Questions over tripling of Gauteng Health's security budget

In just two years, the Gauteng health department's spending on security has more than tripled. Spotlight tries to get to the bottom of the ballooning bills and what it means for governance in the department.
The Gauteng Department of Health's projected R2.54-billion spend on security contracts for 2025/2026 has received the thumbs up, fuelling suspicion in various quarters. It comes as the department claims to lack the funds to fill vacancies, pay all suppliers on time or continue fulfilling doctors' overtime contracts.
The R2.54-billion is more than three times the R838-million the department spent two years earlier in 2023/2024. This was revealed at the end of May in response to questions raised in the Gauteng Legislature by the Democratic Alliance (DA), the official opposition in the province. In 2024/2025, the department's security spending was just above R1.76-billion.
Jack Bloom, DA shadow MEC for health in Gauteng, calls the proposed expenditure 'unjustified', given that the department is failing to meet its health service delivery targets.
According to him, security companies charge R77-million per year for guarding services at Chris Hani Baragwanath Hospital, and more than R72-million annually at Charlotte Maxeke Hospital.
At Tara Hospital, the new security contract costs R14-million a year – a sharp increase from the previous year's R4.2-million contract, which had provided 21 guards for the facility. Bloom says that, according to the department's own assessment, only five additional guards were needed at Tara Hospital, increasing the total to 26. However, the current contract pays for 46 guards. 'This means they are paying about R5-million a year for 20 guards they do not need,' Bloom says. 'They could better use this money to fill the vacancies for 13 professional nurses, as Tara Hospital cannot use 50 of its 137 beds because of staff shortages. It is a clear example of excessive security costs squeezing out service delivery,' he says.
'The numbers simply don't add up,' Bloom says. He points out that the written responses provided in the Gauteng Legislature – signed off by MEC for Health and Wellness, Nomantu Nkomo-Ralehoko – cite an internal security assessment and compliance with Private Security Industry Regulatory Authority (PSIRA) salary increases for guards as reasons for the higher costs. However, the internal assessment has not been shared with either Bloom or Spotlight, despite requests from both.
The PSIRA-approved annual increase is 7.38%. In contrast, the department's security spending rose by more than 100% from 2023/2024 to 2024/2025, and is projected to increase by another 40% from 2024/2025 to 2025/2026.
According to a statement released by the Gauteng health department in April 2024, it had 113 security companies under contract at the time, providing a total of 6,000 guards across 37 hospitals and 370 clinics and institutions in the province.
'Very fishy'
Bloom says security guarding contracts have been 'very fishy for at least the past 10 years'. He claims: 'There are certain security companies that keep popping up. These companies will get two-year contracts, then have their contracts extended for something like 10 years. Then we have these new contracts, which have soared in costs. The auditor general has said that there is irregular expenditure. Security contracts have always been suspect and have always been corruption territory.'
In March this year, the DA lodged a complaint with the Public Protector over a R49-million guarding contract for five clinics in Tshwane and the MEC's offices. The contract was awarded to a company called Triotic Protection Services.
The DA alleges that the company was founded by City of Tshwane's deputy executive mayor, Eugene Modise, who also previously served as its director. When the company was awarded the contract, it was allegedly in the crosshairs of the South African Revenue Service because it owed R59-million in tax over five years.
This has raised concerns about the company's tax compliance status and its eligibility to tender for the contract. Spotlight approached Modise for comment through Samkelo Mgobozi, spokesperson for the office of the executive mayor, but had not received a response by the time of publication.
Other security companies that have contracts with the department have also made headlines for allegedly flouting labour laws. These include not paying guards for months and withholding employees' pension and provident fund contributions. It leaves questions about due diligence and the proper vetting of companies.
A review under way?
In the weeks since Bloom's questions were answered in the legislature, he says Nkomo-Ralehoko conceded to a review of the security spend at the province's hospitals.
However, the Gauteng health department has not announced anything formally and no further details have been provided.
