
Most COVID-19 Drugs Fall Short, Meta-Analysis Confirms
No treatment reduced mortality, although some treatments were associated with a shorter duration of symptoms. These findings are expected to guide future World Health Organization (WHO) treatment recommendations.
SARS-CoV-2, the virus responsible for COVID-19, emerged in late 2019 and continues to circulate worldwide. In 2024, Italy recorded 3154 COVID-19-related deaths, according to the Istituto Superiore di Sanità, despite a sharp decline in surveillance. Since the start of the pandemic, more than 8000 clinical trials have been conducted to identify effective treatment options.
Disease Phases
COVID-19 progresses through two distinct phases: An early viral replication phase (mild disease) and a later inflammatory phase (severe disease), which may lead to a cytokine storm, respiratory failure, and multiorgan dysfunction. As each phase involves different biological mechanisms, the treatment effectiveness must be assessed accordingly. This meta-analysis specifically focused on patients with mild-to-moderate COVID-19, a population for which therapeutic evidence remains limited.
Study Design
Researchers conducted a systematic review and network meta-analysis using the COVID-19 Living Overview of Evidence database, which compiles all randomized clinical trials related to COVID-19. The final analysis included 187 trials with data from 166,230 participants, which compiled all randomized clinical trials related to COVID-19.
Mild-to-moderate COVID-19 was defined according to the WHO criteria as follows: oxygen saturation ≥ 90% on room air; respiratory rate ≤ 30 breaths per minute; and no signs of respiratory distress, acute respiratory distress syndrome, sepsis, or septic shock. The primary outcomes included mortality, hospitalization, mechanical ventilation, adverse events leading to treatment discontinuation, and time-to-symptom resolution. The certainty of evidence was assessed using the GRADE approach.
Hospitalization and Duration
Paxlovid reduced hospitalizations by 25 per 1000 patients treated, with an estimated range of 20-28 fewer cases and moderate-certainty evidence. Remdesivir had a slightly smaller effect, preventing 21 hospitalizations per 1000 patients (range, 7-28 fewer), also with moderate certainty. Molnupiravir and corticosteroids may help reduce hospitalizations, but the evidence is of low certainty. Doxycycline shortened hospital stay by 1.33 days, with an estimated range of 0.03-2.63 days shorter, supported by moderate-certainty evidence.
Lopinavir-ritonavir (Kaletra) increased hospital stay by 1.77 days (range, 0.34-2.19 days longer) and was the only treatment clearly linked to adverse events that led to stopping treatment, both with high-certainty evidence.
Symptom Duration
Azithromycin showed the greatest benefit in shortening symptom duration, reducing it by an average of 4 days (typically from 5 days to 1 day), with moderate certainty. Other agents that shortened symptom duration by 2-3.5 days included molnupiravir, systemic corticosteroids, favipiravir, and umifenovir. All these findings were supported by moderate- to high-certainty evidence.
Limitations
The main limitation of this review was the small number of events for many drugs and outcomes, which resulted in serious imprecision. This limitation may be due to the study's focus on patients with mild or moderate COVID-19, who were less likely to be hospitalized or die. Consequently, many estimates remain uncertain. Symptom resolution data were also limited because of inconsistencies in how the studies measured and reported outcomes.
Conclusions
No drug demonstrated a significant benefit for most of the outcomes. Conversely, high-certainty evidence indicates that few treatments are ineffective.
'According to our classification, no drug was convincingly different from standard care for the outcomes of mortality, mechanical ventilation, venous thromboembolism, or clinically important bleeding. However, rating certainty using a different target may provide important insights. For example, some interventions suggested little or no effects. For mortality, low-certainty evidence suggested that nirmatrelvir-ritonavir and molnupiravir provided little or no benefit. For mechanical ventilation, moderate-certainty evidence indicated that JAK inhibitors provide little or no benefit to patients. Moderate-certainty evidence suggests that prophylactic-dose anticoagulation provides little or no benefit for venous thromboembolism and results in little or no increase in clinically important bleeding. Several interventions do not appear to offer any benefit for patient-important outcomes, including angiotensin-converting enzyme inhibitors/angiotensin II receptor blockers, aspirin, colchicine, fluvoxamine, hydroxychloroquine, ivermectin, and lopinavir-ritonavir. Lopinavir-ritonavir also increases the risk of adverse effects leading to treatment discontinuation and prolongs hospital stay,' the authors said.
