Iowans express frustration, concern about state plan for expanded Medicaid work requirements

Iowans stressed the importance of health coverage and the difficulties of obtaining government recognition of medical conditions at the first public comment session on state's effort to add work requirements to its expanded Medicaid program.
The Iowa Department of Health and Human Services is seeking a waiver from the federal government allow it to implement work requirements for the Iowa Health and Wellness Plan, or IHAWP. The public hearing Wednesday at the Urbandale Public Library was the first of three meetings for public comment required by the waiver submitted to the U.S. Centers for Medicare & Medicaid Services.
IHAWP is the Medicaid expansion program signed into law by former Gov. Terry Branstad, a Republican. It provides public health insurance for low-income Iowans at or below 133% of the federal poverty level who do not qualify for regular Medicaid coverage — people between the ages of 19 to 64 who do not have a disability.

The waiver, announced by Gov. Kim Reynolds earlier in April, proposes requiring IHAWP recipients to work 100 hours per month to remain eligible for coverage. There are other ways to meet this requirement — a person can continue receiving state health coverage if they are enrolled in an education or job skills program, or if they earn the equivalent in wages to working 100 hours a month at the state minimum wage, $7.25 per hour.
There are some exceptions for the work requirements. People who are considered medically frail or medically exempt under Medicaid would not have to meet the work requirements, nor would individuals caring for a child under age 6, who have a high-risk pregnancy, or are in substance abuse treatment programs for up to six months. The waiver request also states that individuals who 'provide proof of good cause as defined by HHS' would be exempt, but did not include details on what standards would be used by the state department for assessing these claims.
At the hearing, several speakers talked about their own experiences or time helping others applying for public assistance coverage under the current Medicaid program. Nancy Lynch of Des Moines, a former school social worker, said she worked with parents of children with disabilities who were denied services and assistance — and said she believes work requirements would add another hurdle for people in need accessing health coverage and care.
'My job was to help support parents who had children with very special needs that needed help — needed help with health care, needed help with services — and I saw how difficult it was for them to receive those services,' Lynch said. 'No one got it on the first time. Everybody had to apply three and four times. They'd say the same thing, but they had to reapply and reapply and reapply. So they had to kind of beat down the system until they were finally accepted. … And my big concern is that these work requirements will add more barriers to people who need these services, they will lose the support they need.'
Robbin Mensching of West Des Moines said she was concerned about how HHS would assess mental illnesses and provide exemptions. Speaking about her own experience struggling to receive an official diagnosis of having a mental illness, she said it can be difficult for people with mental health issues to be recognized as having a medical issue. After receiving care, Mensching said she was able to more fully participate in life, but that denying that coverage could impact people's ability to join the workforce or care for their families.
'I want to know how they're going to handle covering these people, because there are a lot of people that have been like me,' Mensching said 'Anxiety, depression, panic attacks — it's disabling.'
Work requirements would be enforced through reporting. HHS stated that people on IHAWP would be required to report information showing the work requirements were met every six months. The department plans to create a mobile application for IHAWP members to submit required information showing their compliance with the work requirements if HHS does not already have the information available. If the required information is not provided, the individual's coverage would be suspended.
Mary Nelle Trefz, the advocacy network director of Iowa ACES 360, called for the state to share information about the cost of implementing work requirements, pointing to the need for the Iowa state government to create a new information technology system and train staff to implement and support the reporting requirements.
As Reynolds and HHS pursue a waiver for implementing work requirements, the Iowa Legislature has also moved this session to pass a bill implementing expanded Medicaid work requirements. There are some differences between the HHS and legislative policies — the bill proposes an 80-hour monthly work requirement, and includes 'trigger' language that would require the state to end IHAWP coverage for all recipients if the federal government revokes work requirements in the future after they were approved.
The legislation also includes an appropriation of roughly $8.2 million, according to the Legislative Services Agency, to set up the needed IT system for administering reporting requirements under the proposal. The funding to create this IT system is not a part of the HHS waiver request, Trefz said.
'That pending legislation also included support for IT, funding for that — but that legislation has not passed yet,' Trefz said. 'Those dollars have not been appropriated. So how will the state ensure the necessary infrastructure is in place?'
Multiple speakers expressed frustration that the HHS staff who hosted the meeting did not answer several of the questions or direct responses from HHS or other state leadership posed about certain details of the policy or how it would be implemented, or if there would be changes made to address any of their concerns. Staff at the meeting said the comments and questions would be shared with state leaders, and would be used to answer questions about work requirements in future communications.
