Air India crash: Not easy to ‘accidentally' cut off fuel switches, expert says

The London-bound Boeing 787 Dreamliner crashed into a medical college shortly after taking off from Ahmedabad Airport on June 12, killing 241 people on board.
Another 19 other people also died and 67 were seriously injured.
A preliminary report from India's Aircraft Accident Investigation Bureau, published on Friday, said both of the plane's fuel switches moved to the 'cut-off' position 'immediately' after take-off, stopping fuel supply to the engine.
'In the cockpit voice recording, one of the pilots is heard asking the other why did he cut-off. The other pilot responded that he did not do so,' the report reads.
The switches were then returned to their normal inflight position, which triggered an automatic engine relight and a thrust recovery procedure.
While the engines started to recover, one of the pilots transmitted 'mayday, mayday, mayday', before the plane was seen by air traffic control to crash outside the airport.
The fuel switches are used at the end of every flight and in emergency scenarios such as a fire, director of aerospace and aviation at Cranfield University, Professor Graham Braithwaite said.
They are designed so they cannot easily be 'accidentally' turned off and pilots would generally run through a checklist before doing so, he said.
'For obvious reasons, the two switches are a distance apart, so not a huge distance, but enough that you couldn't accidentally switch two when you're trying to switch one.
'So it's not like the lights in your house, where they're right next to each other, so there is some space between them.
'They're in that centre console, so that's in between the two pilots, so they can each reach them with the same ease.'
Professor Braithwaite said that if the switch had been moved by a person, that would have been a 'very unusual thing to do' at below 1,000 feet.
'It's not the point of flight where you try and call for your coffee, it's a period of flight where your focus is very, very clear, and that first 1,000 feet, it's about keeping the airplane climbing and that's not about clicking switches.'
He added: 'I could see why a nervous flyer would be nervous at the thought that it's possible to shut both engines down at a critical stage in flight, but for whatever reason, and that there are a number of things that are in place to stop it from accidentally happening.'
The aircraft was about 11 years old, the fuel switches had been changed two years ago, and the crash was a 'really, really unusual event', Professor Braithwaite said.
Of the next stage of the investigation, he said: 'If somebody did wilfully move a switch, then was it on the one hand a wilful active sabotage, in which case the investigation changes considerably, because this safety investigation that published the report yesterday will not be leading on that, that would be a police investigation.
'So that would change at that point, but if it wasn't clear that it was that wilful action, you'd be looking at, well, what kind of mistake might somebody have made?'
Of his general thoughts on the preliminary report, he said the authors have been 'very careful in their wording' to demonstrate they remain 'open minded' about what could have happened.
A lawyer advising some of the families affected by the crash said the findings were 'deeply concerning'.
Demetrius Danas, an aviation lawyer at Irwin Mitchell, said: 'Our thoughts and sympathies remain with those who've lost loved ones in such devastating circumstances.
'The findings of the initial report are deeply concerning and certainly merit further scrutiny to establish the full facts around how this tragedy happened.
'We're continuing to speak with and advise families who've been affected by the Air India crash. Understandably, all they want is to be provided with answers.'
The only surviving passenger on the plane was Briton Vishwash Kumar Ramesh, who previously told The Sun it was a 'miracle' he was alive but felt 'terrible' he could not save his brother Ajay.

Some 169 Indian passengers and 52 British nationals were killed, making it one of the deadliest plane crashes in terms of the number of British fatalities.
If the final investigation takes more than a year to complete, an interim report would be issued on the anniversary of the crash, Professor Braithwaite said, adding that interim recommendations could be made at any time.
The preliminary report says that all crew members had been breathalised on their arrival at Ahmedabad airport, which found they were 'fit to operate the flight'.
No significant bird activity was spotted near the flight path and the aircraft started to lose altitude before crossing the airport perimeter wall.
Both engines were retrieved from the wreckage and quarantined at a hangar in the nearby airport.
Investigators have identified 'components of interest for further examinations', the report says.
Fuel samples taken from tanks used to refuel the aircraft were 'satisfactory'.
Statements have been taken from Mr Ramesh and witnesses by investigators, who will also be looking at the post-mortem examination reports of the crew and the passengers.
This is being done 'to corroborate aeromedical findings with the engineering appreciation', the report adds.
An Air India spokesman said: 'Air India stands in solidarity with the families and those affected by the AI171 accident. We continue to mourn the loss and are fully committed to providing support during this difficult time.'
The spokesman added: 'Air India is working closely with stakeholders, including regulators.
'We continue to fully co-operate with the Aircraft Accident Investigation Bureau (AAIB) and other authorities as their investigation progresses.
'Given the active nature of the investigation, we are unable to comment on specific details and refer all such inquiries to the AAIB.'