The authors added that drug efficacy is not the only factor that clinicians should consider.
'Some of the included drugs have downsides that were not captured in this review; however, further details are available in the guidelines. For example, molnupiravir may be carcinogenic, nirmatrelvir-ritonavir has numerous critical drug-drug interactions, and remdesivir is administered intravenously. Thus, while this review provides an overview of the evidence, individual considerations — such as patient values and preferences, contraindications, drug-drug interactions, and drug-specific adverse events not included in this review — must be considered when deciding whether to use any medication' the authors concluded.

The prunes found on your grocer's shelves are typically made from one plum varietal, an improved offshoot of the legendary Petit d'Agen plum, which originated in France and is grown in California. Though integral to many Americans' diets, prunes are especially beloved throughout France, Eastern Europe, Germany, and Italy, where they're oftentimes incorporated into baked treats and savory dishes. Broadening your prune horizons can give you a leg up healthwise—and on the culinary front. The Benefits of Prunes Prunes are delicious, nutritious, and brimming with benefits. "I feel like the term 'superfood' is a wee bit overused—but I do think folks would be surprised to know how nutrient-dense prunes actually are!" says Desiree Nielsen, RD, host of The Allsorts podcast, and author of the cookbooks Plant Magic and Good for Your Gut. Bone Health The antioxidants help protect cells from damage, and they also figure prominently in bone health, notes Lauren Manaker, MS, RDN, LD, CLEC, CPT. "Studies show that prunes not only support bone density, but also have anti-inflammatory and antioxidant properties, which may play a role in protecting bones thanks to their unique combination of nutrients, and phenolic compounds, or natural plant-based antioxidants that help reduce inflammation, and oxidative stress, both of which are associated with weaker bones over time," Manaker says. Demographics Data indicates that consistent prune intake has positive bone health outcomes among various populations, including younger females who take certain oral contraceptives, older men, and post-menopausal women. Low bone mass or osteoporosis is of particular concern to the latter group, who may be disproportionately affected. "Studies suggest that 50–100 grams of prunes daily, alongside proper calcium and vitamin D intake, helps post-menopausal women maintain their bone health," Nielsen says. Supporting nutrients: "Prunes are packed with a unique matrix of bone-supporting nutrients, like vitamin K, magnesium, and boron, that work together to help maintain strong, healthy bones," Manaker adds. Muscles and More Vitamin K: This vitamin is also important for blood clotting and bone metabolism. Each serving of prunes provides 20 percent of the DV for vitamin K, Manaker notes. Potassium: A serving of prunes has about 280 milligrams of potassium (6 percent of the DV). In addition to maintaining healthy bones, this mineral is important for muscle contractions and fluid balance, she adds. Heart Health The potassium in prunes also aids in regulating blood pressure. And prunes are linked to heart health outcomes. "One study suggests that daily consumption of 50–100 grams of prunes improves cardiovascular health risk factors in postmenopausal women as exhibited by lower total cholesterol, oxidative stress, and inflammatory markers," Manaker says. Blood Sugar Regulation Prunes have a lower glycemic index (GI) than dried apricots, pears, dates, figs, and other fruits. "Dried fruits tend to be higher on the glycemic index, but prunes are an exception: they have a low GI in the range of 29–40," Nielsen says. A lower GI means they cause a slow and steady rise in blood sugar, rather than a dramatic "Contrary to popular belief, eating fruit—rich in fiber, vitamins, and antioxidants—is a smart choice for maintaining healthy blood sugars and reducing your risk of type 2 diabetes," Nielsen with proteins: People with diabetes can still enjoy fruit, says Nielsen. She suggests pairing fruits with foods rich in protein and fiber for better blood sugar support. "Eating 4 to 5 fiber-rich prunes with some almonds makes an easy blood sugar-friendly snack," Nielsen says. Digestion Prunes are a classic go-to for digestion support. Relief from constipation: "Prunes contain the soluble fiber pectin, polyphenols, and a naturally occurring sugar