
How cruise ship passengers can stay safe from the latest version of norovirus

For Americans planning cruise ship vacations this spring or summer, there could be reason to worry about more than rough seas, experts say.
Recent government cuts included top staff at the Vessel Sanitation Program, which is run by the U.S. Centers for Disease Control and Prevention and conducts inspections and investigates illnesses.
The reductions, part of larger Trump administration cuts, come amid a surge in cruise ship outbreaks fueled by a new strain of norovirus.
So far this year, there have been 16 illness outbreaks reported on cruise ships in the CDC's jurisdiction, mostly from norovirus, compared with 18 outbreaks in all of 2024 and 14 in 2023, VSP reports show. U.S. officials conducted nearly 200 inspections of 150 ships last year.
'Certainly it's something that would be on my mind if was getting ready to get on that cruise ship,' said Donald Schaffner, a food science expert at Rutgers University.
Here's what you need to know about staying safe on board:
U.S. Health Secretary Robert F. Kennedy Jr. launched a broad restructuring of the nation's health agencies in April. The move eliminated the CDC's Division of Environmental Health Science and Practice, which housed the cruise ship vessel program, along with others focused on asthma, air quality, lead exposure in children and other issues.
Four full-time CDC staff, including scientists, were dismissed, said Erik Svendsen, the division's fired former director. The program also employed 12 U.S. Public Health Service commissioned officers, who remain.
The officers, however, lack the expertise of the scientists who were let go, Svendsen said. In addition, the program was already understaffed, he said.
'It's going to put them back many months before they're really functional again,' he said.
A CDC spokesperson said in a statement that the VSP work 'has not stopped.'
'Critical programs in the CDC will continue under Secretary Kennedy's vision to streamline HHS to better serve Americans,' the statement said.
About 18 million people board cruises in North America each year, part of nearly 32 million passengers worldwide, according to the Cruise Lines International Association, an industry trade group. Cruise industry officials say the chance of contracting a gastrointestinal illness is far greater on land than on a ship.
Norovirus is the most common ailment tracked on ships, though other outbreaks do occur.
Of the 19 million to 21 million norovirus illnesses that occur each year in the U.S., less than 1% are tied to cruises, said Lee-Ann Jaykus, a North Carolina State University food microbiologist and virologist who has studied norovirus.
But even that fraction of cases can mean misery for many hundreds of people on ships who come down with symptoms that include sudden onset of vomiting, diarrhea and stomach pain that can last three days, according to the CDC.
Norovirus is highly contagious, often spread by food or on surfaces, particularly in crowded conditions. It is a short-lived illness for many people, but it can be dangerous for people with underlying health conditions, young children and those aged 65 and older.
There are many different types of norovirus, but typically one dominant strain causes illness outbreaks, Jaykus said.
This year, a new epidemic strain has emerged, called GII.17. This version has circulated at low levels for years, but it was behind a surge in U.S. norovirus outbreaks this past winter.
Between Aug. 1 and April 9, more than 2,400 suspected or confirmed norovirus outbreaks were reported to a CDC monitoring system, roughly double for the same period a year earlier. The GII.17 strain has been responsible for nearly 80% of them, the CDC reported.
'It's new to the population,' Jaykus said, which means most people don't have immunity to the germ, so it can spread more widely.
People infected with norovirus typically shed 'literally billions of viral particles,' said Schaffner, the food safety expert. And it only takes a few viral particles to make someone sick.
The first rule should be for sick people to stay home, he said. That's not possible on a cruise.
If people get sick on a cruise ship, they're required to report the illness and isolate themselves, but many fail to do so.
'You spent all this money for a fancy cruise and you're feeling a little bit under the weather, so you tough it out,' Schaffner said. 'But in the meantime, now you spread the virus."
Passengers should be alert for signs of illness, even in other travelers. Vomit, for instance, can spread norovirus particles into the air, Schaffner noted.
'If you're in a situation where you see someone vomit, immediately walk away from them, ideally into the wind,' he said.
Washing hands frequently is key, especially after using the bathroom or before eating and drinking. Using soap and water is best; wash for at least 20 seconds, scrubbing well. Hand sanitizer alone doesn't work well against norovirus, the CDC notes.
Cruise lines have extensive sanitation protocols in place that are monitored through the CDC's vessel program, which is paid for by industry through inspection and other fees that total tens of thousands of dollars per ship per year.
