An Overdiagnosis Epidemic Is Harming Patients' Mental Health

Neurologist Suzanne O'Sullivan thinks that modern health care is overdiagnosing people but not necessarily making them healthier—and in fact, that it might be doing more harm than good.
In her new book, The Age of Diagnosis , she backs this assertion with some sobering facts. For instance, between 1998 and 2018, autism diagnoses jumped by 787 percent in the UK alone; Lyme disease has an estimated 85 percent overdiagnosis rate, including in countries where it's impossible to contract the disease; and there's still little evidence that many cancer screening programs actually reduce cancer-related death rates.
Ahead of her keynote speech at WIRED Health later this month, O'Sullivan spoke to WIRED to talk about the boundaries between illness and health, the nocebo effect, and the dangers of early detection. This interview has been edited for length and clarity.
WIRED: You've been a neurologist for nearly 35 years. When did you start seeing this new phenomenon of overdiagnosis?
Suzanne O'Sullivan: In the book I write about Darcy, a young lady who came to me with seizures. She's only 24 and she had ten other diagnoses. I'm an epilepsy specialist, and that should mean that I'm only seeing people with epilepsy, but unfortunately seizures are a thing that happen very frequently for psychosomatic reasons. When I started, people came to me with seizures that had a psychological cause. That's all they had.

But over the course of the last ten years, that particular group of patients started to gather a long list of diagnoses. What seems to be happening now is that if you go to different doctors with multiple symptoms, you will get a name for them all. These symptoms always existed, but the naming of them has been detrimental to patients. It's caused them to pay a lot of attention to their bodies and that makes the symptoms worse. That's the nocebo effect.
Every week now I see at least one Darcy, a 24-year old with different medical labels. Most of those labels have no treatment and aren't making them better. That's a very concerning trend for me.
Why are doctors over-labeling symptoms rather than trying to get to the root cause of the disease?
We did have an underdiagnosis problem in the past, particularly with learning or behavioral problems such as autism and ADHD. We didn't recognize people who needed help, so we've been trying to correct that. But we've overcorrected. We have been working on the assumption that the more we diagnose, the healthier you can make the population. That was probably true to a point, but the improvement isn't sustained when you get into the milder end of any disease spectrum.
Patients come to us wanting answers. People want to understand why they are the way they are. A satisfying consultation between a doctor and a patient is often one where the patient asks about a symptom and the doctor can explain what it is. I think it's a little bit of a collusion between what patients want and how doctors can satisfy them with labels.
The increase in the diagnosis rates for ADHD and autism, in particular, have exploded over the past decade. Is this the result of the shifting boundaries between what we now consider to be health and sickness?
That's my concern. Autism has risen from like one in 2,500 people to one in 36 children in the UK and one in 20 in Northern Ireland. That happened because people realized that there must be kids out there who've got milder forms of this, and if we help them, they'll get better.
However, what has happened now is that we have a massive increase in autism, and it is not having the downstream effect of making children better. We should be seeing a slightly happier population, but all we're seeing is worse mental health. We did something well-intentioned but there's no evidence that it's working.
The reason it's not working is because when you get to the very mild end of a spectrum of behavioral or learning problems, you have a balancing act between the benefit of being diagnosed along with the help you can get, and the drawbacks of being diagnosed, which is telling a child that they've got an abnormal brain. What does that do to a child's belief in themselves? How does it stigmatize them? How does it affect their identity formation? We thought it would be helpful to tell children this, but the statistics and the outcome is suggesting it isn't helpful.
You're also worried about another aspect of diagnostics, which is overdetection. One example you give in the book relates to modern cancer screening programs that detect the disease at earlier and milder stages. But so far there's little evidence that these are actually beneficial to patients.
Every cancer screening program will lead to some people getting treatment when they didn't need to be treated. That will always be the case. What we're desperately wrangling with is that we want to make sure we keep the number of overdiagnosed people down and the number of people who need the treatment up. However, the more sensitive you make those tests, the more overdiagnosed people you will have. I read in a Cochrane review that if you screen 2,000 women, you save one life, and you over treat somewhere between 10 or 20 women. You're always overtreating way more people than lives you're actually saving. So the suggestion that we should do even more of these tests before we've perfected the ones we have does not make sense to me.
I do multiple brain scans a week and so many of them show incidental findings. Even though I'm a neurologist and I see brain scans all the time, I don't know what to make of half of them. We just don't yet know how to properly interpret these scans. We need to pay more attention to detecting symptomatic disease early, rather than trying to detect asymptomatic diseases that may never progress.
In some cancers—prostate cancer, for instance—patients can opt for watchful waiting rather than treatment. Should this be the norm for early detection?
If you're going to go for screening—and I don't want people not to go for the suggested screenings—you do need to understand the uncertainties and realize you don't have to panic. Of course, the minute you hear there's some cancer cells, the panic kicks in, and you want it out and you want the maximum amount of treatment. But actually, in medicine, a lot of decisions can be made slowly. There are watchful waiting programs.
I want to suggest to people that, before you go for the screening, know these uncertainties exist, so that you can decide before the test comes back positive what you think you'd likely want to do, and then you can take time to think about it afterwards, and you can ask for a watchful waiting program.
I think one of the solutions would be to call these abnormal cells that we find on screening something other than 'cancer.' The moment you hear that word, people's immediate reaction is to get it out, because otherwise they think they will die of it. Watchful waiting is just something people find hard to do.

