Study links air pollution to preterm births, low birth weight in India

Air pollution, a hazard endured everyday by millions across India in varying degrees, has long been associated with a range of respiratory diseases, heart conditions, and a growing list of health issues. Now, a new study reveals the damaging effects of air pollution extend far beyond the lungs and heart, affecting people before they are even born.
Published in PLoS Global Public Health, the study was carried out by researchers from institutions in India, Thailand, Ireland, and the UK, with data from the National Family Health Survey (NFHS) along with satellite data. The team assessed the influence of ambient air quality on birth outcomes, specifically preterm births (PTB) and low birth weight (LBW). The dataset included children aged 0 to 5 years; 52% were female and 48% male.
The results suggest that exposure to fine particulate matter (PM2.5) during pregnancy significantly increases the likelihood of these adverse outcomes. PM2.5 consists of airborne particles less than 2.5 micrometres in diameter.
According to the study, mothers exposed to increased levels of PM2.5 had a 70% higher chance of delivering prematurely compared to those who weren't exposed. The odds of giving birth to a baby with low birth weight rose by 40% for mothers who faced higher air pollution levels.
Northern states at more risk
A particularly significant finding in the study is the regional disparity: specifically, Delhi, Punjab, Haryana, Uttar Pradesh, and Bihar bear the brunt of the consequences of air pollution. These regions are known for being heavily industrialised with high vehicular emissions and the widespread use of solid fuels for cooking.
This conclusion aligns with previous reports. Another recent study in The Lancet reported that the average PM2.5 concentration in Delhi was 13.8-times higher than that in Kerala.
PTB was most prevalent in Himachal Pradesh (39%) and Delhi (17%) while LBW was most common in Punjab (22%) and Delhi (19%). Female children were more likely to be born with LBW (20%) compared to males (17%) — although both conditions were found to be more frequent among children of illiterate and poorer mothers.
Households that used solid fuel to cook also reported higher rates of both LBW and PTB.
Higher levels of PM2.5 during pregnancy significantly increased the likelihood of both LBW and PTB by 1.37x and 1.67x, respectively, with even a slight rise in temperature linked to an increase in LBW cases, though not PTB.
Higher temperatures have previously been linked to maternal dehydration, heat stress, and increased cardiovascular strain, all of which impair placental function and disrupt foetal growth. Conversely, excessive rainfall, especially during the monsoon, raises the risk of waterborne infections, which can further hinder foetal growth, the study suggests.
Flooding and displacement associated with heavy rains can also disrupt healthcare access, leading to delayed medical interventions and increasing the likelihood of pregnancy complications.
Region-specific interventions
The study's findings clarify that while India has made some progress in tackling air pollution with initiatives like the National Clean Air Programme, more needs to be done to mitigate the risks posed by poor air quality. The study's results suggest the government's target to reduce particulate matter concentrations by 20% by 2024 is a step in the right direction, although it may not be enough to address the public health crisis growing in tandem.
The authors call for more comprehensive interventions to reduce exposure to air pollution, particularly in North India, which has been identified as the most vulnerable region. In addition to addressing outdoor air pollution, the study highlights the importance of tackling indoor pollution, particularly from the burning of solid fuels in households. Cleaner cooking technologies and better access to clean energy could significantly reduce the harmful effects of indoor air pollution, especially in rural areas.
The research also calls for greater integration of air quality data with health surveillance systems to identify at-risk populations and implement targeted interventions.
A more localised approach, focusing on the specific needs of high-risk regions, is necessary to mitigate the impacts of air pollution on maternal and child health. Public health initiatives should prioritise the development of climate-resilient healthcare strategies, such as heat action plans and improved water management systems, to protect pregnant women from the dual threats of air pollution and extreme weather events, the study says.

