We must plan for our ageing society, with AI tips from Taiwan

As much of the world confronts the complex implications of ageing populations, middle-income countries like Malaysia face a particularly difficult balancing act: preparing for the demographic pressures of an aged society without the fiscal space or institutional maturity of more developed economies.
By 2030, an estimated 15% of Malaysians will be aged 60 or over – a milestone that marks the transition to an 'aged society'.
With overall fertility rates continuing to decline and life expectancy increasing, the country's population structure is undergoing a fundamental shift.
Unlike many high-income nations, however, Malaysia is ageing before it has become fully affluent, a dynamic that threatens to amplify labour shortages, economic stagnation, and health system overload.
Taiwan, facing many of the same headwinds, has responded with an assertive and future-oriented strategy – one that places artificial intelligence (AI) at the centre of its longevity planning.
Though smaller in scale, Taiwan's experience offers practical and timely lessons for Malaysia and other Southeast Asian economies navigating the challenges of population ageing in the digital age.
Technology not as a fix, but as infrastructure
In recent years, Taiwan has embedded AI into the heart of its public service delivery model, particularly in the care economy.
The government's AI Taiwan Action Plan and AI Basic Act have paved the way for a cross-sectoral push involving healthcare, labour policy, urban design and elderly care.
This includes investment in smart home technologies, AI-enabled telemedicine, fall detection systems, and cognitive health tools designed to delay dementia and promote social interaction among older adults.
At the same time, Taiwan's '10-minute care circle' model – where essential services are located within walking distance for elderly residents – is being reinforced by AI-powered logistics and remote monitoring platforms.
These initiatives reflect a deeper principle: technology must not be a bolt-on, but a core part of the social infrastructure required for an ageing population.
Malaysia's equivalent efforts remain more fragmented.
Digital health initiatives exist, but are typically pilot-based, with uneven adoption across states and districts.
A national AI-in-ageing strategy, explicitly linked to healthcare reform, urban planning and human capital policy, could provide the structure currently lacking.
Reframing older adults as economic actors
A second lesson lies in how Taiwan views its older population – not merely as dependents, but as contributors.
Labour-force participation among Taiwanese aged 65 and above, while still modest, exceeds that of most regional peers.
Public policies incentivise phased retirement, part-time reemployment and reskilling opportunities for older workers.
Many Malaysians, by contrast, still view retirement as an exit rather than a transition.
Our Employee Provident Fund (EPF) system was not designed for the longevity most Malaysians can now expect.
At the same time, the national average savings balance at retirement remains dangerously low.

AI can play a role here, too.
From intelligent job-matching platforms to modular, AI-driven upskilling tools, there is scope to create flexible pathways for seniors to remain economically engaged.
The key is to shift policy incentives – from exit to extension – and to build digital tools that support this transition.
AI adoption requires trust, not just tools
Perhaps the most subtle insight from Taiwan's experience is cultural: technology uptake depends as much on trust and usability as it does on innovation.
Studies among older Taiwanese show high interest in AI tools, tempered by concerns over data privacy, surveillance and replacement of human contact.
Taiwan's approach has been to build user trust through public education, community workshops and policy clarity on how data is handled.
Malaysia needs to follow suit.
With its highly diverse population and digital literacy gaps, trust will be a determining factor.
National campaigns that introduce AI tools in a practical, culturally sensitive manner – across languages and communities – will be essential.
Digital inclusion must become a core part of both ageing and AI policy.
Just as important is regulation.
Legislation must include clear protections on data transparency, opt-in consent and explanations in high-risk use cases such as health monitoring.
If Malaysia is to adopt AI across sensitive domains like eldercare, it must ensure legal frameworks keep pace.
Turning challenge into competitive advantage
Malaysia's window of opportunity is short but real.
If we can act early, we have the potential to turn our demographic challenge into a new wave of economic and social innovation.
The so-called 'silver economy' – ranging from elderly care robotics to financial services for retirees – is a US$15 trillion (RM63.5 trillion) global opportunity by 2030, according to the Asian Development Bank.
With a young tech sector, a growing middle class and a robust health tourism industry, Malaysia is well positioned to capitalise – provided it makes ageing a national priority.
