Once a niche childhood condition, attention deficit hyperactivity disorder (ADHD), has become one of the most common mental health problems.
The increase in diagnosis is not just in children, but also in adults. The numbers are staggering. In the US, ADHD diagnoses in children increased by 42% between 2003 and 2011, and, according to the US Centres for Disease Control and Prevention (CDC), adult diagnoses have risen even faster in the past decade.
According to ADHD Ireland, as of October 2024, ADHD affects approximately 5% of children and 3% of adults, amounting to 230,000 people.
The spike in ADHD diagnoses has prompted a vigorous debate about the legitimacy of these presentations. Are we finally recognising a condition that has long gone undiagnosed, or are we pathologising normal variations in behaviour and attention? Is this surge a reflection of increased awareness, or is it a symptom of something deeper?
Much of the polarisation stems from an inability to consider both perspectives. My 25 years of working in the mental health field have taught me that there is rarely one explanation for any condition or presentation, and being open to multiple causes can offer far more insight.
Technology, education, and work have undergone seismic changes in the past two decades, which may be reshaping not just how we live but also how we function and, in turn, how we diagnose dysfunction.
Mental illness is not a static, universal phenomenon. It is dynamic, context-bound, and shaped by society's values, expectations, and structures. So, while mental illnesses are ever-present, their presentation may change as society changes.
One of the most significant societal shifts contributing to the rise in ADHD diagnoses is the advent of the 'attention economy'. In the past two decades, our lives have become saturated with digital technologies designed to capture and monetise our attention.
Social media platforms, video streaming services, gaming apps, and news feed notifications ping throughout the day, demanding our attention.
The result is that we are constantly switching tasks, scanning headlines, and skimming content. This always-on environment does not just cater to a short attention span, it cultivates it.
It's not hard to see how typical traits associated with ADHD, such as difficulty sustaining attention, impulsivity, and restlessness, could be exacerbated or even produced by the demands and distractions of the modern world. Our brains are neuroplastic; they adapt to the environments they inhabit. And, increasingly, those environments are designed to create hyper-distraction.
Children growing up in this environment face particular challenges. Screen usage has soared, and opportunities for unstructured play and outdoor activity, which were once crucial outlets for youthful energy, have declined or become non-existent.
Classrooms, meanwhile, are increasingly focused on standardised testing and prepping young people for state examinations, leaving little room for creativity, varied learning styles, or divergent thinking.
Explosion in adult diagnosis
The story of ADHD in the 2020s isn't just about children. The explosion in adult diagnoses is one of the most remarkable aspects of the recent surge.
For many adults, an ADHD diagnosis can feel like a revelation that explains years of difficulty with organisation, time management, emotional regulation, or meeting deadlines.
Online forums and social media communities are awash with stories of people, particularly women, overlooked as children, but who now recognise symptoms in themselves, due to greater public awareness.
I worked in CAMHS (Children and Adolescent Mental Health Services) for years, and ADHD was primarily associated with hyperactive boys. In hindsight, girls, who are more likely to present with inattentive symptoms (daydreaming, disorganisation) rather than hyperactive and disruptive ones, and especially those who were academically functioning, tended to slip through the cracks.
An expanded understanding of ADHD was long overdue. Many parents who bring their child for an ADHD assessment see themselves in the questions being asked, prompting them to seek an evaluation. It's a legitimate reason, as ADHD can run in families.
However, the speed and scale of the diagnostic surge also raise reasonable questions about whether we are medicalising behaviours, which, in a different era, might have been seen as personality quirks, lifestyle mismatches, or responses to stress.
So, instead of asking if the diagnosis spike reflects an actual neurological difference, or whether people are seeking a diagnosis to explain their struggles with lifestyle management, why not ask if it is a reflection of an increasingly fragmented and demanding world?
Modern work culture may also significantly shape perceptions of attention and productivity. The rise of remote work and the 24/7 hustle culture have created environments where individuals are expected to be constantly available, responsive, and self-motivated. This flexibility is liberating for some, but overwhelming for others, particularly those who struggle with executive function: The ability to plan, prioritise, and stay on task.
What constitutes 'normal' productivity is being unrealistically redefined by cultural, social, economic, and technological standards. Any deviation can feel like failure when the baseline expectation is constant output. For individuals who struggle to focus or stay organised, this pressure can lead to burnout, anxiety, and a search for answers, which could culminate in them suspecting that they might have ADHD.
I see this in the many young people who feel overwhelmed by the demands of life and ask me whether I think they have ADHD. Some young people ask this not because they genuinely believe they have ADHD, but because they desire to belong to an ADHD community as a means of providing identity.
Digital platforms, such as TikTok and Instagram, contribute to this identity-seeking phenomenon as they are hubs for mental health content, including ADHD awareness.
While these platforms have democratised information and helped to de-stigmatise mental health struggles, the algorithms tend to reward dramatic, relatable content, which can sometimes lead to oversimplification, self-diagnosis, and misinformation. Messaging like, 'Do you forget where you left your car keys? Then you probably have ADHD' is deeply problematic.