Luckily, experts have shared their fool-proof top tips to stop your drains from smelling this summer. And one handy trick only costs 35p - and you might even already have it in your kitchen cupboard. Peter Jones, Director of specialist plumbing firm ADI Leak Detection, revealed that all you need is some white vinegar. Explaining why the issue is worse in summer, he said: 'Most people don't realise that hot weather is like rocket fuel for the bacteria living in their drains. 'What starts as a minor issue in spring can become a real problem by July if you don't know what to do about it. Peter JonesDirector of ADI Leak Detection 'When it's hot outside, the temperature in your pipes goes up too. 'Bacteria love warm, moist environments, so they multiply much faster in summer. Summer weather also causes water in your drain traps - those U-shaped bends under sinks and in floor drains - to evaporate more quickly. He added: 'Those water traps are there for a reason. They create a barrier that stops sewer gases from coming up through your drains. The cable-tie-scissor trick plumbers swear by to get hair clogs out of your drain & it's WAY better than pricey liquids "When that water evaporates faster in hot weather, you lose that protection and start smelling what's lurking down below.' Peter said that white vinegar is the one product that beats expensive drain cleaners every time. He revealed: 'White vinegar is brilliant for this job. "It's antibacterial, it neutralises odours instantly, and it's completely safe for your pipes. "Best of all, it costs pennies compared to shop-bought drain cleaners.' The trick is simple but effective, and Peter recommends mixing one cup of white vinegar with warm water. 'The vinegar kills bacteria on contact and breaks down any gunk that's built up in your pipes. "Plus, it's eco-friendly and won't damage your plumbing like some harsh chemicals can.' Peter breaks down exactly how to do this properly for maximum effect. Mix Your Solution - Combine one cup of white vinegar with an equal amount of warm water. He says: 'The warm water helps the vinegar work more effectively." Pour Slowly Down Each Drain - Start with your sink drain, then move to floor drains if you have them. Pour the mixture slowly to ensure it coats the sides of the pipes. Let It Sit - 'Give it at least 15 minutes to work its magic,' advises Peter. 'The vinegar needs time to kill the bacteria and break down any buildup." Flush With Hot Water Finish by running hot water down the drain for about 30 seconds to wash everything away. Other ways to keep your drains fresh PETER says that prevention is key for problems like this, as it's easier to nip it in the bid than it is to get rid of it once it starts. So he has shared additional tips to prevent problems before they take hold. He suggests: "Keep your water traps topped up, especially if you have drains you don't use often. 'Run water down them at least once a week to make sure that protective barrier stays in place.' Peter also recommends cleaning drain covers regularly. He adds: 'Hair, soap scum, and other debris collect around the edges. "A quick wipe with antibacterial cleaner once a week makes a huge difference.' Most importantly, Peter warns against letting grease and food waste go down bathroom sinks. He said: 'I know it's tempting to rinse greasy face creams or makeup down the sink, but that stuff builds up and creates the ideal conditions for bacteria to thrive.' The expert added: "What's great about this vinegar method is its simplicity. "You don't need special equipment or expensive products, just something most people already have at home. "I've been in the plumbing business for over a decade, and I've seen homeowners spend hundreds on professional drain cleaning when a weekly cup of vinegar would have solved the problem. 'During peak summer months, I recommend doing this every week, without fail. "If you're dealing with particularly stubborn smells, you can safely do it twice a week until the problem clears up. "The key is consistency, as it's much easier to prevent bacteria buildup than it is to get rid of it once it's established. "A few minutes of prevention each week could save you hundreds in repair bills down the line.'

Public urged to look out for exotic Jersey Tiger moths in parks and gardens

The public is being encouraged to look out for the exotic-looking species of Jersey Tiger moths that have been surging in numbers. Wildlife charity Butterfly Conservation said the day-flying moths appear to be doing well as people record butterflies they see for its annual butterfly count. The initiative, which takes place over three weeks in summer and is currently under way until August 10, gets members of the public to spend 15 minutes recording those they see in their garden, parks or countryside. This year, the charity is urging people to take part in what it describes it as a 'nationwide rescue mission' for the UK's ailing butterfly species. As the count reached its mid-way point, Butterfly Conservation said people have recorded seeing 5,300 Jersey Tiger moths, compared with a total of 3,496 for the whole of last summer's count. This marks a 78% rise from 2024 when measured as an average per county, with the increase attributed to the warm, dry conditions, which likely helped the species spread their wings in abundance and settle in parts of the UK they usually would not. The moths are recognisable by their black and white tiger stripe forewings and vibrant red-orange or yellow hindwings adorned with bold black spots. Butterfly Conservation said the Jersey Tiger was largely restricted to the Channel Islands, the south coast of England, London and south-east Wales just a decade ago. But now they are frequently spotted across southern England and are continuing to spread through East Anglia and into the Midlands, with the charity saying they have been photographed this year in Cardiff, Birmingham, Cambridgeshire and Essex. It comes in the wake of last year's record low numbers which were partly down to the wet spring and cool summer but conservationists warned they come on top of long-term declines in the UK. More than 80% of butterfly species have declined since the 1970s, with experts warning they have been hit by damage to their habitats, climate change and the use of pesticides. Butterfly Conservation said that while the looks of Jersey Tiger moths have caught the public's attention this year, sightings in unexpected places also tell an important story about climate change. Dr Richard Fox, the charity's head of science, said: 'Butterfly and moth numbers fluctuate naturally each year depending on the weather, and this summer's warm, sunny conditions have created a much more favourable environment than last year's cool, wet season, which saw numbers recorded during Big Butterfly Count plunge to record lows. 'It's certainly uplifting to see more butterflies and moths this summer, but one good year can't undo the long-term decline many species are facing.' Dr Fox said populations peaks during good years have grown smaller over the past five years, while bad years have seen low numbers dip further. He added that the increase in Jersey Tiger moth sightings is an example of how climate change 'is reshaping the distribution of wildlife'. 'While it's a delight to spot such a vibrant moth in gardens, parks and green spaces, it's also a reminder of how rising temperatures are altering our natural environment,' he said. 'This is why the Big Butterfly Count matters. The more people who take part, the more we can learn about how species are adapting, or struggling, in the face of climate change and extreme weather.' Data collected by members of the public during the Big Butterfly Count directly informs national conservation strategies, helping protect not just butterflies and moths, but the wider ecosystems they support.
