
Tamil Nadu first to integrate ‘predicted possibility of TB deaths' in patients to its State TB elimination programme
The predictive model, developed by the Indian Council of Medical Research (ICMR)-National Institute of Epidemiology (NIE) and launched last week, will aid the State in bringing down the TB mortality rate, Asha Frederick, State TB Officer, Tamil Nadu, said.
The predictive death model has been developed using data from nearly 56,000 TB patients diagnosed in public health facilities across Tamil Nadu between July 2022 and June 2023, Dr. Frederick said.
Research shows TB is among the leading causes of morbidity and mortality worldwide, and more than 70% of TB deaths occur in the first two months of TB treatment.
Globally, India has the highest burden of TB, with two deaths occurring every three minutes from TB. These deaths can be prevented. With proper care and treatment, TB patients can be cured, the World Health Organization has said.
The addition of the new feature in the Tamil Nadu TB elimination programme, Dr. Frederick said, would merge it with the existing TB SeWA (Severe TB Web Application), in use since 2022 under the Tamil Nadu Kasanoi Erappila Thittam (TN-KET) differentiated care model initiative.
'What this new feature will do is to alert frontline staff on how to recognise a severely ill TB patient from a given list of medical indications, including body weight, ability to stand without support, etc., so that they are given priority in hospital admissions, and treatment is initiated without delay. The predicted probability of death ranges widely — from 10% to as high as 50%, depending on how many of the five risk factors are present. In contrast, for patients not flagged as severely ill, the predicted probability drops sharply to just 1% to 4%,' Hemant Shewade, senior scientist at the NIE, said.
The average time from diagnosis to admission of a TB patient in Tamil Nadu is one day, and around a quarter of severely ill patients still face delays of up to six days in the State, Dr. Shewade said.
All 2,800 public health facilities in Tamil Nadu — from primary health centres to medical colleges — currently use the TB SeWA application alongside a paper-based triage tool.
Old age, TB/HIV co-infection, and a baseline body weight of <35 kg increase mortality during TB treatment, a study titled 'Time to Death and Associated Factors among Tuberculosis Patients in Dangila Woreda, Northwest Ethiopia' has found. Special follow-ups of TB patients during the intensive phase, and of older patients and TB/HIV co-infected cases, as well as nutritionally supplementing underweight patients, may be important interventions to consider in order to reduce deaths during TB treatment.

India in grip of silent salt consumption epidemic: ICMR-NIE

The scientists have initiated a community-led salt reduction study to address the issue and are focusing on low-sodium salt substitutes. While the World Health Organisation recommends less than 5 grams of salt per person per day, studies show that urban Indians consume around 9.2 grams/day, and even in rural areas, it is around 5.6 grams/day - both higher than recommended. One promising tool in this effort is low-sodium salt substitutes -- blends where part of the sodium chloride is replaced with potassium or magnesium salts, Dr Sharan Murali, a senior scientist at National Institute of Epidemiology (NIE) and principal investigator of the study, said. "Lesser sodium consumption helps reduce blood pressure and improves overall heart health, making low-sodium alternatives a meaningful switch, especially for those with hypertension," Dr Murali stated. "Just switching to low-sodium salt can lower blood pressure by 7/4 mmHg on average, a small change with a big impact," Dr Murali said. To tackle the issue of high salt consumption, the NIE has launched a three-year intervention project in Punjab and Telangana, supported by the Indian Council of Medical Research. The goal is to evaluate the effectiveness of structured salt reduction counselling in reducing blood pressure and sodium intake among individuals with hypertension, senior scientist at NIE, Dr Ganesh Kumar, who is also a part of the study, said.

