
Investigation launched after man dies at Welsh prison
It is the fourth death at the prison this year being examined

An investigation has been launched after the death of a man at a Welsh prison. Prisoner Darren Watts, who was 51, died last month at HMP Berwyn in Wrexham.
The Prisons and Probation Ombudsman (PPO) has confirmed it is examining the circumstances surrounding his death, which happened on Sunday, July 13. It is the fourth inquiry launched by the PPO in 2025 following the death of a prisoner at HMP Berwyn. A spokesperson for the PPO said: "We can only investigate issues that may be related to someone's death. This could include how their physical or mental health, suicide, self-harm and substance misuse risks have been managed."
Darren Watts was born on November 3, 1973. As an inmate at a category C prison that can accommodate up to 2,100 men, the PPO will assess his post-mortem report and speak to prison staff before reaching its conclusions.
The investigation will also consider clinical reviews, records of interviews and other relevant documents.

A PPO report will be sent to the bereaved family before a final version is issued. A coroner will then conduct an inquest to establish how he died.
A Prison Service spokesperson said: "Our thoughts remain with the friends and family of Darren Watts. As with all deaths in custody, the Prisons and Probation Ombudsman will investigate."

Video footage from her bicycle camera shows Klaudia braking in front of the couple, with one of them seemingly yelling at her to 'slow down'. The 34-year-old can be heard objecting about their dog, to which the man responds, 'yeah but you're on a f**king bike'. The man then labels her 'a f***king idiot', and the video appears to show Klaudia being pushed into the water. Klaudia, who cannot swim, expressed her fear of 'drowning' as her feet were strapped to the bike pedals, making it difficult for her to escape, reports the Manchester Evening News. The mother of two remained in the canal for five minutes until a passer-by noticed her and assisted her out of the water. The police were subsequently called to the scene. Klaudia stated that she believed she could have died from the push, and the couple didn't even glance back to see if she was alright. The incident sparked thousands of comments and reactions on social media, with some voicing strong views on who they believed was at fault. Klaudia Mitura Lancashirelass66 commented: "The lady on the bike was going way too fast and needed to slow down, maybe the dog was frightened of bikes - what if it had chased you?" She added: "On the other hand pushing someone into the canal is not the done thing and very irresponsible as in this case the lady couldn't swim and the outcome could have been far worse. At the end of the day it's a public path for everyone to use and a bit of consideration on both sides was needed." RedMist expressed: "His actions were a shocking assault and he definitely needs a visit from the police, but canal paths are for everyone and the video looks like the cyclist was going at some speed, so much so, that she had to break quickly when encountering the walkers and their dog." They concluded: "Slow down! A bit of thought for others and everyone can enjoy these paths safely." Ancoats blue shared similar sentiments, posting: "His reaction was a bit reckless, she could have drowned if she'd hit her on something in the water. Having said that, they had a large dog, and cyclists should ALWAYS slow down near animals... and especially so on a tight canal pathway." While Hamstersparadise stated: "Far too fast, Klaudia, especially as he has an animal with him. Having said that, pushing someone in a canal is a definite no no." They continued: "They might not be able to swim (as in this case), they might get injured on underwater debris. Just no. And if it was a spur of the moment reaction, he should have checked she was okay." Sheila Brotherton remarked: "Any normal person would just step aside and let the cyclist past, maybe say a good morning too". Shaw Crafty commented: "Common courtesy to go single file when someone is coming in the opposite direction, whether on a bike or not. Would he have done the same to a walker?". Sarah Horne observed: "The first thing he said was 'slow down' - quite rightly, as she was approaching them far too fast." Sue Sue noted: "The man was protecting his dog and shouted for her to slow down, which she did when she was a bit too close. She saw them from a from a long way off and should have slowed down sooner as to not scare the dog." Collette Twist concurred, stating: "Cyclist was approaching way to fast. And don't know what cyclist said to dog walkers first." Nevertheless, Collette added: "But no excuse to push her in. All of them seemed ignorant, and inconsiderate". Possibly viewing the cyclist as the victim in this situation, Jason Quarmby suggested: "Bet he wouldn't have the same reaction if it was a bloke". Greater Manchester Police have confirmed they are examining the incident, though no arrests have been made at this time.