The department has also neither responded to Spotlight's questions nor provided supporting documentation of its assessment criteria for the security contracts, the tender requirements, tender processes and how it measures value for money and the impact of increased guarding in improving safety and security for patients, staff and visitors to its hospitals. It has also not made available a list of the companies with successful contracts and what their services entail.
As Spotlight previously reported in some depth (see here and here), there are serious security problems at many health facilities in Gauteng. It ranges from cable theft disrupting hospital operations to assaults on healthcare workers. The department has also been criticised from some quarters for its plans to train healthcare workers to better handle violent situations.
That steps need to be taken to better secure the province's health facilities is not controversial. But previous reporting has also shown a pattern of questionable contract management, with, for example, contracts being extended on a month-to-month basis for years after the original tenders had technically expired. It appears that the widespread use of these month-to-month security contracts came to an end when the department finally awarded a series of new security tenders in 2024, but it also seems likely that these new contracts are driving the department's ballooning security spending.
'Has to be justified'
The department's massively increased security spend must be fully explained and is essential for transparency, several experts told Spotlight.
'This kind of escalation in cost has to be justified, especially when the department has no money,' says Professor Alex van den Heever, chair of social security systems administration and management studies at the University of Witwatersrand.
He says the specifics of the tender process and the contracts awarded need to be publicly available to be openly scrutinised. The processes must meet Treasury's procurement guidelines and must follow the Public Finance Management Act, which regulates financial management within the national and provincial governments. Where there is wilful non-compliance, Van den Heever says criminal charges should be laid.
'This is a department that has routinely had around R3-billion a year in irregular expenditure. It means procurement procedures have been bypassed. This is not an isolated incident; it's systematic,' he adds.
The latest Auditor-General report into the Gauteng health department was released in September last year for the 2023/24 financial year. It showed that of its R60-billion budget, the department underspent by R1.1-billion, including R590-million on the National Tertiary Service Grant that was meant to help fund specialist services. The report highlighted R2.7-billion in irregular expenditure, which is R400-million more than the previous year, and R17-million in fruitless and wasteful spending – an increase of R2-million from the year before.
Equally damning, the report highlighted the lack of credible information provided. 'This is likely to result in substantial harm to the operations of the department as incorrect data is used for planning and budgeting and the effectiveness of oversight and monitoring are reduced as a result of unreliable reported performance information on the provision of primary healthcare services,' wrote the Auditor-General.
Van den Heever says leadership and management within the health department need to be seriously questioned. Questions should be asked of why 'bad apples' are not being removed, why there are no consequences for conflicts of interests and collusions, and why webs of enablers within the department are not exposed for insulating wrongdoers, he says.
Van den Heever says that over nine years of monitoring, the Gauteng Health Department's irregular and wasteful spending ranged between 3.6% and 6.6% of its total budget. In contrast, during the same period, the Western Cape's irregular spending ranged from 0% to just 0.1%.
Lack of transparency
The Gauteng health department's spike in security spending demands deeper investigation, says advocate Stephanie Fick. She is executive director for accountability and public governance at the Organisation Undoing Tax Abuse and serves on the Health Sector Anti-Corruption Forum.
This forum was launched in 2019 as an initiative to combat corruption within the healthcare system. It falls under the Special Investigating Unit and brings together a range of stakeholders, including law enforcement agencies, government departments, regulators and the private sector.
Fick says the health department's failure to provide easy access to information on tenders, contracts and contracted companies undermines transparency and accountability. She encourages more people to come forward with insider information.
'We want to see the details right down to line items and who signed off on things. We encourage people to use our protected whistle-blower platforms to share information,' Fick says.
'For civil society, there is a growing role to mount strategic challenges to things like this kind of excessive and irregular expenditure; to demand transparency and to expose people who are responsible.
'This must be done so ordinary people can better understand what's been happening with their tax money and so they choose more carefully when they go to the ballot box, starting with next year's municipal elections,' she says.