Mensching said she felt work requirements were already a 'done deal' in Iowa, but said she believed it was important for Iowans — especially those who would be impacted by the change — to speak out.
'I would have ended up living on the street at one point in my life (without health coverage), and then you've got all these other things that go with that — somebody getting health care for me, the police would be involved — I mean, it just goes on and on,' Mensching said. 'It's so much more efficient and humane to just give people coverage so that they can live a normal life.'
Trefz, alongside Stacy Frelund with the American Heart Association, said they were creating a 'protect Medicaid' coalition to coordinate organizing efforts to oppose the new work requirements.
There will be another public hearing held on the waiver request, on April 29 at 2 p.m. at Marshalltown Public Library. Comments can also be submitted through email and by mail through May 15, with information available on the public notice website for the waiver.

While House Speaker Mike Johnson recently claimed millions "will not lose their Medicaid unless they choose to do so," the reality is the bill's work requirements will create administrative barriers that will result in coverage losses due to poorly informed Medicaid beneficiaries. These communication barriers can be addressed by modifications to the Telephone Consumer Protection Act (TCPA). The TCPA, a decades-old law governed by the Federal Communications Commission (FCC), prohibits automated text messages without prior consent. The law has created a culture of confusion and risk aversion in healthcare administration. Healthcare administrators like lawyers and compliance officers view text messaging as a litigation minefield, while patients and families who need timely, accessible communication freely use digital messaging for everything else. This creates a glaring disconnect: people communicate through text messaging across all demographics, including Medicaid beneficiaries, yet Medicaid managed care organizations and state agencies continue relying on antiquated communication methods because they fear litigation from murky TCPA regulations. The FCC has already provided guidance permitting digital communication for Medicaid eligibility and enrollment. However, this guidance needs to extend further and establish an explicit, indisputable exemption for all Medicaid programs and Medicaid-related services. This clarity will provide authority that helps prevent unnecessary coverage losses due to communication failures. It will also lead to a welcome alignment between how health agencies communicate and how people want to communicate. As new work requirements loom, we can either continue prioritizing risk management gray areas over beneficiary communication needs, or we can adapt our approaches to align with the digital channels people use and prefer. We don't need to speculate about the impact of poor communication with beneficiaries because we witnessed it recently. During the 2023 Medicaid redetermination process, people were required to re-enroll to demonstrate their eligibility. The problem wasn't eligibility; it was communication. According to Kaiser, half of Medicaid enrollees were unaware of the process or knew little about it. Millions lost coverage not because they were ineligible, but because they didn't know about re-enrollment deadlines or couldn't navigate the bureaucratic documentation requirements. Many only discovered they'd lost coverage when they showed up for medical care. These coverage losses were avoidable with better, more efficient communication. After 25 years serving a patient population that is 90-95% Medicaid beneficiaries, I understand these are busy people juggling multiple responsibilities. The majority work in gig economy jobs, minimum wage positions, or jobs that don't provide affordable health insurance. Many work multiple jobs and communicate primarily via text message. When Medicaid agencies rely on outdated methods like snail mail and email, critical information gets lost. We have an excellent, accessible tool to address these communication inefficiencies. It is text messaging and we are afraid to use it. To address texting limitations and improve communication with the Medicaid population, Abner Mason, a digital health entrepreneur and longtime advocate for text messaging in healthcare, aims to establish the Medicaid Texting Coalition. The coalition will collaborate with and advise federal agencies to exempt Medicaid from TCPA restrictions. Mason says, "Texting is the primary and preferred mode of communication for all Americans, including Medicaid beneficiaries, and we must use it to improve health outcomes and, if the time comes, to allow efficient communication to verify work status." If we care about reducing the impact of the BBB on Medicaid enrollment, an obvious and politically feasible strategy is to exempt Medicaid programs from TCPA restrictions immediately. The technology exists and the solution is straightforward. Medicaid needs a TCPA exemption now because the freedom to text is among our best hopes for minimizing the unnecessary impacts of potential work requirements.