This directive, detailed in a circular (No. Acad-45/2025) issued on July 15, builds on CBSE's earlier initiative mandating 'Sugar Boards' and reflects its broader campaign to instill good eating habits and encourage physical activity in alarming data from the National Family Health Survey (NFHS-5) and the Lancet Global Burden of Disease study, the board noted that more than 20% of urban adults in India are overweight or obese. Projections suggest this number could soar to over 44.9 crore by 2050, up from 18 crore in 2021. 'The rise in childhood obesity is closely linked to poor dietary habits and reduced physical activity,' the circular part of this initiative, CBSE has instructed schools to display 'Oil Boards'—both digital and static—in high-traffic areas such as cafeterias, corridors, and lobbies. These boards will serve as daily visual reminders of the health risks posed by excessive consumption of oil and fats. Moreover, the board has asked that health messages be printed on all official school stationery like letterheads, notebooks, and folders to keep the message constant throughout the school year.'Schools are not just centres of learning, but also places where children build lifelong habits,' said Dr Praggya M Singh, Director (Academics), CBSE. 'Through visual campaigns and practical steps, we aim to make students more conscious of what they eat and how they live.' Dr Mannan: Infertility now affects nearly 10–15% of couples in India, and the trend is steadily increasing. Medically, rising cases of Polycystic Ovary Syndrome (PCOS), endometriosis, thyroid disorders, diabetes, and obesity in women are significant contributors. In men, conditions like varicocele, hormonal imbalances, and poor sperm parameters are commonly seen. Environmentally, increasing exposure to endocrine-disrupting chemicals (EDCs), air pollution, and unhealthy lifestyle habits—such as poor diet, lack of exercise, smoking, alcohol consumption, and chronic stress—are worsening reproductive health. Additionally, delayed marriages and late pregnancy planning have become major socio-cultural factors contributing to declining fertility. How are declining sperm quality and ovarian reserve due to air pollution and sedentary lifestyles impacting IVF outcomes, especially in urban India? Dr Mannan: In urban settings, declining sperm counts and ovarian reserves have become major challenges for successful conception—both naturally and via assisted reproduction. Studies show increasing DNA fragmentation in sperm and diminished anti-Müllerian hormone (AMH) levels in women exposed to high-pollution environments. These directly reduce embryo quality and negatively impact IVF success rates. Sedentary habits, obesity, and poor sleep hygiene further impair hormonal health. As a result, we often require multiple IVF cycles and personalised stimulation protocols to achieve a viable pregnancy, especially in metropolitan cities. How accessible and affordable is IVF treatment across India, particularly in tier-2 and tier-3 cities? Dr Mannan: While IVF is gaining popularity and acceptance, affordability and access remain significant hurdles outside metro cities. In tier-2 and tier-3 areas, there is limited availability of advanced infrastructure, skilled embryologists, and fertility specialists. Many couples are forced to travel to metro cities, which increases emotional and financial burdens. Moreover, infertility is still considered taboo in several regions, leading to delayed medical intervention. Although some states are considering subsidised treatment or insurance coverage for infertility, there is still a long way to go in terms of accessibility and affordability at the national level. Dr Mannan: IVF success rates typically range from 30% to 50%, depending on factors like the woman's age, the cause of infertility, and the clinic's expertise. For women above 35, success rates start to decline due to reduced ovarian reserve and egg quality. It is crucial for couples to understand that IVF is not a guarantee of pregnancy in the first cycle. It may require two or more attempts, and in some cases, advanced techniques like donor eggs or preimplantation genetic testing (PGT) may be recommended. Emotional and psychological support also plays a big role during this journey. Since the implementation of ICMR guidelines and ART legislation, how have clinical practices, patient safety, and ethical standards changed? Dr Mannan: The Assisted Reproductive Technology (Regulation) Act and ICMR guidelines have brought much-needed standardisation to IVF practices across India. Clinics now require mandatory registration, ensuring quality control and accountability. There are clear protocols for consent, gamete donation, surrogacy, and embryo storage. This has significantly improved patient safety, reduced unethical practices, and built greater transparency in treatment. While compliance can sometimes increase operational costs, it ultimately enhances trust between patients and healthcare providers. STORY CONTINUES BELOW THIS AD Are clinics in India adopting single embryo transfer more frequently to minimise complications from multiple pregnancies? Dr Mannan: Yes, there is a growing shift towards single embryo transfer (SET), especially in younger women with good-quality embryos. While earlier practices often involved transferring two or more embryos to boost chances, we now understand that multiple pregnancies increase maternal risks such as hypertension, preterm birth, and NICU admissions. With improved embryo culture systems, blastocyst transfers, and technologies like vitrification (rapid freezing), SET has become a safe and effective option. However, adoption still varies across clinics and regions based on patient preference, cost considerations, and expectations. What does current evidence suggest about egg freezing and fertility preservation in working women and cancer patients? Dr Mannan: Egg freezing has emerged as a valuable tool for fertility preservation. For working women who wish to delay childbearing, freezing eggs before the age of 35 offers the best chance of future success. In cancer patients, fertility preservation before undergoing chemotherapy or radiotherapy is becoming standard practice. Evidence shows that frozen eggs, when used within 5–10 years, can lead to pregnancy rates comparable to fresh eggs—provided they were retrieved at an optimal age and under proper protocols. The key is early counselling and timely decision-making. Dr Mannan: Advanced technologies such as AI-based embryo selection and time-lapse imaging (like EmbryoScope) are being increasingly integrated into top-tier IVF centres in India. These tools offer real-time monitoring of embryo development and help identify embryos with the best implantation potential. AI algorithms analyse thousands of data points to assist embryologists in decision-making. While the evidence is promising—especially in reducing subjectivity and improving selection accuracy—more long-term data is needed to confirm significant improvements in live birth rates. Cost is another limiting factor, making these technologies less accessible in smaller cities.

