The contagion scale: From measles to TB which disease spread fastest?
She also considered how each infection is given a value called R0 (or R-nought) based on how many other people are likely to become infected from another. So, for an R0 of two, each infected patient will spread the disease to two others. Who will collectively then give it to four more. And so a breakout unfolds.
The R0 measure indicates how an infection will spread in a population. If it's greater than one (as seen above), the outcome is disease spread. An R0 of one means the level of people being infected will remain stable, and if it's less than one, the disease will often die out with time.
Circulating infections spread through a variety of routes and differ widely in how contagious they are. Some are transmitted via droplets or aerosols – such as those released through coughing or sneezing – while others spread through blood, insects (like ticks and mosquitoes), or contaminated food and water.
But if we step back to think about how we can protect ourselves from developing an infectious disease, one important lesson is in understanding how they spread. And as we'll see, it's also a lesson in protecting others, not just ourselves. Here is a rundown of some of the most and least infectious diseases on the planet.
In first place for most contagious is measles.
Measles has made a resurgence globally in recent years, including in high-income countries like the UK and US. While several factors contribute to this trend, the primary cause is a decline in childhood vaccination rates. This drop has been driven by disruptions such as the Covid pandemic and global conflict, as well as the spread of misinformation about vaccine safety.
The R0 number for measles is between 12 and 18. If you do the maths, two cycles of transmission from that first infected person could lead to 342 people catching the illness. That's a staggering number from just one patient – but luckily, the protective power of vaccination helps reduce the actual spread by lowering the number of people susceptible to infection.
Measles is extraordinarily virulent, spreading through tiny airborne particles released during coughing or sneezing. It doesn't even require direct contact. It's so infectious that an unvaccinated person can catch the virus just by entering a room where an infected person was present two hours earlier.
People can also be infectious and spread the virus before they develop symptoms or have any reason to isolate.
Other infectious diseases with high R0 values include pertussis, or whooping cough (12 to 17), chickenpox (ten to 12), and Covid, which varies by subtype but generally falls between eight and 12. While many patients recover fully from these conditions, they can still lead to serious complications, including pneumonia, seizures, meningitis, blindness, and, in some cases, death. Low spread, high stakes
At the other end of the spectrum, a lower infectivity rate doesn't mean a disease is any less dangerous.
Take tuberculosis (TB), for example, which has an R0 ranging from less than one up to four. This range varies depending on local factors like living conditions and the quality of available healthcare.
Caused by the bacterium] Mycobacterium tuberculosis, TB is also airborne but spreads more slowly, usually requiring prolonged close contact with someone with the active disease. Outbreaks tend to occur among people who share living spaces – such as families, households, and in shelters or prisons.
The real danger with TB lies in how difficult it is to treat. Once established, it requires a combination of four antibiotics taken over a minimum of six months. Standard antibiotics like penicillin are ineffective, and the infection can spread beyond the lungs to other parts of the body, including the brain, bones, liver and joints.
What's more, cases of drug-resistant TB are on the rise, where the bacteria no longer respond to one or more of the antibiotics used in treatment.
Other diseases with lower infectivity include Ebola – which is highly fatal but spreads through close physical contact with bodily fluids. Its R0 ranges from 1.5 to 2.5.
Diseases with the lowest R0 values – below one – include Middle East respiratory syndrome (Mers), bird flu and leprosy. While these infections are less contagious, their severity and potential complications should not be underestimated.
The threat posed by any infectious disease depends not only on how it affects the body, but also on how easily it spreads. Preventative measures like immunisation play a vital role – not just in protecting people, but also in limiting transmission to those who cannot receive some vaccinations – such as infants, pregnant women and people with severe allergies or weakened immune systems. These individuals are also more vulnerable to infection in general.
This is where herd immunity becomes essential. By achieving widespread immunity within the population, we help protect people who are most susceptible.

