Air safety reporting under scrutiny as crashes lie unresolved
SEOUL/PARIS - Next month marks three years since a China Eastern jet plunged into a hillside killing 132 people, with relatives still waiting to learn what caused China's deadliest air disaster in three decades.
It is one of dozens of accidents worldwide in which investigators have yet to issue a final report designed to help prevent new accidents, despite a target of one year.
While recent deadly crashes in Kazakhstan, South Korea and the United States and the non-fatal flip of a crash-landed jet in Canada have thrust safety into the spotlight, the industry is worried that too many past accidents remain unresolved.
According to airlines, almost half of 268 accidents involving fatalities or major damage between 2018 and the end of 2023 lacked a final report.
"That's a really big concern," said Mark Searle, head of safety at the International Air Transport Association (IATA).
"The lessons that we learn from accident reports are absolutely critical to avoid future events."
Aviation safety has improved markedly over decades based on open sharing of information, with investigations intended to draw lessons rather than assign blame.
The "brace position" for emergency landings, for example, was refined over years thanks to such investigations. By pure chance, the least injured person in a fatal 1976 crash in New Jersey had his head between his knees due to air sickness.
Technology to avoid collisions, the importance of not inflating life jackets inside planes and improved seat design are all lessons learned from past crashes.
Coordinated by the U.N.'s International Civil Aviation Organization (ICAO), global guidelines call for an initial report in 30 days and a final one ideally within a year. Failing that, investigators should issue statements on each anniversary.
But in a recent paper, IATA and six other aviation bodies raised the alarm over delayed or non-existent final reports.
"I think a number are being held up at the political government level because they are narratives that perhaps they are not too keen to hit the public eye," Searle said.
Others blame judicial interference or a shortage of resources for independent investigations in many countries.
The warning comes as social media has transformed how the public interact with air disasters.
Fuelled by statements from U.S. President Donald Trump that January's mid-air collision in Washington stemmed from diversity policies and a helicopter flying too high, a flurry of rumours circulated online about pilot identities and accident causes.
It is often only when publicity fades that forensic work can unlock the multiple factors in an accident, experts say.
The resulting final reports carry particular weight because they are based on validated data.
Three years on, however, the China Eastern crash remains shrouded in speculation and no final report has been published - only a preliminary one and two anniversary updates.
At three and seven paragraphs long, these contain scant data compared to other major accidents, such as the 150-page interim statement Japan published on a fatal collision in January 2024.
Following the China Eastern crash, investigators examined crew actions after finding no malfunctions, two people briefed on the matter said at the time.
In May 2022, the Wall Street Journal reported that black box data indicated someone had intentionally crashed the Boeing 737, citing a preliminary assessment from U.S. officials.
China's regulator CAAC has said speculation "gravely misled the public" and interfered with accident investigation work.
Chinese lawmakers will now consider significant changes to the country's civil aviation law, including measures against spreading rumours about aviation safety, CAAC said this week.
CAAC did not respond when asked if it would issue a final report for the March 21 anniversary, or if the proposed law revisions were linked to the China Eastern crash.
ICAO, without commenting on any specific probe, said there can be challenges in releasing reports "particularly in cases involving security sensitivities or political considerations".
The guidelines have recently been amended to allow more information to be published earlier to curb speculation.
UNEVEN REPORTING
Even when reports are published, recommendations are sometimes ignored or findings disputed, experts say.
A pair of hotly debated Egyptian accidents almost a decade ago illustrates the uneven reporting practices.
In 2016, EgyptAir 804 plunged into the Mediterranean, killing 66 people. After eight years, a final report identified explosives as the probable cause.
But France's BEA agency said the report's accuracy was "questionable" and a cockpit fire likely initiated events.
Even so, the eventual publication of the report - which allowed those dissenting comments to be published - was seen as a victory for the diplomatic machinery behind safety reporting.
By contrast the Sinai crash of an Airbus operated by Russian airline Metrojet in 2015, which Moscow blamed on a bomb while Egypt doubted a terrorist link, has not produced a final report but suspicions of a criminal act were reported to prosecutors.