GUWAHATI Global childhood vaccination rates, hit by the COVID-19 pandemic, did not return to the pre-pandemic levels by 2023, a study published in The Lancet has revealed. The study also marked India among eight countries that accounted for more than half of the world's 15.7 million 'zero-dose' children in 2023. Zero-dose refers to children who did not receive the first dose of diphtheria, tetanus, and pertussis (DTP). The other seven countries are Brazil, the Democratic Republic of Congo, Ethiopia, Indonesia, Nigeria, Somalia, and Sudan. 'The study highlights the progress and challenges in global childhood vaccination efforts from 1980 to 2023. it shows that the coverage for key vaccines for DTP, measles, polio, and tuberculosis nearly doubled over this period, but the positive trend hides some worrying developments,' Hemen Sarma, an associate professor at Assam's Bodoland University, said on July 24, 2025. A GBD (Global Burden of Diseases) senior collaborator with the University of Washington's Institute for Health Metrics and Evaluation, Mr Sarma is one of the authors of the study involving researchers worldwide. The study underscored the slowdown in the vaccination progress in many countries and territories between 2010 and 2019, with 21 out of 36 high-income nations witnessing a drop in coverage for at least one vaccine. It said the COVID-19 pandemic made things worse, 'with global rates for these vaccines declining sharply since 2020, and still not returning to pre-COVID-19 pandemic levels as of 2023'. 'Vaccines introduced more recently, such as those for pneumonia, rotavirus, and the second dose of the measles vaccine, continued to expand during the pandemic, but at a slower pace. Only the third dose of DTP is expected to meet the global target of 90% coverage by 2030, and that too in the most optimistic scenarios,' Mr Sarma said. One of the concerns underlined by the study was the rise in the number of zero-dose children during the pandemic after dropping sharply from 1980 to 2019, peaking at 18.6 million in 2021. Most of these children live in conflict zones or areas with limited healthcare access, especially in sub-Saharan Africa. The study has warned that unless urgent action is taken, global immunisation targets will not be met. It called for focused efforts to strengthen healthcare systems, fight vaccine misinformation, and reach the most underserved communities, especially in Africa and South Asia. Campaigns like the World Health Organisation's 'Big Catch-Up' and efforts to restore routine immunisation must prioritise these areas to regain lost ground and ensure every child is protected, the study said. The study has provided updated global, regional, and national estimates of routine childhood vaccine coverage across 204 countries and territories for 11 vaccine-dose combinations recommended by WHO for all children globally. Employing advanced modelling techniques, this analysis accounted for data biases and heterogeneity and integrated new methodologies to model vaccine scale-up and COVID-19 pandemic-related disruptions. To contextualise historic coverage trends and gains still needed to achieve the WHO's Immunisation Agenda 2030 coverage targets, the researchers assessed the effect of the COVID-19 pandemic on vaccine coverage, forecast coverage of select life-course vaccines up to 2030, and analysed the progress needed to reduce the number of zero-dose children by half between 2023 and 2030.