It remains to be seen how cuts to the program will affect inspections and outbreak investigations in the future, Schaffner said.
'If you want to have no disease outbreaks, all you have to do is fire all the epidemiologists,' he said. 'And there'll be no one there to investigate.'

By LAURAN NEERGAARD It's official: Older Americans worried about cognitive decline can stay sharper for longer by exercising both their bodies and their brains and eating healthier. That's according to initial results released Monday from a rigorous U.S. study of lifestyle changes in seniors at risk of developing dementia. People following a combination of healthier habits slowed typical age-related cognitive decline — achieving scores on brain tests as if they were a year or two younger, researchers reported in JAMA and at the Alzheimer's Association International Conference. It's not too late to get started -- study participants were in their 60s and 70s -- and it doesn't require becoming a pickleball champ or swearing off ice cream. 'It was the first time I felt like I was doing something proactive to protect my brain,' said Phyllis Jones, 66, of Aurora, Illinois, who joined the study after caring for her mother with dementia and struggling with her own health problems. It's too soon to know if stalling age-related decline also could reduce the risk of later Alzheimer's or other forms of dementia. But Jones and other study participants underwent brain scans and blood tests that researchers now are analyzing for clues – such as whether people also saw a reduction in Alzheimer's-related protein buildup. 'We're all on a cognitive aging clock and anything we can do to slow that clock down, to me, that is a significant benefit,' said Laura Baker of Wake Forest University School of Medicine, who led the study. Doctors have long encouraged physical activity and a healthy diet for brain fitness. Those steps fight high blood pressure and cholesterol, heart disease and diabetes, factors that increase the risk of dementia. But until now the strongest evidence that specific lifestyle changes later in life could improve how people perform on brain tests came from a study in Finland. Would it work for a more sedentary and culturally diverse U.S. population? With funding from the Alzheimer's Association and the National Institute on Aging, Baker's team tested the strategy for two years in 2,100 adults ages 60 to 79. Half of participants were randomly assigned to group classes for exercise and dietary changes plus brain-challenging homework – with peer support and coaches tracking their progress. They did a half-hour of moderately intense exercise four times a week -- plus twice a week, they added 10 to 15 minutes of stretching and 15 to 20 minutes of resistance training. They followed the 'MIND diet' that stresses lots of leafy greens and berries plus whole grains, poultry and fish. Nothing is banned but it urges limiting red meat, fried or 'fast food' and sweets, and substituting olive oil for butter and margarine. They also had to meet someone or try something new weekly and do brain 'exercises' using an online program called Brain HQ. Other study participants, the control group, received brain-healthy advice and minimal coaching — they chose what steps to follow. Both improved but the groups fared significantly better. Combining social engagement with exercise and dietary steps may be key, said Jessica Langbaum of the Banner Alzheimer's Institute, who wasn't involved with the study. 'Americans want to have that one easy thing – 'If I just eat my blueberries,'' Langbaum said. 'There is no one magic bullet. It is a whole lifestyle.' Moderately intense physical activity means raising your heart rate and panting a bit yet still able to talk, said Wake Forest's Baker. Pick something safe for your physical capability and start slowly, just 10 minutes at a time until you can handle more, she cautioned. Make it something you enjoy so you stick with it. Likewise there are many options for brain exercise, Baker said – puzzles, joining a book club, learning an instrument or a new language. Jones, a software engineer-turned-tester, learned she loves blueberry-spinach smoothies. Her favorite exercise uses an at-home virtual reality program that lets her work up a sweat while appearing to be in another country and communicating with other online users. Researchers will track study participants' health for four more years and the Alzheimer's Association is preparing to translate the findings into local community programs. Will people with stick with their new habits? Jones lost 30 pounds, saw her heart health improve and feels sharper especially when multitasking. But she hadn't realized her diet slipped when study coaching ended until a checkup spotted rising blood sugar. Now she and an 81-year-old friend from the study are helping keep each other on track. Hogan, whose real name was Terry Bollea, previously had leukemia and atrial fibrillation, an irregular heart rhythm, the report from the District Six Medical Examiner said. Hogan, 71, was pronounced dead at a hospital less than 90 minutes after medics arrived at his home in Clearwater, Florida to answer a call about a cardiac arrest on the morning of July 24, police said. Hogan was perhaps the biggest star in WWE's long history, known for both his larger-than-life personality and his in-ring exploits. Tsunami warnings fading after one of the largest earthquakes ever recorded. Here's what to know