Sarah Paquin says her 9-year-old son, Declan, takes dextroamphetamine for his ADHD. It's a commonly prescribed stimulant that Declan had been taking for years. As Sarah tells PEOPLE, her husband, David, was about to give him the medication, which they'd picked up from Shoppers Drug Mart pharmacy in Comox, B.C., on May 28. "It's very chaotic in the morning, and if my husband had just been distracted with the other two, or been tired from being up with the baby the night before, this could have ended totally differently," Sarah, who has a 3-year-old and 10-month-old, tells PEOPLE. That's when David looked at the bottle, and realized he was holding someone else's prescription — and it was for hydromorphone. It's a profoundly strong opioid, the U.S. Drug Enforcement Agency explains, with 'potency approximately two to eight times greater than that of morphine and has a rapid onset of action.' As the National Library of Medicine says, 'Accidental ingestion or intentional abuse can lead to overdose and potentially life-threatening respiratory depression.' In a statement to PEOPLE, Loblaw Companies Ltd., which owns Shoppers Drug Mart, said, 'Upon review with the store, we have learned this was a case of human error, one that never should have happened. We have controls in place to minimize risks like this – where the patient was handed the wrong prescription bag – and the Associate will review these with employees to avoid a similar situation in the future. The owner of this location has reached out to the patient's parents to apologize for any undue stress this may have caused, and to outline the corrective steps.' "To do that to a 9-year-old child, it would have been devastating," she tells PEOPLE. She says her family has filed an official complaint with the College of Pharmacists of British Columbia a regulatory agency, and part of the complaint, she says, is "we would like some more strong and strict procedures put in place for picking up such strong heavy medications, especially when they pertain to a child." Her son's medicine, she said, was eventually found in the pharmacy's outgoing box. However, they won't be returning there for their prescriptions. "Trust has been broken," she tells PEOPLE, and urges everyone to inspect their medications before leaving the pharmacy: "Some simple little mistake like that can end somebody's life or cause serious harm." Invasive tick species found in Berrien County

GRAND RAPIDS, Mich. (WOOD) — The first in the state have been found in Berrien County. Calvin University researchers found the two ticks during routine surveillance at Grand Mere State Park, according to the Michigan Department of Agriculture and Rural Development. 'Calvin research students have been studying the ecology and distribution of ticks in West Michigan since 2020,' said William Miller, assistant professor of biology at Calvin, in a statement. 'The students worked with our close partners at Michigan State University and collaborating state agencies to initially identify these ticks as nymphal longhorned ticks.' The ticks were sent to the U.S. Department of Agriculture's National Veterinary Services Laboratories, according to MDARD. On Wednesday, they were confirmed to be Asian longhorned ticks. City crews to start oak wilt treatment at Aman Park The ticks are native to east Asia but were first found in New Jersey in 2017, according to MDARD. In the following years, they have spread to more than 20 other states. Females can lay up to 2,000 eggs at a time. The ticks have been known to form large infestations on a single animal, according to MDARD. This can stymie the animal's growth, and in severe cases, the blood loss can even kill the animal. Officials say these ticks are not known to carry Lyme disease, but they can cause diseases that impact humans and animals, like Rocky Mountain spotted fever, Powassan virus and heartland virus. Outside of a lab, MDARD says Asian longhorned ticks collected in the U.S. haven't been confirmed to carry these diseases — but they have been found to carry a pathogen that causes bovine theileriosis. That's why experts are especially concerned about how these ticks can affect livestock. If you have animals, you're urged to take preventative measures. MDARD says you should have your animals treated regularly, check your animals often for ticks and keep your lawns and pastures mowed and clear of brush. Is It Actually Organic? A Mattress Testing Expert Breaks Down Natural and Organic Certifications