Consider the recent surge in the diagnosis of gluten intolerance. The awareness of gluten intolerance has led to many people with coeliac disease being identified sooner and receiving treatment. At the same time, we have seen a raft of self-diagnoses of gluten intolerance.
That doesn't mean that coeliac disease is not real; it means that some people have taken it upon themselves to align with this group without the testing or clinical assessment.
Cultural causes
Ultimately, we need to consider how the surge in ADHD diagnoses reflects more than just medical or psychological trends. It is a cultural mirror, revealing how our society defines 'normal' behaviour, and how that definition is being strained by modern life.
We live in a time of relentless overstimulation and declining downtime. We expect children to sit still for hours, adults to juggle multiple roles, and to be constantly connected and perpetually productive. In such a world, is it any wonder that more and more people feel like their brains are not functioning 'correctly'?
This is not to say that ADHD doesn't exist. Of course it does. For many, diagnosis and treatment can be transformative, and going through life without a diagnosis or understanding can be detrimental. But as with all aspects of over-correction, it is also true that we might be diagnosing more people more quickly and broadly than ever before. And instead of only asking what is wrong with individuals, perhaps we should also ask what is wrong with the world they live in.
We need a more nuanced approach to ADHD that balances awareness and compassion with critical thinking and understanding. We need to better distinguish between genuine neurological conditions and socially induced struggles. We must also rethink how schools, workplaces, and digital environments are structured to allow for a broader range of human attention styles and learning modes.
Not everyone who struggles to focus has a disorder. Sometimes, the problem is not the person, it's the system.
As the conversation around ADHD evolves, let's make room for complexity. Let's respect the stories of those who have found clarity and healing through a diagnosis, while remaining vigilant about the forces, such as economics, technology, and culture, that may be shaping our understanding of what it means to be 'normal'.
The surge in ADHD diagnoses may tell us as much about society as it does about individuals.
I hope we find the insight needed to regain control over our attention and build a world that demonstrates an understanding of our brain's limitations, leading to fewer people struggling to keep up with an unattainable pace.

‘The system is under real pressure': why do so many Leaving Cert pupils need special supports?

Last Friday afternoon school principal Craig Petrie finally got the call he had been waiting for. Two students who injured their hands in recent weeks were finally approved by the State Examinations Commission for access to a scribe to write down their answers during the Leaving Cert exams which begin on Wednesday. 'It's a huge relief,' he says. 'If it had been any later, we might not have found out until the day of the exams.' Last week, the school also received late confirmation that two students who had been diagnosed with ADHD in recent months would have access to a small classroom instead of a large exam hall. READ MORE 'If they had been turned down for any reason, it would have been too late to appeal the decision in time. Appeals can take two or three weeks,' Petrie says. East Glendalough School in Co Wickow, where Petrie is principal, is a relatively small secondary school of 360 pupils. The process of preparing for the exams, however, is increasingly complex. [ Classroom to College: our essential newsletter on navigating the Leaving Cert for parents, guardians and students Opens in new window ] It has four main exam halls for the 120 students sitting Junior Cycle and Leaving Cert exams this year, as well as 12 smaller so-called special examination centres. They are provided under the 'reasonable accommodation' scheme, which allows for a range of adaptations for students with additional needs (such as anxiety, learning difficulties or physical disabilities) who would struggle to sit exams under normal conditions. It means students may be sitting the same exam but they won't be in the same room. Some will be in the traditional sports hall with large numbers of students, while others may be in a classroom, one-to-one with an invigilator or with a small number of other students. Some will be using a traditional pen and paper, while others will have laptops, 'reading pens' or access to scribes. The aim of the scheme is to remove barriers and provide a level playing field. The problem is the number of pupils who have these arrangements has increased so significantly that schools are struggling to find enough classrooms or qualified staff. In many cases, schools say they end up waiting until just days before the exams to find out if students have been approved for special supports. [ 'Early mornings worked best. Ten years later it had climbed to 13 per cent. Last year, the total number reached to a record 24 per cent, or 31,800 State examination candidates. If current trends continue, numbers for this year are expected to climb higher still. The vast majority of candidates availing of the reasonable accommodation scheme have learning difficulties, while vision- and hearing-impaired candidates make up a small proportion of the overall numbers (1-2 per cent). The most common type of special measures students receive include spelling or grammar waivers, reading assistance, use of a word processor or exemption from a subject or component. The sharp rise appears to reflect the growing prevalence of conditions such as autism and other learning difficulties. Most education experts put this down to an increased awareness of symptoms, better diagnosis and a significant widening of assessment boundaries for disorders. More controversially, experts also say the fact that a diagnosis can unlock supports means that clinicians are more likely to diagnose a child, even if they on the borderline of clinical criteria. In 2018, for example, a little more than 18,600 students sat their State exams in special centres. Last year, it climbed to 31,800 candidates, a 70 per cent increase. With such sharp growth in numbers, can the system continue to cope into the future? 'I think the system is under real pressure, but I do think it's fit for purpose,' Petrie says. 'The SEC [State Examinations Commission] delivers an extraordinary service when you think of the numbers. They will always answer the phone and respond, under huge pressure.' Another principal, who declined to be named, said we are at risk of reaching a point where a minority of students will be in traditional exam halls: 'We're just about responding to the needs. Too many students are finding out if they are approved late in the day. If the trends continue, it will pose serious logistical problems. A spokesman for the SEC said it operates a number of Race (Scheme of Reasonable Accommodations at Certificate Examinations) application processes with various closing dates, including a 'late application' process with a deadline of April 2025, and a separate 'emergency application' for candidates with broken bones, hospitalisations or other issues. Given the circumstances in which emergencies arise, the spokesman said the application and decision-making process for reasonable accommodation is 'time constrained'. SEC staff, he said, were working throughout the bank holiday weekend finalising applications, issuing decisions and putting arrangements in place with school authorities. 'All emergency applications are handled as a priority and all decisions will be issued in time for the start of examinations,' he said. Petrie feels we need to tread carefully before changing eligibility rules. 'I do worry that maybe we are over-diagnosing. The system does bestow benefits in terms of reasonable accommodation, access to the Dare scheme [lower CAO points requirement] and supports in college ... but this system is also here to help students. I'd be very wary of changing rules that would not make students entitled to supports aimed at levelling the playing field.'

