A retired managing director killed himself and a married couple after accidentally pressing the accelerator instead of the brake pedal on his powerful new Audi, an inquest heard today.
Eye-witnesses said Humphrey Pickering, 81, mistakenly 'floored' the Audi A8, which hit a group of pedestrians walking along the seafront in the town of Beaumaris, on the island of Anglesey, at 55mph – 35mph over the 20mph speed limit - last summer.
Reverend Stephen Burch and his wife Katherine, both 65, who were visiting North Wales from their home, in Alcester, Warwickshire, died at the scene. Mr Pickering, who was not wearing a seat belt, also suffered fatal chest injuries.
Police crash investigator Meilir Hywel told an inquest, in Caernarfon, that Mr Pickering had pressed the accelerator pedal of his automatic grey car three times, causing it to accelerate sharply from 25mph to 55mph in just five seconds before the fatal collision, close to the town's historic pier.
CCTV footage showed no brake lights were activated as the car swerved erratically for 100 metres (330ft) along Alma Street in the resort, ploughing into four people, at 2.45pm on August 28 last year.
'Mr and Mrs Burch didn't have enough time to react and move to safety,' Mr Hywel said. 'For them the collision was probably unavoidable.'
Two other pedestrians suffered minor injuries.
The court heard that Mr Pickering had been trying to move his car from a parking space on Wall Street, near the Bulkeley Hotel, so his wife could get into the passenger side when the accident happened.
'The most likely explanation for the collision is a pedal misapplication,' Mr Hywel added.
Holidaymaker Jamile Serrano, who witnessed the accident, said the Audi came out of the narrow road slowly at first, but suddenly picked up speed. He was knocked to the floor and then heard a loud bang.
One woman said it was 'like a firework had gone off'.
Another eye-witness, Francis O'Rourke, described the car as being 'floored' and swerving erratically. Mr Pickering avoided a parked car and a horse-drawn carriage but collided with Mr and Mrs Burch before crashing into a nearby property, causing structural damage.
'The scene was complete chaos,' Mr O'Rourke said in a statement. 'It was just so shocking.'
Pathologist Dr Mark Atkinson said Rev Burch, who had recently retired, and his wife, a charity worker, died of multiple injuries. The couple had two sons and a daughter and were grandparents, the court heard.
Kate Robertson, senior coroner for North West Wales, recorded all three deaths due to a road traffic collision.
She said: 'The most likely explanation is the pedal misapplication which has led to the sudden, unintended acceleration. There's no evidence before me the incident was deliberate on the part of the driver.'
The coroner told relatives who attended the hearing: 'This is one of the most traumatic cases I am sure I have come across in the circumstances before me.
'There's nothing I can say which will ease the pain and suffering you will all no doubt be feeling.'
Mr Pickering, a former librarian, of Colwyn Bay, was described as being 'active' and was involved in his local Rotary Club.
But in a statement, Mr and Mrs Burch's daughter, Sarah Packwood, said the family had concerns about his ability to handle and operate such a powerful car 'given the driver's age.'
Her father was Vicar of St James, in Fletchamstead, Coventry, for 19 years before he retired at the end of 2023. He was also the chairman of the Great Alne Cricket Club, Ms Packwood said.
Mrs Burch, described as a talented musician and baker, also worked with the Church Pastoral Aid Society (CPAS) and worship ministry.
Describing her mother as 'a woman of deep Christian faith,' Ms Packwood added: 'She was a 'loving and dedicated wife, mother and grandmother. She had a servant heart….and was passionate about serving her community.'
Following the couple's deaths, a spokesman for the Diocese of Coventry said: 'It is with deep sadness that we share the news that Steve and Kathy Burch have been tragically killed while walking along Beaumaris Pier on the Isle of Anglesey. Steve was a popular member of our diocese and well known for his good humour and unwavering faith and many will also know Kathy well from her work with CPAS and worship ministry. As a couple they were still very active in retirement, running an alpha course leading to baptism and confirmations.
'Our thoughts and prayers are with Steve and Kathy's family at this time especially their children, David, Jonathan and Sarah as they come to terms with this devastating, unexpected loss.'
A tribute from the Great Alne Cricket Club read: 'Steve has been a GACC member for well over 20 years, holding various positions including Sunday captain, secretary, and most recently, chairman. A cherished member of the club, Steve racked up 1190 runs since records began.
'Best known for his unperishable forward defensive, he also took 15 wickets during his bowling days. His efforts in the field were unquestionable, and commitment to ensuring that the club succeeded in whatever format shall remain legendary.
'Both Steve and Kathy were vital members off the field, too. Steve would regularly score for the Saturday, Sunday and Wednesday teams; a role which is invaluable but often forgotten. Kathy was well known in Great Alne for her tremendous cricket teas, enjoyed by all who were able to tuck in.'