Rambau cited one specific case of a youth-headed household with four orphaned children, all without identity documents. She said the eldest, now the sibling carer, had never attended school. 'Over the weekend, they had nowhere to stay. Their mother had been allowed to stay temporarily in Extension 8, but after she passed, the owners wanted their space back. One of our caregivers had to volunteer to take them in.' In some of the worst cases, Rambau said, young sibling carers were themselves ill. She described a young woman caring for two siblings while battling tuberculosis. 'She defaulted on her medication and was admitted. Now she's pregnant. No one is taking care of them.' Rambau said Ratang Bana would sometimes receive food parcels from a local church to give to families facing such challenges, but with more than 200 households currently in need, they were forced to rotate support. The situation worsened after USAID withdrew its funding. 'It affected us a lot. We had to reduce staff. It also affected our beneficiaries,' she noted, adding that programmes such as HIV testing, prevention, and youth education, especially the YOLO (You Only Live Once) initiative for 12–15-year-olds, were halted as a result of the funding withdrawal. DA MP Bridget Masango recently visited the centre and expressed concern. 'They employed 135 young men and women but have lost half of them because of the withdrawal of funding by USAID,' she said. Ratang Bana is one of the few organisations in Alexandra with deep, daily insight into the lived experiences of vulnerable children. But without urgent support, its ability to assist is slipping and with it, the safety net for hundreds of young lives. Trailblazer at 74: Princess Anne-Sheilah Makhado's inspiring journey in nursing and advocacy

With humour, warmth and steely resolve, Princess Anne-Sheilah Makhado speaks to Spotlight about her long journey as a nurse, of creating solutions in the public health system, and her burning passion for motivating women and girls to unlock their talents. Princess Anne-Sheilah Makhado has worn many hats: that of midwife, professional nurse, motivational speaker, farmer, seamstress, mother and author. Her latest achievement was graduating last year with a PhD in advanced nursing science from the University of Venda at the age of 74. And Makhado is still learning. She recently returned home to Louis Trichardt in Limpopo armed with fresh insight after a month in Beijing visiting her son, who is teaching English in China. 'In China, I observed a lot,' she says. 'It's peaceful and very clean. I've never seen such beautiful red roses right in town, and no one steals these roses. And the elderly there, no one I saw was limping; people are working and they have purpose.' Reflecting on this, she recalls telling her group of retirees in Louis Trichardt that if they want to stay healthy, they need to make an effort to exercise more and keep busy. Keeping busy has been a hallmark of Makhado's life. Fittingly, she describes herself as 'results oriented' and 'thirsty'. She adds: 'I ask God to send me where there is a need.' Makhado's competing priorities came to a head in 2015, when she applied for a Master of Nursing course at the University of Venda. She was 65 at the time. 'The dean said: 'I can't take you' and I said: 'Why?' And she said: 'You got 59.8 marks for your Honours degree.'' This is just below the 60% minimum typically required to qualify for a Master's programme. Looking back, Makhado explains that during her Honours studies, which she completed in 2010, she was also juggling a demanding role as nursing services manager at Midlands Provincial Hospital in Graaff-Reinet, a position she held for six years. 'I had been running an institution where I found nursing issues to not be up to a good standard,' she recalls. The rejection hit hard. 'I cried then, I cried like a baby,' she says. 'Then in 2017, I went back to the university, and I said: 'I am back to repeat my Honours now'.' She completed the degree a second time, earning her graduation in 2018. That same year, she enrolled for her Master's, which she completed in 2020. Child-headed households Her Master's research, which focused on the experiences of child-headed households, was inspired by her work with a non-profit organisation she founded in 2018 called Voice of the Voiceless. Building on this, her PhD explored strategies to improve support for these children. She interviewed 15 child heads of households, aged 14 to 19, along with 15 of their relatives, and conducted additional focus group discussions within the community. 'Many of these children, when asked about their parents, would say: 'We heard that our mother died but we're not sure when, we don't even have a picture of our mum.' And many of them didn't know their father,' she says. A key finding in Makhado's research was a need to educate men around family values. 