Calmer? Now you can focus on what's at the heart of your decision. It may sound simple, but Sheena suggests clearly defining the decision, asking: 'What do I need to decide?' Step Two: Free Yourself From People's Opinions Now comes the hard part. 'Ask yourself: 'If no one else's opinion mattered, what would I truly want?'' says Sarah. "Many women have been raised to prioritise others' needs over their own, often without realising it. "Even if you consider yourself independent, societal messaging can still shape your decision-making process.' Advertisement But that doesn't mean you need to make decisions alone. 'Gather relevant information to understand your options,' says Sheena. For example, try talking to people who have chosen each option, so you can listen to their experiences, then decide for yourself. 'Fiction' is often the voice of self-doubt. ' Once you've looked at each limitation and have clarity that what's holding you back is a set of false, limiting beliefs, you are left with facts and data to help make an informed decision.' Step Four Let Go Of Perfection What if limiting beliefs come from a trait or mindset, such as perfectionism? Advertisement 'Many people believe a decision must be 100% right or else it's a mistake, but this kind of thinking keeps you stuck,' says Sarah. 'Instead of searching for the perfect choice, ask yourself: 'What is a good enough option that moves me forward?' "Also, it's OK to change your mind.' Step Five: Test It Out To put your decision to the test, use the 'future self' visualisation, which involves imagining yourself six months or even a year from now, having made the big decision. Advertisement 'Picture your day-to-day life,' says Sarah. 'What's changed? How do you feel? "Now, do the same for an alternative decision. "How does that version of your future feel in comparison? Advertisement "Often, your emotional response will provide valuable insight,' she says. You could even 'try on' your decision mentally for 48 hours. 'Act as if you have already chosen a path,' she says. "If you're debating moving abroad, tell yourself: 'I'm moving abroad', and notice how you feel. Advertisement 'Does this bring a sense of excitement, relief or peace? "Or does it trigger anxiety and resistance? "Your emotions can offer more clarity than over-analysing ever will,' she says. Step Six: Move things on Once you've decided, let go of the idea of possibly taking the 'wrong' path. 'It's important to make a decision and then learn from it, rather than stay in paralysis and let opportunity pass you by, or change your mind if it is not working out,' she says. Podcast reviews: Busting ADHD myths, using mosquitoes for good and the psychology in our everyday decisions

Neurodiversity is in the news a lot lately, though often for the wrong reasons, with some public figures having dismissed the increased incidence of autism as a 'fad', where others contend that diagnoses have increased due to improved clinical evaluation. As such, podcasts such as Climbing the Walls (Apple, Spotify) are a valuable resource, this new limited investigative series focusing on ADHD in women and girls which is historically misdiagnosed due to presenting very different symptoms to men and boys – who have been widely presumed as the default in trials and treatment. It's hosted by health and science journalist Danielle Elliott who, informed by her own experience of ADHD, talks to experts in the field about why there has been a surge in women and girls being diagnosed, the pros and cons of social media in raising awareness, and the impact of fluctuating ­hormones on symptoms. Alisha Wainwright, after ­studying botany in Florida, moved to Panama for her PhD research. 'Science is one of the toughest ­industries because everyone is vying for the same pot of money to fund their research,' she told W magazine. 'I had a very weird anthropological-­botanical focus, and I couldn't find anyone to give me the money to study it. I had to shift my focus to something a little broader, and I wasn't as interested in it.' And so she swapped botany for... acting, starring in TV series Shadowhunters and Raising Dion.