Many mattress and bedding brands use 'natural' and 'organic' in their descriptions, but what does that really mean? Here's what to look for. As a professional mattress tester, I've seen an upward trend in the past few years of people looking into organic mattresses, toppers, and sheets. Whether it's because of a renewed focus on what is truly 'healthy' and toxin-free, or a concern over the use of fiberglass (I know we all remember that blowing up on TikTok), more brands are increasingly getting products certified. However, just because someone's throwing on a label and using buzzwords like 'natural' and 'organic' doesn't mean it's the real deal. Nor does it mean it truly qualifies as organic—you really have to look closely to verify. If I see a brand doing only that, I give them some serious side-eye. When it's truly the natural stuff, organic sleep products aren't cheap because they involve top-of-the-line materials. Here are common logos and certifications you'll see and what materials they're often associated with, so that you can discern what kind of bedding you're buying like a pro. If You See This, Know It's the Real Deal If any of these logos pop up on your search, it's a good sign that the product you're looking at is truly certified organic at a global scale. Each of these organizations has a database where you can look up brands and products, so to be 100 percent certain, I recommend you take that extra step just to verify. Courtesy of Global Organic Textile Standard Global Organic Textile Standard (GOTS) is a certification that upholds rigid guidelines on the production of organic cotton and other textile fibers. Bottom line: GOTS verifies that non-GMO plants are used, along with no potentially poisonous farming pesticides. The organization also evaluates the environmental impact of growing these materials, production and human rights standards, manufacturing, and distribution. There is also third-party certification used to verify that materials are truly of the utmost quality. You can look up GOTS products within the organization's database to cross-check. Courtesy of Global Organic Latex Standard Global Organic Latex Standard, otherwise known as GOLS, is a certification specifically pertaining to latex products and the purity and quality of the raw material used. Latex is analyzed at every stage of the game—from being harvested on rubber sap farms to being processed into materials used in mattresses or other latex-involved products. GOLS-certified products have to prove that they followed these steps to a T, and that they generated minimal environmental impact in the process. Eco-Institut is a German-based organization and laboratory that will test products for dangerous substances that can infiltrate air quality and compromise human health. These substances can include VOCs and formaldehyde. It also serves as cross-verification for other organic certifications, like GOTS. You can look up certified products within its database as well. Not Organic, but Health-Focused The following certifications are still good signs to see, but they aren't going to qualify a product as truly 'organic' by itself. This could be because the evaluation process is handled at the final stages when the product is already made, as opposed to checking in throughout the entire supply chain process. You can also check on each site's database to see if a product or brand made the cut. Oeko-Tex Standard 100 international certification is awarded to products that are confirmed not to have 1,000 plus substances that are harmful to human health. The list is reviewed yearly for upkeep, and you can verify if a brand is truly certified by checking the label within the organization's database. Can never be too careful! UL GreenGuard Gold certification is tied to air quality—particularly the emissions generated by volatile organic compounds (VOCs). Since air circulation is a lot more retained inside than outdoors, there really is no 'breathing room' so to speak for emissions to air out. For mattresses offgassing, followed by you then sleeping on them nightly, you may not feel so hot if there are high emissions. This is something to consider, especially with 'natural' memory foams. There is a standard Greenguard certification as well, but the Gold certification passes higher going to see GreenGuard Gold certifications a lot in the organic certification sphere. It's very important, but it's equally as vital to understand that this doesn't qualify a product as 'organic' alone. You can use the UL SPOT database (Sustainable Products of Trust) to verify certified products. UL also has a Formaldehyde-Free certification that gives a stamp of approval to products that don't incorporate this harmful substance into its makeup. Courtesy of Environmental Working Group EWG (Environmental Working Group) is an organization dedicated to understanding the long-term ramifications of certain chemicals and how they interact with our bodies, especially when they make skin contact. EWG measures whether these kinds of chemicals are used in products. Specifically, it looks for over 10,500 substances that could result in bodily harm, like cancer, and potential damage to your endocrine and reproductive systems. It's a certification used for cosmetics, skincare, and of course, mattresses because they all directly touch your skin. What About the USDA? You may be wondering, why no US Department of Agriculture organic certification on this list? The bottom line is that many mattress and sleep product manufacturers have an international presence, which requires a separate set of rules and regulations outlining product quality.