A whole-of-government approach that coordinates ministries and agencies such as the Health Ministry and Malaysia Digital Economy Corporation (MDEC) alongside entities such as EPF, local governments and the private sector will be essential.
This will require political will, but also narrative change.
Ageing should not be treated as a looming crisis, but as a structural transition – one that, with the right mix of policy and innovation, can lead to a healthier, more inclusive and productive society.
Taiwan's story shows that the integration of AI into longevity planning is not only possible – it is practical.
For Malaysia, the alternative is to wait, underprepare and find ourselves overwhelmed by the demands of a demographic shift we knew was coming.

At the same time, 84% don't participate in sports or use walking or cycling for transportation. The Diet Problem Our eating habits aren't helping either. A shocking 95.1% of Malaysian adults don't eat enough fruits and vegetables daily, consuming only two servings instead of the recommended five. Meanwhile, one in five adults doesn't drink enough plain water. These alarming health statistics were presented at Monash University during a ceremony marking a significant new research partnership. Recognising this health crisis, nutrition company The collaboration, witnessed by Minister of Science, Technology and Innovation, Chang Lih Kang, will study how specific nutrients—including plant proteins, fibre, probiotics, and postbiotics—can improve digestion, reduce inflammation, and strengthen the immune system. CEO Emphasises Mission-Driven Transformation Speaking at the signing ceremony, Jason Leng, General Manager of Amway Malaysia & Brunei, highlighted the urgency of addressing Malaysia's health challenges through scientific innovation. We are here today because non-communicable diseases (NCDs) remain one of the most pressing health challenges in Malaysia. They are rising at an alarming rate, demanding not only lifestyle changes but also stronger scientific understanding and intervention. He emphasised that gut health has evolved from a niche interest to a critical area of study, now recognised as central to managing inflammation, immunity, metabolic function, and even emotional wellbeing. By combining Monash University's scientific expertise with Amway's long-standing investment in nutrition and innovation, it is setting the foundation for research that can truly impact lives by developing solutions tailored to the Malaysian population's gut microbiome and health profile, he explained. KUALA LUMPUR: In a major step forward for Malaysia's health and wellbeing landscape, Amway Malaysia and Monash University Malaysia have signed a three-year Master Research Agreement (MRA) to advance microbiome science in the fight against non-communicable diseases (NCDs) — a leading cause of death and disability in Malaysia. The pioneering initiative will specifically explore how targeted nutrition can improve gut health and help reduce the risk of chronic lifestyle diseases and NCDs such as obesity, diabetes, heart disease and mood disorders. The gut microbiome plays a vital role in digestion, immunity, metabolism and mental well-being, with NCDs estimated to affect one-third of Malaysians and cause nearly 72% of all premature deaths, making them the leading cause of death and disability in the country. The initiative is closely aligned with the objectives of Malaysia's National Science, Technology and Innovation Policy (NSTIP), which seeks to build a more competitive and socially responsible STI ecosystem. By supporting public health efforts and delivering impactful, community-level solutions, the collaboration reflects NSTIP's core vision of 'Innovation for Well-Being', a commitment to advancing science that brings direct benefits to the people. The signing ceremony was witnessed by the Science, Technology and Innovation Minister Chang Lih Kang. Meanwhile, representing Amway Malaysia at the ceremony was its general manager (Malaysia and Brunei) Jason Leng, while Monash University Malaysia was represented by its vice president (research and development) Prof Datuk Dr Faisal Rafiq Mahamd Adikan. 'This MRA will enable advanced scientific testing and clinical insights to guide the development of future wellness solutions. By combining Monash University's scientific expertise with Amway's long-standing investment in nutrition and innovation, we are setting the foundation for research that can truly impact lives by developing solutions tailored to the Malaysian population's gut microbiome and health profile,' said Leng. Led by Monash University's Dr Chong Chun Wei and Dr Lim Wei Meng, the research will examine how ingredients such as plant protein combinations, fiber combinations, probiotics, postbiotics and phytonutrients affect gut balance, nutrient absorption, inflammation and metabolic health. 'This research applies rigorous scientific methods to uncover how specific nutrients influence the gut microbiome and gut integrity and elucidate related metabolic pathways, with the goal of informing evidence-based strategies to prevent and manage lifestyle diseases,' said Dr Chong. One collaboration study will look at how a mix of soy protein, fiber, probiotics and other gut-friendly ingredients can help improve digestion, support good bacteria, reduce inflammation and strengthen the immune system. At the same time, another study will explore how these ingredients — both on their own and combined — can help keep the gut barrier healthy and support overall metabolism. #HEALTH: Do you really need antibiotics for a sore throat?