alcohol called sorbitol that are all thought to help support better bowel habits," Nielsen says. Sorbitol draws water into the bowel to hydrate stools while pectin adds volume, making them easier to pass. Prune alternative: Prune juice, Manaker adds, is also an effective choice to help support "Sure, prunes can help with constipation, but eating a few prunes won't send you urgently to the bathroom," Manaker says. If you're concerned that things may be moving too quickly, pair them with other foods and space out eating them, she to Eat Prunes Small and snackable, prunes are perfect on their own, but they're also very versatile. Here's how to make them a dietary staple: With chocolate: "I love snacking on prunes straight up, but a little chocolate never hurts!" says Nielsen. She suggests making chocolate-dipped prunes or dark chocolate prune bark by pouring melted dark chocolate over prunes and freezing until firm. Cereal: Add chopped prunes to your granola or morning oatmeal. Smoothies: "I use them to naturally sweeten smoothies while keeping the glycemic index lower than sugary additions," Manaker says. She teams them with cherries and lime zest for a refreshing, tangy-sweet blend. Chicken dishes: Prunes shine in savory dishes, too, especially with chicken, Manaker says. "They add a rich, slightly sweet flavor to recipes like Moroccan-inspired tagines with warm spices or stuffed chicken breasts with goat cheese and herbs for a sweet-savory twist. But just a few minutes of gentle movement outdoors can reduce blood sugar levels after a meal and help you wind down. 'I go for a walk every evening, which allows me to process the day's information as my blood flow increases to my brain,' U.K. psychologist and well-being specialist Lee Chambers told HuffPost. 'This mental tidying up gives my busy mind a level of peace, meaning I go to bed with a quiet environment and a quiet internal dialogue.' You can do this simple mindfulness exercise while lying in bed. 'Bring attention to each part of your body, starting with your head and moving slowly down until you reach your toes,' he told HuffPost. 'Pay attention to the sensations you notice in your body with a sense of warmth and non-judgmental curiosity. This activates the brain network critical for self-regulation and inner balance. It's also a great way to de-stress and let go of all the tension that builds up in our busy lives.' Grosso also suggests practices like yoga nidra (a meditative technique in which you 'cycle your awareness around your body,' she said) or gentle stretching. Either will activate your parasympathetic nervous system — the one responsible for the body's 'rest and digest' response. This method 'helps you feel more calm and drift off to a night of restorative sleep,' she said. 'Over time these practices can help your body more reliably and quickly return to a sense of ease and emotional regulation after a stressful event.' Reflect on the day. Related: Many of us are more likely to fret about all the things we haven't checked off our to-do list on a given day than we are to stop and appreciate the progress we made. 'Whether written or reflected upon, note the steps taken towards a goal, the challenges overcome or tasks completed, no matter how small,' Chambers said. 'And that feeling of progression will create the groundwork for some forward momentum on the following day.' Dahl also called self-reflection 'a great practice to end a busy day.' He suggests taking a moment to think about what you learned and how you grew that day. 'See if you can reframe stressful events as opportunities for self-discovery or to align with your most cherished values or guiding principles,' he said. 'This simple practice strengthens our capacity for insight and can help us to approach the next day with an open mind that is ready to learn.' End your day with gratitude. Before you go to sleep, write down three to five things you're thankful for. Santos pointed to the research of psychologist Robert Emmons, a professor at the University of California, Davis. He and his colleagues 'found that the simple act of listing your blessings in life can lead to significant improvements in your well-being,' she said. 'So make your night-time gratitude list a daily habit.' Dahl also recommends closing out the day with a short gratitude practice. 'If you spend a few minutes reflecting on people you appreciate and things you're grateful for as part of your bedtime routine, you'll naturally feel less stressed and more connected as you drift off to sleep,' he said.
