Do Tattoos Pose a Cancer Risk?

Over recent decades, tattoos have rapidly gained in popularity, with a Narrative Research survey in 2024 showing that 31% of the adult Canadian population has tattoos. Moreover, women are much more inclined to get a tattoo, including permanent makeup, than men (38% vs 28%). In Canada, the US, and Europe, a patient must be about 18 to start accumulating tattoos.

As tattoos grow in popularity, patients increasingly seek advice from general practitioners and dermatologists about potential health risks. While adverse effects such as infections and allergic reactions are well-documented, the longer-term health implications of tattoos, particularly cancer risks, have only recently begun to receive more attention. Although this commentary centers on Canada, its implications extend globally. Let's explore the key concerns tattoos present in terms of cancer risks and how clinicians should approach them.
A patient may wonder whether injecting all manner of inks into the dermal layers of their skin poses any long-term risks. One might assume, wrongly, that since tattoos have been around for a very long time that cancer risks would have been exhaustively investigated. While there have been many case reports noting the occurrence of various types of cancerous lesions within tattoos, they do not tell us whether this association is causal or coincidental. Epidemiological studies exploring this question are few.
On the surface, it seems like an easy question to explore: Just compare cancer risk in those with tattoos to those without. However, one must remember that there are hundreds of brands and thousands of colors of ink on the market — and individual tattoos often contain many colors. Then there is the question of tattoo size. Moreover, if people stopped at one tattoo, that would make it much easier for epidemiologists; however, individuals who were happy with their first tattoo may go on to get many more over subsequent years, adding complexity to the calculation of an individual's overall exposure risk. Then there are the many confounding factors such as smoking, alcohol, socioeconomic status, etc, which must be accounted for.
Before we get into the epidemiological studies, we might want to know how tattoo inks are regulated. Health Canada has created a list of restricted or prohibited ingredients of tattoo inks. This list includes substances that are considered mutagenic, carcinogenic, toxic to reproduction, skin sensitizers, and irritants. The regulator also samples and tests tattoo inks to check for microbial contamination, heavy metals, and labeling accuracy. The European Union has taken a similar approach, but the FDA does not approve tattoo inks. Its regulation is mostly passive (eg, investigating safety concerns when adverse reactions are reported).
So, what can be found in these tattoo inks? Nonorganic inks can contain a virtual smorgasbord of metals, including barium, cadmium, chromium, cobalt, iron, nickel, lead, titanium, and mercury, though the latter largely has been phased out due to toxicity concerns. Some nonorganic inks contain acrylics or synthetic pigments. All these substances aid in long-term color retention. In contrast, organic inks are often made from plant-based or carbon-based pigments but tend to fade faster as they lack the metallic or synthetic stabilizers found in nonorganic inks. While the FDA requires accurate labeling of these products, a recent US study found that 83% of tattoo inks tested (45 of 54) had major label discrepancies — not that clients getting a tattoo would even read the label.
Many of the metals used are known carcinogens (eg, cadmium, chromium, lead, mercury, and nickel) or possible carcinogens (eg, cobalt and titanium), depending on the chemical species. Additionally, some of the pigments used, such as azo dyes, can decompose into carcinogenic aromatic amines. Other harmful substances in inks include polycyclic aromatic hydrocarbons, which have been proven to be carcinogenic to animals and humans.
What about cancer risks? Most studies to date have examined lymphomas, as it has been demonstrated in animal and human studies that most of the pigment from tattoos will be transported to regional lymph nodes. In this mostly final resting place, it could cause chronic inflammation and carcinogenic processes over time. A couple of case-control studies have found a significant increase in lymphoma risk in tattooed subjects relative to comparison groups (see Table). A well-designed twin study noted a higher risk, particularly for large tattoos (ie, bigger than the palm of the hand, which may not seem so large today). While another study from Sweden noted an elevated risk for lymphoma and lymphoma subtypes, no finding was significant. In contrast, a Canadian study found no trend for non-Hodgkin lymphoma, although tattoos were uncommon (around 5%) in the population studied.
Several studies have examined risks for various types of skin cancer. A study on cosmetic tattoos showed a nonsignificant elevated risk for basal cell carcinoma. Clemmensen et al, who conducted a case-control and a cohort study, showed significantly elevated risk for skin cancer associated with large tattoos in the former study and elevated risks for skin cancer and basal cell carcinoma in the latter study. In contrast, a recent study by Liljedahl et al found no evidence of risk for cutaneous squamous cell carcinoma. Aside from lymphomas, a few other hematologic cancers have been studied, but nothing significant has been found.