'That's why, in my recommendations, I said there should be man-to-man programmes because men are [conceiving] children and leaving the children there. Men must learn to take care of their children. Not just dump the mother with the baby. Then the mother has HIV and dies — and the children?' Another key recommendation was that traditional leaders should play a greater role in caring for orphaned children. 'I went to the traditional leaders, and I said: 'You are a traditional leader. What are you doing for these children? You need to have a list of the children who are heading families in your area, and you must visit them.'' With high crime rates, alcohol and drug use, and some child-headed homes unable to secure their doors, Makhado also questioned the role of the community policing forum. '[A]re they aware that these children must be protected and kept safe? The drug sellers know there are no parents and know they can abuse these kids.' Reflecting on conditions in these communities, she says: 'It is painful my darling, traumatic.' From Sophiatown to Limpopo and Chris Hani Baragwanath Hospital Makhado grew up in a tightly-knit family. The second born of seven siblings, soon after her birth in Johannesburg's Sophiatown in 1950, the family moved to Sibasa in Limpopo where her father worked for the then Native Affairs Department and her mother was a school teacher. 'My mother was so neat and clean and organised,' she says. 'My mother taught us never to take no for an answer and to never settle for less.' After attending Shingwedzi Secondary School, she trained and worked first as a teacher, then as a typist. But one day entering a hospital, she recalls: 'I saw the crisp white uniforms, my heart bled and I knew this is what I wanted.' At 27, she started her Diploma in Nursing at the then Groothoek Nursing College in Polokwane, moving to Chris Hani Baragwanath Hospital in Soweto to specialise in midwifery. She says her first job as a nursing sister in Baragwanath's buzzing casualty and neurology sections was 'wonderful, really'. Chuckling, Makhado recalls how one patient grew so enamoured with her, that she suggested Makhado marry her brother: 'So the guy was invited to come and meet me. But he was a short guy and I was not interested in short guys.' How does one become a good nurse? 'Nursing starts with you,' she says. 'The question is: 'How would you like to be treated when you are ill?'' In 1984, Makhado moved back to Limpopo to Elim Hospital and then to the former Transvaal Provincial Hospital — which is now the Louis Trichardt Memorial Hospital — as a clinic sister supervisor, with tasks including managing mobile clinics in the area. 'Working directly with the vulnerable and disadvantaged, women working on farms from a tender age, carrying their babies, I felt a strong need to empower them,' she says. Another issue close to her heart is children with special needs. In 1998, Makhado says she helped set up Tshilwavhusiku Razwimisani, a special needs school for children 25km outside of Louis Trichardt. 'I used to visit schools in that area and picked up so many children who had some type of disability.' As a results-orientated person, she says she couldn't turn a blind eye to this. 'So we sat down and identified an area where we could start a school for the mentally challenged.' Makhado says Tshilwavhusiku Razwimisani started with just 30 pupils and volunteers, including mothers preparing meals. Today, the provincial education department is running the school, which has 298 learners. 'There are moments where I feel like crying. We started from nothing,' she says. Training nurses in Graaff Reinet In 2008, Makhado became the nursing services manager at Midlands Provincial Hospital in Graaff Reinet, in the Eastern Cape. 'There was a lot of unemployment, and many young people would say to me they wanted to do nursing,' she says. But the challenge was the nursing colleges were far away in Mthatha or Gqeberha. The solution came to her: starting a nursing training college at the hospital. 'So I applied to the South African Nursing Council in 2010. Oh, and God has been wonderful, the college was approved in 2012. The next challenge was, where do I get the structures where the school would operate?' Makhado says she then negotiated with the provincial Department of Public Works to renovate buildings for student nurses' accommodation, and with the then mayor of Camdeboo Local Municipality, Hanna Makoba, to secure classrooms. 