Salman Iftikhar was on a flight from London Heathrow to Lahore when he unleashed a tirade of abuse at stewardess, Angie Walsh, and promised to blow up the Avari Lahore Hotel, where cabin crew were due to stay. Footage shows the 37-year-old repeatedly accusing the woman of being a racist and shouting,' You called me a p**i in front of everybody'. Another passenger, who filmed the shocking scenes, can be heard saying 'holy s**t' as the scene unfolded in first class. Iftikhar's wife and three children watched him chugg champagne, film the crew and eventually 'grabbing and squeezing' Ms Walsh's arm and telling her, 'You will be dead on the floor of your hotel'. Isleworth Crown Court heard that the incident on on February 7, 2023, was so bad that pilots even considered detouring the plane to Turkey. Prosecutor Abdul Kapadia detailed how the horror unfolded: 'During the first meal service, the defendant was seen helping himself to ice, leaning over the bar he was drinking at, and taking ice with his hands. 'When told to stop, the defendant became irate, and started to film cabin crew with his phone, telling them: 'Do not tell me what to do you bitch.' 'When asked by the cabin crew to return to his seat, he then said: 'Don't tell me what to do you racist f***ing bitch. I know where you are from in Cardiff.'' The pilot was then contacted, and turned the seat belt lights on, but that only made the situation worse. Kapadia said: 'The defendant then shouted, and said: 'Look at this bitch having the seat belt sign on when there was no turbulence. What the f**k is this bitch doing?' 'Iftikhar returned to his seat and called crew member Angie Walsh a 'f**king bitch.'' Once the passenger was informed that the flight could be diverted due to his behaviour, his threats did not stop. Instead, he replied: 'I don't care. F**k it, go to Turkey. I have contacts.' The court was told how his three young children were crying and pleading for him to stop, but to no avail. Kapadia revealed how Iftikhar then threatened to blow up a floor of the hotel. He added: 'The defendant knew the specific hotel, but also the hotel room numbers, and threatened the cabin crew with this. 'He told Ms Walsh: 'You will be dead on the floor of your hotel'. 'Iftikhar shouted at Ms Walsh and said: 'The white sheep s**ging b***h will be dead. The floor of your hotel will be blown up and it will disappear.' 'He told Ms Walsh: 'You will be dragged by your hair from your room and gang raped and set on fire.'' When the plane landed in Pakistan, no action was taken against Iftikhar. He was eventually arrested – more than a year later – at his £900,000 home in Iver, Buckingamshire. Ms Walsh told the court how after 37 years of flying with Virgin Atlantic, she had to take 14 months off work after the threat of being gang-raped and She said: 'I can't quite believe that one passenger has had this much of an impact on my, my job, my career and my life. 'I am a strong brave, happy stewardess, and loved my job. I am well known within the company. 'I have been flying with Virgin Atlantic for 37 years. I was working when all flights were grounded on 9/11, and I have even flown into a warzone. But this incident has broken me. 'But I don't feel strong enough anymore. I was abused for eight hours and 15 mins. It has broken me. It was a very personal attack. More Trending 'I was doing everything in my power to protect passengers and the crew from him. I felt exposed and vulnerable, especially as we were 39,000 feet in the air. There was nowhere for me to go. 'There was one moment where I felt I could not cope. Ben Walker-Nolan, defending, told the court that his client was suffering from 'amnesia blood loss', which contributed to what happened. It follows questions from opposition leader councillor Matthew Evans around the effectiveness of camera coverage. Councillor Evans said he had received "a number of complaints from city centre traders about the CCTV system and allegations about the cameras either being switched off, facing the wrong way or not capturing anti-social behaviour". He said: "I understand when there have been some shoplifting or anti-social behaviour cases and [when] the police have contacted the control centre, there hasn't been any camera footage because the camera was switched off or it wasn't visible." In a written question to the cabinet members with responsibilities for communities, he asked: "Can you confirm, if this is true, what you intend to do about it? If these allegations are unfounded, why have there been several recent complaints?" In response, councillors Jason Hughes and Mark Spencer said: "We have recently experienced some issues due to a number of power surges on the wider grid causing damage.