IN Malaysia, as in many other countries worldwide, there is growing concern over a silent, yet widespread epidemic caused by the excessive and often inappropriate use of antibiotics. This issue is particularly evident in the treatment of sore throats, which are usually the result of viral infections and do not require antibiotics. The widespread overprescription of these medications has serious and extensive consequences, leading to a troubling increase in antibiotic-resistant superbugs that pose a significant threat to public health, says Universiti Sains Malaysia senior consultant, department of otorhinolaryngology - head and neck surgery, Professor Dr Baharudin Abdullah. The misuse of antibiotics can lead to a rise in mortality rates and exerts a significant burden on healthcare infrastructure and the economy, draining resources and complicating treatment efforts across the board. "It is more critical than ever to enhance public awareness and promote responsible prescription practices to protect public health and ensure the continued effectiveness of existing antibiotics," says Dr Baharudin. The World Health Organisation's Global Antimicrobial Resistance and Use Surveillance System has categorised Malaysia as a high-alert nation due to the increasing threat of antimicrobial resistance (AMR). In comparison to other Asian countries, Malaysia is among the leading nations in terms of antibiotic overuse, akin to Thailand and China, where obtaining antibiotics without a prescription remains a prevalent issue. The Health Ministry has consistently raised alarms about the escalating issue of AMR, yet the trend of overprescribing antibiotics remains prevalent due to several key factors. IN DEMAND Many Malaysians mistakenly believe antibiotics can cure viral infections, and even think they are effective for pain relief. Many physicians in both public and private primary care settings are often influenced by patient expectations and diagnostic uncertainty, leading to the prescription of antibiotics even when they are not clinically indicated. Studies show that in Malaysian hospitals, the challenge of AMR is becoming increasingly apparent, with a surge in drug-resistant strains like Methicillin-resistant Staphylococcus aureus and Carbapenem-resistant Enterobacteriaceae. Dr Baharudin says this predicament results in extended hospital stays, increased healthcare expenses and a greater chance of treatment failures. "Treating a resistant infection can be up to three times more expensive than a non-resistant one, placing a substantial financial burden on both patients and the healthcare system." THE ECONOMIC IMPACT In addition to its effects on healthcare, AMR has major economic and environmental consequences. According to World Bank estimates, by 2050, AMR may cause the world economy to contract by as much as 3.8 per cent, which might result in the poverty of 28 million people. The need for more costly second-line therapies and extended hospital stays will drive up prices for the already overburdened healthcare system. Furthermore, the environmental impact is too great to overlook because overprescribed antibiotics frequently end up in wastewater, contaminating soil and rivers and facilitating the spread of antibiotic resistance in bacterial populations outside of hospital settings. While the statistics paint a grim picture, Malaysia is not without recourse. The country has already made significant strides in public health, from implementing national antibiotic stewardship programmes to conducting public awareness campaigns. However, these efforts need to be scaled up and reinforced with a long-term, multi-sectoral approach, says Dr Baharudin. "Strengthening enforcement of antibiotic sales, improving diagnostic capabilities in primary care settings, and fostering stronger collaboration between healthcare providers, policymakers and the public are critical next steps." Malaysia has a strong track record of advancing healthcare, which puts it in a unique position to lead efforts domestically and throughout Southeast Asia in the regional fight against AMR, he explains. Since Malaysia has already made great progress in improving healthcare - from bolstering universal healthcare laws to spearheading programmes for managing infectious diseases - it can support a regional AMR action plan that unites Asean countries in concerted efforts to stop antibiotic abuse. By advocating for stricter cross-border regulations on antibiotic sales, harmonising prescription guidelines and championing large-scale public awareness campaigns, Malaysia can set the benchmark for responsible antibiotic stewardship in Southeast Asia.