The International Society of Air Safety Investigators is worried that cases of "unlawful interference" are not always investigated from a safety perspective.
Even when a crime is suspected, civil investigations have a place alongside police probes, it said in a recent paper.
French investigators issued a report urging better mental health checks following the pilot-suicide crash of a Germanwings jet, whose 10-year anniversary also falls next month.
Despite the gaps, investigators argue the reporting system remains robust with tangible gains in safety.
ICAO data indicates reporting rates for fatal accidents rose to 76% between 1990 and 2022 from 41% between 1990 and 2016.
But the issue remains a "significant focus area," it said.

The air transport industry delivered another year of strong overall performance in 2024 on safety including showing improvements on the five-year average for several key parameters, the International Air Transport Association's (IATA) 2024 Annual Safety Report said. However, the performance was a step back from an exceptional showing in 2023. Highlights of the report are: • The all-accident rate of 1.13 per million flights (one accident per 880,000 flights) was better than the five-year average of 1.25 but worse than the 1.09 recorded in 2023. • There were seven fatal accidents in 2024, among 40.6 million flights. That is higher than the single fatal accident recorded in 2023 and the five-year average of five fatal accidents. • There were 244 on-board fatalities in 2024, compared to the 72 fatalities reported in 2023 and the five-year average of 144. Fatality risk remained low at 0.06, below the five-year average (0.10), although double the 0.03 reported in 2023. 'Even with recent high-profile aviation accidents, it is important to remember that accidents are extremely rare. There were 40.6 million flights in 2024 and seven fatal accidents. Moreover, the long-term story of aviation safety is one of continuous improvement. A decade ago, the five-year average (2011-2015) was one accident for every 456,000 flights. Today, the five-year average (2020-2024) is one accident for every 810,000 flights. That improvement is because we know that every fatality is one too many. We honor the memory of every life lost in an aviation accident with our deepest sympathies and ever greater resolve to make flying even safer. Notably, there were no controlled-flight-into-terrain (CFIT) accidents. • Airlines on the registry of the IATA Operational Safety Audit (IOSA) (including all IATA member airlines) had an accident rate of 0.92 per million flights, significantly lower than the 1.70 recorded by non-IOSA carriers. Conflict zones: The need for action Accidents and incidents related to conflict zones are considered security-related events and are not included in this report. While they do not appear in this data of this safety report, they, along with growing incidents of Global Navigation Satellite System (GNSS) interference, are a top concern for aviation safety requiring urgent global coordination. 'No civil aircraft should ever be a target—deliberate or accidental—of military operations. Governments must step up, enhance intelligence-sharing, and establish clearer global protocols to prevent such tragedies and safeguard civilian aviation,' said Walsh. Regional safety performance • Middle East and North Africa: With two accidents in 2024, the all-accident rate improved from 1.12 accidents per million sectors in 2023 to 1.08 in 2024 and was also better than its five-year average of 1.09. Fatality risk has remained zero since 2019. While no accidents were related to GNSS interference, it has emerged as a critical area of concern in the region. • North America: With 12 accidents, the all-accident rate improved from 1.53 per million sectors in 2023 to 1.20 in 2024 and was better than the region's five-year average of 1.26. Fatality risk has remained zero since 2020. The most common accident types in 2024 were tail strikes, followed by runway damage and runway excursions. While no accidents have been linked to debris from space operations, the increasing number of rocket launches presents challenges for air traffic management. • Asia-Pacific: With seven accidents in 2024, the all-accident rate increased from 0.92 per million sectors in 2023 to 1.04 in 2024 but remained below the five-year regional average of 1.10. Fatality risk was unchanged from 2023 at 0.15. There was no dominant classification for accidents in the region which included tail strikes, runway damage and turbulence, among others. • Africa: With 10 accidents in 2024, the all-accident rate rose from 8.36 per million sectors in 2023 to 10.59 in 2024, exceeding the five-year average of 8.46. Africa (AFI) recorded the highest accident rate, though the fatality risk remained at zero for the second year in a