Oz made the comments during an interview on Fox Business on Wednesday, arguing that when Medicaid was created in the 1960s lawmakers did not include work requirements because it 'never dawned on anybody that able-bodied people who work would be on Medicaid.' 'We're asking that able-bodied individuals who are able to go back to work at least try to get a job or at least volunteer or take care of loved-one who needs help or go back to school,' he said. 'Do something that shows you have agency over your future.' If Americans are willing to do that, he added, they should be able to be enrolled or stay enrolled in Medicaid. 'But if you are not willing to do those things, we are going to ask you to do something else. Go on the exchange, or get a job and get onto regular commercial insurance. Trump's sprawling agenda bill calls for trillions of dollars' worth of tax cuts, with offsets from sweeping cuts to federal benefit programs, primarily Medicaid. The bill would reduce federal Medicaid spending by $793 billion, according to an analysis from healthcare policy nonprofit KFF. Most of those savings stem from new work requirements for adults trying to enroll in the public health insurance program and more frequent eligibility checks. The bill calls for states to impose work requirements for childless adults between the ages of 19 and 64, with some exceptions, to be eligible for Medicaid. Adults would be required to work or volunteer at least 80 hours a month beginning in December 2026 to qualify for the public health insurance program. Many Trump allies in Congress have sought to downplay the impact of the Medicaid reforms, denying it will reduce access to the program. Meanwhile, several Senate Republicans have raised alarm over Medicaid cuts. Office of Management and Budget Director Russell Vought recently told CNN's Dana Bash that concerns over the bill are 'ridiculous.' 'This bill will preserve and protect the programs, the social safety net, but it will make it much more common sense,' he said. 'No one will lose coverage as a result of the bill.' Democrats have pushed hard against the proposed cuts, while GOP senators such as Lisa Murkowski (Alaska), Susan Collins (Maine) and Josh Hawley (Mo.) have pushed for changes to the bill passed by the House last month.

America's Dental Health Is in Trouble

Fournier, a dentist who practices at an urban hospital in Louisiana, had to extract six of the teen's teeth; he was eventually intubated and admitted to the intensive care unit because his airways had closed up. He survived, but Fournier is worried that there will be more children like him across the country who could come close to death because of the state of their oral health. 'I really worry that someone is going to die because they have an abscessed cavity that develops into an infection, and they won't be able to access care,' she says. In the U.S., 27% of adults don't have dental insurance, according to the most recent State of Oral Health Equity in America by the CareQuest Institute for Oral Health, a nonprofit that advocates on behalf of better oral health care. That's about 72 million Americans. By comparison, 9.5% of adults don't have health insurance. And though many children can get dental care through Medicaid or the Children's Health Insurance Program (CHIP), low reimbursement rates mean that many dentists won't accept those insurance plans, leading to dental-care deserts across the country. Only about half of all children on Medicaid used any dental service in a year, according to an analysis by KFF. Now, dentists say they're worried that a perfect storm of public-policy changes could further worsen oral health across the country. Proposed cuts to Medicaid would mean that fewer people will be able access dental care, as federal government staffing purges target places like the prevention division of oral health at the U.S. Centers for Disease Control and Prevention (CDC). What's more, as states including Florida and Utah vote to ban the addition of fluoride to drinking water and other states consider similar bans, dentists say the oral health of children and adults will suffer. 'We are already facing an oral health crisis,' says Melissa Burroughs, director of public policy for CareQuest. 'Medicaid cuts and water fluoridation rollbacks are the two biggest ways in which the oral health crisis is likely to be exacerbated.' America has long separated dental health from medical health. In most cases, Medicare, the federal health insurance program for older adults, doesn't cover dental care at all. Dental care through Medicaid varies tremendously from state to state, and states are not required to include dental coverage for adults, though they are required to include it for children. People going onto the Affordable Health Care marketplace for health plans can't purchase a dental insurance plan independently unless they also purchase a medical health plan. And subsidies offered to lower-income families on the health marketplace don't apply to dental plans. Even those people with dental insurance coverage often find that their plans don't cover much outside of a dental cleaning and check-up. About 40% of adults who have health insurance don't get regular dental care, according to one recent survey from the PAN Foundation, a health care advocacy organization. Not having dental health care can come with major consequences. Tooth decay and gum disease can exacerbate other health conditions and lead to heart disease, low birth weight in pregnancy, and even respiratory disease. Adults who present to emergency departments for tooth pain often end up with opioid prescriptions, which can lead to addiction. If children's teeth hurt, they may have trouble eating, leading to poor nutrition; if they're in pain, they're likely to sleep poorly. The CDC estimates that 34 million school hours are lost each year because of unplanned dental issues. Read More: The Science Behind Fluoride in Drinking Water 'You can find lots of studies that find associations between poor dental care and things like pneumonia and diabetes and heart disease,' says Dr. Lisa Simon, an internal medicine specialist who started her career as a dentist and then went to medical school to focus on oral health care. 