Ultimately, many uncertainties about cancer risks remain, and thus it is not surprising that some larger cohort studies have been established to answer these questions. A couple of examples include Tattoo inK, a cohort study of about 18,000 tattooed individuals and 160,000 untattooed controls within the German National Cohort, and Cancer Risk Attributable to the Body Art of Tattooing, a cohort study of about 13,000 tattooed individuals and 100,000 untattooed controls, which is integrated into the French Consultants des Centres d'Examens de Santé (Constances) cohort. While these are ample cohorts, one may have to wait one or more decades for findings to accrue.
A patient who smokes or drinks can quit to reduce his or her cancer risk. But what about a patient with a tattoo? The most common removal technique is laser therapy, which uses high-intensity light pulses to break down tattoo ink particles. Can this process remove the potentially hazardous particles? Well, not exactly, as these fragments end up being funneled into the draining lymph nodes. Sending more pigment into this region could increase risk, but we don't know for sure. Dermabrasion, chemical peels, or surgical removal can remove the ink but can also produce significantly more scarring and thus are usually reserved for smaller tattoos.
So, you have a patient with a new tattoo who is eager to hit the beach to show it off. What do you tell them? First, they should be warned that sun exposure will accelerate fading. And those degraded pigments don't just disappear, they release potentially harmful substances into the body. There is also a possible increased risk for skin cancer, so they're better off displaying their tattoos indoors or using sun-protective clothing or sunscreen. Beyond skin issues, tattoos may also increase the risk of lymphomas, but we do not know this for sure. Nor do we know much about risks for other types of cancer. Finally, tattoo removal is not the quick fix that works for other risky habits. Thus, for the patient considering their first tattoo, or more tattoos, one might say that if they can hold off for just another 10 to 20 years, we should be able to give a more definitive answer. Alternatively, given the current evidence, it would be premature to assure patients that tattoos are entirely risk-free.

Canada's measles case count has passed 4,500, with the western province of Alberta -- which has about five million people -- recording more cases this year than the United States, figures updated Thursday showed. The United States, confronting its worst measles epidemic in 30 years, has confirmed 1,375 cases, the Centers for Disease Control said this week. The Pan American Health Organization, WHO's regional office, said this month that 71 percent of confirmed cases occurred in unvaccinated people, with an additional 18 percent among people whose vaccination status was not known. Canadian experts have pointed to several factors driving the outbreak, including the proliferation of vaccine misinformation. Canadian physicians have criticized US Health Secretary Robert F. Kennedy Jr., who has spent decades spreading false information about vaccines. But the bulk of the Canadian epidemic has occurred among Anabaptist Christian communities -- of whom Mennonites are one -- where vaccine hesitancy is historic. The beginning of the outbreak has been linked to a Mennonite wedding in the eastern province of New Brunswick. Outside of Ontario and Alberta, which have larger Mennonite communities, cases have been isolated, with British Columbia the third-hardest hit province with 190 cases. The only suspected measles-related death in Canada during the 2025 outbreak was that of a newborn baby whose mother was unvaccinated, but officials noted the baby was born pre-term and had other medical conditions. bs/aha