row. The most common accident types in 2024 were runway excursions, followed by those related to landing gear. Forty percent of all accidents involving AFI-based operators, were on turboprop aircraft. Through the IATA Focus Africa initiative, the Collaborative Aviation Safety Improvement Program (CASIP) continues to mobilize resources to address key safety challenges. • Europe: With nine accidents in 2024, the all-accident rate increased slightly from 0.95 per million sectors in 2023 to 1.02 accidents in 2024. This rate is on par with the region's five-year average accident rate of 1.02. The fatality risk rate increased from zero in 2023 to 0.03 in 2024. The largest proportion of accidents were related to tail strikes followed by runway excursions. Strengthening safety Delayed or incomplete accident reports deny critical stakeholders — operators, manufacturers, regulators, and infrastructure providers — vital insights that could further improve aviation safety. IATA's analysis of 2018-2023 accident investigations reveals that only 57% were completed and published as obligated by the Chicago Convention. Completion rates vary significantly across regions, with North Asia leading at 75%, followed by North America (70%) and Europe (66%), CIS (65%), Middle East and North Africa (60%), Latin America and the Caribbean (57%), Asia-Pacific (53%), and Africa (20%). 'Accident investigation is a vital tool for improving global aviation safety. To be effective, the reports of accident investigations must be complete, accessible, and timely. Annex 13 of the Chicago Convention is clear that this is a state's obligation. Burying accident reports for political considerations is completely unacceptable. And if capacity is the blocker, then we need a coordinated global effort to provide technical support to countries with limited accident investigation expertise,' said Walsh. Sharp rise in GNSS interference Data from the IATA Incident Data Exchange (IDX) highlights a sharp increase in GNSS-related interference, which can mislead aircraft navigation systems. While there are several back-up systems in place to support aviation safety even when these systems are affected, these incidents still pose deliberate and unacceptable risks to civil aviation. GNSS interference is most prevalent in Türkiye, Iraq, and Egypt, it said. Reports of GNSS interference — including signal disruptions, jamming, and spoofing — surged between 2023 and 2024. Interference rates increased by 175%, while GPS spoofing incidents spiked by 500%. "The sharp rise in GNSS interference events is deeply concerning. Reliable navigation is fundamental to safe and efficient flight operations. It is also important to note that CIS has limited accident information available and may undergo larger revisions than normal once more data becomes available. This may affect accident rate as well as fatality risk calculation. Europe: With nine accidents in 2024, the all-accident rate increased slightly from 0.95 per million sectors in 2023 to 1.02 accidents in 2024. This rate is on par with the region's five-year average accident rate of 1.02. The fatality risk rate increased from zero in 2023 to 0.03 in 2024. The largest proportion of accidents were related to tail strikes followed by runway excursions. North Asia: With a single accident, the all-accident rate increased slightly from zero accidents per million sectors in 2023 to 0.13 in 2024. Executive jet operations, military, and test flights are excluded. The aircraft is turbine-powered and has a certificated Maximum Takeoff Weight (MTOW) of at least 5,700 kg (12,540 lb.). Either the aircraft has sustained major structural damage adversely affecting the structural strength, performance or flight characteristics of the aircraft and would normally require major repair or replacement of the affected component exceeding USD1 million or 10% of the aircraft's hull reserve value, whichever is lower, or if the accident is relevant by ACTF, or the aircraft has been declared a hull loss. An event in which a person is fatally injured, as a result of being in the aircraft, being in a collision with the operating aircraft, being in direct or indirect contact with any part of the aircraft, including parts which have become detached from the aircraft being in direct exposure to jet blast. A hull loss is an accident in which the aircraft is destroyed or substantially damaged and is not subsequently repaired for whatever reason including a financial decision of the owner. In addition to the significant improvements in safety that have taken place since 2005, evident in safety performance statistics, there is a masked improvement from inflationary pressure. As the accident criteria has not adjusted for inflation since 2005, less severe accidents have come to be considered in evaluating safety performance.
