Lieutenant Commander Gillian Hector and her colleagues, Master Warrant Officer William Mathipa and Warrant Officer Mokwapa Mojela, died after being swept off the submarine off the Cape coast.

They were involved in a training drill in stormy sea conditions off Kommetjie in September 2023.
A board of inquiry looking into the cause of the tragic incident has wrapped up its work and the report handed over to the Hawks for further investigations.
However, Hector, who has been looking for answers on what exactly caused his wife's death, said he has still not seen the report.
He has told Eyewitness News that he has been seeking answers from his superiors for over a year now after suspecting negligence, but he's been met with red tape.
Hector said his lawyers wrote to the office of the Navy again to try and obtain a copy to proceed with his civil matter.
He hopes a copy of the board of Inquiry probe will be made available to him as soon as possible.

Speaking from the homes of the bereaved families, National Police Commissioner General Fannie Masemola condemned the killing, calling it 'a declaration of war on the state.' 'This is not just an attack on police, it's an attack on law, order, and the very institutions meant to protect society,' Masemola said. The two officers were both seasoned veterans – Sintwa had 23 years of service, while Masenye had served for 17 years. Their deaths form part of a growing trend of violence against police. Since April, six officers have been killed in targeted attacks, with 27 officers having lost their lives in the past year. The Hawks are currently reviewing surveillance footage from both the Roodepoort Magistrate's Court and a filling station where the officers reportedly stopped before the ambush. Meanwhile, the SAPS also confirmed that a separate R150 000 reward is being offered for information regarding 10 suspects linked to the killing of Sergeant Lawrence Mtshweni, who was gunned down in Mpumalanga during a burglary response within 24 hours of the Roodepoort attack. Police offer R150k reward for information on whereabouts of alleged cop killer Jabulani Thabang Moyo
Police are offering a R150,000 reward for anyone with information on the whereabouts of Jabulani Thabang Moyo who is wanted in connection with the killing of two police officers in Roodepoort on Friday. Moyo allegedly shot dead the two trio task team officers who were transporting him back to the Boksburg prison after his court appearance. The detectives' firearms were taken and the suspect was seen driving the unmarked police car. Police said the car was found abandoned a few kilometres from the scene. The slain officers have been named as 51-year-old WO Vuyisile Sintwa and 45-year-old Sgt Simon Masenye. Sintwa had 23 years of service, having joined the organisation in 2002, while Masenye had 17 years of service. On Saturday, police commissioner Gen Fannie Masemola visited the families of the slain officers. Masemola said the service had lost three police officers in 24 hours in the line of duty- the two detectives attached to the Florida police station and a crime prevention officer attached to the Schoemansdal police station in Mpumalanga. 'This is not only an attack on the justice and crime prevention security cluster, but it is a direct attack on the authority of the rule of law and the state,' he said. Masemola said 27 men and women in blue had died in the line of duty in the past financial year. Six police officers have lost their lives to criminal attacks and ambushes from April to date. 'These are not just numbers, they were mothers and fathers who have left families behind to fend for themselves. Most of them were breadwinners in their households but, most importantly, they were members who dedicated and lay down their lives in the line of duty by ensuring that all people living in South Africa are safe and protected from callous criminals,' he said. He warned that it cannot be business as usual "when criminals have clearly declared war on the authority of the state". Masemola appealed to officers to "not die with the tools and resources that we have given you". 'The men and women in blue must act appropriately when circumstances allow them to act. Remember to always use force that is proportional to the threat., he said. Masemola said the investigation into how these detectives were shot was ongoing and being led by the Hawks. 'We are relying on the bystanders and witnesses as well as evidence gathered at the filling station where they stopped, from the Roodepoort magistrate court and where the shootings happened. We are following up on every piece of information that will assist us in solving this case.' He said whether Moyo was handcuffed and why members were travelling in a sedan were under investigation. Police have warned that Moyo is extremely dangerous and members of the public should avoid approaching him. 'He has no regard for life and we are warning anyone that has information to come forward and not approach this killer. Mathe said police are also offering a separate R150,000 reward to anyone who can assist police to find at least 10 suspects who shot dead 47-year-old Sgt Lawrence Mtshweni. He had 18 years of service and was attached to the visible policing unit.

SAPS offer R300 000 reward after 3 police officers killed since Friday
Less than 24 hours after two police officers were killed in Roodepoort on Friday, a police officer in Mpumalanga was shot and killed in an apparent ambush by robbers. National police commissioner General Fannie Masemola has offered two rewards, of R150 000 each, for information on either case. 18-year veteran officer killed responding to call In the most recent incident, Sergeant Lawrence Dumisani Mtshweni (47) and his colleagues were responding to a burglary in progress near when they were ambushed by about 10 armed suspects. During the attack, Mtshweni was shot in the upper body and rushed to hospital, but he later succumbed to his injuries. Mpumalanga provincial police spokesperson Brigadier Donald Mdhluli confirmed the sergeant's death. He served in the Visible Policing Unit at Schoemansdal police station and had 18 years of service. Following the shooting, Acting Provincial Police Commissioner Major General Dr Zeph Mkhwanazi deployed all available resources to locate the suspects. 'An integrated team of intelligence operatives, Hawks, the provincial tracking team, and private sector units are working around the clock to apprehend the suspects,' said Mdhluli. The police are urging community members not to protect criminals but to provide information that could lead to their arrest. 'Attacking police officers is an attack on the state,' Mdhluli added. Stolen firearm recovered in separate Bushbuckridge incident In a separate incident, on July 28, a police officer based at Nelspruit police station but residing in Bushbuckridge was robbed of his service pistol. The officer was walking towards his vehicle in the Bushbuckridge CBD around 19:00, after using public transport, when two men approached him from behind, overpowered him, and fled with the firearm loaded with 15 rounds of ammunition. Police opened a case and, following a tip-off, arrested three suspects, two men and a woman, on Thursday (August 7). The woman was found in possession of the stolen firearm at her residence. The gun and three rounds of ammunition were recovered. Three suspects appear in court The suspects – Fumani Avoid Mashego (19), Surprise Khoza (25), and Promise Thandi Mashego (23) – appeared briefly in the Mapulaneng Magistrate's Court on Friday (August 8). They face charges of robbery with aggravating circumstances, possession of an unlicensed firearm, and ammunition. All three have been remanded in custody ahead of a formal bail application scheduled for August 15.
