this fateful day, a group of more than five armed males stormed into the church premises, threatening the congregants with firearms. About nine church members, including the pastor and church elders, were present at the time, and were ordered to lie down,' said Mpumalanga provincial police spokesperson, Brigadier Donald Mdhluli. The victims were subsequently searched by the armed intruders, and tied with cable ties before being robbed of their mobile phones, wallets containing bank cards, laptops, as well as a computer screen belonging to the church. 'The suspects further demanded car keys and pin codes for the mobile phones that they took. The victims were also forced into three vehicles belonging to them (the victims). The suspects then drove off to a remote area in the bushes, where two of them remained to guard the victims while the others went away, only to return later and collected their two accomplices,' said Mdhluli. Family members of the victims later reported the incident to the authorities. 'The matter was escalated whereby a collaborative effort between the SAPS and Tracker Connect yielded significant results, as the vehicles were successfully traced (on Wednesday night), one near Vosman and the other in Silverton, Gauteng,' said Mdhluli. 'Thanks to the diligent work of Witbank detectives and Vosman Vispol, the law enforcement agencies managed to locate all victims in a bushy area in Kromdraai. One individual sustained minor injuries and was given medical treatment by the medical personnel.' In total, three vehicles were recovered: a Mercedes-Benz, which was found in Vosman in the possession of a 21-year-old suspect; a Volkswagen Polo, which was abandoned in Vosman; and an Isuzu KB bakkie, which was traced and recovered in Silverton, Gauteng. A 27-year-old man was arrested during the recovery of the Isuzu bakkie. 'The two suspects were arrested, with all the stolen vehicles recovered. The pursuit of the remaining suspects continues. Police urge anyone with information that may assist in apprehending these perpetrators to contact the Crime Stop number at 08600 10111 or send information via MySAPSApp,' said Mdhluli. 'All received information will be treated as confidential, and callers may opt to remain anonymous.' Meanwhile, acting provincial commissioner of police in Mpumalanga, Major General Zeph Mkhwanazi, has applauded the effective collaboration between the police and private security. "We are concerned with the incidences of such crimes, especially in places that should be sanctuaries for individuals seeking solace,' said Mkhwanazi. This comes after a passenger managed to board an Ethiopian Airlines flight from Johannesburg on May 25 with two training grenades in their checked luggage, only for the items to be discovered upon arrival in Addis Ababa. The company also denied that ACSA has the right to terminate its "provision of services" for hold baggage screening at the airport, adding that it has been providing hold baggage screening at the airport for over 25 years and is the legally approved service provider. However, ACSA disputes this claim, arguing that ACS operates without a valid contract and is in breach of procurement laws. In 2023, ACSA decided to insource the hold baggage screening (HBS) function, but was temporarily blocked by a court interdict obtained by ACS. According to the company, the matter is now set for mediation under the Minister of Transport, Barbara Creecy. ACS said that the recent security breach involving training grenades was due to human error, but could have been avoided if ACSA had permitted upgrades to the screening equipment. "For the past eight years, ACSA has prevented ACS from replacing HBS equipment at airports with more advanced and reliable screening technology. "Had ACS been permitted to carry out the long-term HBS equipment upgrade and replacement, the security incident that took place on 25 May may well have been avoided," the company said. Next Stay Close ✕ ACS also claimed that "as a result of ACSA's refusals to allow it to replace the HBS equipment, ACS had to resort to bringing several urgent proceedings before the High Court in an attempt to force ACSA to permit the interim replacement of Level 3 back-up machines". "ACS was successful in these proceedings and obtained three consecutive court orders against ACSA between November 2024 and May 2025". However, ACSA described this claim as "factually incorrect," stating that any replacement of HBS equipment must be approved by the South African Civil Aviation Authority (SACAA). ACSA added that SACAA has previously communicated to ACS that it lacks the authority to replace HBS equipment, as ACS is not an airport operator. "ACSA had issued a tender worth approximately R3.1 billion for the replacement of the entire HBS equipment. However, ACS obtained an interdict blocking ACSA from adjudicating the tender until the court's review of ACSA's decision to insource," ACSA said. ACS also accused ACSA of delaying notification of the incident by 10 days, which slowed their investigation. "Our investigation confirmed that the items were 'Free From Explosive' (FFE) grenades, which are deactivated and used for training purposes and are safe for transportation. While the suspicious baggage was detected at Level 1 and Level 2 screening points, it was mistakenly cleared by a screener at Level 3". In response, ACSA said that Ethiopian Airlines sent communication regarding the incident to an ACSA employee who was not the designated official as stipulated in the regulatory reporting arrangement, and "as a result, the communication was not immediately brought to the attention of the designated official". "When the designated official was made aware, the matter was immediately reported to SACAA, and ACS was notified. The issue of the delayed reporting is being investigated, and appropriate action will be taken". ACSA said. "While the matter of the delayed reporting is being addressed, this should not detract from the fact that the reason for the occurrence of the incident, which is negligent human error, took place under the watch of an unregulated and unlawful operator". ACS added that it has "zero tolerance to any form of security breach and deems this incident as gross negligence by the screener, hence this led to the dismissal".