'I used to go out to furniture shops and ask for whatever furniture… for the nurses,' she recalls. This would become the Midlands Hospital Nursing School, today still an accredited institution within the Eastern Cape health system. During our interview, she checks the spelling of Makoba's name on her phone, saying they have kept in touch. The Master Lock Key Upon retiring from the public sector at Midlands Hospital in 2013, Makhado returned to Louis Trichardt where she focused on motivational speaking, continuing her community outreach work and studies. She also wrote a book called The Master Lock Key. 'It has been and will always be in my deepest innermost thoughts that women are the backbone of the nation. If women can learn to stand firmly in great numbers and become more assertive in issues that concern themselves, their families and the nation, I believe there will be a better future,' she wrote. Makhado is divorced and a mother of three children: two daughters and a son, and three grandchildren. When she struggled to make ends meet on a nursing salary, she says she sewed for extra income. 'My mother, when we grew up, she used to show us how to sew. She gave me that skill. So when times were tough, I did some sewing. I used to produce track suits for schools. When I came back from working as a nurse, I would take out my sewing machines.' Makhado relays how farming and growing food has been another enduring joy. Presently, she heads gardening projects that supply the Spar in Louis Trichardt with spinach, beetroot and garlic. 'It's quite amazing,' she says. 'I encourage all, I urge women and girls, let us work and fight poverty and hunger instead of idleness and being dependent. Women have this dependency syndrome. I said: 'Oh no, we cannot go on like this!'' As our conversation draws to a close, Makhado underscores South Africa's need for women leaders, saying that 'anything is within a woman's power'. She adds: 'This country needs women's listening ears and caring touch, but also thinkers, risk takers. Risk is the spice of life. What we women can achieve is virtually limitless.' DM

During the recent 2025/26 budget and policy speech in the Provincial Legislature, the MEC for health, Sasekani Manzini, said the department's budget of R19.75b is more than just a financial allocation; it is a strategic mandate to deliver essential health services to the people. On July 22, Manzini said the largest portion, 60.36%, has been allocated to district health services, highlighting a strong focus on primary healthcare and community-level interventions. This is followed by central hospital services and provincial hospital services, receiving 10.48% and 10.06% respectively. Emergency medical services was allocated around 3% to deliver emergency medical services and planned patient transport. ALSO READ: Services disrupted at Mbombela licensing centre Manzini said a significant achievement that has changed people's quality of life by restoring their eyesight through the Cataract Marathon. 'In the beginning of the 2023/24 financial year, the department had a backlog of more than 14 000 cases of cataract surgery, so we dedicated Tonga Hospital as the Eye Care Centre.' 'The backlog had been reduced to 4 636 by the end of the last financial year, with a total of 10 292 operations conducted since the 2023/24 financial year. Our aim is to eradicate the backlog by the end of the 2026/27 financial year,' said Manzini. She added that significant obstacles remain, with antiretroviral treatment retention rates, the prevalence of teenage pregnancies, and malnutrition-related deaths demanding urgent, targeted action. 'Capital expenditure execution, though lagging at 22.4%, highlights the need for redoubled efforts in project delivery.' Regarding emergency medical services she said, 'We made a commitment of procuring 10 ambulances annually, but we can report that we managed to double our efforts in the last financial year. We procured 30 ambulances and, this year, we are planning to procure 15 more. We are committed to turning the situation around through the implementation of effective and efficient interventions that will restore and enhance service delivery.' During the debate on the budget, DA provincial chairperson and spokesperson on health, Bosman Grobler, said: 'The mandate of this department is probably the biggest of all departments in the province. One way of getting the basics right in healthcare is to have a proper record of the patients in your care. Unfortunately, a patient went missing from a hospital, only to be found weeks later, dead in a field next to the hospital.' Grobler went on to point to what he called a glaring inconsistency in the number of ambulances the MEC buys on a yearly basis. 'Currently, there are 181 ambulances, with another 16 budgeted for this financial year. But the department has fewer than 75 ambulances in operation due to staff shortages. The MEC could only employ 28 emergency care personnel. All the other positions could not be filled due to financial constraints. Let us get the basics right,' he said.