A 12-year-old girl has tragically died after she fell off an electric scooter and was struck by a passing car. Abby Gillon and a friend were riding e-scooters in Aston Township, a suburb of Philadelphia, on June 14 when they both fell and were hit by the car. Paramedics treated the pair at the scene before transferring them to a children's hospital in Delaware in critical condition, KYW reports. Abby died from her injuries two days later. Her friend, 11-year-old Bella Jones, survived the horrific accident. The driver involved in the crash is understood to have cooperated with police. No criminal charges were filed in the investigation. Abby's mother Lori Kralle said she feels 'broken' after the horror crash took the life of 'my best friend, my only daughter, my only child'. Kralle is working alongside Pennsylvania state Sen. Tim Kearney to pass legislation that would strengthen safety protocols for e-scooters. 'I don't want another family having to get that phone call or having to bury their child,' the grieving mother said. 'It shouldn't have happened'. The bill, dubbed Abby's Law, would ban people under the age of 16 from riding e-scooters. The proposed law would also cap e-scooter speeds at 20mph and require that 16 and 17-year-old riders wear helmets. Kralle, who has vowed to protect other youths from facing the same fate that befell her daughter, wants electric scooters to be formally addressed in motor vehicle codes. 'They're not actually covered in the motor vehicle code at all. So what that means is that they're not legal to drive on the streets,' she told KYW. 'The only thing they say in the motor vehicle code is that you can only operate them on private property, which is kind of crazy, because you know that's not happening.' Abby's Law is expected to be formally introduced to state lawmakers next month. The family has created a Facebook group to gain support for Abby's Law and safety measures the Kralles believe 'need to be put in place for children under the age of 18 who have access to these motorized devices'. Abby's loved ones encourage supporters to 'share Abigail's sacrifice with other parents and children with hopes that she will continue to save lives as we plead for changes to the laws governing these motorized devices'. The group has already amassed more than 3,300 members. A GoFundMe page was set up to support Abby's family which has raised almost $90,000. Another fundraising page was also started to support Bella through her recovery. She was badly hurt in the incident but is now back home recovering, NBC Philadelphia reported last month, a process which was expected to take some six months. Last month, a 21-year-old died in Washington state after falling off the scooter and into the path of a moving semi-truck, police said. Sean Keaton Nolan was one of two passengers on the scooter as they rode along the sidewalk near the university district at around 2.20am. E-scooter bans have been considered in multiple areas across the US in recent years.

One of Switzerland's richest men has been hit with a staggering speeding penalty of $110,000 (£80,000) for driving just 17mp over the limit. The millionaire motorist, and repeat offender, was caught on camera hurtling along a Lausanne street at 48mph in a 31mph zone. But under Swiss law, the faster you are - and the richer you are - the pricier it gets. The French-born tycoon, whose fortune runs into the hundreds of millions according to Swiss financial weekly Bilan, was clocked in August 2024 by an automated speed camera. Prosecutors wasted no time calculating his punishment, with the fine based not only on the severity of the offence, but also on his income, assets and lavish lifestyle. Under the penal code in the canton of Vaud, speeding fines for the super-wealthy can soar into six figures. While poorer offenders might swap their fine for a night behind bars, the Alpine elite can find themselves paying enough to buy a small chalet. In this case, the court ordered him to pay £8,800 immediately and warned a further £73,000 could be added if he is caught speeding again in the next three years. The man did not contest the decision, which was handed down in June. However, this is not the tycoon's first brush with the speed camera. Eight years ago, he was fined £9,000 and faced a suspended penalty of £55,000 if he reoffended within two years. Despite the steep fine from the August incident, this is not regarded as Switzerland's priciest speeding ticket. In 2010, a millionaire Ferrari driver in St. Gallen was fined a jaw-dropping £267,000 for a similar offence. The moment a car slammed into a New York City food truck, killing the driver and two pedestrians, has been captured in horrific surveillance video obtained by the Daily Mail. The New York Police Department said the wreck unfolded on Tuesday a little after 8:30am on 42nd Street near 19th Avenue in Astoria, a neighborhood in Queens. The video shows a gray Toyota Corolla flying down the road before striking two pedestrians and ramming head-on into the food truck. After the impact, the Corolla spun around and hit a Volvo sedan that was traveling northbound on 42nd Street while it was attempting to make a U-turn to go south. Debris from all the vehicles involved was scattered all over the street as police and paramedics rushed to the scene to render whatever aid they could. The 84-year-old man behind the wheel of the Corolla and the 70-year-old pedestrian were both transported to Elmhurst Hospital, police said. Neither of them survived. The other pedestrian who was hit, a 42-year-old man, was pronounced dead at the scene, according to police. The driver of the Volvo was unharmed and no other injuries were reported. Giancarlo Caruso was standing right next to the men who were standing on the street who were killed and said the car 'missed me by a millimeter'. 'All of a sudden I hear loud screeching from tires and a really loud engine roar. It sounded like it was right on top of me. Someone screamed really loudly,' Caruso said. 'As soon as I heard that scream and the screeching, I stepped back and a vehicle going about 60 miles an hour mauled two of the customers that were with me,' he added. 'I felt the wind hit my leg and like I said, the gentleman right next to me got struck really bad.' Caruso said he went into 'fight or flight mode' after the collision. He saw someone trying to rip the door off the Corolla to help the driver out, so he ran over to help out before calling 911. When asked to describe the scene and the victims, Caruso said the two men who were hit were 'laid out and lifeless'. He also saw blood on the windshield of the Corolla. Francisco Neto, the owner of a doggy day care on the street where the crash happened, said he knew one of the pedestrians who was killed. He identified him only as Joaquin and said he managed Astoria Discount Tire right across the street from his business. Neto said he got close to Joaquin after he introduced himself about a year ago when the tire shop first opened. The NYPD declined to confirm the identity of any of the victims when contacted by Daily Mail. 'The guy who died, his name is Joaquin. Very nice guy. He helped us a lot with the cars here. He just replaced all four of my tires the other day,' Neto said, adding that he was in his mid-forties. 'It's sad,' he added. 'Everybody here is a mechanic, tow truck, tire shop, but I know them. I've been here for almost 10 years.' The NYPD told Daily Mail that no arrests have been made in connection to the crash, but clarified that they are still investigating what happened.