The Scientific Benefits of Being a Cat Lover

Research suggests that cats may make us happier and healthier. August 8 is International Cat Day, but for cat lovers, every day is Cat Day. Cats wake us up at 4 a.m. to beg for breakfast and barf on that rug we just cleaned, yet around 23% of us are 'cats only people' — not dog people, not even equal-opportunity cat and dog people. In the United States, about 49 million homes have a cat. Why do we choose to bring these fluffy little beasties into our homes and spend an average of over $1,000 per year on them? All the cat lovers out there probably don't need scientific research to justify their fierce love. But scientists have studied it anyway and found that, while our feline friends may not always be good for our furniture, they likely contribute to our physical and mental health. Well-being Cats can improve your well-being by providing companionship and reducing loneliness, not to mention giving us a reason to laugh at their playful antics when they've had catnip or pounce on a new toy. Cat videos rule the internet for a reason, right? If Millennials (aka the ' wellness generation ') and Gen Z are any indication, cats are more a 'part of the family' than ever before. Millennials make up the largest percentage of current pet owners, and 59% of them own at least one cat. In some countries, like Japan, more people own cats than dogs. Many even throw birthday parties for their cats, and according to a 2024 survey, 82% of pet owners say their pets are 'like their own child.' So, cats are creating a sense of joy and family for many people. Of course, a cat's temperament does play a role in how close people feel to their felines. Interacting with cats has positive effects on their owners' brain. It improves mood and activates the prefrontal cortex and inferior frontal gyrus in the brain, contributing to physiological and psychological health. In one older study, people with cats reported experiencing fewer negative emotions and feelings of seclusion than people without cats. In fact, single people with cats were often happier than people with a cat and a partner. Similar to dogs, cats are honest, faithful, and don't criticize us, no matter what we look like or what we're going through — an ideal best friend. This helps us feel connected to other living beings in a way that might feel safer than being around certain people. A 2024 study confirmed that people who have a pet (a dog or a cat) are generally much happier than those without. Stress Interacting with animals has been shown to decrease levels of cortisol (a stress-related hormone) and lower blood pressure. In one older study, researchers visited 120 married couples in their homes to observe how they would respond to stress, and whether cats were any help. Hooked up to heart rate and blood pressure monitors, people did a series of daunting tasks involving math problems and sticking their hands in a bowl of ice. People either sat in a room alone, with their pet roaming around, with their spouse (who could offer moral support), or both. Before the stressful tasks began, the cat owners had a lower resting heart rate and blood pressure than people who didn't own any pets. During the tasks, the cat owners were more likely to feel challenged than threatened, their heart rate and blood pressure were lower, and they even made fewer math errors. Cat owners looked the calmest and made the fewest errors when their cat was present. They also recovered faster physiologically. There's actually an entire scientific scale that measures how much emotional support you get from your cat, based on how likely you are to seek them out in different stressful situations. Even spending just 10 minutes interacting with a cat can reduce heart rate and blood pressure. This positive effect increases if you've owned your cat for more than 2 years. Cats offer comfort and a distraction from our worries. But they're also not a replacement for therapy or sometimes medication when you're dealing with mental health issues. Despite how much we love the concept behind the novel ' We'll Prescribe You a Cat.' Relationships Domestic cats and humans care for each other (or at least we believe they do). This cross-species bonding may benefit our human-to-human relationships. For cat lovers, their cat/s can be part of their social network, and when someone — human or animal — makes us feel good and connected, it builds up our capacity for kindness and generosity toward others. Research has found that cat-only owners are more open, imaginative, and curious. However, they may also tend to be more self-contained and introverted — though that's not necessarily a bad thing. Owning a cat may also help people with social anxiety by giving them a sense of routine and emotional support. Health It's no secret that getting outside is good for you, and more cat owners are safely taking their indoor cats outdoors with the use of a harness and leash. We might not all have an internet-famous adventure cat like Sigrid or Gary, but taking your cat into the yard in a well-fitted harness can be a great way to bond while getting some fresh air. In a 2022 study, owners and their indoor cats had more emotional closeness compared to indoor/outdoor cats. This is yet another reason to only let your cat outside while being closely monitored on a leash instead of letting them wander the neighborhood, where they're at risk of being hit by a car or becoming prey to larger animals. Another way in which cats can improve your health? That favorite sound in the world to every cat lover: Purring. When a cat purrs, it creates a consistent, patterned frequency between 25 and 150 Hertz. Cats may purr to express hunger, affection, or stress. According to Dr. Leslie Lyons, director of the Feline Genetics and Comparative Medicine Laboratory at the University of Missouri College of Veterinary Medicine, purring at a low frequency of 25 to 50 Hz could stimulate muscles and even promote bone healing. The next time your cat settles next to or on top of you while purring, it may be in self-repair mode, but it wouldn't hurt for you to absorb some of that healing frequency, either. Cats may also improve: Though we need more objective studies, so far we know that cats can improve depression and provide social support, which can impact immune response and motivation to make positive health changes.