UNAIDS 2025 report warns of funding gaps in fight against HIV as India battles stigma, discrimination
Also, in India, more than 80 percent of surveyed women aged between 15 and 49 years were reported between 2020 and 2024 to lack autonomy over their sexual choices and reproductive healthcare. Additionally, over 90 percent of them lacked information about contraceptive use. The report titled 'AIDS, Crisis And The Power To Transform', released on 10 July, also highlights that the prevalence of intimate partner violence among partnered or married women living with HIV in the same age group in India stood at 24 percent—just 10 percent less than Liberia, where the prevalence was found to be highest globally. The figures are based on data from 2020-2024. New Delhi: The 2025 Global AIDS Update by UNAIDS (Joint United Nations Programme on HIV/AIDS) shows that 30 percent of people surveyed in the 15-49 age group in India reported facing HIV-related stigma and discrimination. The 2025 update notes the impact that massive funding cuts by international donors are having on countries most affected by HIV, and how the threat of a funding crisis may hurt global progress in the fight against HIV/AIDS unless countries alter their programming. According to the UNAIDS Executive Director Winnie Byanyima, the Global HIV response shows a decline of newly HIV infected persons by 40 percent, and of 56 percent deaths since 2010. However, the withdrawal of the US President's Emergency Plan for AIDS Relief (PEPFAR), the biggest contributor to global response, may lead to 6 million additional people being infected, and an additional 4 million deaths by 2029. The funding crisis has disrupted treatment and prevention programmes around the world, the report says. But it also points to the emergence of new prevention tools, like Lenacapavir, a long-acting injectable pre-exposure prophylaxis (PrEP), 'which has shown near-complete efficacy in clinical trials'. It can prevent HIV with injection doses twice a year. The manufacturer of Lenacapavir, Gilead Sciences, has licenced six generic manufacturers, of which Dr Reddy's Laboratories, Emcure, Hetero, and Mylan—subsidiary of Viatris—operate in India. However, affordability and access are still big challenges. According to the report, in terms of region-wise distribution, Eastern and Southern Africa saw the highest new HIV infections in 2024 at 37 percent, the lowest being in the Caribbean at one percent. In Asia and the Asia Pacific, of which India is a part, the figure stands at 23 percent. With respect to people living with HIV in 2024, the highest was reported in Eastern and Southern Africa at 52 percent, or 21.1 million. The lowest number of people was in the Caribbean—one percent, or 3,40,000. In Asia and Asia Pacific, the number was 17 percent, or 6.9 million. India is among the 13 of the 35 reporting countries that have reached safe injecting targets—90 percent using sterile equipment at last injection. It is clubbed with South Africa, Thailand and Brazil under Low-Dependency Countries in the matrix of dependency on PEPFAR funding. The report also mentions that social enterprises help diversify revenue resources. In India, the Mist LGBTQ Foundation runs an online platform to market merchandise, and offer HIV self-testing and PrEP consultations, either free or at a low rate. Another organisation, the Network of Maharashtra People Living with HIV TAAL+ Pharmacy sells antiretroviral and other medicines at discounted prices via corporate social responsibility partnerships. The goal is to evaluate the effectiveness of structured salt reduction counselling, delivered by health workers at Health and Wellness Centres (HWCs), in reducing blood pressure and sodium intake among individuals with hypertension, senior scientist at NIE, Dr Ganesh Kumar, who is also a part of the study, said.. "We are currently in the first year of the project, focused on baseline assessments and field preparations," Dr Kumar said. "Counselling materials are not yet finalised; rather, we aim to co-create the intervention package with the community health workers, drawing on their experiences and incorporating their suggestions. It's not just about delivering health education - it's about listening, understanding, and building together," Dr Murali said. To ensure interventions are grounded in reality, the NIE conducted a market survey across 300 retail outlets in Chennai to assess the availability and pricing of low-sodium salt (LSS). They found that LSS was available in only 28 per cent of retail outlets. It was seen in 52 per cent of supermarkets, but a dismal 4 per cent in small grocery shops. The price of LSS averaged Rs 5.6 per 100g, more than twice the price of normal iodised salt (Rs 2.7 per 100g). These findings highlight a critical supply-demand disconnect, Dr Murali said. "The lower demand for low sodium salt might be leading to its lower availability - it's a proxy indicator of awareness and access," noted Dr. Murali. To spark a public conversation around salt reduction, the NIE has also recently launched the #PinchForAChange campaign on Twitter and LinkedIn through ICMR-NIE. Using infographics, facts, and simple messages, the campaign aims to raise awareness about hidden salt sources, promote low-sodium alternatives, and empower individuals to make heart-healthy choices. "If successful, this project could lead to the integration of sustainable dietary counselling models into the existing public health system. It can bridge the gap between knowledge and action, improve health literacy, and ultimately reduce the burden of hypertension-related diseases. "This is not just about reducing salt. It's about restoring balance in our diets, our systems, and our hearts. Together, one pinch at a time, we can create lasting change," Dr Murali added.