'But even if you didn't think about any of those things, how important is it to have a central feature in our face look the way we want to, and not live with pain, and be able to take in nutrition?' Simon practices in Massachusetts, a state with one of the best dental safety nets in the country, and generous Medicaid benefits compared to those in other states. But she still sees people who have ended up in the ICU because of life-threatening sepsis from a tooth infection, patients who can't start chemotherapy because they can't pay to remove their infected teeth, people who won't even let her look into their mouths because they're so ashamed. In Massachusetts, fewer than one third of dentists accept Medicaid, which is close to the national average. 'I have gone down to Haiti nine times, and I have never seen the level of decay that I saw when I worked in Florida,' says Fournier, the Louisiana dentist, who previously practiced in Florida. She and other dentists worry that looming Medicaid cuts would exacerbate the problem; when state budgets are tight, dental care is often one of the first things to go. Massachusetts, for instance, cut Medicaid coverage for adult dental care in 2010 in the aftermath of the Great Recession; dental-related visits at a safety-net hospital increased 14% in the two years after the Medicaid cuts. Fournier recently testified before the Louisiana House of Representatives about Senate Bill 2, which sought to make it more difficult for localities to add fluoride to their drinking water. (In Louisiana, only about 38% of people are served by community water systems that fluoridate their water.) The bill was voted down in committee, but bills to restrict access to fluoride have been introduced in other states, including North Carolina, Massachusetts, Ohio, and Nebraska, according to CareQuest. Bills to ban the addition of fluoride in public drinking water have already passed in Utah and Florida. Some local counties have already voted in 2025 to ban fluoride independently. They are likely influenced by the Make America Healthy Again movement, led by Robert F. Kennedy Jr., Secretary of the Department of Health and Human Services. He has called fluoride a 'dangerous neurotoxin' and has said he wants the CDC to stop recommending fluoridation. In May, the FDA announced that it was trying to remove ingestible fluoride tablets from the market. Read More: What to Do If Fluoride Is Removed From Your Water Dentists predict long-term and costly health problems if communities continue to remove fluoride from the water. One recent study published in JAMA Health Forum found that the elimination of fluoride from the public water supply would be associated with a 7.5% increase in tooth decay and cost about $9.8 billion over five years. Places that have taken fluoride out of their water supply have seen an increase in dental problems; in Canada, for instance, Calgary removed fluoride in 2011, saw a significant increase in cavities, and is now reversing course and adding fluoride back in. Dr. Jeff Otley, a practicing dentist in Florida's panhandle, says he noticed when his region stopped fluoridating its water in 2014. He saw an increase in the number and severity of cavities in kids. The recent ban on fluoridation in Florida is going to affect kids and adults, he says, especially because Florida's Medicaid program offers barely any benefits for adults. 'We are going to have more disease, larger cavities, and some of these kids are going to have to go to the hospital because their cavities are going to be so bad,' he says. Oral health advocates say that in recent years, the country had been making some progress in improving access to dental care. For instance, a bill introduced in the Senate in March would require Medicare to cover dental, vision, and hearing. And some states have, in the last few years, expanded Medicaid benefits to cover adult dental services. This can end up saving money in the long run; when Colorado chose to expand Medicaid adult dental benefits under the Affordable Care Act, one safety-net provider saw a 22% decrease in tooth extractions, according to CareQuest. When states increase how much dentists can be reimbursed through Medicaid, more dentists sign up as Medicaid providers, which has been shown to increase children's dental visits. But advocates say they're worried that all of this progress is now going to be reversed, and that oral health in the U.S., especially for children, is going to suffer. Read More: How Having a Baby Is Changing Under Trump 'I think we're at this balancing point where if we can keep things moving forward, there is the real opportunity for millions of people to get dental care,' says Simon, the Boston doctor and dentist. 'But we've seen this before—anytime there's a budget shortfall, dental care is the first thing on the chopping block.' The irony of this to many dentists is that providing people with preventative care can actually save states money over time. Children on Medicaid who received fluoride treatments saved between $88 and $156 each for their state programs, one study found. Water fluoridation is another preventative policy that saves money: In 2024, the CDC estimated that providing communities with fluoridated water for one year saves $6.5 billion in dental treatment costs and leads to 25% fewer cavities. But some of these preventative ideas aren't likely to go far, says Amy Niles, the chief mission officer of the Pan Foundation. 'In this country, we don't always embrace the importance and value of preventative care to prevent disease later on,' she says. Fournier, the Louisiana dentist, is relieved that her testimony and that of other medical professionals helped persuade Louisiana legislators to ditch the fluoride bill. But she still chafes at a health care system that makes it so hard to provide preventative care for oral health. 'Our goal is aligned with RFK Jr.'s, which is to make Americans healthy,' she said in her testimony. But, she says, America doesn't seem interested in waging a war on the No. 1 chronic disease in children: tooth decay. Contact us at letters@