Build readiness against rising dirty-war risks
This August 6 marks 80 years of the US's nuclear bombing of Hiroshima, Japan, which permanently altered the trajectory of global security and warfare. The two bombs dropped in 1945, on Hiroshima and on Nagasaki three days later, killed over 350,000 people instantly and left another 400,000 dead within a few months — introducing the world to the devastating potential of weapons of mass destruction (WMDs). At the time, the yield or intensity of these weapons was relatively modest compared to the nuclear arsenal that exists today. Today, the world faces far more advanced, lethal, and widespread threats under the growing umbrella of 'dirty wars'. The scale of suffering and devastation is beyond comprehension, and today's weapons are exponentially more powerful. There are estimated to be over 12,000 nuclear warheads in the world. Many of the modern strategic nuclear weapons are thermonuclear weapons, having yields equivalent to a 100 kilo tonnes (KT) of TNT and above. This has the potential to destroy the world as we know it many times over. Tactical nuclear weapons can be 50 KT and less. Each WMD category brings its own horrors: While nuclear weapons incinerate cities, biological and chemical agents work invisibly, silently, and over time. (AFP) The India-Pakistan conflict captures the world's attention given both have nuclear capabilities. A nuclear bomb's destruction extends far beyond its immediate blast radius. It generates an intense fireball, a blinding flash, a shockwave and, most dangerously, ionising radiation. The radioactive mushroom cloud not only contaminates the target, but reaches far-flung regions, depending on wind conditions — rendering vast areas uninhabitable for decades. In the Cold War era, WMDs were broadly categorised under the NBC triad — nuclear, biological, and chemical weapons. Over time, radiological was added to this, creating the term CBRN. Each category brings its own horrors: While nuclear weapons incinerate cities, biological and chemical agents work invisibly, silently, and over time. Chemical weapons, although banned under international law, are still accessible to States and non-State actors. They can be created using widely available industrial and pharmaceutical chemicals, turning everyday compounds into agents of mass death. Sarin, VX, chlorine, and mustard gas are examples. Equally alarming are biological weapons — viruses and bacteria designed to infect and kill populations indiscriminately. The Covid-19 pandemic gave the world a preview to the havoc a simple virus can unleash, even if accidental. But when weaponised and intentionally released, pathogens could cripple societies, economies, and militaries in a matter of days. The proliferation of biological research facilities worldwide, some poorly regulated, raises grave concerns. The Wuhan lab controversy underlined the thin line between scientific exploration and potential dual use for bio-warfare. Biological and radiological warfare has become the preferred 'silent weapon' of the future, and many nations are covertly advancing such capabilities under the guise of health research. A critical issue lies in the imbalance of offensive and defensive capacities. While many nations possess the capability to launch CBRN attacks, few are adequately prepared or capable of defending themselves. Countries like China have built subterranean tunnel networks housing military assets in case of nuclear or chemical strikes. Such preparation indicates an evolving mindset — wars will no longer be limited to borders or frontlines. They will target critical infrastructure, command-and-control (C2) nodes, urban populations, and national willpower itself. Missiles, aircraft, and now drones can deliver munitions deep into the hinterland. In a dirty war, collateral damage will be immense — not just military installations but power grids, hospitals, transport systems, and even the financial sector can be targeted through kinetic, cyber, or biological means. Even conventional missile warheads will be toxic enough to cause permanent damage to the lungs, heart and brain. A dirty war is now a creeping reality. Technology has empowered even smaller nations and insurgent groups to wield devastating power. Leaders today appear less tolerant, more reactive, and increasingly reliant on first-strike doctrines. What can nations do? Preparedness must be comprehensive. This includes building CBRN shelters in key cities and military bases and training first responders and civilians for CBRN scenarios. It should be essential to stockpile antidotes, protective suits, and detection systems. The civil defence and NDMA should conduct regular drills and heighten awareness. Securing borders and labs against bio-accidents/terrorism, investing in early warning, air defence and missile defence systems, especially empowered by space and cyber domains would be essential. Civil defence needs revival. Strategic deterrence must go hand-in-hand with societal resilience. Dirty wars may not be declared, but their impact will be undeniable. Let us prepare, not panic. Delhi HC forms panel to determine autistic students school
Last Updated: New Delhi, Aug 5 (PTI) The Delhi High Court on Tuesday formed a committee of experts to evaluate whether a girl diagnosed with autism can be admitted in a private school in the capital or she needs a school for children with special needs. A bench of Chief Justice Devendra Kumar Upadhyaya and Justice Tushar Rao Gedela passed the order on an appeal filed by GD Goenka Public School against an order directing it to admit the child in Class 1, or in an age-appropriate class, as a fee-paying student. 'To allay apprehension in the mind of the appellant we propose constitution of a committee of experts, along with her mother and school counsellor, to evaluate the girl and give its opinion as to whether she can be admitted in G D Goenka or a school of children with special needs," the bench said. The bench also directed the doctor concerned to examine the child to conduct any study that was required to come to the conclusion. The court was hearing appeal against a July 1 order of a single judge of the high court, saying 'inclusive education" was about belongingness, recognising that every child had a place in the classroom not because they were the same, but because they were different, as the difference enriches the learning environment for all. The petition before the single judge bench claimed the child was born in May 2017, and in November 2019, a doctor suspected her to be suffering from autism and started her therapy, which was interrupted by Covid-19. The child was admitted to the school in academic session 2021-22 under the 'sibling clause" and her parents informed the institution about the speech delay in the admission forms. In December 2021, the child was diagnosed with mild autism and was recommended various therapies. When the offline classes resumed in April 2022 after the Covid-19, the parents submitted the diagnosis to the school and requested support in the form of a shadow teacher, or allowing 'special educator" to assist her in the classroom, the petition said. Due to constant pressure and lack of support from the school, education of the petitioner was stated to have been discontinued from January 2023. The court said the child should be permitted to attend school with the assistance of a parent-appointed shadow teacher, subject to the school's basic norms of decorum and safety. It further directed the Directorate of Education to monitor the reintegration of the child and ensure the school provides an inclusive and non-discriminatory environment. Maharashtra reports 5 new COVID-19 cases
Last Updated: Mumbai, Aug 5 (PTI) Maharashtra reported five COVID-19 cases, including four in Mumbai and one in Chhatrapati Sambhajinagar civic limits, on Tuesday. With the fresh additions, the tally of infections since January rose to 2,745, the public health department said. The count of fatalities remained unchanged at 43. The department said 42 of the deceased had comorbidities while one person died due to some other disease since January, an official statement said. Out of 43,526 samples tested by the health department, 1, 2,670 returned positive. With the addition of four infections, Mumbai's tally stands at 1,091 since January.