(AP Photo) HONOLULU--One of the most powerful earthquakes ever recorded sent a tsunami crashing into a Russian port and smaller waves barreling across the Pacific to the Americas and south to New Zealand. The danger had largely passed more than a day after the 8.8 magnitude quake, which was centered off the coast of the Kamchatka Peninsula in Russia's Far East. No major damage was reported and tsunami warnings were canceled in most areas. Chile, however, raised its warning to the highest level early Wednesday for most of its lengthy Pacific coast and started evacuating hundreds of people. Here's what to know: Tsunamis are waves triggered by earthquakes, underwater volcanic eruptions and submarine landslides. After an underwater earthquake, the seafloor rises and drops, which lifts water up and down. The energy from this transfers to waves. Many people think of tsunamis as one wave. But they are typically multiple waves that rush ashore like a fast-rising tide. 'Tsunamis cross the ocean at hundreds of miles an hour — as fast as a jet airplane — in deep water,' said Dave Snider, tsunami warning coordinator with the National Tsunami Warning Center in Alaska. 'But when they get close to the shore, they slow down and start to pile up.' It could take minutes for waves to hit land next to the site of a major quake. It could take hours for tsunamis to cross the Pacific Ocean. The speed of tsunami waves also depends on ocean depth. They travel faster over deep water and slow down in shallow water. People were urged to stay away from coastlines until any wave surges passed in places as far away as Fiji, Samoa, Tonga, Federated States of Micronesia and Solomon Islands. Some tsunamis are small and don't cause damage. Others can cause massive destruction. In 2004, a 9.1 magnitude earthquake off Indonesia caused waves that leveled remote villages, ports and tourist resorts along the Indian Ocean across Southeast and South Asia. Some 230,000 people died. A 9.1 magnitude quake and tsunami ravaged parts of Japan's northeastern coast on March 11, 2011, killing about 20,000 people and triggering a nuclear meltdown. The 8.8-magnitude quake, which struck early Wednesday local time, was among the four strongest earthquakes this century, according to the USGS. It was also the sixth-biggest quake ever recorded, said Simon Boxall, a principal teaching fellow at the University of Southampton's Physical Oceanography Research Center. The earthquake, which was followed by multiple aftershocks as strong as 6.9 magnitude, occurred along the Pacific Ring of Fire, the ring of seismic faults around the Pacific Ocean where more than 80% of the world's largest quakes occur. Several tectonic plates meet there. The ring gets its name from the volcanoes that surround it. While not all earthquakes lead to tsunamis, this one generated a series of them spreading outward from the epicenter off the coast of Russia's Kamchatka Peninsula. 'It's a bit like throwing a very, very large rock into the sea and then watching the waves propagate away from that rock, that splash,' Boxall said. 'And so that's what's happened in this case. And that's why this particular one has generated a tsunami. It's not huge. It's not one that's going to cause mass devastation. But it will cause coastal flooding and it will cause damage, and it does put lives at risk if people don't move to high ground.' A tsunami height of 3 to 4 meters (10 to 13 feet) was recorded in Kamchatka, while tsunami waves about 2 to 5 feet (0.61 to 1.52 meters) high reached San Francisco early Wednesday, officials said. Other areas have seen smaller waves. The International Atomic Energy Agency said initial reports showed there had been no safety impact on nuclear power plants along Japan's Pacific coast. Damage and evacuations were reported in the Russian regions nearest the quake's epicenter, and officials declared a state of emergency in several areas. Several people were injured, but none gravely, and no major damage has been reported. Additional aftershocks are possible. A tsunami advisory remained in effect for parts of the northern California coast. In Hawaii, emergency authorities blast alerts to people's cellphones, on TV and radio and sound a network of sirens. In Alaska, some communities have sirens, and information is available on weather radio or public radio broadcasts. In the U.S., the National Weather Service has different levels of alerts: — A warning means a tsunami that may cause widespread flooding is expected or occurring. Evacuation is recommended and people should move to high ground or inland. — An advisory means a tsunami with potential for strong currents or dangerous waves is expected or occurring and people should stay out of the water and away from beaches and waterways. — A watch means that a tsunami is possible and to be prepared.
